■ Paper- Mutations in leucine-rich repeat kinase 2 (LRRK2) are the most common cause of familial and sporadic Parkinson's disease (PD). While the clinical features of patients with LRRK2-PD resemble those of typical PD, there are significant differences in the pathological findings. The pathological hallmark of definite PD is the presence of α-synuclein (αSYN)-positive Lewy-related pathology; however, approximately half of patients with LRRK2-PD do not have Lewy-related pathology. Lewy-related pathology is a late-stage αSYN aggregation that can be visualized with hematoxylin and eosin stains or conventional immunohistochemistry (IHC). Increasing evidence has indicated that αSYN oligomers, which represent the early-stage of αSYN aggregation, may have neurotoxicity. Visualization of αSYN oligomers requires specialized staining techniques, such as αSYN-proximity ligation assay (PLA). Distribution and severity of αSYN oligomers in the brain of patients with LRRK2-PD remain unknown. In this study, we performed phosphorylated αSYN-IHC and αSYN-PLA staining on postmortem brain sections of patients with three pathogenic LRRK2 mutants: p.G2019S (n = 5), p.I2020T (n = 5), and p.R1441C (n = 4). The severity of Lewy-related pathology and αSYN oligomers was assessed semi-quantitatively in the brainstem, limbic lobe, basal ganglia, and cerebral cortex. αSYN oligomers were detected in patients with LRRK2-PD even in those without Lewy-related pathology; a negative correlation was observed between Lewy-related pathology and αSYN oligomers (r = - 0.26 [- 0.39, - 0.12]; P < 0.0001). Our findings suggest that αSYN oligomers may represent a common pathological feature of LRRK2-PD. Notably, patients harboring p.G2019S and p.I2020T had significantly higher levels of αSYN oligomers in those without Lewy-related pathology compared to those with Lewy-related pathology. These patients also had a trend toward shorter disease duration. These results imply that in LRRK2-PD, αSYN oligomers may initially accumulate in the brain but do not progress to form Lewy-related pathology. - BackgroundSubjective cognitive decline represents an early stage of mild cognitive impairment, with the Cognitive Function Instrument (CFI) serving to subjectively evaluate the decline in daily living activities associated with this minor cognitive decline.ObjectiveTo examine how CFI scores change with apolipoprotein E ε4 (ApoE4) carriage, objective cognitive decline, and dementia prevention intervention. We aimed to assess CFI's usefulness in the early dementia risk identification.MethodsThis study involved 196 older adults with normal cognition in a randomized controlled intervention trial. CFI was assessed every six months from baseline to 18 months, using the Alzheimer's Disease Cooperative Study-Preclinical Alzheimer Cognitive Composite (ADCS-PACC) to measure cognitive decline. We employed a mixed model for repeated measures to compare the CFI scores at 18 months in the ApoE4, ADCS-PACC, and allocation groups.ResultsCFI scores increased in ApoE4 carriers and decreased in the intervention group, with significant differences observed in the CFI score changes at 18 months between carriers and non-carriers and among the allocation groups (p = 0.002, p = 0.026, respectively). However, there was no significant difference in the CFI score change among ADCS-PACC groups (p = 0101).ConclusionsWe observed CFI scores worsening over time in individuals with ApoE4 and showing a tendency to deteriorate over time in those with objective cognitive decline. These findings suggest that the CFI may be able to identify high-risk individuals for dementia at an early stage. - BACKGROUND: The number of people with dementia is expected to grow substantially across the world due to population aging, but cognitive impairment remains undetected and undiagnosed, especially in early stages. Newly available diagnostic tools such as digital cognitive assessments and blood biomarker tests may be well suited to increase the rates of early detection of dementia in primary care. OBJECTIVES: The objective of the Davos Alzheimer's Collaborative Healthcare System Preparedness (DAC-SP) Early Detection Flagship Program was to improve the rate of early detection of cognitive impairment in primary care and non-specialty settings. We aimed to understand the program's impact on clinician attitudes, engagement, and confidence in diagnosing and managing cognitive impairment. DESIGN: Survey of participating healthcare professionals before and after the intervention. SETTING: The DAC Healthcare System Preparedness Early Detection Flagship Program was implemented in seven sites across six countries: Brazil, Jamaica, Japan, Mexico, Scotland, and the United States (2 sites). PARTICIPANTS: 110 healthcare professionals, including, primary care physicians, specialists (neurologists and psychologists), nurses, nurse practitioners, physician assistants, social workers, and healthcare support workers completed the pre-intervention survey. 68 healthcare professionals completed the post-intervention survey. INTERVENTION: Participating sites implemented a digital cognitive assessment tool and a blood biomarker test for the Alzheimer's pathology and were trained in the administration of the digital cognitive assessment tool. The intervention was adapted to each site for cultural relevance and operational feasibility. MEASUREMENTS: Participants completed the General Practitioners Attitude and Confidence Scale for Dementia (GPACS-D), a 15-item scale with three subscales: Attitude to Care (six items), Confidence in Clinical Abilities (six items), and Engagement (three items). In addition to the subscale scores, the total GPACS-D score was reported. RESULTS: Across all sites, there was a significant increase in the Confidence in Clinical Abilities score from 2.98 (SD = 0.77) pre-intervention to 3.27 (SD = 0.72) post-intervention (p = 0.01), and in the total GPACS-D score from 3.48 (SD = 0.48) to 3.65 (SD = 0.39) (p = 0.01). There were non-significant increases in the Attitude to Care and Engagement scores across all sites. CONCLUSIONS: The implementation of digital cognitive assessment tools and a blood biomarker test was associated with an increase in healthcare professionals' confidence in diagnosing and managing patients with cognitive impairment in primary care and non-specialty settings. Digital cognitive assessments and blood biomarker tests are promising tools that could be utilized in primary care to increase clinicians' confidence in detecting dementia and lead to timely clinical evaluation, treatment, and referral to supportive resources.Mar. 2025, The journal of prevention of Alzheimer's disease, 12(3) (3), 100038 - 100038, English, International magazineScientific journal
- BACKGROUND: The number of people with dementia is increasing in Japan, and establishing evidence for preventing dementia is necessary. METHODS: This study was a randomized controlled trial in cognitively normal community-dwelling older adults aged 65 to 85 with diabetes and/or hypertension. Participants were randomly assigned in a 1:1 ratio. The intervention group underwent 90 min of group-based weekly physical exercise, cognitive training, nutritional counseling, and vascular risk management for 18 months. The primary endpoint was the change in a cognitive composite score calculated by averaging the z-scores of seven neuropsychological tests from baseline to 18 months. RESULTS: We randomly assigned 203 participants to two groups, and 178 (87.7%) completed the 18-month follow-up. There was a significant group difference in the cognitive composite score change at 18 months (mean difference 0.16, 95% confidence interval: 0.04 to 0.27; p = 0.009). DISCUSSION: An 18-month multimodal intervention for older adults at risk of dementia could improve their cognitive function. The trial was registered in the Clinical Trial Registration System (UMIN000041938). HIGHLIGHTS: Japan-Multimodal Intervention Trial for Prevention of Dementia (J-MINT) PRIME Tamba was a randomized controlled trial to prevent dementia. We provided a multifactorial intervention based on the Finnish Geriatric Intervention Study to Prevent Cognitive Impairment and Disability (FINGER) trial methodology. The primary outcome, the cognitive composite score, improved with our intervention. Executive function/processing speed and memory improved in the intervention group. - Flexible pulse-by-pulse regulation of sensorimotor synchronization is crucial for voluntarily showing rhythmic behaviors synchronously with external cueing; however, the underpinning neurophysiological mechanisms remain unclear. We hypothesized that the dorsal anterior cingulate cortex (dACC) plays a key role by coordinating both proactive and reactive motor outcomes based on contextual mental imagery. To test our hypothesis, a missing-oddball task in finger-tapping paradigms was conducted in 33 healthy young volunteers. The dynamic properties of the dACC were evaluated by event-related deep-brain activity (ER-DBA), supported by event-related potential (ERP) analysis and behavioral evaluation based on signal detection theory. We found that ER-DBA activation/deactivation reflected a strategic choice of motor control modality in accordance with mental imagery. Reverse ERP traces, as omission responses, confirmed that the imagery was contextual. We found that mental imagery was updated only by environmental changes via perceptual evidence and response-based abductive reasoning. Moreover, stable on-pulse tapping was achievable by maintaining proactive control while creating an imagery of syncopated rhythms from simple beat trains, whereas accuracy was degraded with frequent erroneous tapping for missing pulses. We conclude that the dACC voluntarily regulates rhythmic sensorimotor synchronization by utilizing contextual mental imagery based on experience and by creating novel rhythms.Jul. 2024, Brain sciences, 14(8) (8), English, International magazineScientific journal
- BACKGROUND: Maximizing the efficiency to screen amyloid-positive individuals in asymptomatic and non-demented aged population using blood-based biomarkers is essential for future success of clinical trials in the early stage of Alzheimer's disease (AD). In this study, we elucidate the utility of combination of plasma amyloid-β (Aβ)-related biomarkers and tau phosphorylated at threonine 217 (p-tau217) to predict abnormal Aβ-positron emission tomography (PET) in the preclinical and prodromal AD. METHODS: We designed the cross-sectional study including two ethnically distinct cohorts, the Japanese trial-ready cohort for preclinica and prodromal AD (J-TRC) and the Swedish BioFINDER study. J-TRC included 474 non-demented individuals (CDR 0: 331, CDR 0.5: 143). Participants underwent plasma Aβ and p-tau217 assessments, and Aβ-PET imaging. Findings in J-TRC were replicated in the BioFINDER cohort including 177 participants (cognitively unimpaired: 114, mild cognitive impairment: 63). In both cohorts, plasma Aβ(1-42) (Aβ42) and Aβ(1-40) (Aβ40) were measured using immunoprecipitation-MALDI TOF mass spectrometry (Shimadzu), and p-tau217 was measured with an immunoassay on the Meso Scale Discovery platform (Eli Lilly). RESULTS: Aβ-PET was abnormal in 81 participants from J-TRC and 71 participants from BioFINDER. Plasma Aβ42/Aβ40 ratio and p-tau217 individually showed moderate to high accuracies when detecting abnormal Aβ-PET scans, which were improved by combining plasma biomarkers and by including age, sex and APOE genotype in the models. In J-TRC, the highest AUCs were observed for the models combining p-tau217/Aβ42 ratio, APOE, age, sex in the whole cohort (AUC = 0.936), combining p-tau217, Aβ42/Aβ40 ratio, APOE, age, sex in the CDR 0 group (AUC = 0.948), and combining p-tau217/Aβ42 ratio, APOE, age, sex in the CDR 0.5 group (AUC = 0.955), respectively. Each subgroup results were replicated in BioFINDER, where the highest AUCs were seen for models combining p-tau217, Aβ42/40 ratio, APOE, age, sex in cognitively unimpaired (AUC = 0.938), and p-tau217/Aβ42 ratio, APOE, age, sex in mild cognitive impairment (AUC = 0.914). CONCLUSIONS: Combination of plasma Aβ-related biomarkers and p-tau217 exhibits high performance when predicting Aβ-PET positivity. Adding basic clinical information (i.e., age, sex, APOE ε genotype) improved the prediction in preclinical AD, but not in prodromal AD. - BACKGROUND: Early diagnosis of dementia is important for both initiation of non-pharmacological activities to slow cognitive decline as well as the development of disease-modifying drugs; however, it appears there may be a tendency for formal diagnosis to be delayed. Since the current status of diagnosis in Japan is unclear, we conducted a survey with family caregivers of patients with dementia using questionnaires and interviews to investigate the factors regarding the dementia diagnosis process in Japan. METHODS: We distributed questionnaires to family caregivers of people with dementia and conducted additional follow-up interviews with approximately half of them. We calculated odds ratios for the time to diagnosis using logistic regression analysis for each characteristic from the questionnaire data. We also created co-occurrence networks from the interview data in order to provide qualitative context to the questionnaire data. RESULTS: We collected 68 questionnaires and conducted 32 interviews. The median time to diagnosis was 12 months, and logistic regression analysis showed a significant trend toward shorter time to diagnosis in the absence of other caregivers. In addition, there were significant differences in age, relationship with patients and the time from the first visit to the final diagnosis between groups with and without other caregivers. CONCLUSIONS: The results of this study suggest that the presence or absence of other caregivers may affect caregivers' behaviour and the time taken to receive a diagnosis of dementia. - BACKGROUND: The demand for more accurate and early diagnosis of mild cognitive impairment (MCI) patients due to Alzheimer's disease (AD) has increased after disease-modifying drugs were launched. Among these needs, there is a requirement for tools that can easily assess the ability to recall memories, which changes early in the disease. OBJECTIVES: We established Self Assessment Memory Scale (SAMS) method before, which includes 8-picture recall test and 16-word recognition test. We adopted this method to software that can be operated on a tablet computer so that participants can perform the method independently. The purpose of this study was to validate this method. DESIGN: Cross sectional research. SETTING: Some of the participants were recruited from hospitals for patients diagnosed with AD or MCI. The others were recruited from three regional cohorts of healthy older adults. PARTICIPANTS: The total number of participants was 304 (20 of whom had AD or MCI), and the mean age was 71.2 years. 64% of the participants were women. MEASUREMENTS: We used the logical memory subtest of the WMS-R as the standard for memory evaluation and assessed the relationship between this score and the SAMS score calculated by the software. RESULTS: The 2nd SAMS score were higher than the 1st SAMS score in some participants, on the other hand, the intraclass correlation coefficient was good. Since the number of false recognition in the 16-word recognition test was higher in participants with lower LM II scores, we developed a new score to reflect the ratio of false recognition, SAMS-R, and we observed it has good correlation with LM II. The mean SAMS-R score decreased gradually after the age of 65 years, indicating that age-related changes in memory recall can be detected. The ROC curve analysis was conducted to evaluate the detectability to determine whether if the WMS-R LM II score is above or below 10, showing that the AUC was greater than 0.9. CONCLUSION: SAMS-R, which can be performed on a tablet literally by himself/herself independently, shows a high correlation with the WMS-R Logical Memory II score, and has the advantage of being performed in a short time without the need for a clinical psychologist or other personnel.2024, Frontiers in aging neuroscience, 16, 1512947 - 1512947, English, International magazineScientific journal
- Importance Recent evidence indicates the efficacy of β-amyloid immunotherapy for the treatment of Alzheimer disease, highlighting the need to promote β-amyloid removal from the brain. Cilostazol, a selective type 3 phosphodiesterase inhibitor, promotes such clearance by facilitating intramural periarterial drainage. Objective To determine the safety and efficacy of cilostazol in mild cognitive impairment. Design, Setting, and Participants The COMCID trial (A Trial of Cilostazol for Prevention of Conversion from Mild Cognitive Impairment to Dementia) was an investigator-initiated, double-blind, phase 2 randomized clinical trial. Adult participants were registered between May 25, 2015, and March 31, 2018, and received placebo or cilostazol for up to 96 weeks. Participants were treated in the National Cerebral and Cardiovascular Center and 14 other regional core hospitals in Japan. Patients with mild cognitive impairment with Mini-Mental State Examination (MMSE) scores of 22 to 28 points (on a scale of 0 to 30, with lower scores indicating greater cognitive impairment) and Clinical Dementia Rating scores of 0.5 points (on a scale of 0, 0.5, 1, 2, and 3, with higher scores indicating more severe dementia) were enrolled. The data were analyzed from May 1, 2020, to December 1, 2020. Interventions The participants were treated with placebo, 1 tablet twice daily, or cilostazol, 50 mg twice daily, for up to 96 weeks. Main Outcomes and Measures The primary end point was the change in the total MMSE score from baseline to the final observation. Safety analyses included all adverse events. Results The full analysis set included 159 patients (66 [41.5%] male; mean [SD] age, 75.6 [5.2] years) who received placebo or cilostazol at least once. There was no statistically significant difference between the placebo and cilostazol groups for the primary outcome. The least-squares mean (SE) changes in the MMSE scores among patients receiving placebo were –0.1 (0.3) at the 24-week visit, –0.8 (0.3) at 48 weeks, –1.2 (0.4) at 72 weeks, and –1.3 (0.4) at 96 weeks. Among those receiving cilostazol, the least-squares mean (SE) changes in MMSE scores were –0.6 (0.3) at 24 weeks, –1.0 (0.3) at 48 weeks, –1.1 (0.4) at 72 weeks, and –1.8 (0.4) at 96 weeks. Two patients (2.5%) in the placebo group and 3 patients (3.8%) in the cilostazol group withdrew owing to adverse effects. There was 1 case of subdural hematoma in the cilostazol group, which may have been related to the cilostazol treatment; the patient was successfully treated surgically. Conclusions and Relevance In this randomized clinical trial, cilostazol was well tolerated, although it did not prevent cognitive decline. The efficacy of cilostazol should be tested in future trials. - INTRODUCTION: The Kobe project, which utilizes prospective data from the national health insurance system, focuses on early detection and preventive strategies through the Frail Kenshin health check-up program. Previous research has underscored the correlation between tooth loss and the decline in physical and cognitive functions. In this study, using Kobe project data, we examined the link between remaining teeth and long-term care needs in individuals aged 64-65 years, with primary and secondary objectives involving various health parameters and quality of life. METHODS: We analyzed baseline data from a prospective study conducted alongside the Frail Check program for generally healthy individuals aged 64-65 years to examine the relationship between the number of remaining teeth and various health indicators. This study focused on citizens aged 64-65 years to identify those at risk of needing long-term care by the age of 65 years. RESULTS: Data from 1,530 participants were obtained, excluding eight individuals for specific reasons. At the end of the follow-up period, 41 (2.7%) individuals required support and 15 (1.0%) needed long-term care alone. The data revealed a significant association between the number of remaining teeth and the need for long-term care or support, as demonstrated by the Cochran-Armitage trend test (p<0.001). Although trends were noted for nutrition and total Cognitive Functional Instrument Self scores, they did not reach statistical significance. Additionally, a decrease in the number of remaining teeth was significantly associated with worse European Quality of Life Five Dimensions (EQ-5D-5L) visual analog scale scores, mobility, and regular activities (p<0.001). CONCLUSION: Tooth loss indicates the potential long-term care needs of older adults. - AIM: To investigate the association between locomotive syndrome and anemia among community-dwelling older adults. METHODS: This cross-sectional study was conducted at specific health checkup centers in Japan between 2019 and 2020. We sent a questionnaire to older adults aged ≥65 years who participated in health checkups. A total of 2507 community-dwelling older adults were included in this study (mean age = 72.3 years, 51.4% women). Locomotive syndrome was measured using a 25-question Geriatric Locomotive Function Scale with a score range of 0-100, and was defined as ≥16 points. Anemia was defined using World Health Organization criteria, as a hemoglobin concentration of <13.0 g/dL in men and <12.0 g/dL in women. Logistic regression analyses were performed to investigate the association between locomotive syndrome and anemia, adjusted for age, sex, body mass index, depression symptoms, self-reported comorbidities (cancer, rheumatoid arthritis, knee osteoarthritis, fractures, and spinal disease), hypertension, and renal function. RESULTS: Of all the participants, 11.6% had locomotive syndrome and 12.8% had anemia. Even after adjustment, a relationship between locomotive syndrome and anemia was observed (adjusted odds ratio = 1.9, 95% confidence interval = 1.3-2.7). CONCLUSIONS: Community-dwelling older adults with anemia had a significantly higher prevalence of locomotive syndrome than those without. This finding suggests that older adults with anemia are at risk of locomotive syndrome, and appropriate measures should be taken for prevention. - INTRODUCTION: The human brain can flexibly modify behavioral rules to optimize task performance (speed and accuracy) by minimizing cognitive load. To show this flexibility, we propose an action-rule-based cognitive control (ARC) model. The ARC model was based on a stochastic framework consistent with an active inference of the free energy principle, combined with schematic brain network systems regulated by the dorsal anterior cingulate cortex (dACC), to develop several hypotheses for demonstrating the validity of the ARC model. METHODS: A step-motion Simon task was developed involving congruence or incongruence between important symbolic information (illustration of a foot labeled "L" or "R," where "L" requests left and "R" requests right foot movement) and irrelevant spatial information (whether the illustration is actually of a left or right foot). We made predictions for behavioral and brain responses to testify to the theoretical predictions. RESULTS: Task responses combined with event-related deep-brain activity (ER-DBA) measures demonstrated a key contribution of the dACC in this process and provided evidence for the main prediction that the dACC could reduce the Shannon surprise term in the free energy formula by internally reversing the irrelevant rapid anticipatory postural adaptation. We also found sequential effects with modulated dip depths of ER-DBA waveforms that support the prediction that repeated stimuli with the same congruency can promote remodeling of the internal model through the information gain term while counterbalancing the surprise term. DISCUSSION: Overall, our results were consistent with experimental predictions, which may support the validity of the ARC model. The sequential effect accompanied by dip modulation of ER-DBA waveforms suggests that cognitive cost is saved while maintaining cognitive performance in accordance with the framework of the ARC based on 1-bit congruency-dependent selective control.2023, Frontiers in human neuroscience, 17, 1239207 - 1239207, English, International magazine[Refereed]Scientific journal
- BACKGROUND: This study investigated how cognitive function-related simple questions can be used to identify older individuals who are at risk of needing long-term care. METHODS: This cohort study was conducted in Kobe city, Japan. In 2015, the municipal office distributed the Kihon Checklist by post, a 25-item questionnaire including three cognitive function-related questions (questions 18, 19, 20) to citizens aged ≥ 70 years. Need certification is routinely done by Kobe city as part of the national Long-Term Care Insurance Act. The answers to the 2015 questionnaire were merged with need certification data between the questionnaire delivery and the end of December 2019. RESULTS: Of the 77,877 citizens (age: 72.9 ± 2.7 years) who received the questionnaire, 50,154 responded (response rate: 64.4%). During the study period, the cumulative incidence of the need for long-term care was higher in those who did not respond than in those who did (12.5% vs 8.4%; P < 0.001). Among those who responded, the incidence of the need for long-term care was progressively greater as the number of negative answers to cognitive function-related questions increased (5.0%, 8.4%, 15.7% and 30.2% at 4 years' follow-up, for respondents with, respectively, 0, 1, 2 and 3 negative answers). Similarly, when the need certification for long-term care was confined to that accompanied by dementia, the incidence also rose as the number of negative responses to the cognitive function-related questions increased (3.4%, 6.5%, 13.7% and 27.9% for respondents with, respectively, 0, 1, 2 and 3 negative answers). Using multivariate Cox regression analysis, all three cognitive function-related questions were predictive of the need for long-term care, and question 18 (about memory loss) had the highest hazard ratio for predicting the need for long-term care accompanied by dementia. - The pathological hallmarks of Parkinson's disease (PD) are α-synuclein (αSYN)-positive inclusions referred to as Lewy bodies and Lewy neurites, collectively referred to as Lewy-related pathology (LRP). LRP is thought to propagate in an ascending manner throughout the brain as the disease progresses. LRP is visible with histologic methods and is thought to represent a later stage of the disease process, while αSYN oligomers, which are not visible with routine histologic methods, are considered earlier. There is increasing evidence to suggest that αSYN oligomers may be more toxic than visible LRP. Detecting αSYN oligomers requires special techniques, and their distribution and association with clinical features are important research objectives. In this report, we describe the distribution of αSYN oligomers in multiple cortical and subcortical regions of PD using a proximity ligation assay (PLA). We observe widespread distribution of αSYN oligomers with PLA and more restricted distribution of LRP with αSYN immunohistochemistry. The distribution of αSYN oligomers differed from LRP in that αSYN oligomer burden was significantly greater in the neocortex, while LRP was greater in vulnerable subcortical regions, including the brainstem. We also found that cognitive impairment was associated with αSYN oligomers in the hippocampus. - We have developed a new method for easy self-assessment of changes in memory recall impairment, which can be used during the very early stages of dementia. An 8-picture recall and a 16-word regression were assessed, respectively, and the index was calculated by adding up the ratio of correct responses to both tests. A total of 85 subjects including 12 MCI, 8 AD, and 65 older persons with normal cognitive function were evaluated, and the correlation with the WMS-R Logical Memory II score was examined. The results showed that there was a statistically significant correlation between the 8-picture recall (R = 0.872, p < 0.0001) and the index (R = 0.857, p < 0.0001), respectively, with the Logical Memory score. - A 62-year-old man showed abnormal behavior. Brain magnetic resonance imaging revealed multifocal lesions on T2-weighted images. Initial screening revealed that he was seropositive for antibodies against glutamate decarboxylase, which usually indicates treatment resistance to autoimmune encephalitis. Intensive immunosuppressive therapies, however, improved the neurological symptoms. In line with this, we also detected seropositivity for antibodies against leucine-rich glioma-inactivated 1 and gamma-aminobutyric acid A receptor (GABAAR). Brain imaging and treatment responsiveness suggested that antibodies against GABAAR were the main cause of symptoms. - OBJECTIVE: The purpose of this study is to investigate the effect of an intervention combining exercise and cognitive activity on cognitive function in healthy older adults. METHODS: This pilot randomized controlled trial recruited 33 eligible, healthy communitydwelling older adults (mean age, 77.1 years old; women, 51.5%), who were divided into intervention and waitlist control groups. The intervention group was engaged weekly in a group activity comprising exercise and discussions of homework, which included reading aloud, simple arithmetic, and simple activities, like spotting differences, for cognitive stimulation. They were also required to complete cognitive activity homework twice a week. The waitlist control group received no intervention. The main outcomes were cognitive function assessed using the Mini-Mental State Examination, delayed recall score on the Logical Memory IIA of the Wechsler Memory Scale Revised, Trail Making Test, and digit symbol substitution test. RESULTS: According to the results, Mini-Mental State Examination scores were maintained in the intervention group but declined in the control group [Mean change in outcomes in control group (95% confidence interval): -1.68 (-2.89 to -0.48)]. Additional mean change in outcomes in intervention group were found [1.68 (0.02 to 3.35)]. - OBJECTIVE: We conducted a 3-year prospective study on olfaction of patients with Parkinson's disease (PD) in order to examine the severity and frequency of smell disorder in PD using odor identification test, Open Essence (OE) and to verify the validity of olfactory tests as a predictor of cognitive symptom onset of PD. PATIENTS AND METHODS: We conducted a prospective study by performing an annual examination over a 3-year period. For 56 cases diagnosed with PD by the Department of Neurology at our hospital, OE and Jet Stream Olfactometry (JSO) were performed to assess the olfactory function, and Mini-Mental-State Examination (MMSE) was conducted to measure cognitive impairment. RESULTS: At the beginning, 56 cases were examined, of which 42 remained to be followed up for 3 years. Based on the results of baseline, we found a correlation between OE and the average cognitive thresholds of JSO, but did not find any correlation between OE and MMSE. OE (median 4.0→4.0) and the average cognitive thresholds of JSO (median 2.2→1.6) decreased after 3 years, and MMSE (median 29→29) also declined, but not significantly. At the 3rd year, 6 cases with MMSE score of 23 or less were identified as suspected dementia and 36 cases with more than 24 points were defined as an invariant group. In order to distinguish these two groups, OE scores of baselines were evaluated with a combination of 12 odors. Sensitivity 1.0 and specificity 0.722 were obtained and the sensitivity+specificity value (1.722) was the highest when the number of correct answers was 4 or less using an odor combination of lumber, menthol, Japanese orange, gas for household use, Hinoki cypress and condensed milk. CONCLUSION: When the number of correct answers of 6 odors (lumber, menthol, Japanese orange, gas for household use, Hinoki cypress and condensed milk) is 4 or less in patients with PD, there is a possibility that MMSE declines in 3 years.Sep. 2019, Auris, nasus, larynx, 47(5) (5), 899 - 904, English, International magazine[Refereed]Scientific journal

- OBJECTIVES: Parkinson disease (PD) is frequently associated with olfactory disorder at early stage, which is caused by deposition of Lewy bodies emerging from the olfactory bulb to higher olfactory centers. Early detection of olfactory disorder in the patients with PD may lead to the early diagnosis and treatment for this refractory disease. METHODS: Visual analog scale (VAS), Jet Stream Olfactometry, and Japanese smell identification test, Open Essence (OE), were carried out on 39 patients with PD. Thirty-one patients with postviral olfactory disorder (PVOD), which was caused by the olfactory mucosal dysfunction, were also enrolled in this study as control. RESULTS: There were no significant differences in detection thresholds (2.2 vs. 1.4, P=0.13), recognition thresholds (3.9 vs. 3.5, P=0.39) and OE (4.8 vs. 4.2, P=0.47) between PVOD and PD, while VAS scores of PVOD and PD were significantly different (2.0 and 6.2, P<0.01). In OE, significant differences were observed in the accuracy rates of menthol (68% vs. 44%, P=0.04) and Indian ink (42% vs. 15%, P=0.01) between PVOD and PD. Of particular interest, patients with PVOD tended to select "no detectable," while patients with PD tended to select wrong alternative other than "no smell detected." - Objective: Here we investigate the process of neurodegeneration in amyotrophic lateral sclerosis (ALS). The relationship between the cortical field spreading of glucose metabolic decreases in the primary motor cortex (PMC) and the progression of corresponding extremity dysfunction was evaluated using [18F] fluoro-deoxyglucose (FDG)-positron emission tomography (PET). Methods: Patients with ALS underwent [18F] FDG-PET and the resulting datasets were individually contrasted against healthy controls using the NEUROSTAT software. The extent ratio was defined as the proportion of pixels with a significant Z-score decrease within regions of the primary motor cortex (precentral gyrus or paracentral lobule) opposite to the impaired upper extremities (UEs) or lower extremities (LEs), respectively. Clinical symptoms in all extremities were assessed using an upper motor neuron (UMN) score and the MRC (Medical Research Council) sum score upon physical examination. Cross-sectional correlations were analysed between clinical symptoms, the duration of these symptoms, and the extent ratio. Results: Nineteen regions of the primary motor cortex corresponding to symptomatic limb in 10 participants were evaluated. In the corresponding region of the primary motor cortex, the extent ratio increased (same meaning as hypometabolic area spread) in association with symptom duration (rs = 0.5, p = 0.03). Neither UMN nor lower motor neuron (LMN) scores were correlated with symptom duration. Three out of 19 impaired regions did not show upper motor neuron (UMN) signs upon physical examination. The extent ratio and UMN score-controlled symptom duration were partially correlated (rs = 0.5, p = 0.05). Conclusions: In patients with ALS, glucose metabolism decreased in the impaired side of the primary motor cortex depending on the clinical symptom progression in the corresponding extremities, regardless of the presence of clinical UMN signs. - The patient was a 46-year-old woman having a history of multiple sclerosis (MS) for 14 years. She had been treated with interferon β-1b since 2001, but discontinued because of psychiatric problems in 2006. Thereafter relapses were observed 1-2 times a year, and EDSS became 2.5 to 6.5. In April 2012, relapse of MS was noticed and the patient received introduction of fingolimod (FTY) after methylprednisolone (mPSL) pulse therapy. Twenty days later, dysarthria and lower limb weakness were appeared. Brain MRI showed more than 20 several millimeter Gd enhanced lesions in periventricular white matter, juxta-cortical white matter, and cerebellum. - A 76-year-old female was referred to our department because of diplopia for two months and intermittent claudication for five months. She showed medial longitudinal fasciculus (MLF) syndrome. Brain MRI (T2WI) showed multiple infarctions in the right pontine tegmentum and left paramedian midbrain. A biopsy of superficial temporal artery showed the characteristic findings of glanulomatous inflammation indicative of giant cell arteritis. We thought the mechanism of this cerebral infarction as artery to artery embolization or intracranial arteritis. - 日本学術振興会, 科学研究費助成事業, 基盤研究(C), 神戸大学, 01 Apr. 2023 - 31 Mar. 2026認知症における新規血中バイオマーカーの探索 ~炎症誘導性マイクロRNAの意義~現在、認知症の診断、特にアルツハイマー型認知症を診断するにあたり、保険適用となる唯一の検体は髄液材料である。髄液採取は腰椎穿刺によって実施されるが、侵襲性が高く、被検者の負担が非常に大きいことから、経過観察のために受診毎に実施する検査材料ではない。今回、炎症と関わりのあるmiRNAを対象とし、認知症を診断できる、および認知症の進行や治療効果を評価できるバイオマーカーの探索研究を開始した。 [R5年度の研究実績]・脳脊髄液中と血漿中のmiRNA発現量の比較:まず、炎症に関連する3種類のmiRNA(Candidate (Cand) miRNA-A, -B, -C)に焦点を当て、軽度認知障害(以下MCI)患者を対象として脳脊髄液と血漿中のmiRNA発現量を定量した。その結果、miRNAの種類によって、脳脊髄液と血漿における発現量(分布)が異なることが明らかとなった。具体的には、Cand miRNA-Aは脳脊髄液において、Cand miRNA-B, -Cは血漿中において有意に発現していた。 ・脳脊髄液中ADマーカーと脳脊髄液/血漿中miRNA発現の相関:次に、脳脊髄液中のADマーカー(アミロイドβ42/40比、リン酸化タウ、総タウ)と脳脊髄液/血漿中miRNA発現の相関を調べた。脳脊髄液中ADマーカーとの相関係数がもっとも高かったのは血漿中Cand miRNA-Cであり、脳脊髄液中アミロイドβ42/40比と正の相関、脳脊髄液中リン酸化タウ、総タウと負の相関を示した。 ・血漿中miRNAにより認知機能低下を予測できるかの評価:さらに、MCI患者の初診時と2回目の受診結果から、認知機能の低下を示した群と認知機能が維持されていた群に分け、初診時の血漿中miRNA発現量を解析した。その結果、認知機能が低下した群は初診時の血漿中Cand miRNA-Aが低下していた。
- 日本学術振興会, 科学研究費助成事業 挑戦的研究(開拓), 挑戦的研究(開拓), 神戸大学, 30 Jun. 2022 - 31 Mar. 2026視線の文法化による脳機能・脳内病理の推定手法本研究の目的は、視線の文法化による脳機能・脳内病理推定という新たな分野への挑戦・開拓である。本研究では、視線の時系列的変化を文法や単語の意味等のようなものに捉え(視線の文法化)、それを基に脳機能・脳内病理を推定する新たな学術体系の構築に挑戦する。 昨年度は、250Hzの視線計測装置を用いて、神戸大学医学部付属病院等で患者24名を対象として、5つの課題(プロサッカード、アンチサッカード、滑動性眼球運動、視空間記憶、絵の説明)を実施する際の視線とMMSEスコアの計測を行った。 本年度は、上記データを用いて、gazeNetという視線を固視、サッカード、滑動性眼球運動などに分類するために学習したニューラルネットワークを転移学習することで、他の推定に利用できないかを検討した。はじめに、視線と感情を計測した60人分のデータを用いて、上記ネットワークを転移学習することによる感情の推定を試みた。瞳孔径などを用いずに偶然より高い推定精度を示すことができた。この知見を用いて、より人数の少ない認知症患者の視線を計測したデータについて、gazeNetを転移学習することで、認知症かどうかを推定する手法について検討を行った。プロサッカード、アンチサッカード課題を実施した際のデータを用いて、検討を行った結果、アンチサッカード課題を実施した場合、視線からMMSEスコアを推定できる可能性が示唆された。これについては6月にACM Symposium on Eye tracking research & Applicationsで発表予定である。 さらに詳細なデータを取得できるように実験システムの改善を行った。1000Hzで計測できる視線計測装置(Eyelink)を導入しマイクロサッカードを計測できることを確認中である。脳波計も導入し何人かの被験者で予備実験を行うなど、実験環境を整備しているところである。
- 日本学術振興会, 科学研究費助成事業, 基盤研究(C), 神戸大学, 01 Apr. 2022 - 31 Mar. 2025地域在住高齢がんサバイバーコホートの構築:健康寿命延伸に向けて高齢化率30%に近づいている本邦において、悪性腫瘍のインパクトは、65歳以上の高齢者の死因の第1位であること、新規罹患の7割は65歳以上の高齢者であることである。我々は、高齢期のがん罹患が、がんを罹患していない群と比較して、要支援・要介護の発生が高くなることを明らかにしてきた。高齢がんサバイバーは、治療終了後健康寿命短縮の可能性が考えられる。しかし、国内外において高齢がんサバイバーの健康寿命短縮とそのプロセスについては十分に検討されておらず、高齢がんサバイバーは対策されないまま地域で日常生活を営んでいる。 本研究の目的は、地域在住高齢がんサバイバーのコホートを構築することにより、高齢がんサバイバーの生活習慣の現状を明らかにするとともに、フレイル・転倒の発生、骨折発生・認知症発生、要支援・要介護発生、死亡をがんに罹患していない高齢者と比較することで、外来でのがんサバイバーのリハビリテーション実施に向けたエビデンスを提供することである。 2023年度は、特定健診情報と医療介護レセプトデータをリンケージし、保有データセットから高齢がんサバイバーを同定した。また、高齢がんサバイバーにおいて、運動習慣を有している場合(がん罹患後に特定検診にて運動習慣ありと回答したもの;1回30分以上、週2回以上、1年以上継続している)、有していない場合と比較して、死亡率が低下(ハザード比;0.24)し、要支援・要介護の発生が低下する(ハザード比;0.23)ことを明らかにできた。

In particular, it was revealed that learning characteristics such as individual and group risk attitudes and exploitation/exploration ratio have a significant effect on creativity. Hence, it would facilitate creativity to create an environment that promotes these characteristics. In particular, we argued that the development of a virtual environment that allows low-cost and risk-free experiments is instrumental in improving creativity at the individual and organizational levels.

Our multi-omics analyses revealed gene expression, DNA methylation, and protein phosphorylation changes after aerobic and resistance exercises, providing the theoretical basis for exercise. A pharmacogenetic approach using the Connectivity Map and the results of multi-omics analyses identified some compounds that mimic all molecular events after exercises. Administration of these compounds enhanced exercise performance and improved the disuse syndrome and osteoarthritis, suggesting that they are promising exercise mimetics.
- 日本学術振興会, 学術研究助成基金助成金/基盤研究(C), 基盤研究(C), 神戸大学, Apr. 2013 - Mar. 2016認知症患者に対する嗅覚障害の評価法の確立を目的に神戸大学医学部附属病院神経内科と連携して、認知症専門外来の責任者(分担研究者)により軽度認知障害と判断された症例を対象として研究を行った。登録された対象症例に対して、耳鼻咽喉科外来の嗅覚専門外来において嗅覚の評価を行った。評価方法は、実臨床で用いる基準嗅覚検査に加えて、VASスケール評価・においアンケートとカード型嗅覚同定検査(オープンエッセンス)を行った。登録以降は3か月後に神経内科と耳鼻咽喉科に受診を指示し、認知症の原因疾患の診断結果と嗅覚検査の結果の集計を行った。初期に登録された10例について、軽度認知障害例の嗅覚評価方法として、新規の嗅覚同定検査であるオープンエッセンスが妥当か否かを検討した。その結果、オープンエッセンスが従来からの嗅覚機能検査(基準嗅覚検査)と相関性を認める結果が得られた。(スペアマン相関係数 有意差5%)さらにオープンエッセンスはVASスケールやにおいアンケート(嗅覚検査検討委員会作成)に対しても相関関係を認められた。この結果によりオープンエッセンス検査が認知症の嗅覚障害に対する評価方法として妥当であることが示唆された。また、認知症においてオープンエッセンスの12種類の嗅素別の正解率を解析を行った。その結果、特異度と感度の両面から数種類の嗅素を組み合わせにより、スクリーニング検査として利用ができる可能性が確認された。これらの結果は、関連学会(第52回日本鼻科学会)において発表を行った。Competitive research funding

We took 10 stool samples and extracted DNA from patients and compared with 10 control samples. DGGE analysis revealed three distinctive bands in the patient group, which was sequenced and identified. Further analysis of large number samples are needed to confirm the result.

It is believed that most are caused by an immune response against onconeural antigens. It is important to detect these antibodies against neurons and/or glial cells for the diagnosis of PNDs. To establish high-sensitive detection system for paraneolplastic anti-neuronal antibodies, patients clinically diagnosed as PNDs were examined using dot-blot kit, immunohistochemistry (IHC) on frozen mouse brain sections, Western blot analysis of wild-type mouse brain homogenate and immunocytochemistry (ICC) on primary culture of mouse fetus brains. The combination of IHC and Western blot analysis is the high-sensitive tool to detect anti-neuronal antibodies. ICC is supposed to be useful for detecting how antibodies work to cause PNDs.Competitive research funding
