- OBJECTIVE: In Sweden, primary healthcare centers play an important role in the performance of general practice, education, and clinical research. In Japan, general physicians or general practitioners are expected to be more active in the small-scale hospitals and clinics in rural areas. This study aimed to explore the differences in attitudes toward general practice and clinical research among medical students in Japan and Sweden to present solutions to help doctors stay in rural areas of Japan. MATERIALS AND METHODS: This cross-sectional study was conducted at two medical schools in Japan and Sweden in 2018, using an anonymous and self-administered questionnaire survey that comprised 16 items including 9 items on clinical research. RESULTS: Participants were 154 medical students (response rate: 69.4% for 222 students) in Japan and 56 (27.1% for 201 students) in Sweden. The proportion of medical students who wanted to become general physicians was greater in Japan than in Sweden (Japan:Sweden=36.4%:17.9%; P=0.012). Although fewer Japanese students wanted to conduct research in rural areas than Swedish students (43.5%:57.1%; P<0.001), the positive proportion of Japanese students working in clinical research and/or taking an academic degree in rural areas was greater than that of Swedish students (52.0%:23.2%; P=0.032). CONCLUSION: As Swedish medical students and young doctors learn considerably from primary healthcare centers, their attitudes toward clinical research are more developed than those of their Japanese counterparts. However, more Japanese medical students than Swedish students wish to become general practitioners, and they are likely to strive to conduct clinical research at small-scale hospitals/clinics in rural areas. - Purpose: The Yoitoko check-up, a novel health check-up providing positive feedback, has been developed to promote health among older adults, and consists of several comprehensive geriatric assessment items. This report aimed to describe the details of the Yoitoko check-up and to explore the future possibility of the check-up by evaluating the participants' short-term behavioral changes in terms of comprehensive functioning, using a before-after study design. Patients and Methods: Four Yoitoko check-ups were conducted, at 3-month intervals, between December 2018 and September 2019. Study participants aged ≥65 years included those who had undergone ≥2 Yoitoko check-ups. The results of each visit after the second check-ups were retrospectively compared with those of the baseline, and the mean changes and the odds ratios were calculated using a paired t-test or a McNemar test, respectively. Results: Of 84 participants, the results of 16 (19.0%) participants were analyzed. The mean (standard deviation) age was 75.3 (4.7) years. The mean Tokyo Metropolitan Institute of Gerontology Index of Competence score, a measure of high-level functional capacity, increased 0.9 (95% confidence interval; range, 0.2-1.5) points between the first and second visits. Conclusion: We developed the Yoitoko check-up and introduced the details of it. Our study findings suggested that the Yoitoko check-up may further motivate older adults to improve their health and promote positive behavioral changes. - PURPOSE: To assess the factors involved in oral intake discontinuation in elderly patients with recurrent aspiration pneumonia. PATIENTS AND METHODS: This study included patients with pneumonia who were treated at Jichi Medical University Hospital between 2007 and 2013, at Toyooka Public Hospital between 2011 and 2013 and at Yuzawa Community Medical Center between 2010 and 2012. We consecutively enrolled patients with aspiration pneumonia. The primary study point was oral intake discontinuation after the initiation of oral intake during hospitalization in cases of recurrent aspiration. Various parameters were recorded at admission, at the initiation of intake, and during hospitalization; these parameters were statistically evaluated. RESULTS: A total of 390 patients were assigned to either a "no reaspiration of intake" group (n=310) or a "reaspiration of intake" group (n=80), depending on whether intake was discontinued owing to aspiration during hospitalization. At admission, the following items significantly differed between the groups: level of consciousness, respiratory rate, oxygen saturation, CURB-65 score, extent of infiltration/opacity on chest radiography, albumin levels, blood urea nitrogen levels, and application of swallowing function assessment. At the initiation of intake, level of consciousness, pulse rate, and albumin levels significantly differed between the groups. The following items did not significantly differ between groups: systolic blood pressure, pulse rate, C-reactive protein, bacteremia, use of ventilator at admission, oxygen administration, respiratory rate, and systolic blood pressure at initiation of intake. Multivariate analysis revealed that application of swallowing function assessment, level of consciousness at the initiation of intake, and extent of infiltration/opacity on chest radiography were significant predictive variables for discontinuation of intake. CONCLUSION: A low level of consciousness at the initiation of intake and a greater extent of infiltration/opacity on chest radiography and the application of a swallowing function are important factors. - Background: When analyzing regional disparities in healthcare resources, hospital accessibility is given little consideration. We surveyed accessibility from residential districts to medical institutions using GIS (Geographic Information System) and estimated Gini coefficient for each hospital distribution.Japan Primary Care Association, 2012, Gen Med, 13(1) (1), 25 - 29, English
Methods: The subjects were 2,688 census mesh blocks ( "Cho-cho-aza" ) and 109 hospitals in Tochigi prefecture. The number of hospitals located within the road distances of 5 km, 10 km and 15 km from the geometrical center of each block was calculated using GIS. The Gini coefficient of each hospital per 100 residents was calculated among the regions located within 5 km, 10 km and 15 km from the geometrical center of the census mesh block.
Results: The population of each block was 748±1,067 (mean±SD), and the road distance to the nearest hospital from the center of each block was 4.3±4.5 km. The number of census mesh blocks with distances from the center of each block to the nearest hospital within 5 km, 5-10 km, 10-15 km and more than 15 km were 1909 (71.0%), 561 (20.9%), 139 (5.2%) and 79 (2.9%) respectively. The number of hospitals located within 5 km, 10 km and 15 km were 3.3±4.7, 8.3±8.6 and 14.4±11.4. Gini coefficients were 0.65, 0.52 and 0.43.
Conclusion: When analyzing regional disparities in healthcare resources, it is necessary to take into account not only the number of physicians and beds, but also accessibility. - Objectives : To determine factors related to peoples' preference for visiting home-doctors when experiencing new health problems.Japan Primary Care Association, 2011, Gen Med, 11(2) (2), 71 - 77, English
Method : A questionnaire survey was conducted of people receiving annual health checkups in municipalities in the vicinity of Jichi Medical University Hospital. We surveyed personal characteristics, test equipment, having of a home-doctor, and answers to an assumed scenario (asking about willingness to visit a home-doctor in case of getting certain health problems).
According to the responses to the scenario, we divided the subjects into two groups (a home-doctor group: visiting a home-doctor; and a specialist group: not visiting a home-doctor) and statistically compared the two groups.
Results : In the analytic sample of 1,829, the home-doctor group numbered 1,097 individuals (60%) and the specialist group numbered 732 individuals (40%). The home-doctor group statistically had more home-doctors than the specialist group (adjusted odds ratio, 95% confidence interval: 2.47, 2.00-3.05).
More home-doctors in the home-doctor group had test equipment than home-doctors in the specialist group: Gastrointestinal test equipment (gastroscopy, colonoscopy, or ultrasonography) (adjusted odds ratio, 95% confidence interval: 1.39, 1.06-1.83).

- OBJECTIVE: To show the relationship between the personal and educational backgrounds of rural doctors and their intention to continue a rural career. DESIGN: Nationwide postal survey. SETTING: Public clinics or hospitals in municipalities that are classified as 'rural' by the national government. SUBJECTS: A total of 4896 doctors working for 828 public clinics and hospitals. MEASUREMENTS: A questionnaire was mailed. The questionnaire inquired about the subject's age, sex, hometown, exposure to rural practice in undergraduate education, postgraduate training, continuing medical education, current position and affiliation status with a medical school, as well as his or her intention to continue a rural career. RESULTS: Response rate was 64%; 26% answered that they intended to continue a rural career. Postgraduate training in general internal medicine, general surgery, anaesthesiology, paediatrics and gastroenterology were positively related with the intention to continue a rural career (odds ratio = 2.045, 1.59, 1.30, 1.48, 1.38). Rural background, undergraduate exposure to rural practice, multispecialty-rotation in postgraduate training and current administrative position had positive correlations with the intention to continue in logistic regression analysis (odds ratio = 1.80, 2.47, 1.54, 2.17). Affiliation with a medical school department was negatively related with the intention to continue (odds ratio = 0.45). - OBJECTIVES: This study was conducted to investigate the association between municipality expectations of retaining physicians in public medical facilities and their evaluation of the physicians from key aspects considered important for practicing community medicine. METHODS: A cross-sectional study using a self-reported postal questionnaire survey was conducted with all 3,152 local governments in Japan from July to September, 2000. Three thousand and fifty-nine (94%) local governments responded. Of the responders, 1,315 (42%) operated public medical facilities. These local governments evaluated their expectations of retaining physicians in medical facilities and the physicians themselves focusing on the following aspects considered important for practicing community medicine: degree of government satisfaction with the physician's involvement in public health and welfare services; primary care for emergency patients; human relationships between the physician and the population, staff of the facility or the municipal officials; public estimation of the facility; income and expenditure of the facility; understanding of the local government's system. RESULTS: The subjects were 1,092 local governments (83.0%) that responded to all of the questions. The proportion of municipalities expecting to retain physicians was 56% overall, 61% in rural areas and 44% in urban areas (P<0.001). The proportion of governments satisfied with the physicians' human relationships was higher than the other factors and the proportion satisfied about income and expenditure of facilities was the lowest. Multiple logistic regression analysis revealed that the degree of local government expectation of retaining physicians was significantly associated with the physicians' involvement in public health and welfare services (Odds ratio (OR) 1.8; 95% confidential interval (95%CI) 1.3-2.5), their human relationship with the population and public estimation of facilities (OR 1.6; 95%CI 1.1-2.2), and income and expenditure (OR 1.3; 95%CI 1.01-1.8 in all, OR 1.7; 95%CI 1.7-2.4 in rural areas). Primary care for emergency patients demonstrated a significant association in rural areas (OR 1.6; 95%CI 1.1-2.3). CONCLUSIONS: Municipal expectation of retaining physicians in public medical facilities was significantly associated with factors related to the integration of medical, public health and welfare systems, human relationship with the population, public estimation and income and expenditure of the facilities overall. In rural areas, there was a closer association with income and expenditure and a significant association with emergency care. - OBJECTIVES: The purpose of this paper was to discover to what degree Japanese rural doctors are satisfied with various aspects of their jobs and lives, and to find out whether they intend to continue their rural careers. DESIGN: Nationwide postal survey. SETTING: Public clinics or hospitals in municipalities that are authorised as 'rural' by the national government. SUBJECTS: A total of 4896 doctors working for public clinics or hospitals. INTERVENTIONS: Self-evaluation questionnaires were mailed. The rural doctors were asked to evaluate their satisfaction with 19 items related to their job conditions and 10 items concerning life conditions, using a four-point scale. They also were asked to evaluate their intent to stay in rural practice until retirement. RESULTS: The response rate was 64%. Overall, rural doctors were satisfied with both their work and life conditions. However, only 27% of respondents hoped to continue rural practice beyond the usual age of retirement. Among job-related items, continuing medical education and interactions with municipal governments were rated as least satisfactory. Among lifestyle-related items, duration of holidays and workload were unsatisfactory. Subgroup analysis revealed male doctors showed greater intent to stay in rural practice. Doctors aged > 50 years were more satisfied with most aspects of their job and lifestyle than younger doctors. A strong correlation was found between the degree of intent to stay and several items such as interactions with municipal government, human interactions salary and job fulfillment. CONCLUSIONS: Strategies, based on the results of this survey, should be implemented. Particularly in Japan, positive interaction between doctors and municipal governments is crucial. WHAT THIS PAPER ADDS: Though rural doctors in Japan in general were satisfied with their lifestyle and work conditions, they have low intentions to stay in rural areas. - OBJECTIVES: In order to practice appropriate community healthcare activities, it is essential for healthcare providers to integrate medical, public health, and welfare systems into one comprehensive healthcare body. In Japan, local governments are obliged to provide their entire population with public health and welfare services And physicians who work as government employees at public medical facilities play an important role in combining the three systems. It has been reported that the level of subjective evaluation by the municipality of the degree of involvement by physicians in the region's public health activities and welfare services is associated with the standardized mortality rates for strokes in the area's population. However, little is known about which specific public health and welfare services are more important as factors contributing to the level of evaluation of physicians by the people or municipality. The present study was therefore conducted to determine associations between levels of participation by community physicians in each specific public health or welfare service and the respective levels of evaluation by their municipalities. METHODS: A cross-sectional study design using a self-reported postal questionnaire survey was conducted with all 3,152 local governments in Japan from July to September, 2000. A total of 3,059 local governments (94%) responded, and 1,315 (42%) that operated a public medical facility were analyzed. The aspects from which local governments evaluated the physicians working for their public medical facilities were: the frequency of involvement in each of 13 services related to public health and welfare; the degree of satisfaction of governments with the physicians' involvement in each of the services; and the relationship between the physicians and the municipal healthcare officials. RESULTS: The services in which the local governments responded that they 'always' appreciated the physicians involved was significantly associated with the degree of municipal satisfaction. In the multiple logistic regression analysis, the degree of municipal satisfaction was significantly associated with their perception of consistent involvement by the physicians in case conferences on homecare management, health counseling, health education, life style instruction after basic health screening (odds ratio > 2.0 respectively), and school healthcare, vaccination, school health screening, and basic health screening (0 < odds ratio < 1.5 respectively). There was no significant difference in the level of satisfaction between large and small municipalities. CONCLUSIONS: The findings showed that high levels of municipal appreciation of consistent participation by physicians in certain public health and welfare services is associated with a higher degree of local government satisfaction with the physicians. - To elucidate the present condition about health, medicine and welfare, and to evaluate factors associated with relief for elderly life in each community, a questionnaire survey was conducted among officers of all the municipalities in Japan. We asked officers about their recognition whether older people can live feeling relieved or not, and other factors about health, medicine and welfare condition in each municipality. Demographic data of each municipality were also used as dependent variables. Odds ratios and their 95% confidence intervals for the officers' evaluation of relief for life of the elderly were calculated using unconditional logistic models. Of the 3.252 municipalities, 3,059 (94%) responded to the survey. Of the respondents, 2,957 municipalities (91%) answered questions about relief for elderly life. "Receiving satisfactory medical home care service", "cooperative function between medicine, health, and welfare sections", and "receiving complete welfare service" were associated with their recognition about relief for elderly life in both of cities and rural towns. In addition, "the rate of elderly households" and "the easiness to maintain nursing staff" were associated in the cities. This study indicated that more arrangements are needed not only about health and welfare aspects, but also about medical service, especially medical home care service systems to support older peoples' life in the community.6, Nov. 2003, Nihon Ronen Igakkai zasshi. - BACKGROUND: Medical care in pediatric-related areas in which physicians other than pediatricians, such as surgeons, otolaryngologists, ophthalmologists, orthopedists and dermatologists, can also participate is one of the fields of pediatric primary care. Independent factors associated with the practice of specific procedures in pediatric-related areas by certified pediatricians in Japan were evaluated. METHODS: Data were collected from a nationwide survey of 1116 pediatricians who were selected by systematic sampling from the list of certificated members of the Japan Pediatric Society. The survey included questions relating to full- or part-time practice, types of practice setting, practice with or without pediatric subspecialty interest, numbers of procedures performed among 15 procedures in pediatric-related areas, and attitude toward practice of those procedures. RESULTS: Of 79% of respondents, 73% were analyzed (n=810). Among 15 procedures in pediatric-related areas, seven procedures performed by more than 30% of certificated pediatricians were evaluated. In comparison with certified pediatricians who annually performed three or less procedures (n=410), certified pediatricians who annually performed four or more procedures (n=400) were more likely to be young and male, working full-time and in a general hospital setting, practicing with a pediatric subspecialty interest. In multiple logistic regression analyses, young age (odds ratio [OR]: 0.8 per 10 years of age; 95% CI: 0.6-0.9), positive attitude toward practice (OR: 1.7; 95% CI: 1.6-1.9), male (OR: 1.7; 95% CI: 1.1-2.5), full-time practice (OR: 3.2; 95% CI: 1.5-6.9), and practice in general hospital setting (OR: 1.7; 95% CI: 1.2-2.4) were independently associated with more extensive practice of procedures in pediatric-related areas. CONCLUSIONS: A positive attitude toward medical care in pediatric-related areas is one of the most important factors associated with practice of procedures in these areas. - 日本学術振興会, 科学研究費助成事業, 挑戦的研究(萌芽), 神戸大学, 28 Jun. 2019 - 31 Mar. 2023地域創成まちづくり事業を活用した医療の枠組みを超えた地域医療教育の検証町づくり事業:夢前町花街道事業の一つである加点式健診事業(「よいとこ健診」)は、学生主体に実施されている。姫路市夢前町地区の住民(主に65歳以上)を対象として、高齢者総合的機能評価(CGA)で用いられる項目:社会背景とライフスタイル、手段的日常生活動作、意欲、社会活動、運動機能、口腔機能、体脂肪、認知機能を評価し、良い結果に対してポジティブフィードバックを学生が行う形式でよいとこ健診は実施されている。よいとこ健診は、「健康に良い習慣・努力」を「発見して、ほめてもらう体験」を通して、健康づくりへのモチベーションアップとフレイル予防(介護予防)、家族・地域の社会関係を育むこと、および高齢者健診受診率の引き上げを目的としている。実施体制は、神戸大学医学部、経済学部、文学部、甲南女子大学栄養学部の学生による実行委員会と学生のコアメンバーと教員とで構成されるよいとこ健診共同研究会となっている。実施方法は、学生委員会で検討し、共同研究会で決定した。2022年度は、9月にオンライン形式で、3月は対面形式での実施となった。学生は、実施前に地元自治会の役員と意見交換会にも参加し、健診受診者の募集等にも関与している。学生が実施した住民による健診の評価および地区自治会との反省会での意見を参考に、次回の実施方法を検討する、いわゆるPDCAサイクルを学生が実施している。このように、他業種による学生による連携事業が構築された。オンライン形式と対面形式の違いとして、「現場の雰囲気が肌で感じることができ、より地域の理解が深まった。」「対面でフィードバックすることによって、より受診者との距離が近くなった感じがする。」「他の健診項目の実施状況が把握でき、健診への理解が深まった」「対面での協働作業ができたことで、学生間の繋がりが強まった感じがした」などの意見が多かった。

Studies of patients decision process are very important from the point of view of self-realization, self-determination informed-consent. The research methods of this field are not established, so our main research is establishment of methods. We recorded patient and doctor separately during consultation using two videocameras by informed-consent of patient and doctor. The two records are united in one picture plane using video-editor. We interviewed structurely the patients about the thinking process and contents while showing the editored videotape of consultation. The time/point and contents of question are structured. At the change of verbal, semi-verbal and non-verbal communication we interviewd the "why", "what" and "how". The decision of patients were expressed on semi-verbal and non-verval commnications.
