- OBJECTIVES: The effects of systemic inflammation on the temporomandibular joint (TMJ) are poorly understood. This study aimed to establish a mouse model to study the effects of systemic inflammation on the TMJ. MATERIALS AND METHODS: SKG mice, a BALB/c strain with spontaneous onset of rheumatoid arthritis-like symptoms due to a spontaneous point mutation (W163C) in the gene encoding the SH2 domain of ZAP-70, were treated with zymosan (β-1,3-glucan). Synovitis, bone erosion, and cartilage damage in the TMJ were evaluated using established scores for animal models of inflammatory arthritis. Myeloperoxidase-positive areas and numbers of tartrate-resistant acid phosphatase (TRAP)-positive cells were compared between naive and zymosan-treated SKG mice. Correlations between TMJ inflammation scores and clinical scores for extremities were also assessed. RESULTS: There were significant differences in TMJ inflammation scores, including synovitis, bone erosion, and cartilage damage, between naive and high-dose zymosan-treated mice. There were significant differences in myeloperoxidase-positive areas and numbers of TRAP-positive cells between naive and zymosan-treated mice. There were significant correlations between TMJ inflammation scores and clinical scores for extremities. CONCLUSIONS: Systemic administration of zymosan efficiently induces TMJ inflammation in SKG mice. Zymosan-treated SKG mice offer a useful tool to investigate the effects of systemic inflammation on the TMJ.Dec. 2024, Oral diseases, English, International magazineScientific journal

- OBJECTIVE: Although benign, ameloblastoma is a locally aggressive lesion in some patients and the development of additional treatments is needed. Verteporfin (VP) is a photosensitizer exhibiting considerable photocytotoxicity in various tumor cells. We aimed to investigate the effects of verteporfin photodynamic therapy (VP PDT) on ameloblastoma. METHODS: Eighteen patients who underwent surgery for ameloblastoma were randomly selected. We performed an immunohistochemical assessment to investigate the expression of low-density lipoprotein receptor (LDLR) and Yes-associated protein (YAP), targets of VP, in human ameloblastoma tissues and cultured human ameloblastoma cell line (HAM1). The effect of VP PDT on cell proliferation and apoptosis in HAM1 was analyzed. RESULTS: The expression of LDLR and YAP were detected in human ameloblastoma tissues and HAM1. LDLR expression was significantly higher in patients who had previously undergone surgery than in patients who were receiving it for the first time. The cytotoxic effect of the combination of low-concentration VP administration and laser irradiation was comparable to high-concentration VP administration with and without laser irradiation. The addition of laser irradiation to VP administration significantly accelerated apoptotic bleb formation compared with VP administration alone. CONCLUSION: VP PDT has the potential to become an additional treatment for large-sized ameloblastoma.Oct. 2023, Oral diseases, English, International magazineScientific journal
- OBJECTIVE: This study aimed to reveal characteristic condylar movements in patients with jaw deformities. STUDY DESIGN: Thirty patients with jaw deformities before surgery were enrolled and instructed to chew a cookie during 4-dimensional computed tomography (4DCT). The distance between the most anterior and posterior positions of the bilateral condyles on 4DCT images was measured and compared among patients with different skeletal classes. Correlations between the condylar protrusion and cephalometric values were also determined. RESULTS: The distances of condylar protrusion during mastication were significantly greater in the skeletal class II group than in the skeletal class III group (P=0.0002). Significant correlations were found between the distances of condylar protrusion during mastication and the sella-nasion-B point angle (r=-0.442, P=0.015), A point-nasion-B point angle (r=0.516, P=0.004), sella-nasion plane to ramus plane angle (r=0.464, P=0.01), sella-nasion plane to occlusal plane angle (r=0.367, P=0.047), and condylion-gonion length (r=-0.366, P=0.048). CONCLUSION: Motion analysis with 4DCT images revealed that condylar movement in patients with retrognathism was larger than in patients with mandibular prognathism. - Although perineurium has an important role in maintenance of the blood-nerve barrier, understanding of perineurial cell-cell junctions is insufficient. The aim of this study was to analyze the expression of junctional cadherin 5 associated (JCAD) and epidermal growth factor receptor (EGFR) in the perineurium of the human inferior alveolar nerve (IAN) and investigate their roles in perineurial cell-cell junctions using cultured human perineurial cells (HPNCs). In human IAN, JCAD was strongly expressed in endoneurial microvessels. JCAD and EGFR were expressed at various intensities in the perineurium. In HPNCs, JCAD was clearly expressed at cell-cell junctions. EGFR inhibitor AG1478 treatment changed cell morphology and the ratio of JCAD-positive cell-cell contacts of HPNCs. - Oral lichen planus (OLP) is a type of chronic and refractory stomatitis characterized by abnormal keratinization, which is often painful. There is no consensus regarding treatment options for OLP, particularly in the presence of pain. The current study protocol focuses on the short-term efficacy and long-term safety of an ibuprofen gargle for pain management in patients with OLP. Patients (n = 24) with painful OLP will be enrolled. During a crossover study period, patients in the ibuprofen–placebo (IP) group will receive an ibuprofen gargle (0.6%) on day 1, a placebo gargle on day 2, and an ibuprofen gargle on days 3–5 at least once daily. Patients in the placebo–ibuprofen (PI) group will receive a placebo gargle on day 1, an ibuprofen gargle on day 2, and an ibuprofen gargle on days 3–5 at least once daily. The primary endpoint of the crossover study period is the change in pain level as measured by a visual analogue scale score from before gargle administration to 5 min after gargle administration on days 1 and 2. The primary endpoint of the long-term extension study is assessment of long-term safety. - Purpose Recurrence in oral squamous cell carcinoma (OSCC) is not rare. Due to lack of studies assessing characteristics of recurrent OSCC, including time to recurrence and outcomes, we sought to investigate its characteristics, time to recurrence, and outcomes in Japanese OSCC patients. Methods This study was a nonrandomized retrospective cohort study in a tertiary referral centre. It included 208 (117 men and 91 women) patients with recurrent oral cancer who underwent major curative surgery in the Department of Oral and Maxillofacial Surgery at Kobe University Hospital between January 1999 and April 2017. The outcomes were disease-specific survival (DSS) and overall survival (OS). Results In multivariable Cox proportional hazards analysis, the time to recurrence (hazard ratio [HR] 3.55, 95% confidence interval [CI] 1.69-6.63; P = 0.001), extranodal extension (ENE, HR 2.72, 95% CI 1.51-4.89; P = 0.001), and high T stage (HR 2.00, 95% CI 1.01-3.97; P = 0.046) were independent predictors of DSS. The time to recurrence (HR 3.29, 95% CI 1.82-5.96; P < 0.001) and ENE (HR 2.64, 95% CI 1.52-4.56; P = 0.001) were independent predictors of OS. - BACKGROUND: Prognostic biomarkers provide essential information about a patient's overall outcome. However, existing biomarkers are limited in terms of either sample collection, such as requiring tissue specimens, or the process, such as prolonged time for analysis. In view of the need for convenient and non-invasive prognostic biomarkers for oral cancer, we aimed to investigate the prognostic values of neutrophil-to-lymphocyte ratio, lymphocyte-to-monocyte ratio, and platelet-to-lymphocyte ratio in patient survival. We also aimed to explore the associations of these ratios with the clinicopathologic characteristics of Japanese oral squamous cell carcinoma patients. METHODS: This study was a non-randomized retrospective cohort study in a tertiary referral center. We included 433 patients (246 men, 187 women) who underwent radical surgery for oral cancers between January 2001 and December 2013. We evaluated various risk factors for poor prognosis including neutrophil-to-lymphocyte ratio, lymphocyte-to-monocyte ratio, and platelet-to-lymphocyte ratio with univariate and multivariate analyses. The disease-specific survival and overall survival rates of patients were compared among the factors and biomarkers. RESULTS: In multivariable Cox proportional hazards analysis, high neutrophil-to-lymphocyte ratio (hazard ratio 2.87, 95% confidence interval 1.59-5.19, P < 0.001), moderately or poorly differentiated histology (hazard ratio 2.37, 95% confidence interval 1.32-4.25, P < 0.001), and extranodal extension (hazard ratio 1.95, 95% confidence interval 1.13-3.35, P = 0.016) were independent predictors of disease-specific survival. High neutrophil-to-lymphocyte ratio (hazard ratio 2.30, 95% confidence interval 1.42-3.72, P < 0.001), moderately or poorly differentiated (hazard ratio 1.72, 95% confidence interval 1.07-2.76, P = 0.025), and extranodal extension (hazard ratio 1.79, 95% confidence interval 1.13-2.84, P = 0.013) were independent predictors of overall survival. - PURPOSE: This study retrospectively investigated relationships among risk factors and post-hepatectomy surgical site infection (SSI) and other complications in patients who underwent hepatectomy for hepatocellular carcinoma (HCC). METHODS: We included 334 patients who underwent hepatectomies for liver cancers between January 2011 and December 2015 in this study. We evaluated risk factors for SSI and other post-hepatectomy complication, including demographic factors, preoperative factors, and preoperative intervention including oral management, perioperative factors, and length of hospital stay, with univariate and multivariate analyses. The oral management intervention included self-care instructions, extraction of infected teeth, removal of dental plaques and calculus (scaling), professional mechanical teeth cleaning, removal of tongue coating, and cleaning of dentures. SSI was defined in accordance with the guideline issued by the Centers for Disease Control and Prevention; it included purulent discharge from any incision or organ space within 30 days postoperatively, with or without microbiological evidence. Complications of grade II or greater, according to the Clavien-Dindo classification, were regarded as postoperative complications. RESULTS: We found bacterial infection of ascites (Odds ratio (OR) = 13.72), lack of preoperative oral management intervention (OR = 10.17), and severe liver fibrosis (OR = 2.76) to be associated with SSI and severe liver fibrosis (OR = 2.28), hypoalbuminemia (OR = 2.02), blood transfusion (OR = 1.86), and longer operation time (OR = 1.80) to be associated with postoperative complications. - Exploring a Novel Screening Method for Patients with Oral Squamous Cell Carcinoma: A plasma Metabolomics Analysis.AIM: This study aimed to explore novel metabolite biomarker candidates for screening oral squamous cell carcinoma (OSCC). PATIENTS & METHODS: We collected plasma samples from 48 patients with OSCC and 29 with an oral disease and conducted a plasma metabolomics analysis of patients with OSCC using gas chromatography mass spectrometry. Then, we used the cross-validation procedure to ensure the accuracy of biomarker candidates. RESULTS: We selected four biomarker candidates against OSCC. Their sensitivity was more than 90%, and the AUC was over 0.9 according to the receiver operating characteristic curve analysis. - The aim of this study was to clarify the masticatory function and oral health-related quality of life (OHRQoL) for different ranges of mandibulectomy and also continuity resection of the mandible. A total of thirty one patients who underwent mandibulectomy and were classified into AT, body, and marginal resection (MR) groups according to the CAT classification were investigated. The masticatory function was evaluated using a color-changing gum, a gummy jelly, and a modified Sato's questionnaire. The OHRQoL was evaluated using the Oral Health Impact Profile (OHIP), which included seven subscales. There were significant differences between the AT group and body group in the findings from the color-changing gum and the OHIP (P < 0.05). There was also a significant difference between the AT group and body group regarding the degree of physical disability and psychological disability (subscales of the OHIP) (P < 0.05). There were no significant differences between the body and MR group in any of the tests. Consequently, the body group showed a higher masticatory function and OHRQoL than the AT group despite having the same category of mandibulectomy defect however, the masticatory function and OHRQoL in the body and MR groups were found to be closely similar, at least, regarding the chewing performance.Elsevier Ltd, May 2018, Journal of Oral and Maxillofacial Surgery, Medicine, and Pathology, 30(3) (3), 220 - 224, English[Refereed]Scientific journal

- Sagittal split ramus osteotomy and intraoral vertical ramus osteotomy are commonly performed for the correction of jaw deformities. However, during mandibular orthognathic surgeries such as sagittal split ramus osteotomy and intraoral vertical ramus osteotomy , the authors sometimes encounter exposure of the buccal fat pad (BFP), which decreases the surgical field. The exposed BFP makes it difficult to perform these operations, may result in unexpected complications, and may increase the operation time. - The aim of this study was to evaluate the effects of early removal of fixed plates in patients with occlusal discrepancies after sagittal split ramus osteotomy (SSRO) with a focus on the positional relationship of the temporomandibular joint (TMJ). Sagittal split ramus osteotomy with/without Le Fort I osteotomy was performed on 98 patients with mandibular prognathism. Of these 98 patients, 15 with occlusal discrepancies and/or TMJ symptoms underwent early plate removal after SSRO. Finally, 12 consecutive patients were evaluated in this study: 7 underwent bilateral SSRO, 1 underwent unilateral SSRO, and 4 underwent bilateral SSRO with maxillary advancement. The axiolateral TMJ Schuller method was used to evaluate the TMJ position. The authors measured 3 spaces (anterior, superior, and posterior joint spaces) between the condyle and glenoid fossa in the sagittal plane. The anterior and superior joint spaces were significantly larger immediately after the operation than before the operation. After early plate removal, these spaces significantly decreased in size. The posterior joint space increased, but with no significant difference. Three months after SSRO, the size of each of the 3 spaces was closely related to its preoperative size. - Objective: The purpose of this study was to retrospectively evaluate the postoperative results of partial glossectomy for early tongue cancer using a carbon dioxide laser (CO2 laser). Background data: CO2 lasers are frequently used for the excision or treatment of soft tissue in a range of diseases, including oral cancer, leukoplakia, mucocele, anomalies of the labial and lingual frenum, and peri-implantitis. Materials and methods: We retrospectively reviewed 31 primary cases of early superficial tongue cancer that were treated using CO2 lasers. In this study, early superficial cancer of the tongue is defined as a T1 or T2 tumor (TNM classification, NOMO; type, superficial spread, or exophytic; depth, <5 mm). The lesions were stained with 10% Lugol's solution and excised with a 5-or 10-mm safety margin from the nonstained area or induration using a CO2 laser. The raw surface was covered with a polyglycolic acid sheet using fibrin glue spray (n = 23), sutures (n = 6), or both (n = 2). Five of the patients showed a bleeding tendency: 1 was taking warfarin 100 mg per day, 1 was taking 350 mg per day, 2 were taking aspirin 100 mg per day, and 1 was taking aspirin 200 mg per day. Results: There were no cases of postoperative bleeding. Regarding postoperative pain, all patients could stop taking analgesic drugs by 1 month after undergoing the operation. In regards to postoperative difficulty to swallow, all could start swallowing rice gruel 2 days after the operation. The surgical margin was unclear in two cases due to the thermal denaturation of the excisional margin. The 2-year local control rate was 100% and subsequent cervical lymph node metastasis rate was 6.5%. - Objective In this study, we investigated the degree of concordance in the histopathological diagnosis among lesions clinically diagnosed with oral lichen planus (OLP) to understand the importance of the histopathological examination. Methods In total, 169 patients clinically diagnosed with OLP on their initial visit to our hospital between 2001 and 2012 were experienced. Of them, histopathological examinations were carried out for 77 patients (83 lesions), and they were selected as the subjects of this study. The age, gender, location of the lesion, clinical type of OLP determined via visual inspection, and histopathological findings were investigated. Results Of the 77 patients, 12 were male and 65 were female, with a mean age of 63.9 years. Histopathological examinations were performed in 83 lesions, of which 54 were diagnosed as OLP. Among the diagnostically discordant 29 lesions, most were histopathologically diagnosed as leukoplakia (15 lesions, 51.7%), whereas one was diagnosed as squamous cell carcinoma (3.4%). The most frequent location of discordant lesions was the tongue (discordant rate: 77.8%), and the most clinical type was plaque (discordant rate: 90.0%) all of which were atypical types of OLP. Conclusions The rate of discordance between the clinical and histopathological diagnoses was 34.9%. Lasers are widely used in the oral and maxillofacial region surgeries. The CO2 laser has been used in the treatment of diseases that occur in soft tissue because of its excellent incision and hemostatic abilities, and good results have been obtained. The Nd:YAG and KTP lasers have an affinity for pigmented tissues and penetrate soft tissues deeply. Using these characteristics, vascular malformations (hemangiomas) can be treated by photocoagulation with these lasers. In this paper, current status of oral and maxillofacial laser surgery is reported with some case presentations.

- Objective: Previous studies have described the clinical statistics and treatment outcomes of leukoplakia of the oral cavity. However, because leukoplakia is often clinically diagnosed without any histopathological examination, current opinion regarding treatment methods and preoperative pathological diagnosis is inconsistent and no standard site-specific treatment guidelines exist for oral leukoplakia. The purpose of this study is to clinically evaluate carbon dioxide (CO< inf> 2< /inf> ) laser treatment for leukoplakia of the tongue. Methods: We examined 38 lesions in 35 patients of lingual leukoplakia who underwent CO< inf> 2< /inf> laser treatment with using Lugol staining at our department over a 10-year period, i.e., from 2001 to 2010. We investigated gender-related differences, age distribution, left and right differences, clinical classification, histopathological diagnosis (presence or absence of epithelial dysplasia), treatment technique (excision or evaporation), recurrence rate, residual rate, and malignant transformation among these cases. Results: Of the 38 lesions that fulfilled the inclusion requirements, 35 were excised and 3 were evaporated. After CO< inf> 2< /inf> laser treatment, the recurrence and residual rates for lingual leukoplakia were 10.5% and 7.9%, respectively. The recurrence rate of cases examined in this study was lower than that reported by our hospital previously (2004) and many other hospitals. None of the lesions exhibited malignant transformation. - Clinical study of 19 cases resulting in dental implant removalDental implant treatment is a highly anticipated therapy with widespread use. However, various accidental symptoms and complications have been reported, and we may reluctantly have to remove an implant body out of necessity. This study evaluated 19 cases (21 jaws) that resulted in removal after the enforcement of implant treatment in our hospital from January 2003 to August 2013 and clarified the details of the removals clinically for the purpose of improvement of future treatment results. Implants were removed at a higher frequency in elderly patients, particularly those 70 years of age or older. The removal rate regarding the tooth missing style was high in edentulous jaw cases in both the maxilla and mandible, and was low in defective cases of middle teeth. Regarding the duration and cause of removal, approximately half of the implants were removed due to defective primary stability within 6 months for periods greater than 6 months, all cases except 1 jaw were removed within 30 months and many cases were due to peri-implantitis. - 電気メス、レーザー等を用いた手術に関連するインシデントについて日本医療機能評価機構による医療事故情報収集等事業第1〜37回報告書(平成17年4月〜平成26年3月)から狩猟し得た「電気メス・レーザー等を使用した手術に関連するインシデント事例」86件を対象に、使用された医療機器の内訳、患者への影響、原因について調査した。結果、使用された医療機器は、「電気メス」61件(71%)、「サージビームや消化管内視鏡などの光源」11件(13%)、「ラジオ波焼灼治療機器」7件(8%)、「レーザー」5件(6%)、「超音波凝固切開装置」2件(2%)であった。患者への影響は、「熱傷」46件(53%)、「異物残存」8件(9%)、「近接組織(臓器)の損傷」6件(7%)、「穿孔」3件(3%)、「その他」5件(6%)、「影響なし」18件(21%)であった。原因は、「使用方法の誤り」33件(38%)、「機器の不具合・破損」20件(23%)、「突然の発火」17件(20%)、「対極板の使用に関連したもの」9件(10%)、「その他」7件(8%)であった。患者への影響・原因のうち比較的多くみられた以下のケースについて改善策を中心に考察した。1)発火(薬剤等の併用による発火、ガーゼや綿球への引火、挿管チューブへの引火)。2)機器の置き場に関連する熱傷。3)機器の不具合・破損(機器の劣化が原因のもの)。(一社)日本レーザー歯学会, Dec. 2014, 日本レーザー歯学会誌, 25(3) (3), 136 - 139, Japanese[Refereed]Scientific journal

- Cystadenocarcinoma is an uncommon salivary gland tumor that occurs in the palate. We report a rare case of cystadenocarcinoma arising in the palate. A 80-year-old man visited a certain hospital because of a painless swelling in the palate. A biopsy was performed, and adenocarcinoma was diagnosed histopathologically. He was therefore referred to our hospital to receive treatment for adenocarcinoma. He had a painless swelling, measuring 25 × 15 mm, in the left side of the palate. Computed tomography showed no evidence of cervical metastasis. Partial maxillectomy was performed, and the final histopathological diagnosis was cystadenocarcinoma. - The long-term effects of red light-emitting diode irradiation on the proliferation and differentiation of osteoblast-like MC3T3-E1 cellsLow level laser therapy (LLLT) affects various biological processes, and it is said that the non-coherent light of the light-emitting diode (LED) has a similar action. The purpose of this study was to examine the effects of LED light on the proliferation and differentiation of osteoblasts-like MC3T3-E1 cells cultured in osteogenic differentiation medium (ODM) over the long term. Cells were irradiated with red LED light of 630 nm at three doses 0.5J/cm2, 1.5J/cm2 or 3.0J/cm2 for the cell proliferation activity assay, and at 0.5J/cm2 for the osteogenic differentiation activity assay. The former activity was checked by counting the number of viable cells using Trypan blue dye. The latter activity was evaluated by alkaline phosphatase (ALP) staining and examining the mRNA expression of the osteopontin (OPN) gene using real-time quantitative PCR. The number of viable MC3T3-E1 cells showed a tendency to increase after the irradiation at all three energy densities in comparison with a non-irradiation group (control group). In particular, there was a remarkable 3.34-fold increase in the group irradiated with 3.0J/cm2 on day 7 after starting the culture. On culture day 15, there was a tendency for the red LED irradiation group (0.5 J/cm2) to exhibit more staining for ALP than the control group, and the expression of OPN was significantly higher in the irradiation group on culture day 16. - Leiomyoma is a benign tumor of smooth muscle origin, and usually arises in the uterus, alimentary tract, and skin. Since the first report by Blanc in 1884, most reported leiomyomas have been angioleiomyomas. In the present report, we describe a rare type of leiomyoma arising in the tongue of a 25-year-old woman. The tumor was an 18 × 7 mm, well-demarcated, and slow growing mass located in the right margin of the tongue. The entire lesion was excised, and the pathological diagnosis was solid leiomyoma. - Abstract: Patients with rheumatoid arthritis often receive methotrexate (MTX). Recently, it has been shown that MTX can cause B cell lymphoma, non-Hodgkins lymphoma. B cell lymphoma caused by MTX is referred to as a methotrexate-associated lymphoproliferative disorder (MTX-LPD) and is recognized by the World Health Organization and included among immunodeficiency-associated LPD. - A case of Ewings sarcoma of the mandible in a young child is presented. A 12-year-old boy was referred to our hospital because of swelling of the cheek, which he noticed 2 months previously. Clinical examination revealed diffuse swelling of the cheek, and a fistula of the gingiva. CT examination revealed a sun-ray apiculated image. MR showed a contract-enhancing lesion in the left mandibular angle. FDG-PET and Ga-scintigraphy revealed no metastasis. Based on the results of histopathological analysis, Ewings sarcoma was diagnosed. The patient received combined chemotherapy, surgery, and radiotherapy. - We describe a plexiform perineurioma, which is an extremely rare variant of soft tissue perineurioma. A 43-year-old Japanese man presented with a painless, well-demarcated, elastic, soft tumor measuring 2.0 × 1.9 cm on the upper lip that had persisted for three years. Microscopically, a plexiform tumor composed of minimally atypical spindle cells with wavy nuclei was located in the lamina propria of the oral mucosa. Tumor cells were concentrically arranged around small vessels and aligned in parallel with delicate collagen fibers on a fibromyxoid background in the hypocellular area. Tumor cells were immunohistochemically positive for EMA, GLUT-1, claudin-1, and CD34 and negative for S-100 protein. The histopathological and immunohistochemical profiles of the excised tumor indicated a diagnosis of plexiform perineurioma. The patient has remained free of recurrence for sixteen months. Intraoral soft tissue perineurioma is rare and two of the four reported plexiform perineuriomas, including that described herein, notably involved the intraoral area. According to previous reports, plexiform perineuriomas largely develop in middle-aged women without a history of type 1 or type 2 neurofibromatosis. - 白板症や早期舌癌で舌部分切除術が行われた68例(男性35例、女性33例、平均61.7歳)を対象に、術後開放創にコラーゲン使用人工皮膚貼付の31例(コラーゲン群)と、フィブリン糊でポリグリコール酸(PGA)シート貼付の37例(PGA群)に分け、その効果について比較した。病理診断は異型上皮・上皮内癌28例、扁平上皮癌40例、平均切除面積はコラーゲン群:837.3mm2、PGA群:736.0mm2、術後鎮痛剤服用平均日数はコラーゲン群:7.2日、PGA群:5.9日であり、PGAシート付着最終確認日は平均15.4日であった。コラーゲン群とPGA群の瘢痕拘縮の程度は、「なし」がそれぞれ5例、13例、「軽度」が16例、23例、「中等度」が10例、1例、「高度」は両群とも0例であった。なお、術後の栄養摂取はコラーゲン群が1週間、PGA群が2日間経鼻チューブを使用した。舌部分切除後のフィブリン糊とPGAシート使用は早期摂食、疼痛緩和、瘢痕拘縮予防に有用と考えられた。日本口腔外科学会, Jul. 2011, Japanese journal of oral and maxillofacial surgery, 57(7) (7), 394 - 400, Japanese

- 日本学術振興会, 科学研究費助成事業, 基盤研究(C), 神戸大学, 01 Apr. 2021 - 31 Mar. 2024統合オミックス解析による口腔がん制御機構の解明と創薬標的の網羅的探索口腔がんの大部分は扁平上皮癌であり、標準的治療は手術とされている。また、補助療法として放射線治療、化学療法が併用されることが多いが、進行がんに対しては予後が悪く、制御が可能であった場合でも患者自身のQOLを著しく低下させる。そのため、根治治療を目的としながらも、可能な限り機能を温存することも口腔がんにおいては重要である。現在、口腔がんに対する化学療法は、シスプラチン(CDDP)や5-FU、ドセタキセルが使用されている。しかし、進行がんを制御するまでは至っておらず、副作用も多い。本研究では、統合オミックス解析を行い、口腔がんに特異的な代謝経路を探索する。また、それに関わる代謝酵素のうち、がんの増殖の制御に関わるものを導き出し、その阻害剤の効果を検討することで、将来的な口腔がんの新たな治療薬の開発をめざす。 今年度は、まずヒト由来口腔がん細胞株(OSC-19、HSC-3)のガスクロマトグラフィー質量分析を実施した。また、口腔がん患者の血漿メタボローム解析を実施し、得られたデータを統計ソフトを用いて多変量解析することで、口腔がん患者に特異的な代謝物の変動を探索している。次に、ヒト由来口腔がん細胞株と血漿メタボローム解析の結果と比較することで、特に著明な変化を示す代謝経路の探索を開始している。今後は、これらに加え、口腔がん生体材料のメタボローム解析を実施することで、さらに口腔がん特異的な代謝経路を絞っていく予定である。
- 日本学術振興会, 科学研究費助成事業, 基盤研究(C), 神戸大学, 01 Apr. 2019 - 31 Mar. 2023血管・リンパ管新生に着眼した歯性感染症-顎骨骨髄炎移行メカニズム解明と治療法探索骨粗鬆症などに対し用いられる骨吸収抑制薬や、口腔癌に対し施行される放射線照射等の医療行為が素因となる顎骨骨髄炎は、近年患者数の増加に伴い顎口腔外科領域において最も新たな治療法開発が期待される難治性疾患の一つである。本研究の目的は、近年感染の進展にも関与していることが明らかとなりつつある血管・リンパ管新生をターゲットとし、歯性感染症-顎骨骨髄炎移行メカニズムを解明することである。 研究の端緒として、まず血管新生のマーカーとなりうる分子の検討を行なった。近年動脈硬化や高血圧、腫瘍の増大における病的な状態における血管新生に関与している可能性が示唆されている血管内皮細胞間接着複合体構成分子に注目した。この分子についてはマウスやヒト培養細胞を用いた研究はこれまでに行われてきたが、ヒトの組織内での発現や局在部位を確認した報告は稀少であった。昨年度の研究結果より、この分子がヒトの組織内でも特に非炎症部位の血管に比べ炎症部位の微小血管等に発現が強く、病的な血管のマーカーとなり得る可能性が示唆され、この結果を英文査読付き学術誌に投稿し、採択された。次に、放射線性骨髄炎で顎骨内を感染がどのように侵入部から拡散し、特に顎関節方向へと拡散するのかを調査した。放射線性顎骨壊死では放射線性の影響と思われるダメージが皮質骨のすぐ内側で強く認められ、同部を介して感染が顎関節方向へ進展する可能性が考えられた。こうした下顎骨内をどのように感染が進展するかを手術検体を用いて詳細に調査した研究は希少であり、当該年度この研究成果を英文査読付き学術誌に投稿し、採択された。
