- BACKGROUND: Abdominal contrast-enhanced computed tomography (CT) is the gold standard for diagnosing acute focal bacterial nephritis (AFBN). However, contrast-enhanced CT is contraindicated for patients with contrast medium allergies, impaired renal function, pregnancy, metformin use, and thyroid disease. Diffusion-weighted whole body imaging with background body signal suppression (DWIBS) is a safe imaging modality for such patients. Although DWIBS can quickly evaluate inflammation, its role in diagnosing AFBN remains unclear. Therefore, we aimed to compare imaging modalities for diagnosing AFBN and evaluate the efficacy of DWIBS in patients receiving inpatient treatment. METHODS: In this retrospective cohort study, we included individuals aged ≥18 years hospitalized for AFBN under the Diagnosis Procedure Combination system from January 1, 2013, to December 31, 2022. Exclusion criteria included imaging findings not suggestive of AFBN. The primary outcome was the proportion of patients who underwent abdominal ultrasonography, contrast-enhanced CT, and DWIBS and their respective diagnostic rates for AFBN. The secondary outcome was the accuracy of AFBN detection using abdominal ultrasonography and DWIBS compared to that of the standard contrast-enhanced CT. RESULTS: The study cohort included 123 patients, with an average age of 51±20.4 years; 107 (87.0%) were women. Abdominal ultrasonography was used in 74.0% (91/123) of the patients, yielding a diagnostic rate of 31.9% (29/91). Contrast-enhanced CT was used in 90.2% (111/123) of the patients, with a diagnostic rate of 100% (111/111). DWIBS was used in 11.4% (14/123) of the patients, with a diagnostic rate of 85.7% (12/14). The accuracy of AFBN detection (correct response rate) was 26.6% (21/79) for abdominal ultrasonography and 75.0% (6/8) for DWIBS, indicating that DWIBS had a significantly higher accuracy rate than that of abdominal ultrasonography [Chi-square test, χ2(1) =7.96, P=0.0048]. CONCLUSIONS: DWIBS demonstrated promising diagnostic accuracy for AFBN compared with abdominal ultrasonography, suggesting it could be a valuable diagnostic tool for this condition. Given its diagnostic accuracy, DWIBS may be considered over abdominal ultrasonography in patients who are unable to undergo contrast-enhanced CT due to allergies or pregnancy. - OBJECTIVE: This study examined the barriers to writing case reports among Japanese physicians who participated in a web study series titled "Let's Write a Case Report! What you need to do to be Accepted" and explored differences in these barriers based on physicians' years of experience. METHODS: The study design was cross-sectional in nature. Throughout August and November 2020, the respondents participated in the web study sessions. Using an 11-point scale, from -5 (not a barrier at all) to 0 (neither) to 5 (very much of a barrier), the 16 items interrogated which elements could be barriers to case report writing. Participants were divided into two groups: those with 10 or fewer years of postgraduation experience and those with 11 or more years of postgraduation experience. The basic attributes and possible barriers were compared between the groups using chi-squared and t-tests. RESULTS: In total, 53 (62.4%) participants responded to the web-based questionnaire. In a comparison between the two groups, the following barriers were significantly higher in the 10 years or fewer group: having sufficient medical documentation to write a report, knowing how to write a case report, determining the main case points and clinical message, knowing how to search the literature, knowing how to obtain literature, meeting the financial cost of accessing literature. Although there were no significant differences, only the lack of motivation to write papers was perceived as a barrier by the 11 years or more group. CONCLUSIONS: Determining the main case points and clinical message, selecting an appropriate journal for submission, and having adequate time to write a paper were the major barriers to writing a case report. Paper-writing motivation was crucial for those with longer clinical experience and experience in writing papers. These barriers were significantly more pronounced among those with fewer than 10 years of clinical experience than among those with 11 or more years of experience. - INTRODUCTION: This study intended to evaluate the medium-term effectiveness of a community-based medical education (CBME) program and to determine the program's influence on the application rates of regional-quota students seeking to become residents in Tamba, Japan. MATERIALS AND METHODS: We conducted a cohort study of regional-quota students. Exposure factors included (1) experience compared to no experience of CBME in the Tamba area; (2) CBME experience compared to no experience in Tamba in the senior years (4-6 years of medical school) and experience in the junior years (1-3 years of medical school); and (3) experience in the senior years compared with those in the junior years. Outcome measures were applications to become a medical resident and actually becoming a medical resident at the Hyogo Prefectural Tamba Medical Center. RESULTS: Of 94 participants, 58 (61.7%) were male and 37 students (39.4%) had previous CBME experience in the Tamba area. In applying to become a resident at the Hyogo Prefectural Tamba Medical Center, students who had experienced CBME in the Tamba area in their senior years had significantly higher adjusted risk ratios compared to those who experienced it in their junior years. Regarding applications to become a resident, students who had experienced CBME in the Tamba area in their senior years had a significantly higher adjusted risk ratio than students who had not experienced CBME and students who had experienced CBME in their junior years. CONCLUSIONS: There was a statistically significant application rate for residency programs among medical students who participated in the CBME program in their senior years compared with those who did not. - Amyloid light chain (AL) amyloidosis is a disease in which ALs, which are proteins with fibrous structures, are deposited in systemic organs, causing functional impairment. Diagnosis is often difficult because of non-specific and varied symptoms. We report a case of systemic AL amyloidosis that was diagnosed as a result of repeated syncope. A 76-year-old woman was brought to the emergency room with multiple episodes of loss of consciousness over the past five years. She visited the major hospital, where pulmonary thromboembolism and symptomatic epilepsy were considered possible causes. Orthostatic hypotension was observed after being transferred to our hospital for rehabilitation. We performed diagnostic tests, including blood tests, imaging, and a head-up tilt test, which confirmed severe orthostatic hypotension. A gastrointestinal biopsy with Congo red staining confirmed the presence of amyloid deposits. AL amyloidosis (λ) was diagnosed using immunohistochemical staining. Given her age and prolonged bed rest, she was determined that she could not tolerate chemotherapy and was discharged upon her request. To the best of our knowledge, this is the first report of systemic AL amyloidosis presenting with orthostatic hypotension severe enough to cause syncope due to autonomic neuropathy. - BACKGROUND: Sublingual immunotherapy (SLIT) leads to the long-term remission of allergic rhinitis and requires long-term daily adherence. There are limited studies on the treatment burden or satisfaction of SLIT among caregivers of children treated using SLIT. We aimed to evaluate the association between the treatment burden and satisfaction for pediatric allergic rhinitis caregivers and the clinical factors of their children's SLIT. METHODS: We conducted a questionnaire survey with 115 caregivers of SLIT-treated children from May to August 2023 and retrospectively reviewed their children`s medical records. A single caregiver had at least one child, all undergoing SLIT at the time of the survey. Of the total 139 SLIT-treated children, 78 children over eight years of age were interviewed regarding adherence to SLIT and subjective symptoms of allergic rhinitis. We performed a comparative analysis of caregivers' impressions of SLIT and their children's clinical factors regarding SLIT. RESULTS: A runny nose was the most common symptom experienced by children before the start of SLIT. After the children started SLIT, 74 (64.3%) of the 115 caregivers felt satisfied and 30 (26.1%) felt burdened by SLIT. The most frequent reasons for being satisfied and burdened by SLIT were because it improved or relieved allergic rhinitis and the preparation and encouragement for children to take SLIT tablets, respectively. Of the 115 caregivers, 96 (83.4%) recommended SLIT to others. A comparative analysis showed that caregivers had already felt burdened from the first one to six months of SLIT initiation and the proportion of caregivers who answered that they felt burdened by SLIT was significantly lower for children with appropriate adherence than for those with inappropriate adherence (11.1% vs. 30.9%,P = 0.034). CONCLUSIONS: Most caregivers of SLIT-treated children reported higher treatment satisfaction than treatment burden. Caregiver burden emerged shortly after the start of SLIT and was also associated with lower adherence in children. - A 63-year-old man with schizophrenia presented to our hospital with a chief complaint of anorexia and edema in the lower legs for the past month. Upper gastrointestinal endoscopy revealed a gross type 3 gastric carcinoma (pathology: tubular adenocarcinoma, moderately differentiated type) from the upper gastric body to the gastric antrum. His serum total protein level was 4.7 g/dL and serum albumin concentration was 1.5 g/dL. Contrast-enhanced computed tomography, magnetic resonance imaging, and examination of ascites showed no evidence of distant metastases. Hypoalbuminemia was refractory to albumin administration with no evidence of cancerous peritonitis on staging laparoscopy. Gastrointestinal scintigraphy confirmed protein leakage from the gastric tumor into the gastrointestinal tract. We reviewed the literature and identified 34 other cases of protein-losing gastric cancer, which were used to comprehensively elucidate its clinical features. Borrmann type 0 or 1 tumors of the gastric body or antecubital area, with a maximum tumor diameter of nearly 10 cm and a cauliflower-like appearance, are highly suggestive of protein-losing gastric cancer. - BACKGROUND: Kounis syndrome is defined as the concurrence of acute coronary syndromes in the setting of allergic or anaphylactic reactions. It primarily affects men aged 40-70 years and is often associated with chest pain. This syndrome is often unrecognized and undiagnosed in clinical practice due to a low level of awareness. Herein, we present a case of type I Kounis syndrome in a young woman without chest pain. CASE PRESENTATION: A 28-year-old Japanese woman with a history of atopic dermatitis received a glycyrrhizin, glutathione, and neurotropin preparation (a preparation of inflamed skin extract from rabbits inoculated with vaccinia virus) at a dermatology clinic to treat pruritus caused by atopic dermatitis. Immediately after the administration, the patient developed abdominal pain and generalized body wheals. The patient was diagnosed with anaphylaxis and was transported to our hospital. She had no chest pain on arrival at our hospital; however, a 12-lead electrocardiogram showed ST elevation in leads I, aVL, V2, and V3, and an echocardiogram showed decreased wall motion in the anterior and lateral walls of the left ventricle. Sublingual nitroglycerin administration improved ST-segment elevation and left ventricular wall motion abnormalities. The patient underwent emergency coronary angiography, which revealed no significant stenosis, and was diagnosed with type I Kounis syndrome. CONCLUSION: Kounis syndrome without chest pain is rare in young women. Since it can be fatal in cases with severe allergic symptoms such as anaphylaxis, the possibility of concurrent acute coronary syndrome should be considered when treating systemic allergic reactions, regardless of age, sex, or the presence or absence of chest symptoms.Sep. 2024, BMC cardiovascular disorders, 24(1) (1), 467 - 467, English, International magazineScientific journal
- BACKGROUND: Desulfovibrio desulfuricans (D. desulfuricans), a commensal anaerobic gram-negative rod endemic to the soil environment and human gastrointestinal tract, rarely causes bloodstream infections. We report two rare cases of bacteremia caused by D. desulfuricans in which the intestinal tract was the portal of entry. In addition, we summarize findings on D. desulfuricans. CASE PRESENTATION: Case 1: A 51-year-old man presented to the emergency department with the chief complaints of fever and right lower abdominal pain. He was admitted to the hospital with ascending colonic diverticulitis and received empirical antibacterial therapy with piperacillin/tazobactam. Blood culture revealed D. desulfuricans. The patient was discharged after 2 weeks of antimicrobial therapy. Case 2: A 95-year-old woman presented to our hospital with a chief complaint of fever. Owing to an elevated inflammatory response and pyuria, the patient was diagnosed with pyelonephritis and treated with ceftriaxone. Klebsiella pneumoniae was detected in her urine culture, while D. desulfuricans was detected in her blood culture. The patient was then treated with ampicillin/sulbactam for 14 days. The fecal occult blood test result was positive, suggesting a colonic mucosal lesion, such as a malignant tumor, may have been the portal of entry for D. desulfuricans bacteremia. Previous literature reviews indicate that D. desulfuricans bacteremia often results from liver or renal abscesses, intestinal lesions, among others, serving as the portal of entry. Although no specific underlying disease has been reported, it is more common in the older population. We encountered two cases of D. desulfuricans bacteremia and combined them with 15 cases from previous studies to explore the characteristics of the disease. The proportion of patients aged [Formula: see text]60 years was 73.7%; overall, 73.7% had gastrointestinal complications, and 63.2% had abdominal symptoms at the time of presentation. CONCLUSIONS: We encountered two rare cases of D. desulfurican bacteremia. - A 68-year-old man presented with a headache that had started 1 month earlier. The scalp vein dilatation was observed at presentation. The findings of computed tomography and magnetic resonance imaging raised suspicion of a dural arteriovenous fistula, leading to the definitive diagnosis by digital subtraction angiography. - BACKGROUND: A Bartholin's gland abscess is one of the most common infections in women of reproductive age. Although Bartholin's gland abscesses have been reported in prepubertal children, they are rarer in prepubertal children than in adults. Herein, we report a case of bilateral Bartholin's gland abscesses in a 4-year-old girl with vitamin A deficiency. CASE PRESENTATION: A 4-year-old girl diagnosed with autism spectrum disorder was admitted to the hospital for close examination and treatment because of persistent fever and malaise. The child was a marked fussy eater and was diagnosed with corneal ulceration and night blindness secondary to vitamin A deficiency. Both of the patient's labia were swollen, and a diagnosis of a bilateral Bartholin's gland abscess was made using computed tomography. Incisional drainage was performed under general anesthesia. The patient's postoperative course was uneventful, and she was discharged from the hospital on day 8 after the surgery. During hospitalization, attempts were made to correct the vitamin deficiency by adding nutritional supplements to the diet. Three months after the surgery, no recurrence of abscesses was noted. CONCLUSIONS: Decreased immunocompetence and mucosal barrier function due to vitamin A deficiency is thought to be the underlying cause of Bartholin's gland abscesses. Although prepubertal Bartholin's gland abscesses have been reported, they are rare. To the best of our knowledge, no reports of bilateral Bartholin's gland abscesses potentially caused by vitamin A deficiency have been reported. When prepubertal girls present with Bartholin's gland abscesses, the presence of immunodeficiency due to vitamin or trace element deficiency should also be considered.May 2024, BMC infectious diseases, 24(1) (1), 487 - 487, English, International magazineScientific journal
- Because the β-lactam ring has a molecular structure similar to that of gamma-aminobutyric acid (GABA) neurotransmitters, it binds to GABA A receptors and inhibits GABAergic transmission, causing AAE. - A 2-year-old girl developed dermatitis with atypical brown crusts after ethanol disinfection. - BACKGROUND: Roseomonas mucosa (R. mucosa) is a pink-pigmented, Gram-negative short rod bacterium. It is isolated from moist environments and skin, resistant to multiple drugs, including broad-spectrum cephalosporins, and a rare cause of infection with limited reports. R. mucosa mostly causes catheter-related bloodstream infections, with even fewer reports of skin and soft tissue infections. CASE PRESENTATION: A 10-year-old boy received topical steroid treatment for sebum-deficient eczema. A few days before the visit, he was bitten by an insect on the front of his right lower leg and scratched it due to itching. The day before the visit, redness, swelling, and mild pain in the same area were observed. Based on his symptoms, he was diagnosed with cellulitis. He was treated with sulfamethoxazole/trimethoprim, and his symptoms improved. Pus culture revealed R. mucosa. CONCLUSIONS: We report a rare case of cellulitis caused by R. mucosa. Infections caused by rare organisms that cause opportunistic infections, such as R. mucosa, should be considered in patients with compromised skin barrier function and regular topical steroid use. - Dengue fever (DF) can be complicated by hemophagocytic lymphohistiocytosis (HLH). Steroid administration is markedly effective for this hematologic complication, and for other viral infections. We present a rare case of DF-associated HLH that improved with steroid-sparing supportive care. A 47-year-old Japanese male with diabetes mellitus and no history of DF traveled to the Philippines 10 days before his hospitalization. Three days before emergency admission, he experienced fever and joint pain and was referred to our hospital for suspected DF, after blood tests indicated liver damage and thrombocytopenia. Erythema of the extremities and trunk appeared on day 2, and the next day neutrophils were 550 cells/μL, platelets 29,000 cells/μL, ferritin 9840 ng/mL, and fibrinogen 141 mg/dL. Bone marrow aspirate revealed hemophagocytic lymphohistiocytosis, and he was diagnosed with HLH. On day 4, the symptoms and findings improved; only supportive care without steroids was continued. He tested positive for dengue virus antigen on admission. He was discharged on day 9 of hospitalization in good general condition with no vascular leakage or bleeding and recovery of blood cells. Although steroid administration is markedly effective in cases of DF complicated by HLH, this case suggests that such cases can resolve with steroid-sparing supportive care.Nov. 2023, Tropical medicine and infectious disease, 8(11) (11), 497, English, International magazineScientific journal
- Parvimonas micra (P. micra) is a gram-positive anaerobic coccus endemic to the oral cavity and intestinal tract. We report a case of pyogenic spondylitis caused by P. micra and summarize the clinical features of previous case reports. An 81-year-old man with a history of lumbar vertebral compression fracture two years previously presented to the emergency department with low back pain. He was clinically diagnosed with pyogenic spondylitis due to difficulty in moving his body, spinal tapping pain, and signs of inflammation. He was hospitalized, and aerobic and anaerobic blood culture samples were collected, but the results were negative. Computed tomography and magnetic resonance imaging revealed inflammation in the second and third lumbar vertebrae and L2/3 and L3/4 intervertebral discs, and culture of the infected disc biopsy showed P. micra growth. After six weeks of treatment with ampicillin-sulbactam and ampicillin, the patient's symptoms improved, and he was discharged. During hospitalization, he was diagnosed with periodontitis and type 2 diabetes; his dentures were adjusted, and he was started on an oral hypoglycemic agent. Pyogenic spondylitis caused by P. micra tends to be associated with oral infections. This case illustrates the importance of appropriate detection and treatment of the source of infection to prevent recurrence.Nov. 2023, Cureus, 15(11) (11), e48665, English, International magazineScientific journal
- OBJECTIVE: To examine the significant events experienced by initial trainees during community medicine training, evaluate their impact on community medicine practice, and support improvements in rural community medicine training. RESULTS: Three faculty teachers independently evaluated the reports of 25 residents who had completed a four-week community medicine training in a rural area to analyze major events. The reports were analyzed using topics from the Model Core Curriculum for Medical Education that relate to rural medicine. The most frequently reported items were identified as follows: Primary care: 9 (36.0%); integrated community care systems: 8 (32.0%); medical care in the local community: 7 (28.0%); home health care and systems, patient-physician relationship, and end-of-life medical treatment and care: 6 each (24.0%). - BACKGROUND: Mamushi bites are the most common venomous snake bites in Japan, with known complications including rhabdomyolysis and acute kidney injury; however, adrenal insufficiency as a result of snake bites has not been previously reported. We report a case of empty sella with transient adrenal insufficiency during hospitalization for a Mamushi bite. CASE PRESENTATION: An 84-year-old man was admitted to our hospital with a Mamushi bite on the right fifth finger. Serum sodium (Na) level remained in the normal range. On the ninth day of admission, he developed hyponatremia, with a serum Na level of 114 mEq/L and serum cortisol level of 4.0 μg/dL (reference value 4.5-21.1 μg/dL). His serum Na level was restored within the normal range after administration of corticosteroids with 3% NaCl solution. Both rapid adrenocorticotrophin and corticotropin-releasing hormone loading tests showed low cortisol response. Based on the results of the hormone loading tests, a diagnosis of pituitary adrenal insufficiency was made. Contrast-enhanced pituitary magnetic resonance imaging (MRI) showed primary empty sella. After discontinuation of corticosteroids, the hyponatremia did not recur, and the patient was discharged on the 24th day of hospitalization. After discharge, the patient visited an outpatient clinic, but hyponatremia recurrence was not observed. CONCLUSIONS: This is the first report of hyponatremia due to pituitary adrenal insufficiency during hospitalization for a Mamushi bite in a patient with empty sella. When hyponatremia occurs during hospitalization for a Mamushi bite, cortisol measurement, hormone loading test, and head MRI should be performed to search for pituitary lesions because of the possibility of adrenal insufficiency caused by snake venom.Oct. 2023, BMC endocrine disorders, 23(1) (1), 219 - 219, English, International magazineScientific journal
- BACKGROUND: Angioedema is a disorder characterized by edema of the face, lips, tongue, and extremities due to increased vascular permeability. Angioedema of the tongue usually occurs bilaterally, and the incidence of unilateral angioedema of the tongue is rare. This study reports a rare case of unilateral angioedema of the tongue with no identifiable cause and repeated recurrence even after discontinuation of an angiotensin-converting enzyme inhibitor. CASE SUMMARY: The patient was a 65-year-old woman with pre-existing hypertension and hyperlipidemia and had been receiving 20 mg/d of lisinopril. She was diagnosed with angioedema due to unilateral swelling of the tongue. No airway obstruction was observed, and the symptoms improved following the administration of 0.3 mg of epinephrine intramuscularly and 125 mg of methylprednisolone intravenously. Although lisinopril was discontinued, unilateral angioedema of the tongue continued to recur every 2-5 mo, with the symptoms improving following the administration of prednisolone and an antihistamine. Daily oral administration of 500 mg of tranexamic acid after dinner prevented the recurrence of angioedema. - Milk-alkali syndrome, which is characterized by hypercalcemia, metabolic alkalosis, and renal dysfunction, typically results from the ingestion of large amounts of calcium and absorbable alkaline products. However, these symptoms can also manifest when alkalosis and calcium loading occur simultaneously, owing to other factors. We report a case of milk-alkali syndrome caused by loop-diuretic-induced alkaline load and polypharmacy in an 85-year-old Japanese woman with multiple comorbidities, including osteoporosis, hypertension, type 2 diabetes, dyslipidemia, and Parkinson's disease. The patient regularly took 14 drugs, including calcium L-aspartate, eldecalcitol, celecoxib, and a fixed-dose combination of losartan and hydrochlorothiazide. Immediately before admission, furosemide was administered for the treatment of edema. The patient presented with chest discomfort, general malaise, and clinical signs of dehydration, hypercalcemia, hypophosphatemia, hypokalemia, and hypomagnesemia, accompanied by electrocardiogram abnormalities, renal dysfunction, and chloride-resistant metabolic alkalosis. The hypercalcemia was specifically induced by calcium L-aspartate and eldecalcitol. The hypomagnesaemia and hypophosphatemia were caused by diuretics and hypercalcemia. Thus, all the oral medications were discontinued, and rehydration and electrolyte correction therapy were administered. The final diagnosis was milk-alkali syndrome caused by the concomitant use of loop diuretics and other medications, without absorbable alkaline preparation use. - BACKGROUND: Myocardial abscess is often associated with infective endocarditis (IE), and isolated myocardial abscess without IE is rare. Echocardiography and computed tomography (CT) are often used to diagnose myocardial abscess; however, to the best of our knowledge, diffusion-weighted whole-body magnetic resonance imaging with background body signal suppression (DWIBS) has not been used. Here, we present a case of myocardial abscess without IE that was diagnosed using DWIBS. CASE PRESENTATION: A 72-year-old Japanese man with a history of hypertension, dyslipidemia, and retinitis pigmentosa presented to our hospital with malaise and a fever lasting 10 days. Blood test results showed elevated inflammatory marker levels (white blood cell count 18,700/µL and C-reactive protein level 23.0 mg/dL). Infection was suspected; however, the source of the infection could not be identified. DWIBS, which was performed on day 7 of admission to determine the source of infection, showed a high signal surrounding the right wall, suggesting inflammation. Contrast-enhanced CT performed on day 1 of hospitalization revealed a low-density area in the same region; however, the pathological implications of this finding could not be determined. Based on DWIBS findings, we concluded that the condition presented as a myocardial abscess that was confined specifically to the right atrial wall. Three sets of blood cultures revealed negative findings, and echocardiography showed no vegetation or valve regurgitation. Therefore, the patient was diagnosed with an isolated myocardial abscess uncomplicated with IE. An electrocardiogram on admission showed no P waves, and the patient had a junctional rhythm. However, on day 20 of hospitalization, he developed a complete atrioventricular block. After complete myocardial abscess healing following antibiotic treatment was confirmed, the patient underwent pacemaker implantation. Ten months after surgery, the patient had no signs of infection recurrence. CONCLUSIONS: Based on history and physical examination alone, diagnosis of an isolated myocardial abscess can be challenging. - Background: Renal infarction is an extremely rare disease. Although more than 95% of cases are symptomatic, there have been no previously reported asymptomatic cases, without any abnormal blood and urine test findings. Furthermore, the efficacy of long-term treatment of idiopathic renal infarction remains unknown. Case Presentation: A 63-year-old Japanese male underwent laparoscopy; a very low anterior resection of the rectum for lower rectal cancer (stage II) four years and five months prior to diagnosis with renal infarction. During the follow-up imaging studies, asymptomatic idiopathic renal infarction was found incidentally. The blood and urine test findings were normal. Contrast-enhanced computed tomography revealed a linearly bordered area of poor contrast in the dorsal region of the right kidney; however, no renal artery lesions, thromboembolic disease, or coagulation abnormalities were observed. Initial treatment with rivaroxaban 15 mg/day resulted in the remission of the infarcted lesion. The anticoagulation therapy was terminated after about 18 months without any incidences of re-infarction or bleeding events. Conclusions: We reported a very rare case of asymptomatic idiopathic renal infarction where blood and urine tests revealed no abnormal findings, and it was diagnosed incidentally during a post-treatment follow-up examination for lower rectal cancer. - Background and objective: Unilateral agenesis of pulmonary arteries (UAPA) is a rare disease, with approximately 400 cases reported to date. UAPA is often associated with congenital heart disease, and the uncomplicated form is isolated UAPA, which accounts for approximately 30% of all cases of UAPA. The incidence of pulmonary hypertension due to UAPA has been reported to range from 19 to 44%. There is no consensus treatment for pulmonary hypertension associated with UAPA. We present the first reported case in which a three-drug combination, comprising of iloprost inhalation, riociguat, and ambrisentan, was administered to a patient with UAPA, and was followed-up for 3 years post-diagnosis. Case presentation: A 68-year-old Japanese woman presented to our hospital with dyspnea and chest discomfort. She underwent chest radiography, blood tests, and echocardiography; however, the cause of the patient's symptoms could not be identified. During regular follow-up, an echocardiography 21 months after the initial visit revealed elevated right ventricular pressure (peak tricuspid regurgitation velocity: 5.2 m/s and right ventricular systolic pressure: 120 mmHg) and a diagnosis of pulmonary hypertension was made. Contrast-enhanced computed tomography (CT) of the chest and a pulmonary blood flow scintigram were performed to investigate the cause of pulmonary hypertension, and isolated UAPA was diagnosed. The patient was treated with a three-drug combination of iloprost inhalation, riociguat, and ambrisentan and followed up for 3 years with good therapeutic outcomes. Conclusions: We present a case of pulmonary hypertension caused by isolated UAPA. Although rare, this disease can lead to pulmonary hypertension and should be treated cautiously. - BACKGROUND: Syphilis is a chronic disease that progresses in the primary, secondary, latent, and tertiary stages. Pulmonary manifestations of syphilis are rare, and their histological features have not been well-described. CASE PRESENTATION: A 78-year-old man was referred to our hospital because of a solitary nodular shadow in the right middle lung field on a chest radiograph. Five years prior, a rash appeared on both legs. He was tested for syphilis at a public health center, and the non-treponemal test result was negative. When he was approximately 35 years old, he had unspecified sexual intercourse. Chest computed tomography showed a 13-mm nodule with a cavity in S6 of the right lower lobe of the lung. Robot-assisted resection of the right lower lobe was performed because of suspected localized right lower lobe lung cancer. A cicatricial variant of organizing pneumonia (CiOP) was observed, and immunohistochemistry identified Treponema pallidum inside the macrophages in the nodule cavity. The rapid plasma regain (RPR) value was negative, and the Treponema pallidum hemagglutination assay was positive. The patient was diagnosed as having secondary syphilis with pulmonary involvement. Insidious progression of secondary syphilis may result in CiOP and a negative RPR test result. CONCLUSIONS: We report the first case of pulmonary syphilis with a histological pattern of CiOP. It may be asymptomatic and difficult to diagnose because the RPR test may be negative for a long period of time. - Background: Cardiopulmonary disorders are the most common cause of central cyanosis, and methemoglobinemia is often overlooked in the differential diagnosis of patients with central cyanosis. In most cases, methemoglobinemia is acquired and hereditary congenital methemoglobinemia is rare. Only a few case reports of congenital methemoglobinemia can be found in PubMed. To date, only four cases of congenital methemoglobinemia diagnosed after the age of 50 years have been reported. Case Presentation: A 79-year-old Japanese woman presented at our hospital with the chief complaints of dyspnea and cyanosis. She exhibited cyanosis of the lips and extremities, and her SpO2 was 80%, with oxygen administration at 5 L/min. Blood gas analysis revealed a PaO2 of 325.4 mmHg and methemoglobin level of 36.9%. The SpO2 and PaO2 values were dissociated, and methemoglobin levels were markedly elevated. Genetic analysis revealed a nonsynonymous variant in the gene encoding nicotinamide adenine dinucleotide cytochrome (NADH) B5 reductase 3 (CYB5R3), and the patient was diagnosed with congenital methemoglobinemia. Conclusions: It is important to consider methemoglobinemia in the differential diagnosis of patients with central cyanosis. At 79 years of age, our patient represents the oldest patient with this diagnosis. - Introduction: Duplication of the inferior vena cava (IVC) is a congenital venous malformation that occurs in 0.2%-3% of the population as a result of persistent left and right supracardinal veins. The IVC duplication is prone to deep vein thrombosis due to endothelial dysfunction and associated venous stasis. This is a rare case of recurrent venous thrombosis due to IVC duplication and decreased protein C activity. Case: A 57-year-old male presented with swelling of the left lower limb that gradually developed over a one-week period preceding his visit. He reported a history of superior mesenteric vein thrombosis, approximately three years ago, for which he received anticoagulation therapy for three months. Thoracoabdominal contrast-enhanced computed tomography (CT) revealed thrombi in the locations of the bilateral main pulmonary arteries, IVC duplication, left common iliac vein, left IVC, and left renal vein. Blood work confirmed protein C activity of 21% (baseline 64%-146%), that could have contributed to the recurrent IVC thrombosis and formation of pulmonary artery thrombus. Subsequently, the patient was hospitalized and started on anticoagulation therapy. The swelling in the left lower extremity gradually improved, and the patient was instructed to continue anticoagulation therapy permanently. - BACKGROUND: Trueperella bernardiae is a coryneform, gram-positive bacterium that is a commensal of the skin and upper respiratory tract. It is treated as a contaminant and rarely causes infections. Blood, urine, and abscesses have been previously reported as the most common sites of infection. Infections caused by T. bernardiae are rarely reported in bedridden very old patients with reduced activities of daily living (ADL). In this report, we describe a case of sepsis due to acute pyelonephritis caused by T. bernardiae in a very old patient with impaired ADL. CASE PRESENTATION: A 94-year-old woman had a home visit from her local physician. She was bedridden and used diapers. On the day of admission, she presented with fever and dyspnea and was admitted with a diagnosis of sepsis associated with acute pyelonephritis. T. bernardiae was detected in blood and urine cultures; furthermore, multiple bacteria were detected in a urine culture. She was treated with ampicillin/sulbactam 3 g every 12 h on the day of admission. The fever was controlled, and inhaled oxygen 1 L/min via a nasal cannula was administered for dyspnea until hospitalization day 2. On hospitalization day 2, her fever resolved to 36 °C. Antimicrobials were de-escalated and changed to cephazolin and then to cephalexin on hospitalization days 9 and 16, respectively, and were continued until day 22. On hospitalization day 28, the urinary tract infection flared up; however, her fever resolved by hospitalization day 38 after the re-administration of antimicrobial agents. She was discharged on hospitalization day 60. CONCLUSIONS: We encountered a rare case of sepsis following acute pyelonephritis caused by T. bernardiae infection. When bedridden, diaper-using, very old patients present with urinary tract infections caused by multiple bacteria, the presence of rare opportunistic organisms, such as T. bernardiae, should be considered.Feb. 2023, BMC infectious diseases, 23(1) (1), 121 - 121, English, International magazineScientific journal
- BACKGROUND: Idiopathic chylopericardium is a rare disease characterized by filling of the pericardial cavity with chylous fluid and has no evident cause. Secondary chylopericardium usually results from injury or damage to the thoracic duct. The most common causes of secondary chylopericardium are trauma, thoracic or cardiac surgery, and congenital lymphangiomatosis. Conservative or surgical treatment can be pursued; however, surgical treatment is required if conservative treatment is unsuccessful. Pericardiocentesis plays a crucial role in the definitive diagnosis of chylopericardium. However, although a serious complication, its occurrence is infrequent. Non-invasive methods, such as computed tomography (CT), could be useful in predicting the color or characteristics of pericardial effusion. CASE PRESENTATION: A 37-year-old Japanese woman presented to our hospital with a cough that persisted for 1 week. Echocardiography revealed pericardial effusion, which was diagnosed as acute pericarditis and treated with loxoprofen. However, pericardial effusion increased, and the patient presented to the emergency room with cardiac tamponade 1 month later. Pericardiocentesis was performed, which confirmed that the pericardial effusion was chylopericardium. Lymphatic scintigraphy did not show any connection between the thoracic duct and pericardial cavity, and the patient was diagnosed with idiopathic chylopericardium. The patient underwent continuous drainage for 11 days. After completion of cardiac drainage, the patient was discharged from the hospital without any exacerbation. The CT attenuation value of the pericardial fluid was 11.00 Hounsfield units (HU). Compared with the other causes of pericardial effusions encountered at our hospital, the HU on CT scan of pericardial effusion was low in our study and similar to the values on CT scan of chylous ascites reported in previous studies. CONCLUSIONS: Although idiopathic chylopericardium is rare, it should be considered an important cause of pericardial effusion. - BACKGROUND: Hyperammonemia and hepatic encephalopathy are common in patients with portosystemic shunts. Surgical shunt occlusion has been standard treatment, although recently the less invasive balloon-occluded retrograde transvenous obliteration (B-RTO) has gained increasing attention. Thus far, there have been no reports on the treatment of portosystemic shunts with B-RTO in patients aged over 90 years. In this study, we present a case of hepatic encephalopathy caused by shunting of the left common iliac and inferior mesenteric veins, successfully treated with B-RTO. CASE SUMMARY: A 97-year-old woman with no history of liver disease was admitted to our hospital because of disturbance of consciousness. She had no jaundice, spider angioma, palmar erythema, hepatosplenomegaly, or asterixis. Her blood tests showed hyperammonemia, and abdominal contrast-enhanced computed tomography revealed a portosystemic shunt running between the left common iliac vein and the inferior mesenteric vein. She was diagnosed with hepatic encephalopathy secondary to a portosystemic shunt. The patient did not improve with conservative treatment: Lactulose, rifaximin, and a low-protein diet. B-RTO was performed, which resulted in shunt closure and improvement in hyperammonemia and disturbance of consciousness. Moreover, there was no abdominal pain or elevated levels of liver enzymes due to complications. The patient was discharged without further consciousness disturbance. CONCLUSION: Portosystemic shunt-borne hepatic encephalopathy must be considered in the differential diagnosis for consciousness disturbance, including abnormal behavior and speech.Feb. 2023, World journal of clinical cases, 11(4) (4), 945 - 951, English, International magazineScientific journal
- INTRODUCTION/OBJECTIVES: Social prescribing aims to improve health and well-being by connecting individuals with mental health, housing, and loneliness issues to informal services and support. This approach connects individuals to activities and services in their community to fulfill their practical, social, and emotional needs. However, no reports of community libraries being prescribed as a place to be referred through social prescribing were observed in the literature and the impact of community libraries on community residents and communities in social prescribing. The aim of this study was to identify the functions of a community library operated by medical and social professionals in the social prescribing initiative, its functions, and its impact on community residents and communities. METHODS: Semi-structured interviews were conducted with the users of the Daikai Bunko Library, a community library in Toyooka City, Japan. The library was established by a primary care physician and community residents as a place where visitors can stay and use it as a library, a bookstore, a café, and a consultation place. Interviews were recorded, and the verbatim transcript was analyzed using the Steps for Coding and Theorization. RESULTS: Ten individuals participated. The theoretical description of the interview texts revealed 11 categories as the function of the library and its effect on the individuals and the community: a place to stay, attractive space design, diverse accessibility, choosability of various roles, consultation function, social support, empowerment, mutual trust, formation of connections across generations/attributes, co-creation, and social impact. CONCLUSIONS: We found that a community library run by medical and social professionals was a useful social prescribing site and had various effects on the community residents who used it. - Takotsubo syndrome (TTS) is a disorder characterized by transient cardiac dysfunction with ventricular regional wall motion abnormalities, primarily thought to be caused by the effects of a sudden catecholamine surge on the heart. Although the majority of patients exhibit prompt recovery of their cardiac dysfunction, TTS remains associated with increased mortality rates acutely and at long-term, and there is currently no cure for TTS. Inflammation has been shown to play a key role in determining outcomes in TTS patients, as well as in the early pathogenesis of the disorder. There are also cases of TTS patients that have been successfully treated with anti-inflammatory therapies, supporting the importance of the inflammatory response in TTS. - Objectives: Dexamethasone's (DEXA) beneficial effect on survival when administered to critically ill patients with coronavirus disease 2019 (COVID-19) has been documented in randomized trials and meta-analyses. Here, we conducted this study to clarify the association between time from COVID-19 onset to steroid initiation and mortality and to examine the factors underlying these results.Methods: This was a multicenter, retrospective, observational study of patients enrolled in the Japanese COVID-19 Registry from January 1, 2020, to April 30, 2021. Demographic and clinical factors were extracted from patient records. Patients diagnosed with COVID-19 using polymerase chain reaction, loop-mediated isothermal amplification, or antigen tests were included. Patients aged <18 years, pregnant, with a history of chronic obstructive pulmonary disease or steroid or immunosuppressive drug use, transferred to another hospital, or with an unknown symptom onset were excluded.Results and Conclusion: The analysis included 3692 patients (men, 64.1%; median age, 68 years). Unadjusted comparisons of mortality groups showed significant differences in demographic and clinical characteristics; patients with early dexamethasone initiation had more risk factors for severe disease and significantly higher mortality than did patients with delayed initiation (13.3% vs 7.9%, p < .001). No significant differences were found in intubation rates or duration, length of hospitalization, or time from intubation to death. Multivariate analyses showed significant differences from symptom onset to steroid administration, with an adjusted odds ratio of 0.7 (p = .05) for patients who received steroids for ≥8 days. - This study aimed to investigate the influence of sex, age, and quadrivalent vaccination history on adverse reactions (ARs) to influenza vaccines and the relationship between the occurrence of ARs and the risk of influenza infection. Study participants were employees of three hospitals in the Hyogo Prefecture, Japan, who received the influenza vaccine in 2019. Data were collected using questionnaires. The main factors were age, sex, and history of influenza vaccination as a control. The primary outcomes were the incidence of local and systemic ARs attributable to the vaccine and positive influenza cases among the participants during the influenza season. Logistic regression was used to calculate the odds ratio (OR) and 95% confidence interval (CI). Among the 1493 participants, 80% experienced either local or systemic ARs. ARs were less common among men than among women (OR: 0.28, 95% CI: 0.21-0.37) and less common among those aged ≥60 years (OR: 0.48, 95% CI: 0.26-0.89). ARs were significantly more likely to occur in those with a history of influenza vaccination (OR: 1.96, 95% CI: 1.15-3.33). Those who had ARs, notably localized ones, were significantly more likely to incur influenza infection. - BACKGROUND: All drugs have the potential to cause drug-induced lung injury both during and after drug administration. Acetaminophen has been reported to cause drug-induced lung injury, although this is extremely rare. Herein, we present an extremely rare case of acetaminophen-induced pneumonia. CASE SUMMARY: A healthy 35-year-old Japanese woman visited a neighborhood clinic with complaints of fever and malaise following a tick bite. Her treatment included 1,500 mg acetaminophen (Caronal®) and subsequently minocycline (200 mg) and acetaminophen (2,000 mg; Caronal®) daily when her condition did not improve; the patient was eventually hospitalized. The patient's chest computed tomography (CT) revealed consolidation and ground-glass opacities in the right middle and lower lobes. Minocycline was shifted to sulbactam/ampicillin. However, her fever did not improve during follow-up, and her chest CT revealed extensive ground-glass opacities in the right middle and lower lobes and thick infiltrative shadows in the bilateral basal areas. Drug-induced lung injury was suspected; hence, acetaminophen was discontinued. The fever resolved immediately, and inflammatory response and respiratory imaging findings improved. A drug-induced lymphocyte stimulation test was performed against acetaminophen (Caronal®), and significant proliferation of lymphocytes was noted only for acetaminophen (stimulation index, 2.1). - This study investigated the frequency of adverse reactions to COVID-19 vaccines in Japan and the impact of first-dose adverse reactions on second-dose adverse reactions. Individuals who received an mRNA COVID-19 vaccine at our center in March or April 2021 were included. Data were collected using questionnaires. The main factors were age (<40, 40−59, and >60 years), sex, underlying disease, and first-dose adverse reaction. The primary outcomes were incidence of local and systemic adverse reactions (ARs) attributable to the vaccine. Logistic regression was used to calculate odds ratios (ORs) and 95% confidence intervals (CIs). Among 671 participants, 90% experienced local or systemic ARs. An AR to the first dose was associated with a significantly increased risk of an AR to the second dose (OR: 49.63, 95% CI: 21.96−112.16). ARs were less common among men than among women (OR: 0.36, 95% CI: 0.17−0.76). Local ARs were less common among those aged 60 years or older (OR: 0.35, 95% CI: 0.18−0.66), whereas systemic ARs were more common among those aged under 40 years. - Coronavirus disease 2019 (COVID-19) is an acute respiratory syndrome caused by SARS-CoV-2 and is known to cause respiratory and systemic symptoms. A SARS-CoV-2 infection is involved in aneurysm formation, enlargement, and rupture in medium-sized vessels, such as the cerebral and coronary arteries and the aorta. In contrast, its involvement in forming aneurysms in medium-sized vessels other than the cerebral and coronary arteries has not been reported. An 84-year-old Japanese man with COVID-19 was admitted to our hospital. The treatment course was favorable, and the COVID-19 treatment was completed by the 10th day. On day 14, pancreatic enzymes increased mildly. An abdominal computed tomography revealed a ruptured left gastric aneurysm after spontaneous hemostasis. Arterial embolization was performed. In this patient, a new left gastric aneurysm was suspected of having formed and ruptured during the course of the COVID-19 treatment. - BACKGROUND: On rare occasions, there can be a transition from Hashimoto's to Graves' disease. However, there are no reported cases of transition from Hashimoto's to Graves' disease triggered by the onset of Guillain-Barré syndrome. CASE PRESENTATION: Sixteen years prior, a 55-year-old woman was diagnosed with Hashimoto's disease and followed up without medication. One week after the appearance of signs of intestinal inflammation, weakness in the extremities was observed, and a cerebrospinal fluid test was positive for anti-GM1 IgG antibody, leading to the diagnosis of Guillain-Barré syndrome. In addition, hyperthyroidism was observed at the time of admission, and Graves' disease was diagnosed based on autoantibodies and thyroid echoes. Numbness in the extremities was relieved by high-dose intravenous gamma globulin. CONCLUSION: With the onset of Guillain-Barré syndrome, helper T cells became predominantly type 1, effector B cells increased in number, and thyroid-stimulating antibodies were produced, leading to the conclusion that Hashimoto's disease progressed to Graves' disease. - Objective This study investigated the different learning effects achieved through a clinical reasoning lecture that was simultaneously conducted via two formats: one format involved in-person face-to-face instruction, whereas the other provided remotely conducted online instruction. The lecture was based on a case presentation held at a participating university in 2015. Methods We compared the learning outcomes between the abovementioned formats based on participants' responses. Data were obtained using questionnaires distributed to eligible lecture attendees, including medical students and physicians who had graduated during the preceding 10 years. The questions were about the following aspects: the time duration of the lecture, degree of satisfaction with the online system (online attendees), and degree of satisfaction with the lecture content. The participants then completed a five-question mini-test related to the disease from the presented lecture case to assess their overall degree of understanding. Results Online participants gave significantly higher scores for images in the online system (degrees of satisfaction: online 72.7 ± 18.0 vs. in-person 55.6 ± 24.9), audio in the online system (66.1 ± 20.5 vs. 57.5 ± 25.8), the usefulness of multiple venues (82.1 ± 19.3 vs. 60.5 ± 25.0), intention to attend the next lecture (82.3 ± 19.0 vs. 65.8 ± 21.4), and overall meaningfulness of the lecture in the online interactive format (83.6 ± 16.3 vs. 72.6 ± 19.6) compared with the in-person group. However, similar mini-test scores were documented between the two groups (4.2 ± 0.7 for the in-person group and 4.3 ± 0.7 for the online group; no significant differences were noted). Conclusions The results show similar learning effects, degrees of satisfaction, and degrees of comprehension between online and in-person lecture attendees. Our findings demonstrate that the online format is a suitable pedagogical intervention in the study context. - BACKGROUND: Neuraminidase inhibitor-associated acute hemorrhagic colitis is rare. We report a case of ischemic enterocolitis that was likely caused by laninamivir. CASE SUMMARY: A 54-year-old female patient with influenza type A was administered 40 mg of laninamivir via inhalation once. On the same day, the patient experienced bloody stools and lower abdominal pain. A contrast-enhanced abdominal computed tomography showed edema-like changes from the descending colon to the sigmoid colon, which suggested ischemic enterocolitis. - BACKGROUND: An infected hepatic cyst causes clinical symptoms, such as fever and abdominal pain. A cyst with a diameter > 10 cm increases the likelihood of exclusion symptoms in adjacent organs. Herein, we report a case of pancreatitis caused by an infected hepatic cyst. CASE SUMMARY: The patient was an 88-year-old woman with a history of polycystic liver disease and a cyst > 10 cm in diameter. She was referred to our hospital for upper abdominal pain that persisted for four days before consultation. She had a fever of 37.4ºC, and a blood test showed a C-reactive protein level of 23 mg/dL. An infected hepatic cyst was diagnosed by abdominal ultrasonography, computed tomography, and magnetic resonance imaging. Antibacterial therapy and percutaneous cyst puncture did not elicit sufficient therapeutic effects. As the cyst growth continued, laparoscopic hepatic cyst fenestration was performed on hospitalization day 20. Thereafter, symptoms improved, and she was discharged on hospital day 31. CONCLUSION: To our knowledge, this is the second case report of pancreatitis associated with hepatic cyst growth. Percutaneous cyst puncture and drainage or surgical therapy can be considered if a slight improvement with antibiotic therapy alone or exclusion of surrounding organs is observed. - BACKGROUND: Infectious mononucleosis due to the Epstein-Barr virus is an infectious disease that causes the appearance of atypical lymphocytes in the peripheral blood; it mainly presents with fever, tonsillar pharyngitis, and lymphadenopathy. In addition to hepatitis, splenomegaly, and rashes, it can involve different organs. Here, a case of epididymitis as a rare complication in a patient with Epstein-Barr virus-associated infectious mononucleosis was reported. CASE PRESENTATION: A healthy 23-year-old man visited an outpatient clinic with fever and pharyngitis. Tonsillar pharyngitis, lymphadenopathy, atypical lymphocytes in the peripheral blood, liver dysfunction, and splenomegaly were observed. The patient was diagnosed with infectious mononucleosis based on clinical signs. The next day, the patient developed left testicular pain and was immediately transferred to the emergency outpatient ward. Pain, redness, and swelling were observed in the left scrotum. Ultrasonography revealed swelling of the epididymis and increased blood flow, and the patient was hospitalized with a diagnosis of left epididymitis. The patient's symptoms improved with symptomatic treatment and was discharged on day 16 after admission. Changes in antibody titers established a definitive diagnosis of infectious mononucleosis caused by the Epstein-Barr virus. Based on the disease course, the patient was also diagnosed with infectious mononucleosis associated with unilateral epididymitis. CONCLUSIONS: This is the first case report of Epstein-Barr virus-associated infectious mononucleosis complicated with acute epididymitis. - BACKGROUND Nonocclusive mesenteric ischemia (NOMI) is a life-threatening disease. We present the first case to indicate an association between NOMI and renal infarction caused by renal artery vasoconstriction. CASE REPORT An 84-year-old Japanese man with no relevant past medical history was evaluated in the Emergency Department for disturbance of consciousness. The patient had a consciousness level of E₁V₁M₄ on the Glasgow Coma Scale, temperature of 29.4°C, blood pressure of 90/40 mmHg, regular pulse rate of 48 beats/min, oxygen saturation of 72% while breathing ambient air, abdominal distention, and abdominal tenderness with peritoneal signs. His hemoglobin levels were 3.6 g/dL, blood urea nitrogen per creatinine 54/1.12 mg/dL, plasma glucose 10 mg/dL, and lactate 12.5 mmol/L. Enhanced computed tomography revealed a dilated and distended large transverse colon, spotted poorly enhanced areas in the bilateral kidneys, and poor contrast enhancement of the bilateral renal arteries. The patient died 48 h after admission due to multiple organ failure. An autopsy revealed dappled hemostasis and ischemia in the transverse colon and bilateral kidneys, with no thrombotic infarction in the superior mesenteric artery and/or bilateral renal arteries. We diagnosed NOMI with acute renal tubular necrosis caused by vasoconstriction of the bilateral renal arteries. CONCLUSIONS NOMI can occur simultaneously with renal vasoconstriction, suggesting that NOMI and renal artery vasoconstriction may share mechanisms. This case demonstrates that when enhanced computed tomography shows nonenhancing parenchymal regions in both kidneys, clinicians should check for renal artery vasoconstriction and the life-threatening disease of NOMI.Feb. 2022, The American journal of case reports, 23, e934688, English, International magazineScientific journal
- BACKGROUND: Varicella-zoster virus (VZV) generally causes chickenpox at first infection in childhood and then establishes latent infection in the dorsal root ganglia of the spinal cord or other nerves. Virus reactivation owing to an impaired immune system causes inflammation along spinal nerves from the affected spinal segment, leading to skin manifestations (herpes zoster). Viremia and subsequent hematogenous transmission and nerve axonal transport of the virus may lead to meningitis, encephalitis, and myelitis. One such case is described in this study. CASE SUMMARY: A 64-year-old man presented with dysuria, pyrexia, and progressive disturbance in consciousness. He had signs of meningeal irritation, and cerebrospinal fluid (CSF) analysis revealed marked pleocytosis with mononuclear predominance and a CSF/serum glucose ratio of 0.64. Head magnetic resonance imaging revealed hyperintense areas in the frontal lobes. He had four isolated blisters with papules and halos on his right chest, right lumbar region, and left scapular region. Infected giant cells were detected using the Tzanck test. Degenerated epidermal cells with intranuclear inclusion bodies and ballooning degeneration were present on skin biopsy. Serum VZV antibody titers suggested previous infection, and the CSF tested positive for VZV-DNA. He developed paraplegia, decreased temperature perception in the legs, urinary retention, and fecal incontinence. The patient was diagnosed with meningitis, encephalitis, and myelitis and was treated with acyclovir for 23 days and prednisolone for 14 days. Despite gradual improvement, the urinary retention and gait disturbances persisted as sequelae. - We devised and assessed open recruitment of host families for medical student homestays in a rural area of Hyogo Prefecture, Japan, so that program organizers would not have to depend on professional and personal connections. The duration of the homestays was one night and two days, and they were conducted in August 2016, 2017, and 2018. The purpose of this community-based medical education program was to promote interactions between medical students and residents of Tamba area. The study asked one family member from each host family to complete a questionnaire after the homestay, and their experiences were evaluated in the study. The questionnaire results were analyzed using a visual analog scale (VAS; 0-100 mm). Thirty-three host families participated in the homestay program over three years. Results showed that VAS scores were high for enjoyment of homestays (VAS; 92.4 ± 13.0), continuation of the homestay program (91.7 ± 12.7), continuation of participation in the homestay program (89.2 ± 16.2), and desire for the homestay students to work in the area in the future (95.4 ± 6.3). The recruitment of host families through advertising was an efficient method for this community-based medical education homestay program. The results indicate that it is possible to attract more host families through open recruitment, which will contribute to the sustainability of the homestay program. Further research, including a follow-up of the students who participated and whether they chose a rural area or Tamba to practice is needed in the future. - A 17-year-old female adolescent presented with her mother to our clinic with a two-month history of left-sided chest pain and a one-week history of middle back pain. We diagnosed straight back syndrome based on the chest and thoracic radiographic findings and symptoms of chest pain, palpitations, and dyspnea. We reassured the patient that the disease was benign and advised her to start and continue chiropractic therapy. All symptoms, including back pain, disappeared in three weeks. - We present an extremely rare case of rubella that developed after rubella vaccine administration. A 54-year-old man complained of back and neck pain for some days. He presented with generalized rash and arthralgia that had persisted for two days before his visit. His vital signs were normal, and arthralgia had disappeared during an examination, but lymphadenopathy in the left posterior neck and pink papules were observed throughout the body. He had received his first Rubella vaccination 17 days before this visit and had attended a crowded festival. Owing to the rubella epidemic in that prefecture, we performed a rubella antibody test and polymerase chain reaction assay using blood, urine, and pharyngeal swab specimens. Rubella IgG and IgM antibody titers were 3 and 1.48, respectively. The pharyngeal swab yielded positive results for the 1a vaccine strain. Therefore, he was diagnosed with rubella due to rubella vaccination. His symptoms improved eventually. His clinical course was uncomplicated. Symptoms resolved within one week without specific treatment. The vaccine rubella strain is not as highly infectious as wild-type rubella strains. If rubella symptoms appear after vaccination, it must be investigated whether these are vaccine-specific adverse reactions, wild-strain rubella onset, or other eruptive viral infections.Sep. 2021, Vaccines, 9(9) (9), 1040, English, International magazineScientific journal
- Objective The objective of this study was to evaluate the concerns and anxieties of hospitalized coronavirus disease (COVID-19) patients. Methods A questionnaire was distributed to patients on discharge. The responses were analyzed once they were returned by mail. Results Responses were received from 27 of 39 patients (average age, 50 ± 17 years). Among the participants, 16 were male (59.3%), 19 were symptomatic (70.4%), and two required a ventilator (7.4%). Anxiety about symptom exacerbation was reported by 36.0% of participants. Quarantine-associated stress was experienced by 60.0% of participants, and 72.0% of participants supported the change in federal policy that allows asymptomatic patients and patients with mild conditions to isolate themselves at a hotel or their home. Following discharge, 44.0% of participants experienced anxieties regarding their lives after discharge, and 56.0% were anxious regarding discrimination and rumors. During hospitalization, 68.0% of participants re-evaluated their attitude toward health, 44.0% regretted the preventative measures they took before contracting COVID-19, and 44.0% felt guilty for becoming infected. Conclusions Participants experienced various kinds of stress related to hospitalization for COVID-19. There was a trend of people being more concerned about their relationships than their own health. - INTRODUCTION: Extended-spectrum β-lactamase-producing Escherichia coli (ESBL-EC) reportedly accounts for >20% of E. coli infections and 2.0% of infective endocarditis cases. Nonetheless, there is a global paucity of reports on infective endocarditis caused by ESBL-EC. CASE: An 83-year-old Japanese man who underwent mitral annuloplasty for mitral valve prolapse 5 years ago developed a fever of 38.5°C. The patient was hospitalized the first time for pyelonephritis and bacteremia due to ESBL-EC and treated successfully with the antimicrobial meropenem for 14 days. Two days after discharge, however, the patient was re-admitted with bacteremia due to ESBL-EC. He was treated successfully with the antimicrobial cefmetazole for 14 days. The patient was admitted to our institution for a third time due to bacteremia again, a day after discharge following meropenem antibiotic therapy. Transesophageal echocardiography showed vegetation in the anterior mitral valve annulus. Magnetic resonance imaging of the head showed septic cerebral embolism. The patient was diagnosed with infective endocarditis due to ESBL-EC and underwent mitral valve replacement. After 6 weeks of antibiotic therapy with meropenem and tobramycin, the patient recovered completely. The causative E. coli strain MS6396 was identified as the E. coli clone ST131 by multilocus sequence typing and confirmed the presence of bla CTX-M-27 ESBL gene. CONCLUSION: Only six cases of infective endocarditis associated with ESBL-EC have been reported in the past. Moreover, this is the first report of a patient with infective endocarditis bacteriologically or genetically analyzed for ESBL-EC. - BACKGROUND: Cytomegalovirus (CMV) is reported to have thrombogenic characteristics that activate factor X in vitro and stimulate the production of factor VIII and von Willebrand factor (vWF). Thrombosis associated with CMV infection is prevalent among immunocompromised patients and predominantly presents as a solitary large thrombus in the deep vein, pulmonary artery, splanchnic arteriovenous ducts, or other similar sites. Multiple thrombi, however, are rarely observed in such cases. Here, we report about an immunocompetent man with multiple microthrombi associated with CMV infection. CASE PRESENTATION: A 72-year-old Japanese man who complained of abdominal pain was hospitalized with multiple colonic stenosis. He was later diagnosed with CMV enterocolitis and treated with ganciclover from Day 27 post-admission. During hospitalization, the patient developed thrombi in his fingers. He was initially treated with anticoagulant therapy (rivaroxaban); however, the therapy was discontinued owing to a prolonged activated thromboplastin time and an elevated international normalized ratio of prothrombin time. Instead, vitamin K and fresh-frozen plasma were administered. Nevertheless, his coagulation profile remained abnormal. Eventually, he developed colonic perforation and had to undergo emergency surgery. An intraoperative specimen showed several microthrombi in the middle and small arteriovenous ducts of his small and large intestines. The patient's coagulopathy improved preoperatively, and his overall condition improved postoperatively. Since the activation of ADAMTS13 was reduced remarkably, the thrombotic tendency was determined to be a thrombotic microangiopathy-like condition owing to increased vWF. We could not attribute the coagulopathy to any other cause except CMV infection; therefore, we concluded that this was a case of multiple thrombosis associated with CMV. CONCLUSIONS: We present an extremely rare case of a patient with multiple thrombotic microangiopathy-like microthrombosis caused by CMV infection. - Immunoglobulin A (IgA) vasculitis is characterized by small vessel vasculitis involving immune complexes and IgA deposition. The development of heparin-induced thrombocytopenia (HIT) during IgA vasculitis is extremely rare. An 87-year-old man presented with general fatigue, leg edema, purpura, arthritis and renal disease. He was diagnosed with IgA vasculitis and was admitted to our hospital. Hemodialysis with heparin was initiated thrice a week on post-admission Day 11. On Day 21, during hemodialysis, the pressure in the dialysis circuit increased and the dialysis was interrupted. On Day 24, the platelet count rapidly decreased to 18 000/μl. The patient was diagnosed with HIT after testing positive for HIT antibodies; heparin was discontinued at this time. - BACKGROUND: Acyclovir (ACV) neurotoxicity is a neuropsychiatric condition induced by the anti-herpetic drugs ACV and valacyclovir (VACV). It is presumed that elevated blood levels of ACV and its metabolite 9-carboxymethoxymethylguanine are involved in the development of ACV-induced encephalopathy; age and renal dysfunction are risk factors. Here, we report a case of encephalopathy caused by the administration of VACV for herpes zoster prophylaxis in a patient with renal dysfunction owing to multiple myeloma. CASE PRESENTATION: Renal dysfunction was diagnosed in a 70-year-old man visiting our hospital for a medical checkup. His creatinine clearance rate was 8 mL/min. He was diagnosed with symptomatic multiple myeloma, and bortezomib/dexamethasone (BD) therapy for multiple myeloma and VACV for herpes zoster prophylaxis were initiated. We administered 500 mg/day of VACV three times a week, a lower dosage than recommended, after adjusting for his renal impairment. His renal function was monitored twice per week during therapy. During the second course of BD therapy, 6 weeks after starting treatment, he was hospitalized owing to impaired consciousness (Glasgow Coma Scale score: E2, V4, M4), and his BD and VACV therapy were suspended. Brain magnetic resonance imaging and cerebrospinal fluid analysis showed no abnormalities. Three days after discontinuing BD and VACV therapy, his consciousness recovered completely, and impaired consciousness did not recur after resuming BD therapy. His clinical diagnosis was thus ACV-induced encephalopathy. CONCLUSION: VACV is often prescribed to patients with multiple myeloma receiving BD therapy to prevent herpes zoster. ACV-induced encephalopathy is commonly observed in patients with renal dysfunction; especially among patients with multiple myeloma with Bence-Jones proteinuria, renal tubules are easily damaged and plasma ACV concentrations are likely to increase and induce ACV-induced encephalopathy. - OBJECTIVES: This study investigated what kinds of experiences influence regional quota (chiikiwaku) medical students' motivation to practice community healthcare (CH), and the mechanism of this influence, by focusing on their experiences in a community-based medical education (CBME) programme. DESIGN: A qualitative thematic analysis based on interviews. SETTING: Participants were recruited from the chiikiwaku students of Kobe University, Japan, using purposive sampling. PARTICIPANTS: Fourteen students participated. The median (IQR) age of participants was 23 (23-24); half were sixth-year and half fifth-year students. ANALYSIS: From September to December 2018, the interviews were audiorecorded and transcribed verbatim. Data were analysed according to the 'Steps for Coding and Theorisation' method. Our theoretical framework comprised three internal motives (ie, needs, cognitions and emotions) and their subordinate motivation theories self-determination theory, expectancy-value theories, and positive and negative emotions, respectively. RESULTS: Three mechanisms and corresponding experiences emerged. The first mechanism, envisioning and preparing for practising CH, included corresponding experiences-empathy for the community, grasping the demands for CH, understanding the practices of CH, finding a role model and diminishing the conflicts between personal life and career. The second mechanism, belonging to a supportive community, included the robust construction of students' CH community and harmonisation with community residents. The third mechanism, psychological effects included the affect heuristic and framing effect. Student experiences brought about the changes and influences described in the presented mechanisms, and had both positive and negative impacts on their motivation towards CH. These results can be interpreted through the multifaceted lenses of motivation theories. CONCLUSIONS: The authors revealed that motivation mechanisms of medical students towards CH derived from positive interaction with community residents, healthcare professionals and other students, and from exposure to attractive community environments and cultures. - Haemophilus influenzae causes severe infections such as pneumonia and meningitis. Here, we report the complete genome of H. influenzae type a strain TAMBA230, which was isolated in 2019 from a patient exhibiting bacteremia. - BACKGROUND: Community-based medical education (CBME) has been evolving globally. However, the long-term impacts of CBME programs on career intention are ambiguous. Therefore, this study aimed to reveal the long-term impact of community-based clinical training (CBCT) such as CBME programs in Japan on current community healthcare (CH) practice. METHODS: This cross-sectional study targeted physicians who had graduated from Kobe University School of Medicine between 1998 and 2004 and had over 15 years' experience after graduation. Self-administered questionnaires were mailed to participants between September and November 2019. Of the 793 potential subjects, 325 questionnaires were undeliverable. A total of 468 questionnaires substantially sent to the subjects. The exposure was the undergraduate CBCT defined as clinical training about CH in a community. The primary outcome was the provision of current CH practice. The secondary outcome was rural retention. The odds ratios (ORs) and confidence intervals (CIs) were calculated, and the confounders (age, gender, and attitude toward CH at admission; primary outcome, and age, gender, attitude toward rural healthcare at admission, own and spouse's hometown, and emphasis on child education; secondary outcomes) were adjusted using multivariate logistic regression analysis. RESULTS: A total of 195 (41.7%) questionnaires were analyzed. The mean (standard deviation [SD]) age of study participants was 43.8 (3.5) years and 76.4% were men. A total of 48 physicians (24.6%) experienced CBCT, of which the mean (SD) training period was 26.3 (27.3) days. As many as 148 (76.3%) physicians provided CH at the time of the study, and 12 (6.5%) worked in rural areas. There was no notable impact of undergraduate CBCT on current CH practice (OR, 1.24; 95% CI, 0.53-3.08; adjusted OR [aOR], 1.00; 95% CI, 0.43-2.30) and rural retention (OR, 0.59; 95% CI, 0.06-2.94; aOR, 0.59; 95% CI, 0.11-3.04). CONCLUSIONS: It may be insufficient to use conventional CBCT in Japan to develop CH professionals effectively. Japanese CBME programs should be standardized through a review of their content and quality. - BACKGROUND: Common symptoms of Campylobacter colitis include abdominal pain, vomiting, diarrhea, and fever, among others. However, Campylobacter colitis also has a high incidence of extraintestinal symptoms. CASE SUMMARY: We report the case of a 51-year-old man who presented with bilateral testicular pain. A scrotal examination failed to reveal any physical findings, but the patient exhibited mild tenderness in the right lower abdomen. Computed tomography revealed ileocecal wall thickening. Post-admission, the patient developed diarrhea, and a stool culture was submitted; Campylobacter jejuni infection was confirmed. Testicular pain is known to be caused by appendicitis. Consequently, we suggest that Campylobacter colitis, which causes ileocecal inflammation, caused the testicular pain in this case. - COVID-19 can lead to severe pneumonia, requiring mechanical ventilation. While increased sputum secretion could cause airway obstruction during mechanical ventilation, there are few reported cases in the literature. We report a case of a 65-year-old man with diabetes and severe COVID-19 pneumonia requiring mechanical ventilation and treated with hydroxychloroquine, azithromycin, nafamostat, and prone positioning. Initially, mechanical ventilation consisted of a heat moisture exchanger, endotracheal tube aspiration, and subglottic secretion drainage using a closed suction system. However, endotracheal tube impaction by highly viscous sputum occurred during this mechanical ventilation system. Replacing the endotracheal tube, the use of a humidifier instead of a heat moisture exchanger, and prone positioning contributed to the patient being weaned off mechanical ventilation. Although anti-aerosol measures are important for severe COVID-19 pneumonia, attention should be given to potential endotracheal tube impaction during mechanical ventilation.Jun. 2020, Cureus, 12(6) (6), e8626 - e8626, English, International magazineScientific journal
- [Curd Formation Resulting from a Reaction between Cranberry Juice and Enteral Formula].White solids were observed in the esophagus of a patient with multiple system atrophy. The patient was receiving enteral nutrition with a polymeric formula and cranberry juice via nasogastric feeding. To test the assumption that the precipitates were formed from a reaction between the juice and the formula, a verification experiment was conducted usingformulae of differinging redients, pH and protein content. The results indicated that a precipitate was formed when formulae with lower pH values and higher protein content were used. Mixing a pH neutral enteral formula with cranberry juice, vinegar or their 2- fold diluted solutions may result in the formation of precipitates in the stomach and esophagus.Jun. 2020, Gan to kagaku ryoho. - Adult-onset Still's Disease (AOSD) is a systemic inflammatory disorder characterized by high fever, skin rashes, and joint pains, and is extremely rare in patients over 80 years of age. An 88-year-old woman was admitted with high fever lasting for > 2 weeks and arthritis of the right knee and bilateral wrists. Further examination revealed that the patient fulfilled the Yamaguchi criteria, the most sensitive and extensively used classification criteria for AOSD. After ruling out other causes and considering a greatly raised serum interleukin-18 (IL-18) level, the patient was diagnosed with AOSD. Before prednisolone therapy, active tuberculosis was excluded using chest computed tomography (CT) and an interferon-gamma release assay (IGRA). After starting the treatment, serum levels of IL-18 and acute-phase reactants were decreased gradually. However, during prednisolone tapering, fever relapsed along with increasing serum acute phase reactant levels. Her serum IL-18 level was decreased but remained at a high level, and the neopterin level was further increased. These findings suggested the onset of another disease, but not AOSD recurrence. A chest CT scan revealed new lung infiltrates. Despite the initial negative IGRA result, cultures and polymerase chain reaction tests of bronchoalveolar lavage and sputum were positive for Mycobacterium tuberculosis. She was placed on a 9-month course of anti-tuberculosis therapy and continued prednisolone tapering. She showed steady improvement and her cytokine profile showed a decrease in the IL-18 and neopterin levels. - BACKGROUND: In community-based medical education, opportunities for medical students to interact with local residents are important. To facilitate such interaction, we aimed to evaluate acceptance of a homestay program and attitude toward community medicine among medical students. METHODS: The participants (n = 39) were allowed to stay in the local homes of residents for one night in August 2016, 2017, and 2018. Before and after the homestays, the students responded to a self-reported questionnaire using the visual analog scale (VAS; 0-100 mm). The questionnaire included four questions on homestay/community medical training and community medicine and four questions about attitude toward community medicine in the local areas of medical students. Then, we compared the VAS scores before and after training. RESULTS: The VAS scores for all questions about homestay/community medical training and community medicine significantly increased: "Is it worthwhile for you to have experience in the field of community medicine," "Did you find the homestay enjoyable," "Does the homestay add educational significance to the program," and "Is direct interaction with residents meaningful?" For the two questions about attitude toward community medicine, the VAS scores significantly increased: "Is there a challenge to practicing community medicine" and "In the future, do you want to work in Tamba area where you stayed?" CONCLUSIONS: The medical students were extremely enthusiastic about the educational program for community medicine involving residential homestays, which improved their attitudes toward practicing community medicine. - BACKGROUND: Asplenia, the lack of a spleen, can be congenital and increases susceptibility to severe infections caused by encapsulated bacteria, such as Streptococcus pneumoniae (S. pneumoniae). We report two cases of severe pneumococcal infection in two asplenic family members living in the same household. CASE SUMMARY: Patient 1, a 38-year-old man with a history of congenital hepatitis B infection and hypospadias, was brought to our emergency department with complaints of cyanosis, cough, and edema of his limbs. He was clinically diagnosed as hyposplenic with overwhelming pneumococcal sepsis. He was admitted to the intensive care unit and was administered antibiotics and catecholaminergic therapy but died 2 h after admission. Patient 2, a 63-year-old woman with a history of type 2 diabetes, was brought to our emergency department one month after admission of Patient 1. She was diagnosed as asplenic with overwhelming pneumococcal sepsis. History-taking revealed that she was the mother of Patient 1 and the two had lived in the same household. She was admitted to the intensive care unit and was rapidly provided antibiotics and catecholaminergic intervention but died one day after admission. CONCLUSION: Pneumococcal bacteremia caused by virulent S. pneumoniae may be transmitted within households. All residents of households where individuals with pneumococcal bacteremia are living should be educated about the risk of transmissibility. - Adult-onset Still's disease (AOSD) is a relatively rare systemic inflammatory disorder and is diagnosed using various sets of classification criteria, with the Yamaguchi criteria as the most widely used criteria. Herein, we present the case of a 21-year-old woman admitted with a high fever, lasting for over 1 month, who did not fulfill the Yamaguchi criteria. However, by analyzing the inflammatory cytokine profile, we defined this case as AOSD based on a greatly elevated serum interleukin-18 level. In addition, we predicted the occurrence of macrophage activation syndrome by a characteristic increase in the soluble tumor necrosis factor receptor II level, which allowed a timely intervention for this malicious complication. Cat-scratch disease occurs after a scratch from a cat infected with Bartonella henselae. We report a 64-year-old man with cat-scratch disease who had swollen lymph nodes. Consequently, metastasis of a malignant tumor was suspected as the most likely cause in the differential diagnosis. Therefore, the diagnosis was delayed and several diagnostic examinations were performed. Although diagnosis was difficult in this case, an accurate diagnosis was achieved by taking a detailed medical history, including questioning about pet ownership, and by carefully examining the lower limbs including the femoral region. These steps are important to diagnose cat-scratch disease.
[Refereed] - AIM: To investigate whether GNB3 C825T single nucleotide polymorphism (SNP) contributes to systolic blood pressure (SBP) ≥ 130 mmHg in a large-scale cross-sectional study among the Japanese population with diastolic blood pressure (DBP) < 85 mmHg.METHOD: We analyzed 11008 Japanese subjects, including 2797 cases (SBP ≥ 130 and DBP < 85 mmHg) who were not taking anti-hypertensive medication and 8211 controls (SBP < 130 and DBP < 85 mmHg), all of whom enrolled in the genome banking project of the 21st Century COE (Center of Excellence) Program at Jichi Medical University. Subjects were divided into four groups according to gender (male and female) and age (≤ 49 years and ≥ 50 years). GNB3 gene polymorphism was determined using the TaqMan probe method. We compared the frequencies of alleles and genotypes between cases and controls by chi-squared test. The strength of the associations was estimated by odds ratios (ORs) and 95%CI by using logistic regression analysis. The ORs were adjusted for age and body mass index.RESULTS: Allele and genotype distributions significantly differed between cases and controls only in males aged ≤ 49 years. - 【目的】血管内皮は、脈管トーン、細胞増殖、血小板と白血球の相互作用、血栓形成を管理している多数の因子を分泌する大きなパラクライン器官である。血管内皮機能の障害は、アテローム性動脈硬化症、高血圧および心疾患の病態における重要な要因の一つであると考えられている。血流依存性血管拡張反応(FMD)は、血管内皮機能を評価し、動脈硬化性疾患の早期診断における検査として注目されている。本研究の目的は、女性生活習慣病患者のFMDが通院による運動療法の有無により違いがあるかどうかを検討することである。公益社団法人日本理学療法士協会, Apr. 2008, 理学療法学, 35巻, Suppl.2, pp. 221-221, D0389 - D0389, Japanese
【方法】対象は、高血圧、糖尿病、脂質異常症のいずれかの診断を受けた当院に受診中の女性患者18名である。当院リハビリ室に通院し、週2回以上の運動療法を1年以上継続している10名(年齢69.6±5.1歳)を施行群、運動療法を実施していない8名(年齢59.0±10.0歳)を非施行群として両群の上腕動脈のFMD(ユネクス社製汎用超音波画像診断装置UNEXEF18Gを使用)を比較した。FMDは変化率、拡張量、最大拡張到達時間について測定し、対応のないt検定を用いて統計解析を行った。運動療法はトレッドミル、自転車エルゴメータを用い、無酸素性代謝閾値の範囲内における強度での有酸素運動を中心に実施した。
【結果】両群の比較において、変化率、拡張量に有意な差は認めなかった。最大拡張到達時間は、施行群44.8±9.5秒、非施行群60.3±17.6秒であり、施行群は非施行群に対し有意に短縮していた(p<0.05)。また、年齢は施行群が有意に高かった(p<0.01)。
【考察】安静時血管径は性別により差があり、女性は男性と比較して明らかに小さいとの報告がある。Davidらの行った大規模研究では、FMDは年齢に強く依存していることが判明し、50歳代より変化率は低下し、最大拡張到達時間については延長することが報告されている。女性生活習慣病患者に限定した本研究では、定期的な運動療法を施行している群と非施行群のFMDの違いを検討した。その結果、両群の変化率と拡張量いわゆる血管径の変化に差はなかったが、最大拡張到達時間については、施行群は非施行群に対し、明らかに高齢であるにも関わらず有意に短縮していた。このことは、FMDはずり応力に反応した血管内皮のNO(一酸化窒素)の産生を反映しているとされることから、運動療法はNOの拡散速度など、血管内皮の反応速度の改善に関与している可能性が示唆された。運動療法の介入による血管内皮機能の生理学的変化は十分解明されていないが、1型糖尿病患者を対象にした研究において運動療法が変化率を改善させたという報告がある。今回は横断的な検討であるため、今後、一定期間の運動療法が変化率および最大拡張到達時間に与える効果を見出し、FMDが運動療法の効果判定の一手段となり得るかを検証していきたい。 - Sep. 2007, 中部日本整形外科災害外科学会雑誌, 50巻, 秋季学会, pp. 133-133, Japaneseスキー場直近医療施設でのスキー・スノーボード外傷の検討International conference proceedings

- Difficult Diagnosis of Spontaneous Intracranial Hypotension with Nausea and Lower Abdominal Pain as Main Complaints: A Case ReportBackground and Clinical Significance: Symptoms of spontaneous intracranial hypotension include orthostatic headaches due to decreased cerebrospinal fluid (CSF) levels. Here, we present a 24-year-old female admitted to an obstetrics and gynecology department with primary complaints of lower abdominal pain and dysmenorrhea with subsequent diagnosis of spontaneous intracranial hypotension (SIH). Case Presentation: The patient had experienced nausea and lower abdominal pain independent of her menstrual cycle 5 days before admission, for which she visited the emergency department 3 days later. On admission, her symptoms were temporarily relieved by administering analgesics; thus, she was discharged. However, later, the symptoms worsened. Consequently, she returned to the emergency department for further evaluation, including blood tests, imaging, and endoscopy, which revealed no nausea- or abdominal pain-related organic abnormalities. On day 10, she developed a headache, aggravated by lying in the supine position and improved by sitting. Additional history revealed a diagnosis of SIH owing to the worsening abdominal pain in the supine position. An ¹¹¹In CSF cavity scintigram showed no spinal fluid leakage; early intrabladder radioisotope (RI) accumulation was observed, and the residual 24 h CSF cavity RI was >30%. At a referral specialist hospital, an epidural saline infusion test was performed, which improved her headache and lower abdominal pain. Blood patch therapy improved her lower abdominal pain, headache, and dysmenorrhea. Conclusions: The final diagnosis was SIH, with symptoms attributed to CSF depletion. The patient also experienced rare paradoxical postural-related headaches and lower abdominal pain, aggravated by lying in the supine position, contributing to the final diagnosis.Dec. 2024, Reports, 7(4) (4), 115, English

- リステリア感染症は経口的に摂取されたリステリアが腸粘膜に浸透し全身感染症となる。症例提示および当院におけると侵襲性リステリア感染症のレビューを報告する。症例は 89 歳女性。数ヶ月前に上行結腸癌(cT3N1bM0)を指摘されたが,手術を希望せず経過観察となっていた。入院 1 週前から食欲不振が出現し,悪心が持続するため当院を受診し,入院した。血液培養でListeria monocytogenesが検出され,侵襲性リステリア感染症として,第 6 病日から抗菌薬をアンピシリン 4g/ 日に変更し,計 21 日間治療した。 当院開設後 2 年間で,培養検査で Listeria monocytogenes が培養された症例をまとめた。本症例を含めた 7 例あり,年齢は 65 歳-93 歳(中央値 88 歳)。全例血液培養陽性であった。消化器症状は 4 例のみ,6 例に発熱,5 例で悪性腫瘍合併(4 例消化器癌)を認めた。侵襲性リステリア感染症では,悪性腫瘍の併存を念頭 に置くことが重要である。また担癌状態での発熱では,消化器症状がなくともリステリア感染症を念頭におく必要がある。一般社団法人 日本病院総合診療医学会, 31 May 2022, 日本病院総合診療医学会雑誌, 18(3) (3), 151 - 155, Japanese

Cat-scratch disease occurs after a scratch from a cat infected with Bartonella henselae. We report a 64-year-old man with cat-scratch disease who had swollen lymph nodes. Consequently, metastasis of a malignant tumor was suspected as the most likely cause in the differential diagnosis. Therefore, the diagnosis was delayed and several diagnostic examinations were performed. Although diagnosis was difficult in this case, an accurate diagnosis was achieved by taking a detailed medical history, including questioning about pet ownership, and by carefully examining the lower limbs including the femoral region. These steps are important to diagnose cat-scratch disease.

- 症例は88歳,女性.基礎疾患として肝機能障害はなく,神経因性膀胱とAlzheimer型認知症がある.高アンモニア血症による意識障害で当院に入院した.尿閉があり導尿したところ,速やかに意識障害は消失し高アンモニア血症も改善した.尿培養からウレアーゼ試験陽性のCorynebacterium urealyticumが検出され,ウレアーゼ産生菌が原因の閉塞性尿路感染症による高アンモニア血症と診断した.高齢者の高アンモニア血症の鑑別疾患として,排尿障害,特にウレアーゼ産生菌による尿路感染症を考える必要がある.The Japanese Society of Internal Medicine, 10 Apr. 2013, 日本内科学会雑誌, 102(4) (4), 976 - 978, Japanese

- Virtual Histologyを用いたGrayscale IVUSにおける後方エコー減衰を伴うプラークの検討40MHzのgrayscale血管内超音波検査(IVUS)でエコー減衰を観察した連続10症例(全例男性、平均64.7±11.6歳)について、20MHz Virtual Histology IVUS(VH-IVUS)を用いてその組織構成を検討した。エコー減衰を伴うプラークをその組織構成によりlipid rich群とnecrotic core群の2群に分類し、更にnecrotic core群を石灰化の特徴等から石灰化なし、塊状石灰化、微小石灰化の三つの亜型に分類した。プラークの組織構成で、fibro-fatty成分又はnecrotic core成分を多く含むこと、高いプラーク面積率を有することがエコー減衰の要因となっている可能性がある。また微小石灰化を伴う多量のnecrotic coreはPCI後にslow flowを生じやすいことが示唆された。日本心血管インターベンション学会, 10 Oct. 2008, Jpn J Interv Cardiol, 23(5) (5), 343 - 349, Japanese

- 【目的】血管内皮は、脈管トーン、細胞増殖、血小板と白血球の相互作用、血栓形成を管理している多数の因子を分泌する大きなパラクライン器官である。血管内皮機能の障害は、アテローム性動脈硬化症、高血圧および心疾患の病態における重要な要因の一つであると考えられている。血流依存性血管拡張反応(FMD)は、血管内皮機能を評価し、動脈硬化性疾患の早期診断における検査として注目されている。本研究の目的は、女性生活習慣病患者のFMDが通院による運動療法の有無により違いがあるかどうかを検討することである。<BR>【方法】対象は、高血圧、糖尿病、脂質異常症のいずれかの診断を受けた当院に受診中の女性患者18名である。当院リハビリ室に通院し、週2回以上の運動療法を1年以上継続している10名(年齢69.6±5.1歳)を施行群、運動療法を実施していない8名(年齢59.0±10.0歳)を非施行群として両群の上腕動脈のFMD(ユネクス社製汎用超音波画像診断装置UNEXEF18Gを使用JAPANESE PHYSICAL THERAPY ASSOCIATION, 20 Apr. 2008, 理学療法学, 35(Supplement 2) (Supplement 2), 221 - D0389, Japanese
- 【目的】多くの血管は血流の増加、すなわち、shear stress(ずり応力)によって反応する。この現象を血流依存性血管拡張反応(FMD)と称し、FMDの主要なメディエーターは、血管内皮に由来するNO(一酸化窒素)である。近年、血管内皮機能障害の重症度が心、脳血管イベントの危険性に関係することが報告されており、FMDは早期の動脈硬化性変化としての血管内皮機能を評価することができる。本研究の目的は、年齢とFMDにおける変化率の関係が定期的な運動療法を実行している生活習慣病患者と健常者において違いがあるかどうかを検討することである。公益社団法人日本理学療法士協会, 20 Apr. 2008, 理学療法学, 35(2) (2), D0390 - D0390, Japanese
【方法】対象は、高血圧、糖尿病、脂質異常症のいずれかの診断を受けた当院に受診中の患者15名(年齢68.7±6.7歳、男性3名、女性12名)である。患者は当院リハビリ室に通院し、週2回以上の運動療法を1年以上継続している。運動療法はトレッドミル、自転車エルゴメータを使用し、無酸素性代謝閾値の範囲内における強度での有酸素運動を主体に実施した。一方、41歳以上の健常者51名(年齢50.6±5.9歳、男性13名、女性38名)を対照群とした。FMDは汎用超音波画像診断装置UNEXEF18G(ユネクス社製)を使用し、上腕動脈の変化率を測定した。変化率とは、駆血再灌流後の最大拡張血管径と安静時血管径との差を安静時血管径で除し、その値を百分率で示したものである。患者群、対照群それぞれの年齢と変化率の相関関係を検討し、その回帰直線の位置関係を比較した。
【結果】年齢と変化率の関係について、患者群では有意な相関を示し(r=-0.56、p<0.05)、対照群においても有意な相関を認めた(r=-0.47、p<0.01)。両群の回帰直線の比較では、患者群は対照群よりも上方へほぼ平行にシフトしていた。
【考察】年齢と変化率の関係は負の相関関係にあることが報告されているが、本研究においても同様の結果を得た。生活習慣病の病態では変化率は低値を示すことが予測され、FMDは動脈硬化の早期診断における検査として期待されている。我々は生活習慣病患者に対し、通院での運動療法の実践により病態のコントロールを目指す取り組みを展開しており、本事業において、腹囲の減少およびインスリン抵抗性に関する指標の改善が得られたことを報告した(第42回本学術大会)。運動療法がFMDを改善させることについてはいくつかの報告はあるものの、本装置を用いた知見は十分得られていない。患者群の年齢と変化率の回帰直線が対照群と比較して上方へシフトしていたという本研究の結果から、運動療法は生活習慣病を有していても血管内皮機能を改善させ、加齢による変化率の低下を緩徐にさせる可能性が示唆された。今後は、年齢層をマッチングさせたうえでの検討を行い、FMDが生活習慣病の運動療法教育において活用できるよう検証していきたい。 - (一社)日本循環器学会, 20 Oct. 2007, Circ J, 71(Supplement 3) (Supplement 3), 996 - 996, Japanese心臓MRI検査が病態の理解に有用であった心室中部閉塞性肥大型心筋症の一例

- 【目的】生活習慣病患者の治療の目的は、動脈硬化性疾患の予防にある。これまで、動脈硬化度を評価する検査には頚動脈エコー、脈波伝播速度が用いられてきた。近年、より早期の動脈硬化の危険性を評価できる血流依存性血管拡張反応(FMD)を用いた検査が開発されたが、この方法を使用し運動療法の効果を評価した研究は少ない。本研究の目的は血管内皮機能を反映するとされるFMDを用いて血管径を評価し、有酸素運動前後の急性効果としての反応を観察することである。社団法人 日本理学療法士協会近畿ブロック, 2007, 近畿理学療法学術大会, 2007, 94 - 94, Japanese
【方法】対象は健常男性1名(31歳)、メタボリックシンドロームの男性1名(57歳)の2名であり、それぞれ症例1、症例2とした。FMDの測定は、20分間のエルゴメーターによる有酸素運動の前後に行った。本来、空腹、安静時に測定するが、今回は、上腕動脈を探る5分間を安静の条件とした。運動強度は運動負荷試験にてAT pointを測定し、ATレベルの心拍数にて実施した。検査は、ユネクス社製汎用超音波画像診断装置UNEXEF18Gを使用し、マンシェットで収縮期血圧に70mmHgを加えた値で上腕を5分間駆血し、その後、駆血を開放し、虚血再灌流による血管径の変化を経時的に測定した。今回は、本測定機器で表される安静時血管径を駆血前血管径、最大拡張径を虚血再灌流後血管径とした。
【結果】症例1:駆血前血管径は運動前3.44mm、運動後3.61mm、虚血再灌流後血管径はそれぞれ3.86mm、4.13mm、拡張量は0.42mm、0.52mmであった。症例2:駆血前血管径は運動前4.08mm、運動後4.08mm、虚血再灌流後血管径はそれぞれ4.25mm、4.27mm、拡張量は、0.17mm、0.19mmであった。
【考察】FMDは血管内皮細胞から分泌される血管拡張作用を有するNOの反応を観察しているとされている。今回の結果から、症例1において、有酸素運動の急性効果として、駆血前血管径、虚血再灌流後血管径が拡張する血管内皮機能が忠実に確認できたと思われる。症例2では、運動前後を比較して、駆血前血管径は全く拡張せず、虚血再灌流後血管径の変化が殆んどなかった。この理由としてインスリン抵抗性に起因する動脈硬化の病態を表していることが考えられ、また、加齢による血管の器質的変化の影響も否定できない。今回使用したFMDは、血管の探索は手動でありながらも測定は自動化され、臨床検査として汎化し易い検査機器である。このように、血流が増加するshear stressによる血管拡張が簡便に数値化できることは、運動療法の効果判定に臨床上利用するデータとして有用であると考えられた。しかし現在、基礎データの集積が不十分であり、健常者、生活習慣病患者の血管動態が認識できているとは言えない。今後、長期間の運動療法におけるデータの収集に加え、脈波伝播速度など他のデータとの比較が必要である。 - 2007急性発症の左胸痛で発見された類骨骨腫の1例

- 日本学術振興会, 科学研究費助成事業, 挑戦的研究(萌芽), 神戸大学, Jun. 2019 - Mar. 2023地域創成まちづくり事業を活用した医療の枠組みを超えた地域医療教育の検証町づくり事業:夢前町花街道事業の一つである加点式健診事業(「よいとこ健診」)は、学生主体に実施されている。姫路市夢前町地区の住民(主に65歳以上)を対象として、高齢者総合的機能評価(CGA)で用いられる項目:社会背景とライフスタイル、手段的日常生活動作、意欲、社会活動、運動機能、口腔機能、体脂肪、認知機能を評価し、良い結果に対してポジティブフィードバックを学生が行う形式でよいとこ健診は実施されている。よいとこ健診は、「健康に良い習慣・努力」を「発見して、ほめてもらう体験」を通して、健康づくりへのモチベーションアップとフレイル予防(介護予防)、家族・地域の社会関係を育むこと、および高齢者健診受診率の引き上げを目的としている。実施体制は、神戸大学医学部、経済学部、文学部、甲南女子大学栄養学部の学生による実行委員会と学生のコアメンバーと教員とで構成されるよいとこ健診共同研究会となっている。実施方法は、学生委員会で検討し、共同研究会で決定した。2022年度は、9月にオンライン形式で、3月は対面形式での実施となった。学生は、実施前に地元自治会の役員と意見交換会にも参加し、健診受診者の募集等にも関与している。学生が実施した住民による健診の評価および地区自治会との反省会での意見を参考に、次回の実施方法を検討する、いわゆるPDCAサイクルを学生が実施している。このように、他業種による学生による連携事業が構築された。オンライン形式と対面形式の違いとして、「現場の雰囲気が肌で感じることができ、より地域の理解が深まった。」「対面でフィードバックすることによって、より受診者との距離が近くなった感じがする。」「他の健診項目の実施状況が把握でき、健診への理解が深まった」「対面での協働作業ができたことで、学生間の繋がりが強まった感じがした」などの意見が多かった。

- 文部科学省, 科学研究費補助金(基盤研究(B)), 2012 - 2014地域医療臨床実習で学生が体験している内容および実習内容に対する学生の事前意識の検討を行った。自習での体験項目については、自治医科大学で5年生対象に実施された実習(2週間)の実習報告から、その抽出と分析とを行った。抽出された実習項目は71項目であった。それらの項目は、医療、在宅・介護・福祉、保健、救急、産業保健、地域(現場)の理解、講義、その他に大きく分類された。さらに、医療は、診療の見学、診療の体験、死の体験、他職種業務の体験、施設内カンファランスの参加に区分された。これらから、地域医療臨床実習で提供される実習項目が多岐にわたっていることがわかった。標準プログラムを提案する際には、実習内容を施設別に類型化することが必要であることがわかった。実習内容に対する学生の実習前の意識について、自治医科大学医学部5年生を対象に、実習前の実習内容希望質問紙調査を行って、その回答から分析を行った。実習項目への希望を、Competitive research funding

Thus they may be more easily affected by the tests than others. The questionnaire survey found that the negative genetic test had risk to recurrence an undesirable lifestyle, especial in people restricting their preferred lifestyle. The genetic information should be carefully informed in this case. It is found that the effect of the notification of the genetic test was not observed in most items about the lifestyle. Although we need detailed analyses in future, the genetic tests might not be usable to the health promotion and prevention of disease.Competitive research funding
