- ABSTRACT Background While rapid population ageing is occurring worldwide, its speed is especially prominent in Asian countries. In Asia, cultural diversity might significantly affect care burden; however, few studies have investigated the differences in care burden, and mental, physical and social conditions among Asian countries. This study aimed to clarify the situations of and differences in family caregivers (FCs) of older persons in Japan and Thailand, and evaluate the factors associated with care burden in both countries. Methods A cross‐national survey of 217 in‐home FCs was conducted in Japan and Thailand and mainly included items regarding care burden and psychosocial conditions. Differences between the two countries were obtained, and the factors associated with care burden were analysed by multiple regression analyses. Results The FCs' and care receivers’ mean ages (64.8 ± 11.1 and 84.8 ± 8.6 years, respectively) in Japan were significantly higher than the mean ages (49.1 ± 13.3 and 77.1 ± 9.5 years) in Thailand. In Japan, FCs had more severe care burden, loneliness, and stress compared to those in Thailand. In Thailand, FCs had more social connections and informal support than those in Japan. The factors associated with care burden were different in each country; thus, severe stress, low emotional care preparation, and low willingness to continue care at home were significantly associated with severe care burden in Japan, whereas poor relationships with care receivers, few social connections, low confidence in providing care, low emotional care preparation, and lack of informal support were significantly associated with severe care burden in Thailand. Conclusions Despite there being a long‐term care insurance system in Japan, which is absent in Thailand, care burden and psychosocial conditions of FCs might be worse in Japan. - Abstract Mild cognitive impairment (MCI) is common, affecting 10%-35% of people over 65, and around two-thirds overall develop dementia over their lifetime. Previous studies have reported that family caregivers (FCs) caring for older people with MCI experience considerable burden, which tends to cause various neuropsychiatric symptoms, such as depression, apathy, and irritability. However, it is not clear how FCs’ stress, which leads to increased burden, fluctuates within a day through their daily activities. The aim of this study was to explore the characteristics of stress variability in FCs. We recruited one dyad of an older adult with MCI over the age of 65 and a FC. Data collection was performed continuously for 7 consecutive days. We adopted smart watches (GARMIN vívosmart 4) to measure Heart Rate Variability (HRV)-based stress. Moreover, we asked a FC to talk with a Virtual Agent (VA) on a laptop freely to understand when and what kind of daily activities were done and the FC’s feelings. Detecting change points in time series data was performed using Python 3.9. Results showed that change points in the FC’s daily stress appeared when the FC starts specific activities such as preparing meals and confirming some medicine that an older adult with MCI should take and daily schedules. The FC also expressed anticipatory grief at the prospect of future dementia through the VA. - The current pre-/posttest pilot study investigated the impact of an individual nurse-led active listening intervention for spouses of individuals with depression (herein referred to as patients) on spouses' psychological states and patients' depressive symptoms. Sixteen couples participated in the study. Individual sessions were conducted over 10 weeks to help spouses express their thoughts and feelings. Psychological measurement scale scores did not change markedly postintervention for spouses; however, their subjective evaluations of the intervention were positive. In the qualitative analysis, spouses stated that they were able to express their thoughts and feelings and that the sessions were meaningful. Moreover, postintervention depressive scores of patients improved significantly. Findings suggest that the nurse-led intervention of active listening for spouses may provide a better environment for improving the depressive symptoms of patients. - This study aimed to evaluate the factors related to the mental health of foreign care workers in Japan’s long-term care (LTC) facilities and compare their results with those of native care workers. We conducted a cross-sectional survey covering 80 LTC facilities across Japan between August and November 2021. The survey mainly included mental health, workload, reward, sense of coherence, loneliness, COVID-19-specific factors and sociodemographic variables. The results show that workload was a distinct feature associated with the mental health of foreign care workers (n = 172) when compared with those of native care workers (n = 154). In addition, we found that the relationship between cultural adaptation and mental health in a sample of foreign care workers was mediated by loneliness and sense of coherence (SOC). Finally, we found that reward, loneliness, SOC, and COVID-19-specific factors had significant impacts on the mental health of both foreign and native care workers. These findings highlight the importance of support measures from the workplace for foreign care workers. - Abstract Aims This study was to compare the therapeutic efficacy of Tai chi and conventional exercise on physical performance, sleep efficiency and quality of life among older people with dementia. Methods Eighty-one elderly mild to moderate older people with dementia living in a nursing home in China were screened and randomized to three groups. The intervention groups received a 12-week Tai chi or conventional exercise intervention. Participants were assessed at baseline, week 6, and week 12 between December 2020 and July 2021. The primary outcome was short physical performance battery, secondary outcomes including the sleep efficiency and quality of life. Generalized estimating equations and one-way analysis of variance were the main statistical tests. Results The mean age was 80.7 (SD 7.7) years and 74.1% of participants were women. Analysis indicated that there was a significant difference in both intervention groups in short physical performance battery, sleep efficiency and quality of life relative to control from 0-12 weeks, and the Tai chi group had a substantial advantage over the conventional exercise group. Conclusions Tai chi and conventional exercise could improve physical performance, sleep efficiency and quality of life in older people with dementia. - Abstract The rate of infection and magnitude of the effects of the coronavirus disease 2019 (COVID-19) pandemic has far exceeded the realm of what most people had previously experienced in their lives. There is an urgent need to ensure that strategies are in place to prevent and reduce anxiety in older adults. The present study aimed to assess anxiety in community-dwelling older adults and the related factors during the COVID-19 pandemic in Japan. A community-based cross-sectional study was completed in July 2021. The Japanese version of the Geriatric Anxiety Inventory was used to screen for anxiety symptoms. Demographical data and data for the following five other factors were collected: loneliness, mental well-being, perceived social support, coping behaviors to COVID-19, and fear of COVID-19. The association between anxiety and potential predictors was analyzed using binomial logistic regression. A total of 385 participants aged 65 to 86 years were included in the present study. 17% developed anxiety symptoms. Factors associated with anxiety were age, living alone, loneliness, significant people support, fear of COVID-19, and health monitoring as a COVID-19 coping behavior. Older age, living alone, and significant people support were associated with lower anxiety, and loneliness, fear of COVID-19, and health monitoring as a COVID-19 behavior were associated with higher anxiety in community-dwelling older adults during the COVID-19 pandemic. These findings can help to develop effective measures for reducing anxiety and will support the development of a psychological intervention for tackling the mental health of community-dwelling older adults during the COVID-19 pandemic.Oxford University Press (OUP), Nov. 2022, Innovation in Aging, 6(Supplement_1) (Supplement_1), 463 - 463Scientific journal

- Abstract In Japan, care-giving sons are noted for their high rates of abuse of care recipients. This study revealed, for the first time, the difficulties unique to sons caring for their mothers. All sons used long-term care insurance services. However, they provided care that could not be covered by such public services while harbouring psychological distress. In this study, we interviewed 13 sons living with their older mothers as the primary carer and conducted a thematic analysis focusing on their psychological distress. As part of our study findings, we have identified nine themes during analysis: uncomfortable feelings that cannot be mitigated as a son; feeling of futility in being unrecognised as a carer; fear of losing something important; a strong sense of responsibility for one's mother's life; disappointment in being unable to obtain understanding and co-operation from one's siblings in providing care; irritation and anger towards one's wayward mother; guilt about treatment of one's mother; sense of exhaustion due to demanding care; and experiencing hopelessness and despair. Thus, it is important to understand what psychological distress carers experience and to consider possible approaches for their support. The study findings provide suggestions for long-term care support in regions, such as East Asia, where the numbers of son carers are increasing inevitably owing to declining birth rates and ageing population, as in Japan.Cambridge University Press (CUP), Sep. 2022, Ageing and Society, 1 - 25, English[Refereed]Scientific journal

- AIM: To identify the factors influencing work engagement among psychiatric nurses. BACKGROUND: Work engagement is an essential outcome that promotes nurses' performance, but studies investigating its influencing factors remain limited in psychiatric settings. We hypothesized that job resources and personal resources positively predict psychiatric nurses' work engagement and have a particularly positive impact on engagement when job demand is high. METHODS: This cross-sectional study included 425 Japanese psychiatric nurses. Data regarding demographics, job resources, personal resources (emotional intelligence), job demand (patients' attitude towards nurses) and work engagement were collected followed by data analysis using hierarchical multiple regression. RESULTS: Reward, supervisor support, nurse-physician collaboration, other-emotion appraisal and use of emotions were positively related to work engagement. Patients' unpleasant attitude towards nurses boosted the association of reward and supervisor support with work engagement. CONCLUSIONS: Certain job resources, empathic ability and self-motivation ability may enhance work engagement. Furthermore, reward and supervisor support may be particularly useful when psychiatric nurses face patients' unpleasant attitude. IMPLICATIONS FOR NURSING MANAGEMENT: For enhancing psychiatric nurses' work engagement, nursing leaders should educate nurses' skills related to empathy and self-motivation, consider whether nurses are confronted with patients' unpleasant attitude, and improve the job resources.Corresponding, Mar. 2020, Journal of Nursing Management, 28(2) (2), 306 - 316, English, International magazineScientific journal

- AIM: Several studies have reported a negative correlation between depressive symptoms and family caregivers' (FCs) subjective sleep status. However, there is a paucity of information on the association between objective/subjective sleep status, care burden, and related factors. METHODS: Participants were 23 pairs of care receivers (CRs; Mage = 82.7 ± 8.5 years; 69.6% women) receiving long-term care at home and their FCs (Mage = 66.9 ± 11.0 years; 69.6% women). At baseline, demographic data, subjective sleep status (Pittsburgh Sleep Quality Index; PSQI), WHO-5 well-being, depressive mood, and frequency of going outdoors were collected. FCs wore a small, wrist-worn device with an accelerometer to assess objective sleep status for a consecutive 24-h 2-week period, and they answered the Zarit Burden Interview short version (ZBI) every night before sleep. After 3 months, CR status was collected and analysed retrospectively. RESULTS: The mean total sleep time over 2 weeks was 349.5 ± 69.6 min. The mean ZBI score over 2 weeks was 8.8 ± 6.8, which was significantly correlated with total sleep time (r = -0.42; P < 0.05), total time in bed (r = -0.44; P < 0.05), PSQI (r = 0.62; P < 0.01), frequency of going outdoors by CRs (r = -0.42; P < 0.05), and WHO-5 well-being among CRs (r = -0.50; P < 0.05). Multiple regression analyses revealed that total sleep time (β = -0.51; P < 0.05) was significantly associated with care burden (adjusted R2 = 0.45). At the 3-month follow-up, four CRs had been hospitalised or died, and their FCs displayed significantly severe care burden and slept less than at baseline. CONCLUSIONS: Reduced objective total sleep time is significantly associated with the severity of care burden among FCs. - AIM: To critically analyse the concept of dementia nursing competency in acute care settings. BACKGROUNDS: The nursing care needs of patients with dementia are increasing in acute care settings. However, a framework for the dementia nursing competency has not been explicitly outlined. DESIGN: Rodgers' evolutionary method for concept analysis. DATA SOURCES: Competenc* AND dementia AND nurs* AND acute OR hospital OR clinical were selected from the MEDLINE, CINAHL, PsycINFO, CiNii, and Google Scholar for publication from 2006 to 2017. REVIEW METHODS: All 29 articles were identified. Data were analysed with a particular focus on the attributes, antecedents, and consequences of the concept. RESULTS: Six attributes were identified: respecting patient individuality, applying ethical considerations, providing person-centred care, possessing responsibility, coordinating external environments, and possessing care coordination and facilitation skills. Identified antecedents included building theoretical and clinical knowledge, developing an awareness of dementia and its risk behaviours, performing assessments of dementia nursing practice, developing relationships with patients with dementia, and applying a team approach. Applying the competency led to positive consequences for patients, nurses, and patient-nurse and nursing team relationships. Methods : Semi-structured interviews were conducted for six nurses who had been working for more than five years at community-based clinics in the surrounding areas of Tokyo. Responses were analyzed using the Modified Grounded Theory Approach proposed by Kinoshita. Concept diagrams were drawn from the themes.
Results : Nurses regarded the integrated care of individual patients, their family and community as meaningful and rewarding because they were able to provide truly holistic care. They were also satisfied with "value of individual experiences of nurses", "frontier spirit of community nurses providing pathless integrated care", "appropriate work-life balance", "independent position in the clinic" and "community-based team approach including medicine, health and welfare".
Conclusion : Nurses felt easier providing their psychosocial intervention in a community-based clinic as part of a medical system. Nurses obtained rewards by using their holistic care ability. - [Dementia Care Mapping-Japanese version(DCM-J) as a research evaluation method for measuring quality of life among elderly patients with dementia: reliability and validity of Well-being and Ill-being value].AIM: Dementia Care Mapping is a behavior evaluation tool that can be used to measure and improve the quality of life of elderly patients with dementia. However, the reliability and validity of the Dementia Care Mapping-Japanese version (DCM-J) has not yet been established. Therefore, the purpose of this research was to clarify the reliability and validity of the Well-being and Ill-being (WIB) value of the DCM-J as a method for evaluating quality of life. METHODS: The study was conducted from April 1, 2005 to June 30, 2006. The participants included 130 elderly patients (men 31, women 99, average age 82.65+/-7.69 years) who were given a diagnosis of dementia. We established inter-rater reliability during a parallel observation method and also used test-retest for reliability. The correlation between the WIB value of the DCM-J and the Japanese Quality of Life Inventory for Elderly with Dementia (QOL-D) was used to establish criterion-related validity. RESULTS: Forty-nine (37.7%) subjects were given a diagnosis of dementia of Alzheimer's type, 80 (61.5%) had vascular dementia and 1 (0.8%) had dementia with Lewy bodies. The results showed correlation between the WIB value and social withdrawal measured by the behavioral category code (BCC) on the DCM-J and the three subscales of QOL-D: "interacting with surroundings", "expressing self", and "experiencing minimum negative behaviors". There was good internal consistency among these items. The interclass correlation coefficient was 82.32 (+/-5.85) for the WIB value of the DCM-J. The correlation coefficient of the retest, administered one week later, was 0.836 (p=0.001). The WIB value was significantly correlated with three sub-scales of QOL-D, and the correlation coefficient was greater than 0.53. CONCLUSION: We demonstrated that the WIB value of the DCM-J has good inter-rater reliability and test re-test reliability and criterion-related validity. In this study, the WIB value was shown to have similar reliability to the WIB value of the original DCM. Furthermore, our results suggest that the DCM-J could be useful for evaluating quality of life among elderly Japanese patients with dementia.Jan. 2008, Nihon Ronen Igakkai zasshi. - 日本学術振興会, 科学研究費助成事業, 基盤研究(C), 上智大学, 01 Apr. 2021 - 31 Mar. 2026日本における生活困窮者のホームレス化予防のあり方に関する研究研究チームでの会議5回、文献及び情報収集を行い、今年度の予定の調査枠組みの下準備を進めた。若年層ホームレスの定義を議論し、当初計画書で定義していた友人や知人宅、インターネットカフェや、漫画喫茶で避難している人も含めることになった。日本の文献を参考に若年ホームレスの定義を整理したところ、40歳未満、路上及び安い宿泊施設に泊まり続けている人、最低4日以上路上にいた経験のある人、屋根はあっても家がない状態、及び、屋根がない状態、ユースレフュージにいる人々、友人宅を泊り歩く人々、路上生活者以外の不安定居住層、簡易な小屋やテント、野宿している人であった。若年層ホームレス(ネットカフェ難民などを含めて)へのアクセスの可能性についてアルバイト求人サイト(女性も含めて)の情報を収集、ホームレス当事者へのヒアリングを行い、若年層がアクセスしやすい求人サイトについて情報を整理した。若年層ホームレスが具体的に寝る場所や、休息する場としてどのようなところにアクセスすると対象者にリーチできるか、20人ぐらいにアイデアを聞いてみる案を構想した。生活保護に至るまで/至らなさについては、若年野宿者の傾向として、雇用の不安定さは中高年野宿者と同様の傾向として見られるが、それ以外に「個人的理由」「家族といづらくなり」野宿化に至っている可能性が見出された。1)人間関係の希薄さ、家族との関係性、貧困、2)「前科」、ギャンブル 失業+X=住居の喪失の「X」に相当する要素として、人間関係、安心が検討された。 対象へのアクセスと具体的な調査項目について、先行研究から調査概念図(調査項目枠組み)案を作成し、チーム内で検討を行った。先行研究(欧米諸国)の若年層ホームレスの発生リスクと要因に関するコンセプトを参考に、概念図案を検討した。 コンセプト分析の結果を参考に概念図案を作成したが、先行研究の文献レビューを元に、概念図案の修正を行った。
- Japan Society for the Promotion of Science, Grants-in-Aid for Scientific Research, Grant-in-Aid for Scientific Research (B), Kobe University, 01 Apr. 2020 - 31 Mar. 2024A model development taking advantage of engineering technology for family caregivers caring for people with dementia in Asian Pacific Rim countries本研究では、認知症高齢者と家族介護者の生体情報(ストレス等)、睡眠及び行動・生活環境を、客観的デバイスを用いて24時間継続して縦断的に調査する。これにPC版Virtual Agent(VA)とのコミュニケーションを通した家族介護者の日々の質的データを統合して認知症高齢者と家族介護者の詳細な生活状況を把握する。 本年度は、システム情報学研究科と共同でVAのシステム構築を行った。VAが家族介護者へ声掛けを行う際の具体的発言内容と声掛けの時間・タイミングについて検討し、研究者が実際に試用して最終版を完成させた。Hamon(心拍・ストレス・体調等)、眠りスキャン、アクチグラフ(活動量)の作動とデータ蓄積状況を確認した後、1組の認知症でない高齢者夫婦で1週間すべての機器を用いて予備調査を実施した。環境・行動センシングの作動状況やVAの問いかけ・タイミング、Hamonによるストレスデータ収集のためのトランスミッタとiPhoneの接続状況、質問紙調査の内容と記載日時の確認等を行った。また、機器や使用方法などの理解を容易にするため、VA、アクチグラフ、ストレス測定用ウェア等に関する説明書を作成した。 本研究はタイ王国、中国でも実施予定であることから、中国語とタイ語で音声会話できるMP Agent技術の開発も手掛けた。英語、日本語、中国語、タイ語バージョンを入力し、音声認識とその音声を複製するサンプル試作を行った。現在COVID-19により海外での現地調査や予備調査を実施できない状況にあるが、環境が整い次第、タイ語版・中国語版VAを完成させ予備調査を実施していく予定である。
- Japan Society for the Promotion of Science, Grants-in-Aid for Scientific Research, Grant-in-Aid for Scientific Research (A), Kobe University, 01 Apr. 2019 - 31 Mar. 2023Assisting self care and mutual aid of elderly people with dementia at home based on mind externalization2020年度の成果は,前年度に開発した「こころ」センシング基盤を活用して様々な自助支援サービスを開発したことである.まず,スマートフォンアプリのLINEを活用して,高齢者の内的状態をアセスメントする「こころ」の見守りサービスを実装した.このサービスでは,高齢者の身体,精神,社交の3つの側面から,健康状態を尋ねる質問を作成し,LINEのチャットボットが高齢者に問いかけて状態を取得するものである.実装したサービスを,50代~80代の8名の高齢者,および,20代~40名の19名の非高齢者に対して,14か月の運用評価を行った. また,在宅の認知症当事者とその家族介護者を支援するために,宅内に簡易なセンサ付きスピーカーを設置し,場所と時間に応じた情報提示を行うシステムALPSを提案・実装した.ALPSでは,家族介護者がECAルールによって,情報提示の場所,時刻・曜日,内容を細かく設定でき,世帯ごとに異なる困りごとに柔軟に適応した情報提示が可能になっている. 互助支援サービスとして,高齢者の困り事とその対処法をコミュニティで共有するWebアプリケーションCompass4SLの研究開発も進めた.Compass4SLでは,高齢者がシステムに困りごとを入力し,対処法を検索できるとともに,支援する人が自身の知っている対処法を登録・共有する.対処法を見た高齢者は実際にその方法を試し評価を行うことで,困り事とその対処法が集合知として蓄積されていく. さらに,IoTやセンサを活用した宅内環境・行動センシングの技術開発にも注力した.定点カメラとコンピュータビジョン技術を用いて,寝たきりの高齢者を対象とした身体活動量を骨格センシングによって計測する手法や,宅内画像内で場所を矩形で定義し,そこに利用者が入ったかどうかを検知するBounding Box as a Locationサービス等を開発した.
- 日本学術振興会, 科学研究費助成事業, 基盤研究(C), 園田学園女子大学, 01 Apr. 2016 - 31 Mar. 2023認知症高齢者へのライフコースアプローチを基盤にした生きがい支援プログラムの開発本研究はスピリチュアルペインを抱えている認知症高齢者に対して、失われた本来の「生き方」に焦点をあてたライフコース的アプローチを基盤にした生きがい支援プログラムを開発することにある。 2019年ライフストーリー、ライフコース、ライフストーリ―の認知症高齢者へのケア場面での活用や定義、手法の実際を包括的にレビューし発表した。さらに、その結果をもとに研究協力施設に対して生きがい支援プログラム開発に必要な調査を行い、第1案を作成した。 2020年度はフォーカスグループに対して生きがい支援プログラム第1案を用いて修正を行うべく、施設調整を行っていた。しかし、高齢者施設においては新型コロナの影響による業務負担が大きく、加えて感染対策の面から協力を得ることが困難となった。そのため、実際にプログラムの有効性の検証は困難であった。しかし、施設研究協力者に対して、プログラム第1案(試用)を提示し現実可能性およびプログラム構成等に関する調査を行った。その後、プログラム修正を行い、第2案を作成した。2021年度は緊急事態宣言が長期に発令され、デルタ株も蔓延していたため、高齢者施設におけるプログラムの有効性の検証は、断念せざるえない状況となった。そのため、研究計画やプログラムの精選、文献レビューなど実施し、2022年度に継続して研究計画を再度修正した。
- Japan Society for the Promotion of Science, Grants-in-Aid for Scientific Research, Grant-in-Aid for Scientific Research (C), Mie University, 01 Apr. 2015 - 31 Mar. 2021Construction of an evacuation center management support project that considers the needs of vulnerable people requiring assistance in times of disaster.The purpose of this study is to establish a support method for local residents to manage evacuation center in consideration of vulnerable people requiring assistance in times of disaster. First, we created a program on know-how necessary for evacuation center management and regional disaster prevention in consideration of vulnerable people requiring assistance in times of disaster. Next, in cooperation with the city administration, we implemented a program for local residents and evaluated the effectiveness of the program. As a result, the level of knowledge and skills required for evacuation center management and disaster prevention for local residents has increased. In addition, the vulnerable people's disaster preparedness, knowledge of health management in evacuation center, and lower limb motor function improved. Furthermore, a system of cooperation between the city administration, university, and local residents was established.

According to the first phase of the qualitative survey (interview survey), the subjects had lost their social roles and solidarity with their families, had lost their daily contact with others, and had spent their time alone. Others, however, found new social connections and were able to escape isolation by participating in gatherings at the invitation of trusted supporters or acquaintances. According to the second phase of the questionnaire survey, the longer a person lived in their current residence, the more likely they were to have opportunities to consult with others, contact others, and enjoy interaction with others.

- Japan Society for the Promotion of Science, Grants-in-Aid for Scientific Research, Grant-in-Aid for Research Activity Start-up, Kobe University, 26 Aug. 2016 - 31 Mar. 2018Structuring of the Excercise Model for the Prevention of Sarcopenia among Elderly People with Diabetes in Japan and ThailandMany previous studies have recently been reported the relationship between type2 diabetes and sarcopenia among elderly people, and clarified that the lower muscle mass resulted in the lower mitochondria function of skeletal muscle, which caused the increase of insulin resistance. However, there is a lack of support for the prevention of sarcopenia among elderly people with diabetes. In this study, we examined the incidence and contributing factors of sarcopenia among 163 elderly with diabetes in Japan and Thailand with the high prevalence of diabetes among elderly people in Asia. The prevalence of sarcopenia was about 20% in both countries. Moreover, the results suggest that the incidence of sarcopenia is likely correlated to the weakening of community relathionship in Japan and the lack of consciousness to self-health management in Thailand.

Clinical nurses do not fully comprehend about importance of oral care to prevent oral disuse syndrome by stimulating the oral function and how to select the care methods and tools.In order to expand effective oral rehabilitation, clinical nurses should play central roles in the oral rehabilitation teams and coordinate between other medical professionals, primary care physicians, and patients.We suggest that the oral care learning program for nurses to improve patient’s oral function are need to conclude 5 contents as below. ①purpose and significance of oral care, ②anatomy and structure of oral cavity, ③features of the oral cavity in oral trouble in the elderly, ④ choosing and using of care tools in oral conditions ⑤multi-disciplinary team work.

In this system, maintenance and improvement of health is placed as a part of a disaster prevention measure. First, physical educators and disaster prevention coordinators for elderly people were developed, and next cooperated with administration and build this regional disaster prevention system. In the results, almost of participants were old-old, but motor function and mental function were increased or maintained. Furthermore, the awareness of disaster-prevention measures and the preparations have been strengthened.

Members contributed to grow up social relationship in their community and had been a challenging spirit with purpose and motivation toward everything and with self-awareness pf making a contribution to others. It includs the sense of self-progression and the sense of being spproved of by others.

The findings provided insight into the potential for clinical application and suggested directions for incorporating procedures implemented in other fields into nursing techniques. With the transition to more care-oriented rather than cure-oriented medicine, the theoretical development of care utilizing nurses' hand touch, 'TE-ARTE', and the popularization of this type of care are valuable. Further investigation is required to obtain additional details regarding the approaches toward structuring the science of'TE-ARTE', which constitutes part of the integrative medicine in the future.

Family condition and support methods were clarified from the domestic review ; telecommunication capability as one resource of a support system was clarified from the international review. Then, semi-structured interviews were conducted of 10 family caregivers currently taking care of people with dementia at home and 14 staff members at elderly daycare facilities. The actual conditions and needs of people with dementia living at home, those of their families, and also the actual condition of staff-provided care and the dilemmas of staff, are inductively analyzed.
