    Background: Lymphedema is an intractable disease requiring lifelong treatment. Daily self-care by the patient is essential; however, establishing care is often difficult. Our hospital offers a short-term educational hospitalization program that involves intensive, multidisciplinary intervention to develop self-care. This study aimed to evaluate the effectiveness of this program. Methods and Results: Among the patients diagnosed with primary or secondary lymphedema who underwent educational hospitalization, 31 cases (33 limbs) were included in this study. The upper extremity lymphedema (UEL) index and lower extremity lymphedema (LEL) index were used for objective evaluation, and the Lymphedema Quality of Life Questionnaire was used for subjective evaluation. The therapeutic effects of educational hospitalization were examined before admission, at discharge, and at 6 months postdischarge. The UEL/LEL index significantly decreased for both the upper and lower limbs after educational hospitalization. In addition, the UEL/LEL index change rate from prehospitalization to 6 months postdischarge was compared between the continued and discontinued exercise groups; the former index showed significantly more improvement than the latter. Comparing the quality of life (QOL) assessments pre- and posthospitalization, statistically significant improvements were observed in all categories, including function, appearance, symptoms, and psychological state. Conclusions: Short-term educational hospitalization led to a sustained reduction in the circumference of lymphedema-affected limbs and an improvement in the QOL. The multidisciplinary approach resulted in improved self-efficacy and established daily self-care.

    Nanobubbles are studied for their unique properties and possible applications in wound healing processes. This study investigates the effects of hydrogen (H₂), oxygen (O₂), and ozone (O₃) nanobubbles on fibroblast migration and proliferation using in vitro scratch wound healing assays. Fibroblast cells were treated with Dulbecco's Modified Eagle Medium (DMEM) combined with nanobubble solutions, and cell density was measured at 24 and 48 hours. While no significant difference was observed at 24 hours (p=0.52), ozone nanobubbles significantly reduced cell density at 48 hours (p=0.005), indicating cytotoxic effects. Hydrogen and oxygen nanobubble treatments did not show statistically significant differences from the control. These results highlight the cytotoxic effects of ozone nanobubbles on fibroblasts, which may impact their potential application in wound healing. While the study shows the cytotoxic effects of ozone nanobubbles, in vivo wound healing and antimicrobial impacts remain unexplored and warrant further study.

    INTRODUCTION: Arteriovenous malformations (AVMs) of facial bones are rare conditions. These lesions can cause aesthetic challenges, pain, impact on gums and teeth, facial asymmetry, and life-threatening bleeding. PRESENTATION OF THE CASE: This report described a case involving the resection of AVMs within the maxilla following preoperative embolization and sclerotherapy by using an ultrasonic bone aspirator (Sonopet®, Stryker, Kalamazoo, MI, USA) and a radiofrequency bipolar sealer (Aquamantys™, Medtronic, Minneapolis, MN, USA). The AVMs and deformed bone were resected en bloc through repeated cutting and hemostasis, preserving the maxillary crown and maintaining the maxillary structure by treating only the affected bone. DISCUSSION: Reducing inflow during surgery and controlling intraoperative bleeding requires thorough planning. Preoperative embolization and sclerotherapy can reduce inflow to the tumor, and appropriate surgical devices enable the resection of arteriovenous malformations within the maxilla. CONCLUSION: The method presented in this report is a viable treatment option because it allows for selective bone removal, thus leading to the preservation of crowns and maxillary contour.

    Orbital lymphatic malformations significantly impair visual function, with inflammation contributing to corneal dryness and diplopia due to exophthalmos. Disease progression may lead to blindness, and in severe cases, exenteration may be required. We treated two symptomatic cases of adult orbital slow-flow vascular malformations with oral sirolimus. Although the patients maintained good visual acuity before treatment, prominent exophthalmos posed a substantial risk to visual function. After medication, eyeball proptosis improved significantly in all cases, with magnetic resonance imaging volumetry analysis showing a reduction of 28.5% to 82.1%. This treatment demonstrated minimal side effects and may serve as an initial therapeutic option.

    Reconstruction of large tissue defects following soft tissue sarcoma (STS) resection in the lower extremity presents a significant challenge for plastic surgeons. The optimal method should be selected from a wide array of techniques, depending on the size and location of the defect. Both pedicled and free flaps have demonstrated favorable outcomes. However, due to the nature of STS, there is concern about recurrence and the potential complications for secondary reconstruction. A 78-year-old male with leiomyosarcoma in the proximal third of the leg underwent wide excision and reconstruction using a pedicled medial gastrocnemius muscle flap. Local recurrence occurred at six months postoperatively, necessitating a second extended resection. This was followed by reconstruction using a hemi-split tibialis anterior muscle and hemi-split soleus muscle flaps, combined with a split-thickness skin graft. The postoperative course was uneventful, with complete survival of both the muscle flap and skin graft and no functional limitations in the lower extremity. The hemi-split tibialis anterior and soleus muscle flaps are considered valuable methods for reconstruction, preserving lower extremity function.

    This report highlights two cases of surgical site infections (SSIs) caused by Mycobacterium fortuitum (Mf) following abdominal mesh implantation. The first case involved an 83-year-old male experiencing non-healing erythema and wounds post-operation, which persisted despite multiple treatments, until effective management was achieved with targeted antibiotics after Mf identification. The second case concerned a female patient with a gynecological postoperative hernia, where Mf was quickly detected following SSI onset three weeks after surgery. Prompt mesh removal and appropriate antibiotic treatment led to a rapid and full recovery. These cases emphasize the importance of early detection and intervention in managing Mf infections effectively, illustrating how the timing of diagnosis can significantly influence treatment outcomes.

    It is unclear whether normal human skin tissue or abnormal scarring are photoreceptive. Therefore, this study investigated photosensitivity in normal skin tissue and hypertrophic scars. The expression of opsins, which are photoreceptor proteins, in normal dermal fibroblasts (NDFs) and hypertrophic scar fibroblasts (HSFs) was examined. After exposure to blue light (BL), changes in the expression levels of αSMA and clock-related genes, specifically PER2 and BMAL1, were examined in both fibroblast types. Opsins were expressed in both fibroblast types, with OPN3 exhibiting the highest expression levels. After peripheral circadian rhythm disruption, BL induced rhythm formation in NDFs. In contrast, although HSFs showed changes in clock-related gene expression levels, no distinct rhythm formation was observed. The expression level of αSMA was significantly higher in HSFs and decreased to the same level as that in NDFs upon BL exposure. When OPN3 knocked-down HSFs were exposed to BL, the reduction in αSMA expression was inhibited. This study showed that BL exposure directly triggers peripheral circadian synchronization in NDFs but not in HSFs. OPN3-mediated BL exposure inhibited HSFs. Although the current results did not elucidate the relationship between peripheral circadian rhythms and hypertrophic scars, they show that BL can be applied for the prevention and treatment of hypertrophic scars and keloids.

    BACKGROUND:  Salivary fistula formation is a common and serious complication following head and neck reconstruction. Because it can cause delayed wound healing and infection and carotid artery rupture in severe cases, hence, early detection and treatment are crucial. This study was designed to identify early predictors of postoperative fistula formation. METHODS:  We conducted a retrospective analysis of patients who underwent head and neck reconstruction between 2015 and 2022. Body temperature, serum white blood cell (WBC) count, and serum C-reactive protein (CRP) levels were assessed until postoperative day (POD) 14 and compared between patients with and without fistula. RESULTS:  In this study, 200 patients were included. No significant differences in body temperature and WBC count were observed between the two groups during the entire study period. CRP levels after POD2 were higher in the fistula group than in the without fistula group. From the receiver operating characteristic curves comparing the two groups, the best cutoff level for CRP was 6.27 mg/dL from POD7 to POD8, with 77.1% sensitivity, 69.8% specificity, and 90.1% negative predictive value. CONCLUSION:  CRP is a valuable predictor of fistula formation following head and neck reconstruction. The course of CRP levels in patients with fistulas remains consistently elevated compared to patients without fistulas, and it is particularly useful for the exclusion diagnosis of fistula.

    INTRODUCTION: Sensory disturbance due to injury of the superficial branch of the radial nerve (SBRN) is a donor-site morbidity of the radial forearm (RF) flap. The relationship between the SBRN preservation method and the post-operative sensation at the flap donor-site was retrospectively investigated. METHODS: We included 39 patients who underwent head and neck reconstruction with a free RF flap at Hyogo Cancer Center between April 2014 and March 2018. The patients were classified into the following three groups according to the SBRN preservation method: group 1, zero preservation, excision of the entire SBRN; group 2, main trunk preservation, excision of all branches except the main trunk of the SBRN; and group 3, complete preservation, preservation of the entire SBRN. Objective sensations and subjective symptoms at the flap donor-site were analyzed. RESULTS: The mean objective sensory scores were 3.18, 2.97, and 1.78 in groups 1, 2, and 3, respectively. Differences between groups 1 and 3 and between groups 2 and 3 were significant (p = 0.0035 and p = 0.037, respectively). The mean subjective symptom scores were 2.40, 1.33, and 1.40 in groups 1, 2, and 3, respectively. Differences between groups 1 and 2, and between groups 1 and 3 were significant (p = 0.032 and p = 0.019, respectively). CONCLUSIONS: Zero preservation method had a higher risk of subjective symptoms and objective hypoesthesia development at the flap donor-site than the complete preservation method. Despite inevitable objective hypoesthesia, the main trunk preservation prevented the development of subjective symptoms. Complete preservation is optimal for RF flap harvest; however, in case of perforator crossing, main trunk preservation is another option.

    We report a case of a second free jejunal transfer to treat metastasis in the mesenteric lymph node of the first jejunal flap. A 73-year-old man underwent total pharyngolaryngectomy, bilateral neck dissection, and free jejunal transfer for recurrent hypopharyngeal cancer [left pyriform sinus, pT2N0, moderately differentiated squamous cell carcinoma (SCC)] after radiotherapy. Seven years post-surgery, he underwent transoral videolaryngoscopic surgery for oropharyngeal cancer (soft palate, pT1N0, well-differentiated SCC). Ten years after the first jejunal transfer, metastasis was found in the mesenteric lymph node surrounding the jejunal flap's vascular pedicle. Under general anesthesia, resection of the first jejunum including the affected lymph node, and second jejunal transfer were performed. Lymph node pathological examination revealed poorly differentiated SCC, compatible with pharyngeal cancer metastasis. After neck dissection and jejunal flap transfer, lymphatic collateral pathways toward the flap's mesenteric lymph node might form. Possibly, hypopharyngeal or oropharyngeal cancer metastasized via this pathway.

    This study aimed to elucidate the vasodilatory effects and cytotoxicity of various vasodilators used as antispasmodic agents during microsurgical anastomosis. Rat smooth muscle cells (RSMCs) and human coronary artery endothelial cells (HCAECs) were used to investigate the physiological concentrations and cytotoxicity of various vasodilators (lidocaine, papaverine, nitroglycerin, phentolamine, and orciprenaline). Using a wire myograph system, we determined the vasodilatory effects of each drug in rat abdominal aortic sections at the concentration resulting in maximal vasodilation as well as at the surrounding concentrations 10 min after administration. Maximal vasodilation effect 10 min after administration was achieved at the following concentrations: lidocaine, 35 mM; papaverine, 0.18 mM; nitroglycerin, 0.022 mM; phentolamine, 0.11 mM; olprinone, 0.004 mM. The IC50 for lidocaine, papaverine, and nitroglycerin was measured in rat abdominal aortic sections, as well as in RSMCs after 30 min and in HCAECs after 10 min. Phentolamine and olprinone showed no cytotoxicity towards RSMCs or HCAECs. The concentrations of the various drugs required to achieve vasodilation were lower than the reported clinical concentrations. Lidocaine, papaverine, and nitroglycerin showed cytotoxicity, even at lower concentrations than those reported clinically. Phentolamine and olprinone show antispasmodic effects without cytotoxicity, making them useful candidates for local administration as antispasmodics.

    Artificial intelligence (AI) in medical care can raise diagnosis accuracy and improve its uniformity. This study developed a diagnostic imaging system for chronic wounds that can be used in medically underpopulated areas. The image identification algorithm searches for patterns and makes decisions based on information obtained from pixels rather than images. Images of 50 patients with pressure sores treated at Kobe University Hospital were examined. The algorithm determined the presence of necrosis with a significant difference (p = 3.39 × 10-5). A threshold value was created with a luminance difference of 50 for the group with necrosis of 5% or more black pixels. In the no-necrosis group with less than 5% black pixels, the threshold value was created with a brightness difference of 100. The "shallow wounds" were distributed below 100, whereas the "deep wounds" were distributed above 100. When the algorithm was applied to 24 images of 23 new cases, there was 100% agreement between the specialist and the algorithm regarding the presence of necrotic tissue and wound depth evaluation. The algorithm identifies the necrotic tissue and wound depth without requiring a large amount of data, making it suitable for application to future AI diagnosis systems for chronic wounds.

    UNLABELLED: Abdominal incisional hernia is a complication of the rectus abdominis myocutaneous (RAMC) flap harvest. This study aimed to compare the incidence of abdominal incisional hernia and donor-site closure time between absorbable barbed continuous (ABC) and non-absorbable non-barbed interrupted (nAnBI) methods. METHODS: This study included 145 patients who underwent free RAMC flap reconstruction after head and neck cancer surgery at Kobe University Hospital between January 2012 and March 2020. The nAnBI method was selected between January 2012 and August 2016, and the ABC method was selected between September 2016 and March 2020. The incidence of abdominal incisional hernia and the average time required for donor-site closure were compared between the two groups. RESULTS: Of the 145 patients surveyed, 116 (57 and 59 in the nAnBI and ABC groups, respectively) were followed-up for at least 90 days after the surgery. The incidence rates of abdominal incisional hernia were 0% and 5.1% (n = 3) in the nAnBI and ABC groups, respectively, with no significant differences (p = 0.244). The average donor-site closure times were 127.6 and 111.3 minutes in the nAnBI and ABC groups, respectively, with no significant differences (p = 0.122). CONCLUSIONS: No significant differences in the incidence of abdominal incisional hernia and donor-site closure time were observed between the nAnBI and ABC groups. However, there was a tendency for increased hernia occurrence and shorter wound closure time in the ABC group. A randomized prospective multicenter study is warranted to validate our findings of the ABC method.

    INTRODUCTION: Post-sternotomy surgical site infection (SSI) is a serious complication of cardiovascular surgery. Here, we proposed a new clinical classification and reconstructive strategy for this condition. METHODS: A retrospective study based on medical records was performed on 100 consecutive cases requiring wound management by plastic surgeons for post-sternotomy SSI at Kobe University Hospital between January 2009 and December 2021. We classified 100 cases into four categories according to the anatomical invasiveness of the infection (type 1, superficial SSI; type 2, sternal osteomyelitis; type 3, mediastinitis; and type 4, aortic graft infection). The standard treatment plan comprised initial debridement, negative pressure wound therapy with continuous irrigation, and reconstructive surgery. Reconstructive methods and their outcomes (in-hospital mortality rate, follow-up period, and infection recurrence rate) were investigated for each SSI category. RESULTS: There were nine SSI cases in type 1, 28 in type 2, 25 in type 3, and 38 in type 4. The pectoralis major (PM) muscle advancement flap was mainly selected in types 1 and 2 (100 and 70.4%, respectively), while the omental flap or latissimus dorsi (LD) myocutaneous flaps were mainly selected in types 3 and 4 (77.3 and 81.8%, respectively) for reconstructive surgery. The in-hospital mortality rates for types 1, 2, 3, 4 were 44.4, 3.6, 12.0, and 15.8%, respectively. The mean follow-up periods for types 1, 2, 3, 4 were 542.8, 1514.5, 1154.5, and 831.1 days, respectively. Infection recurrence rates for types 1, 2, 3, 4 were 0, 11.5, 13.3, and 19.2%, respectively. All of these recurrent cases, except for 4 cases of type 4 that required surgical intervention, were treated with conservative wound management. CONCLUSION: A volume-rich flap (omental or LD flap) was required to fill the dead space after debridement in mediastinitis (type 3) or aortic graft infection (type 4), whereas superficial SSI (type 1) or sternal osteomyelitis (type 2) received a less-invasive flap (PM muscle advancement flap). Our new classification method was based on the anatomical invasiveness of the infection, providing both a simple and easy diagnosis and definitive treatment strategy.

    Background  Various methods for monitoring after free flap surgery have been reported in the literature. Among them, pulse oximetry shows a sensitive reaction to vascular issues, and it is easy to interpret visually. However, previous reports used special equipment that was less commonly used and difficult to generalize. In this study, we used a commercial pulse oximeter and a widely used bedside patient monitor to monitor transplanted free tissue and lower extremities of healthy subjects with impaired circulation. Methods  A reflectance pulse oximeter sensor was attached on the flap after free tissue transplantation. The sensor was connected to a bedside patient monitor, and the flap oxygen saturation (SpO2) levels and arterial waveforms were continuously monitored. Additionally, blood circulation disorder was induced in the lower limbs of healthy volunteers using pressure cuff inflation on the thigh, and the waveform and SpO2 levels on the pulse oximeter attached to the lower leg were monitored. Results  Twenty-two patients were included in this study. No postoperative vascular issues were observed in any case. Pulse oximeters showed normal rhythmic wavelengths, and the flap SpO2 level ranged approximately >90%. The pulse oximeter waveform rapidly disappeared during arterial occlusion in the thigh pressure cuff inflation test, and the waveform flattened and the SpO2 level decreased slightly during venous congestion. Conclusion  Flap monitoring using a commercially available pulse oximeter and a bedside patient monitor is a versatile, easy-to-interpret, and useful method. Changes in waveform and SpO2 levels appear during arterial and venous circulation disorders, and these changes can be differentiated.

    We report a case of palliative surgery in a 73-year-old patient with metastatic plantar sarcoma. The patient underwent resection and irradiation of an undifferentiated spindle cell sarcoma in the right plantar region. The wound was not closed and systemic metastases were observed. The chief complaint of the patient on his first visit to our department was difficulty walking due to pain in the right plantar region. Since we were unsuccessful in relieving the pain with conservative treatment, we decided to perform a palliative free tissue transfer to the right plantar. The surgery was successful, the skin ulcer healed, and the pain was relieved after the surgery. When performing palliative surgery, more detailed preoperative management and planning are necessary to achieve a successful outcome. The selection of the flaps according to the local lesion and metastatic lesions and changes in the local hemodynamics should be considered when planning.

    This study aimed to analyze the Hess area ratio (HAR%) in cases of blowout fracture treated in our department and clarify the outline of eye movement disorders in blowout fractures. Patients who underwent surgery for orbital blowout fractures in our department were included. Fracture locations were classified into 5 types (A, outside floor; B, C, anterior and posterior floor; and D, E, anterior and posterior medial wall). The HAR% was compared before and after surgery in eligible cases. The relationship between the fracture location and preoperative HAR% was investigated using multiple regression analysis. The study involved 85 patients. Hess area ratio was higher postoperatively than preoperatively (70.75 ± 18.26 versus 90.06 ± 13.99, P  < 0.01). The postoperative HAR% tended to be higher when the iliac bones were compared to other materials; however, this difference was not significant (90.73 ± 12.91 versus 80.30 ± 17.81, P = 0.178). Fracture locations C and E significantly contributed to the prediction of HAR% as negative regression coefficients (P = 0.024 and 0.013, respectively). The posterior fracture area on both the orbital floor and medial wall contributed to the decrease in preoperative HAR%. This observation indicates that the reconstruction of the posterior region is extremely crucial.

    INTRODUCTION: We hypothesized that hybrid artificial nerves might overcome the limitations of a nerve conduit by isolating nerve fascicles from autologous nerves. Nerve sacrifice during harvest, a drawback of conventional autologous nerve transplantation, may be reduced by the hot dog method. The hot dog method (based on the morphology of hybrid artificial nerves) adds nerve conduits to autologous nerve fascicles. METHODS: Forty-eight rats with a 10-mm sciatic nerve defect were divided into six groups (n = 8 per group) according to the neural reconstruction method: autologous nerve transplantation, the hot dog method, nerve conduit, nerve fascicle transplantation, sham control, and nerve fascicle isolation were classified as Groups I, II, III, IV, V, and VI, respectively. The sciatic nerve function was assessed in these groups, a histological evaluation was performed, and statistical analyses were conducted based on these data. RESULTS: Group III (nerve conduit) and Group IV (nerve fascicle transplantation) showed the lowest functional and axonal regenerative effects, followed by Group II (hot dog method) and Group I (autologous nerve transplantation). Group VI (nerve fascicle isolation) tended to achieve better recovery in motor function and axonal regeneration than Group I (autologous nerve transplantation). CONCLUSIONS: The hot dog method is simple, safe, and easy to execute. This method can serve as a new neural reconstruction method that uses artificial nerves.

    Extra-anatomical bypass grafting is a surgical method used to remove an infected aortic graft and promote revascularization with a new graft in the non-infected area. Here, we report a case of intractable post-sternotomy mediastinitis (PSM) with aortic graft infection which was treated with extra-anatomical bypass grafting. A 56-year-old woman with a history of multiple aortic dissection and prosthetic graft replacement in the thoracoabdominal area developed PSM with aortic arch graft infection. Bacterial culture of the exposed prosthetic graft tissue yielded multidrug-resistant Pseudomonas aeruginosa. Meticulous debridement of the wound and management by negative pressure wound therapy with continuous irrigation was performed. However, the infection of the prosthetic graft could not be controlled. Extra-anatomical bypass was performed between the left common carotid artery and right subclavian artery via the right common carotid artery. Then, the infected graft was removed. After the resolution of infection, the mediastinal wound was reconstructed with a pedicled latissimus dorsi myocutaneous flap, which was harvested from the right dorsum. No recurrence of infection occurred in the nine-month follow-up period. Debridement and removal of exposed artificial graft are considered the gold standard for treating wound infection. In situ replacement of infected aortic grafts carries a risk of re-infection due to residual bacterial contamination of the periprosthetic tissue. Extra-anatomical bypass would be a useful option for reducing the risk of re-infection in patients with intractable PSM and prosthetic aortic graft infection. Further studies are warranted to evaluate the risks and benefits of this operative method.

    Schwannomas commonly occur in the head and neck region as acoustic neuromas. Facial nerve schwannomas are rare and usually occur in the temporal region. A 57-year-old woman presented with a mass at the right mandibular margin. Magnetic resonance imaging revealed a schwannoma located immediately caudal to the mental foramen. We were initially uncertain whether it arose from the trigeminal nerve or the facial nerve. Excision was performed under general anesthesia. The mass was encapsulated and easily detached from the surrounding tissue. The nerve of origin was identified proximal to the tumor. A facial nerve origin was confirmed as the muscles supplied by the marginal mandibular branch of the facial nerve moved on nerve stimulation. Nerve fibers were not found distal to the tumor, possibly because they had been cut during excision. We believe that this is the first report of a schwannoma arising from the peripheral facial nerve.

    Total laryngectomy involves removal of the vocal cords resulting in the loss of vocal function. After laryngectomy, the patient's vocal function can be restored in several ways, including the insertion of a tracheoesophageal (TE) shunt. A TE shunt is considered an effective means of restoring speech due to its high efficacy, low requirement for training, and no need for any equipment while speaking. However, complications such as saliva inflow into the trachea, caused by the widening of the shunt opening, have also been reported. Moreover, the optimal treatment for an enlarged fistula has not yet been established. A fistula may also form at sites of hypopharyngeal reconstruction with free jejunal transplantation. Following its formation, the influx of saliva, infections, and pressure exerted by the act of swallowing make a fistula resistant to closure, and most patients require closure surgery using myocutaneous flaps. We encountered a case where an intractable TE fistula formed due to a TE shunt after the patient underwent total pharyngolaryngeal resection for hypopharyngeal cancer and hypopharyngeal reconstruction with a free jejunum flap. Since the optimal method for the TE fistula closure remains uncertain, we attempted to close the fistula according to the fistula closure of the free jejunal transplantation. Failure to close a TE fistula using a myocutaneous flap necessitates a re-closure procedure. However, because the surgical field around the trachea can be limited in such patients, creating an additional myocutaneous flap may not be feasible. In addition to the myocutaneous flap, ventilation control using a conventional intubation tube may further narrow the surgical field during the re-closure surgery. Based on our experience and existing literature, in this article, we summarize several ways of managing TE fistula when the surgical field around the trachea is limited.

    A congenital cheek fistula is a rare malformation in the buccal area. Here, we report the case of a congenital cheek fistula in a 50-year-old woman who visited our clinic with complaints of swelling and pain in her left cheek. Physical examination revealed a small hole in the left corner of the mouth present since birth. She had no other congenital malformations in the maxillofacial region such as an accessory ear and cleft lip. Manual compression of the cheek mass induced serous discharge from the hole. Magnetic resonance imaging (MRI) showed a cystic lesion in the left cheek and a fistula within the orbicularis oris muscle that opened into the small hole. After immediate incision and drainage of the cyst, both the cyst and fistula were surgically resected. The cystic lesion was completely delineated from the boundary of the parotid gland. The orbicularis oris muscle was partially incised to remove the fistula and the surrounding scar tissue. Histopathological examination of the resected specimen revealed a cavity consisting of epithelium inside the fistula. The postoperative course was insignificant. No recurrence of the cyst was observed six months postoperatively. The operative and pathological findings demonstrated that the ectoderm-derived epithelial tissue was enclosed by the mesoderm-derived muscle tissue. The mixture of different germ layer-derived tissues suggested that the fistula was a type of congenital transverse facial cleft induced by malfusion of the mandibular and maxillary prominences during embryonic development. The differential diagnoses of the congenital cheek fistula included orocutaneous fistulas and salivary fistulas. MRI was useful in delineating the border between the lesion and the surrounding tissue.

    Synovial sarcoma is a rare tumor requiring new treatment methods. A 46-year-old woman with primary monophasic synovial sarcoma in the left thigh involving the sciatic nerve, declining surgery because of potential dysfunction of the affected limbs, received two courses of BNCT. The tumor thus reduced was completely resected with no subsequent recurrence. The patient is now able to walk unassisted, and no local recurrence has been observed, demonstrating the applicability of BNCT as adjuvant therapy for synovial sarcoma. Further study and analysis with more experience accumulation are needed to confirm the real impact of BNCT efficacy for its application to synovial sarcoma.

    INTRODUCTION: Limited literature exists regarding the positional relationship between the pedicled-rectus abdominis myocutaneous (p-RAMC) flap and residual pelvic organs post posterior pelvic exenteration (PPE). PRESENTATION OF CASE: Four patients underwent PPE and reconstruction with a p-RAMC flap. After harvesting the p-RAMC flap with the deep inferior epigastric artery and veins as the vascular pedicle, the intra-pelvic shortest pathway of the flap was created. We dissected the subcutaneous tissues of the flap donor site toward the perineal defect along the inner wall of the pelvis. The pubic origin of the rectus abdominis muscle was preserved. A three-dimensional model was constructed using an image processing software. The vascular pedicle ran almost linearly along the inner wall of the pelvis. The muscle belly was placed on the pelvic floor through the posterior wall of the urinary bladder which filled the dead space of the resected area. All flaps survived without significant complications. DISCUSSION: Assigning the cranial side of the flap to the perineum and caudal side to the pelvic floor could reduce postoperative intrapelvic complications. By preserving the pubic origin of the rectus abdominis muscle, a shock absorber of the pedicle of the flap was created, preventing over-traction of the flap while passing through the intrapelvic pathway. CONCLUSION: A p-RAMC flap via intra-pelvic shortest pathway is an ideal reconstructive method for large skin defect in perineal area after PPE.

    BACKGROUND:  Intraoperative vasospasm during reconstructive microvascular surgery is often unpredictable and may lead to devastating flap loss. Therefore, various vasodilators are used in reconstructive microsurgery to prevent and relieve vasospasm. Lidocaine is a vasodilator commonly used in microvascular surgery. Although many reports have described its in vitro and in vivo concentration-dependent vasodilatory effects, limited studies have examined the pharmacological effects of lidocaine on blood vessels in terms of persistence and titer. METHODS:  In this study, the vasodilatory effect of lidocaine was examined by using the wire myograph system. Abdominal aortas were harvested from female rats, sliced into rings of 1-mm thickness, and mounted in the wire myograph system. Next, 10, 5, 2, and 1% lidocaine solutions were applied to the artery, and the change in vasodilation force, persistence of the force, and time required to reach equilibrium were measured. RESULTS:  The vasodilatory effect was confirmed in all groups following lidocaine treatment. Although strong vasodilation was observed in the 10% lidocaine group, it was accompanied by irreversible degeneration of the artery. Vasodilation in the 1% lidocaine group was weaker than that in the other groups 500 seconds after lidocaine addition (p < 0.05). Between the 5 and 2% lidocaine groups, 5% lidocaine showed a stronger vasodilatory effect 400 to 600 seconds after lidocaine addition (p < 0.01); however, there was no significant difference in these groups after 700 seconds. Additionally, there was no difference in the time required for the relaxation force to reach equilibrium among the 5, 2, and 1% lidocaine groups. CONCLUSION:  Although our study confirmed the dose-dependent vasodilatory effect of lidocaine, 5% lidocaine showed the best vasodilatory effect and continuity with minimal irreversible changes in the arterial tissue.

    The human skin has previously been described to be affected by light; however, the underlying mechanism remains unknown. OPN4 (melanopsin) expression was first identified in the skin of amphibians; however, whether it is also expressed and functioned in the human skin has not yet been identified. Here, we show that OPN4 was expressed in the human skin tissue and cultures of isolated keratinocytes, melanocytes and fibroblasts. Additionally, Ca2+ influx in vitro and ex vivo and phosphorylation of extracellular signal-regulated kinases 1/2 in human fibroblasts were observed by stimulation of blue light irradiation. Notably, our findings showed that this Ca2+ influx and phosphorylation of extracellular signal-regulated kinases 1/2 are promoted in an intensity-dependent manner, indicating that the light signal is converted to an intracellular signal via OPN4 in the human skin. Overall, in this study we showed that the human skin functions as a photoreceptor by demonstrating that in human skin, the photoreceptive protein was expressed, and photoreception was conducted via photoreceptive protein.

    BACKGROUND AND OBJECTIVE: Forearm free flaps are used after hemiglossectomy. However, no investigation has been performed on whether oral functions are better preserved when sizes of the resection and reconstruction flap are exact matches, or whether the size of the resection should be changed. We aimed to retrospectively examine whether size differences between the resection and reconstruction flap affect speech and swallowing functions postoperatively, and to determine whether there are more favorable flap size ratios. METHODS: This is a retrospective cohort study of patients undergoing hemiglossectomy using a forearm free flap between 2006 and 2016 at Kobe University Hospital, Japan. The effect of size difference between the resection and reconstruction flap on maintained oral function was assessed. Speech and swallowing functions were assessed, and their correlation with the ratio of the flap size to that of the resected area was determined. With these data, distribution maps of the relationship between the functional level and reconstructed dimension ratio were prepared. The more suitable reconstructed dimension ratio was examined and evaluated. The Fisher exact test, Kruskal-Wallis test, and Scheffe test were used in statistical analyses. RESULTS: Eighty-eight patients underwent hemiglossectomy using a forearm free flap during a 10-year period. Of these cases, 66 patients were included in this study, while 22 were excluded. The ratio of the area of the reconstruction flap to that of the resection site was 0.59-2.79 (median: 1.61). Sixty patients had flaps greater than the resection area, whereas 6 had smaller flaps. Significant differences were found in speech intelligibility and swallowing function when the reconstructed dimension ratio was categorized as follows: ≤1.3, 1.3-1.8, and ≥1.8. CONCLUSION: Our findings suggest that postoperative deterioration of oral functions after hemiglossectomy could be reduced if reconstruction is performed using a forearm free flap with a surface area 1.3 to 1.8 times greater than that of the resection area.

    OBJECTIVE:  Musculocutaneous flap reconstruction surgery is one of the standard procedures following head and neck cancer resection. However, no previous studies have classified flaps in terms of muscle and fat or examined them after long-term follow-up. The purpose of this study was to estimate the fat and muscle volume changes in musculocutaneous flaps during long-term follow-up. METHODS:  We conducted a retrospective analysis of 35 patients after musculocutaneous flap reconstruction. The total, fat, and muscle volumes of the musculocutaneous flaps were measured using 3-dimensional images. Changes in flap volumes over time (1 month, 1 year [POY1], and 5 years [POY5] postoperatively) were assessed. Flap persistence was calculated using flap volumes at 1 month after reconstruction for reference. RESULTS:  Flap persistence at POY5 was 42.0% in total, 64.1% in fat, and 25.4% in muscle. Muscle persistence was significantly decreased (p < 0.0001). In a multiple regression analysis, decreased body mass index (BMI) of ≥ 5% influenced fat persistence less than muscle persistence at POY1; however, there was no significant difference at POY5. Postoperative radiation therapy was associated with a significant decrease in total flap persistence at POY1 (p = 0.046) and POY5 (p = 0.0097). Muscle persistence significantly decreased at POY5 (p = 0.0108). Age significantly influenced muscle volume at POY1 (p = 0.0072). CONCLUSION:  Reconstruction flaps are well-preserved with high fat-to-muscle ratios. Recommendations for weight maintenance are necessary for patients less than 2 years after surgery due to the influence of BMI on fat persistence. Radiation therapy is necessary for some patients based on their disease state. Intensity-modulated radiation therapy can be offered to reduce scattering irradiation to normal tissues.

    OBJECTIVES: The aim of this study was to evaluate the early and long-term outcomes of our multidisciplinary strategy for treating deep sternal wound infection after aortic grafting, which consisted of debridement by a plastic surgeon, negative pressure wound therapy with continuous irrigation and chest wall reconstruction. METHODS: We performed a retrospective analysis of 18 patients who had a deep sternal wound infection following aortic grafting through a median sternotomy between January 2009 and December 2017. All patients had organisms cultured from mediastinal tissue within 2 months from the initial aortic surgery. The prosthetic grafts were exposed in 15 patients during resternotomy. Our protocol involved repeat debridement and negative pressure wound therapy with continuous irrigation twice a week until the results of the culture were negative and chest wall reconstruction was complete. RESULTS: The mean duration from primary aortic surgery to resternotomy was 23.7 ± 15.9 days. Except for 1 patient, 17 patients underwent chest wall reconstruction. The mean duration from resternotomy to chest wall reconstruction was 31.1 ± 28.0 days. The hospital mortality rate was 16.7% (3 patients), although no patients died of wound-related causes. The mean follow-up period was 2.9 ± 2.5 years. Overall survival was 69.6 ± 11.4% at 1 year and 54.2 ± 13.3% at 5 years. Freedom from reoperation for reinfection was 94.4 ± 5.4% at 5 years. CONCLUSIONS: Our wound care strategy achieved acceptable early and late survival in patients who had deep sternal wound infection following aortic grafting. This strategy may benefit those who experience this devastating complication.

    INTRODUCTION: Skin flap procedures are widely used to reconstruct skin and soft tissue defects. Skin flap necrosis is a serious postoperative complication. Many researchers have introduced pharmacological agents to improve flap ischemia in experimental studies. However, outcomes of these studies remain controversial. We previously demonstrated that transcutaneous CO2 application improves hypoxia in fracture repair. In this study, we hypothesized that improving hypoxia by transcutaneous CO2 application can improve the blood flow in skin flaps and increase angiogenesis. We investigated whether transcutaneous CO2 application can increase the survival of random-pattern skin flaps. MATERIALS AND METHODS: Six-week-old male Sprague-Dawley rats were divided into two equal groups: the control group (n = 6) and CO2 group (n = 6). A random-pattern skin flap was constructed in these rats. Topical CO2 was applied using a hydrogel every day for 5 days in the CO2 group. The flap survival area was measured on postoperative days 1, 3, and 5. The vessel density and expression of vascular endothelial growth factor (VEGF), basic fibroblast growth factor (bFGF), and hypoxia-inducible factor-1α (HIF-1α) were evaluated on postoperative day 5. RESULTS: A statistically significant difference was found in the percentage of the flap survival area between the two groups on postoperative days 3 and 5 (p < 0.05). Furthermore, the expression of VEGF and bFGF was significantly higher and that of HIF-1α was significantly lower in the CO2 than in the control group (p < 0.05). CONCLUSIONS: Transcutaneous CO2 application can improve the blood flow in skin flaps and increase angiogenesis, thus increasing the survival of random-pattern skin flaps.

    OBJECTIVES: Transient receptor potential cation channel (subfamily V, members 1-4) (TRPV1-4) are expressed in skin and neurons and activated by external stimuli in normal mucosae of all oral cavity sites. The oral cavity is exposed to various stimuli, including temperature, mechanical stimuli, chemical substances, and changes in pH, and, notably, the risk factors for oncogenic transformation in oral squamous epithelium are the same as the external stimuli received by TRPV1-4 receptors. Hence, we examined the relationship between oral squamous cell carcinoma (SCC) and TRPV1-4 expression. MATERIALS AND METHODS: Oral SCC patients (n = 37) who underwent surgical resection were included in this study. We investigated the expression of TRPV1-4 by immunohistochemical staining and quantification of TRPV1-4 mRNA in human oral mucosa. In addition, we compared the TRPV1-4 levels in mucosa from patients with SCC to those in normal oral mucosa. RESULTS: The receptors were expressed in oral mucosa at all sites (tongue, buccal mucosa, gingiva, and oral floor) and the expression was stronger in epithelia from patients with SCC than in normal epithelia. Furthermore, alcohol consumption and tobacco use were strongly associated with the occurrence of oral cancer and were found to have a remarkable influence on TRPV1-4 receptor expression in normal oral mucosa. In particular, patients with a history of alcohol consumption demonstrated significantly higher expression levels. CONCLUSION: Various external stimuli may influence the behavior of cancer cells. Overexpression of TRPV1-4 is likely to be a factor in enhanced sensitivity to external stimuli. These findings could contribute to the establishment of novel strategies for cancer therapy or prevention.

  • A Novel Method of Noninvasive Monitoring of Free Flaps With Near-Infrared Spectroscopy.

    Objective: Thrombosis of a site of anastomosis in microsurgical free tissue transfer can result in tissue necrosis. To salvage potentially failing free flap, various methods of monitoring the viability of tissue have been described. We report a novel method of monitoring free flaps using near-infrared spectroscopy. Methods: After microsurgical operation, we monitored the regional oxygen saturation of the flap with using the In-Vivo Optical Spectroscopy. A total of 57 patients participated in this study. Results: Of 57 cases, arterial insufficiency was detected in 1 case and venous insufficiency was detected in 3 cases. Regional oxygen saturation decreased before the flap color changed to a pale or congestive color. We could salvage these 4 patients by re-exploration. Conclusions: The postoperative monitoring with the In-Vivo Optical Spectroscopy is noninvasive, continuous, reliable, and reproducible. This technique could be one of the best methods for monitoring microsurgical free tissue transfers.

  • Low-Grade Myxofibrosarcoma of the Rectus Abdominus Muscle Infiltrating into Abdominal Cavity: A Case Report.

    Objective: Myxofibrosarcoma (MFS) is a relatively rare tumor that is histologically characterized by myxoid stroma and spindle cell proliferation. This tumor most commonly arises as a slow growing, enlarging painless mass in the extremities of elderly patients. Methods: We report a case of a primary, low-grade MFS in the rectus abdominis muscle infiltrating the abdominal cavity of a 75-year-old man. Results: The patient underwent a wide excision of the right abdominal wall mass with a 3-cm surgical margin from the scar due to a biopsy. The tumor infiltrated the urinary bladder, peritoneum, and external iliac vessels. Twenty-six months after the initial operation, he had recurrences in his abdominal wall, urinary bladder, and right iliac vessels. Conclusions: To our knowledge, primary MFS of the muscle in the abdomen has not been documented previously. Although this case was histopathologically classified as a low-grade tumor, it infiltrated the abdominal cavity. The tumor is suspected to have penetrated the abdominal cavity below the linea arcuata, which lacks the posterior sheath of the rectus abdominis muscle; from there, it could easily spread without being blocked by any biological barriers.

    BACKGROUND: The purpose of this study was to analyze outcomes for the treatment of locally advanced temporal bone cancer by means of concomitant chemoradiotherapy (CCRT) with a combination of cisplatin (CDDP), 5-fluorouracil (5-FU), and docetaxel (TPF). METHODS: Between 2006 and 2011, 34 patients with squamous cell carcinoma of the temporal bone were treated at Kobe University Hospital. Medical records were retrospectively reviewed to obtain information concerning patient characteristics, extent of disease, treatment, adverse events, and oncologic results. RESULTS: Ten patients were treated with CCRT using TPF regimen. The 5-year overall survival rate and disease-free survival rate were both 60%. Of special interest is that even for patients with unresectable T4 disease, the 5-year overall survival rate was 56%. CONCLUSION: The results of our study indicate that CCRT with TPF for locally advanced temporal bone cancer is an effective and promising regimen. © 2015 Wiley Periodicals, Inc.     In Japan, negative pressure wound therapy (NPWT) has become an important wound management technique since a NPWT device was approved for use under the Japanese national health insurance system. On the other hand, its utility for treating infected sites is limited because it results in the formation of a semi-enclosed environment and can even exacerbate infections in some cases. Recently, a device with combined NPWT and irrigation functions has been introduced in other countries, although it has not yet been approved for use under the Japanese national health insurance system. We produced a custom NPWT device that allows continuous irrigation and used a preexisting NPWT instrument as a motor source. The system enables patients to retain their ability to perform activities of daily living. The system is able to create stable negative pressure environments while providing continuous focal irrigation, which are associated with good wound bed preparation. Here, we describe the construction of the system.

  • A Study of Internal Thoracic Arteriovenous Principal Perforators by Using Multi-detector Row Computed Tomography Angiography.

    OBJECTIVE: There are numerous reports of perforating branches from the intercostal spaces of the internal thoracic vessels. These branches have varying diameters, and a main perforating branch, the principal perforator, most often found in the second or third intercostal space. We report different results based on multi-detector row computed tomography. METHODS: We evaluated 121 sides from 70 women scheduled for breast reconstruction with free lower abdominal skin flaps who underwent preoperative multi-detector row computed tomographic scan between June 2008 and June 2015. For primary reconstruction, we analyzed both sides, and for 1-sided secondary reconstruction, we analyzed only the unaffected side. We evaluated both early arterial phase and late venous phase 5-mm horizontal, cross-sectional, and volume-rendering images for perforation sites and internal thoracic arteriovenous perforating branches' intercostal space thickness. We analyzed differences in thickness between the internal thoracic arteries and veins and symmetry in cases involving both sides. RESULTS: Venous principal perforators nearly always perforated the same intercostal spaces as accompanying veins of arterial principal perforators (99.2%), forming arteriovenous principal perforators. We found 49 principal perforators in the first intercostal space (37.4%), 52 in the second intercostal space (39.7%), 23 in the third intercostal space (17.6%), 6 in the fourth intercostal space (4.6%), and 1 in the fifth intercostal space (0.7%). Of the 51 cases in which we studied both sides, 25 cases (49%) had principal perforators with bilateral symmetry. CONCLUSIONS: In contrast to findings from past reports, we found that internal thoracic arteriovenous principal perforators were often present in almost the same numbers in the first and second intercostal spaces.

    BACKGROUND: Oroantral fistula may develop as a complication of tooth extraction owing to infection, trauma, or removal of maxillary cyst or tumors. Closure by using a palatal mucoperiosteal flap with the greater palatine vessels is a very traditional and basic technique. The palatal mucosa is thick and is firm. However, deformation can occur with shifting of the mucoperiosteal flap, survival of the flap may be unsuccessful, and patients may have substantial intraoral discomfort felt until healing of the eminence with the arcuation. As a method to relieve these problems, we present a mucoperiosteal tunnel technique for the closure of oroantral fistula by using a palatal mucoperiosteal flap pedicled with the greater palatine vessels. METHOD: A 42-year-old man and a 69-year-old woman each had a palatal fistula after palatal tumor resection and tooth extraction, respectively. We designed a mucoperiosteal flap pedicled with the left greater palatine vessel. We ablated the mucoperiosteum between the fistula and the mucoperiosteal flap, and passed this flap under the ablated mucoperiosteum as a tunnel. RESULT: One year after surgery, the fistula had not reappeared and the mucoperiosteal flap harvest did not generate dyskinesis of the soft palate. CONCLUSION: Tunnel technique for the closure of an oroantral fistula with a pedicled palatal mucoperiosteal flap is obtains good fructification.

    Introduction: Several methods of nerve reconstruction for facial nerve palsy are known. Although the recently introduced method of "cross-linking" of the facial and hypoglossal nerves with a grafted nerve has proved efficacious, the underlying mechanism is unclear. Methods: In this study, we created an animal model with Wistar rats and analyzed the newly reconstructed neural circuit by anterograde and retrograde neural tracer methods. The saphenous nerve was harvested as a graft, and its double end-to-side neurorrhaphy with the facial and hypoglossal nerves with epineural windows was carried out under the microscope. After an appropriate interval, small amounts of fluoro-ruby or fluoro-emerald were injected into the animals and analyzed 5 days later by fluorescent microscopy (Anterograde experiment: fluoro-ruby into the hypoglossal nucleus at 5 weeks; retrograde experiment: fluoro-ruby into the distal facial nerve sheath and fluoro-emerald into the distal hypoglossal nerve sheath, both at two months.). Results: The labeled axons derived from the hypoglossal nucleus were observed passing through the grafted nerve to the facial nerve. On the other hand, retrogradely labeled neurons were observed at both the hypoglossal and facial nuclei with some double-labeled neurons, suggesting that collateral sprouting had occurred. Conclusions: We suggest that the newly constructed neural circuits we observed are conducive to the treatment of facial nerve palsy.

    Negative pressure wound therapy (NPWT) has been introduced as a useful technique for wound treatment. However, some complications have been reported, such as infection, bleeding and pain, which make it impossible for patients to continue further treatment. Air leak is another problem, especially in hairy regions, such as the head and genital area. We have introduced a silicone gel adhesive dressing for NWPT, instead of using the film included in the original kit. The use of the silicone gel adhesive dressing greatly reduces pain, which was occurs when detaching the film. In addition, it plays a role in reducing air leak in hairy regions. Although there remains the problem of its high cost, silicone gel adhesive dressing for NPWT is quite useful.

  • Necessity of Distinguishing Verrucous Carcinoma From Verrucous Skin Lesion Overlaying Residual Skin Staples in an Area of Sensory Loss: A Case Report.

    OBJECTIVE: Verrucous skin lesions on the feet in diabetic neuropathy is a condition usually induced by chronic mechanical stimulation of the feet of patients with diabetic neuropathy and usually occurs at weight-bearing sites. We here present a rare case involving a man with verrucous skin lesions on the feet in diabetic neuropathy at an unusual, non-weight-bearing site. METHODS: A 58-year-old man with diabetic neuropathy presented with a verrucous skin lesion overlaying residual skin staples and an amputation stump of the second metatarsal bone on the dorsal foot. RESULTS: The biopsy findings were inconclusive and suggested the necessity of distinguishing the lesion from verrucous carcinoma. The lesion was resected, and the residual skin staples were simultaneously removed. Investigation of the excisional biopsy confirmed our diagnosis of verrucous skin lesions on the feet in diabetic neuropathy. CONCLUSIONS: Verrucous skin lesions on the feet in diabetic neuropathy is often difficult to distinguish from verrucous carcinoma; in this case, the unusual location of the lesion could be attributed not only to sensory loss but also to the presence of an amputation stump and the persistence of the residual skin staples.

    Background. Postresective mandibular reconstruction is common in cases of oral and mandibular tumors. However, complications such as infection, plate exposure, or plate fracture can occur. We identified several significant risk factors of complications after reconstructive surgery and compared the effectiveness of different surgical techniques for reducing the incidence of complications. Methods. This study is a retrospective analysis of 28 oromandibular cancer cases that required reconstructive surgery between January 1999 and December 2011 at Kobe University Graduate School of Medicine in Japan. All cases were classified using Hashikawa's CAT and Eichner's classification methods. Then, we determined whether these classifications and different treatment or surgical methods were significantly related to complications. Results. Complications after mandibular reconstruction occurred in 10/28 patients (36%). Specifically, five patients had plate fractures, four had plate exposures, and one had an infection. Radiation therapy and closure without any flaps were significantly related to infection or plate exposure. The wrap-around technique of securing reconstruction plates was used in 14 cases, whereas the run-through technique was used in two cases. Conclusions. The success of mandibular reconstruction depends on both mechanical and biological factors, such as the location of defects, presence of occlusions, and the amount of vascularization of the flap.

    BACKGROUND: Tumor thickness and relative depth of invasion are prognostic parameters considered when developing treatment plans for malignant skin tumors. Although conventional magnetic resonance (MR) imaging techniques cannot identify small tumors, use of microscopy coils considerably improves spatial resolution. Some studies have shown that this technique is efficacious in preoperative assessment of relative depth of invasion; however, its ability to provide accurate measurements of tumor thickness remains unconfirmed. OBJECTIVE: The purpose of this pilot study was to evaluate the usefulness of preoperative MR-microscopy in determining tumor thickness and relative depth of invasion of malignant skin tumors. METHODS AND MATERIALS: Magnetic resonance images of malignant skin tumors in seven female patients (six with basal cell carcinoma and one with malignant melanoma) were obtained using a 1.5 T system and a 47-mm or 23-mm microscopy coil. Tumors were then excised, fixed, dehydrated, embedded, and stained with hematoxylin and eosin. We then compared MR-microscopy and pathology values for thickness and relative depth of invasion of each tumor. RESULTS: Both techniques produced similar measurements of tumor thickness and relative depth of invasion. CONCLUSIONS: MR-microscopy is very useful for accurate preoperative estimation of not only relative depth of invasion, but also thickness of malignant skin tumors.

    Umbilical reconstruction after total excision of the umbilicus represents a challenging problem for reconstructive surgeons. We describe herein a new method for one-stage umbilical reconstruction after resection of a urachal cyst. This case series included 6 patients, with laparoscopic urachal cyst removal in 5 and conventional transcutaneous surgery in 1. One-stage umbilical reconstruction was performed in all cases. When a conventional transcutaneous approach is indicated, umbilical reconstruction can be undertaken through the same skin incision used for total resection of the urachal cyst. Two triangular flaps were designed just below the umbilical defect. Flaps were rotated 180 degrees and sutured together to form one big triangular flap. This flap was then folded to create the new umbilicus. A deep umbilicus with good shape was constructed in all cases, and all patients were satisfied with the outcome. This method is simple, easy, and produces a natural-looking umbilicus.

    <p>Prevention of the atrophy of denervated muscles is essential for a good outcome in facial contouring and oral reconstruction. In this study, we compared the effectiveness of end-to-end and end-to-side neurorrhaphy of the motor nerve, and end-to-end neurorrhaphy of the sensory nerve, all of which are frequently used in such reconstruction for the prevention of muscle atrophy. Wistar rats were divided into four groups: group 1, motor nerve division of semi-membranosus without repair; group 2, motor nerve division and end-to-end coaptation to the saphenous nerve; group 3, motor nerve division and end-to-side coaptation to the sciatic nerve; and group 4, motor nerve division and end-to-end repair. Measurement of semi-membranosus volume, histological evaluation and staining of neuromuscular junctions that were carried out 3 months postoperatively revealed that muscle volume preservation was larger in groups 3 and 4 than in the other two groups (p < 0.05), but slightly superior in group 4 (p < 0.05).     The V.A.C. ATS system was listed in the National Health Insurance in Japan in 2010. In this system, direct application of foam to the periwound skin causes skin damage such as erosion, blisters and subcutaneous hemorrhage. The cost of wound-dressing materials used for the prevention of such damage is not covered by insurance. Here, we examine the efficacy of skin protection and compare the cost of wound-dressing materials (hydrocolloid dressing and silicone foam) , protective ointment and specific low-irritative surgical tape. In the V.A.C. ATS system, periwound skin was well protected with hydrocolloid dressing and surgical tape, whereas other materials showed poor efficacy. A comparison of hydrocolloid dressing and surgical tape revealed that the latter was less than 1/30 the cost of the former. In conclusion, low-irritative surgical tape is conducive for periwound skin protection with V.A.C. ATS therapy.

    Toe amputation for chronic ulcers associated with osteomyelitis or malignant tumor requires additional metatarsal bone resection for covering the stump ; alternatively, the stump is kept raw and skin is grafted after the formation of granulation. The fillet toe flap is one of the options for primary coverage of the stump when the lesion is restricted to half the side of the toe and the digital artery is preserved after the lesion is resected. Minimization of the level of amputation is related to improvements in ADL after surgery. Moreover, because long rest causes disuse atrophy, early healing is desirable. Although the fillet toe flap is used only in special cases, it could be used as a sensory flap. We believe the use of the fillet toe flap deserves more widespread consideration in toe amputation.

  • The superficial layers of the superior colliculus are cytoarchitectually and myeloarchitectually disorganized in the reelin-deficient mouse, reeler.

    The causative gene for the reeler mouse is reelin which encodes Reelin protein, an extracellular molecule. In the present study, we have examined the cytoarchitecture, myeloarchitecture, and afferent/efferent systems of the superior colliculus (SC) of the reeler mouse. In the reeler, the laminar structures of the superficial three layers of the SC were disorganized and intermingled into a single layer, i.e., the superficial fused layer (SuF), as previously reported in the reelin-deficient SRK rat (Sakakibara et al., Develop. Brain Res. 141:1-13). Next, we have investigated the course and terminals of visual corticotectal and retinotectal projections with an injection of biocytin into the visual cortex or an injection of cholera toxin subunit B into the retina, respectively. In the reeler, anterogradely labeled visual corticotectal and retinotectal fibers took an aberrant course within the SuF, resulting in abnormal myeloarchitecture of the superficial SC of the reeler. Retrograde labeling of tectospinal tract neurons could not show any differences between the normal and reeler mice, suggesting that the deep layers of the reeler SC are cytoarchitectually normal. In situ hybridization and immunohistochemical studies have shown that reelin mRNA and Reelin protein were both recognized in the normal SC. These results suggest that Reelin protein plays some roles in histogenesis of the superficial layers of the SC.

  • Downregulation of Otx2 in the dedifferentiated RPE cells of regenerating newt retina.

    Cynops pyrrhogaster (the Japanese common newt) regenerates neural retina from retinal pigmented epithelium (RPE) cells. Otx2 is a transcription factor that is involved in RPE cell differentiation. To understand the role of Otx2 during transdifferentiation of RPE cells, we cloned a Cynops Otx2 cDNA, and explored its expression by RT-PCR, immunohistochemistry and in situ hybridization. The expression of Otx2 was compared with the localization of a proliferating cell marker (PCNA), RPE cell markers (RPE65, CRBP) and an RPE and Muller glial cell marker (CRALBP). At the early stage of regeneration, 2 to 3 cell layered regenerating retina consisting of pigmented cells uniformly expressed Otx2 and other markers. Following this stage, 4-cell layered regenerating retina consisted of two distinct layers, pigmented monolayer (the outer layer) attached to Bruch's membrane and presumptive neural retina (the inner layers). In the outer layer, Otx2 and CRBP expression was maintained and majority of cells lost PCNA expression. Some of cells maintained RPE65. In the inner layers, expression of Otx2, CRBP and RPE65 was downregulated, but a majority of those cells maintained PCNA expression. These results indicate that spatiotemporal regulation of Otx2 expression is consistent with those of RPE markers. Otx2 may play a pivotal role in maintenance and specification of RPE cells during neural retina regeneration. In contrast to RPE cell markers, CRALBP was expressed in both the pigmented and the de-pigmented layers. This observation implicates the appearance of Muller glial cells in an early phase of regenerating retina.

  • Ectopic corticospinal tract and corticothalamic tract neurons in the cerebral cortex of yotari and reeler mice.

    Reeler and yotari mice, which are mutant for Reelin or Dab1, respectively, show disorders of cerebral cortical lamination. We injected horseradish peroxidase (HRP) into the upper lumbar enlargement to label corticospinal tract (CST) neurons and wheat germ agglutinin-conjugated HRP (WGA-HRP) into the ventral lateral nucleus of the thalamus to label corticothalamic tract (CTT) neurons in both 19-day-old yotari and reeler mice with the aim of discovering whether or not they show differences in the distribution pattern of layer V or layer VI neurons. Similar injections of tracers were made in normal controls. HRP-labeled CST neurons, which were exclusively distributed in layer V of the normal cortex, were radially scattered in the cortex of both mutants, but those in reeler were more deeply distributed than in yotari. WGA-labeled CTT neurons, which were mainly located in layer VI in the normal cortex, were superficially distributed just beneath the pia mater in both reeler and yotari cortex. The present quantitative study shows that the distribution pattern of layer V neurons, but not layer VI neurons, differs between reeler and yotari mice, suggesting that the Reelin and Dab1 proteins may play different roles in the migration and cell positioning of layer V neurons.

    Shaking Rat Kawasaki (SRK) is a Reelin-deficient rat, that shows significant cytoarchitectural abnormalities in the cerebral and cerebellar cortices in a similar manner to the reeler malformation. In the present study, we investigated the cytoarchitecture and myeloarchitecture of the superior colliculus (SC) of this mutant rat. The Nissl staining clearly showed that neuronal components in the superficial layers of the SC in SRK rat were intermingled with each other and that the boundaries between these superficial layers were blurred. The MBP immunohistochemistry showed an abnormal fiber pattern in the superficial layers of the SC of this mutant rat. In the normal rat, myelinated fibers passed rostrocaudally through the optic layer, and only a few myelinated fibers were recognized in the uppermost two layers, i.e., the zonal and superficial gray layers. By contrast, in SRK rat, the myelinated fibers were distributed throughout the entire thickness of the superficial layers of the SC. Anterograde labeling of retinotectal fibers with an injection of Cholera Toxin subunit B into the retina revealed that this abnormal fiber pattern was associated with the anomalous course of the retinotectal fibers. No distinct differences in the cytoarchitecture and fiber pattern in the deep layers of the SC were seen. In conclusion, the present study demonstrated that the cytoarchitecture and fiber patterning in the superficial layers of the SC were disrupted in SRK rat, suggesting that Reelin protein regulates the formation of the superficial layers of the SC.

  • Opsin expression in adult, developing, and regenerating newt retinas.

    Japanese common newts (Cynops pyrrhogaster) have an ability to regenerate their neural retina even as adults. Although extensive research has been carried out attempting to understand this retinal regeneration, the molecules characterized in newt retina are limited. We isolated cDNAs encoding three putative opsins (Cp-Rh, -LWS and -SWS1), in addition to Cp-SWS2 [Takahashi et al., FEBS Lett. 501 (2001) 151-155] from a cDNA library of adult newt retina. Our immunohistochemical and in situ hybridization studies demonstrated that Cp-Rh is selectively expressed in rods, whereas the other opsins are expressed in cones. The distribution of opsin mRNAs in normal and regenerated retinas is very similar. In both developing and regenerating retinas, Cp-Rh and its mRNA first appeared in immature rods at the beginning or just after the formation of plexiform layers. Cp-Rh was initially found isotropically in the plasma membrane, and then translocalized to the apical region along with the maturation of regenerating rods. This suggests that reorganization of the intracellular structure takes place during maturation of the regenerating newt photoreceptors.

    これまでのわれわれの研究では、生体から得られた口腔粘膜組織(舌、歯肉、 粘膜、口底)の癌細胞と正常粘膜細胞を用いてTRPV1-4イオンチャネルの発現が癌と正常組織でどのように特徴付けられるのかを 証してきた。 (real time PCR法および、免疫組織化学染色法により、①TRPV1,2,3,4全てにおいて、ヒト口腔正常粘膜(舌、歯肉、 粘膜、口底)での発現を確認。②口腔扁平上皮癌と口腔正常粘膜におけるTRPV1,2,3,4の発現量は、扁平上皮癌の方が有意に 増加。③TRPV1,2,3,4全てにおいて、ヒト口腔正常粘膜で‘飲酒あり’群の方が、‘飲酒なし’群よりも有意に増加。④TRPV2,3,4において、ヒト口腔正常粘膜で‘喫煙あり’群の方が、‘喫煙なし’群よりも有意に増加しており、TRPV1も増加傾向。) これまでの研究は、生体から採取した粘膜癌細胞、粘膜正常細胞から採取した組織で行った。培養細胞においても同様な発現が認められるのかを検証中である。生体から採取した組織から細胞を 離しようと試みたが、困難であったため、細胞提供を受けて、培養している。正常粘膜細胞の培地と癌細胞の培地でまずは光刺激でカルシウムイオンチャネルが開くことを確認している。TRPVイオンチャネルの1-4のそれぞれに特有のチャネルを開くアゴニスト (例:カプサイシンやさまざまな温度設定)があるため、おのおので検証してきた。

  • Mechanism of hair cycle with irradiation of blue light

    われわれはヒト皮膚に光受容タンパク質が発現し、この発現はGPCRのセカンドメッセンジャーの1つであるGqを介して、細胞内シグナルへと変換されることを明らかにした。この過程でヒト皮膚の光受容タンパク質の1つであるMelanopsin (以下、OPN4)の発現動態を観察したところ、毛包や脂腺などの付属器にOPN4の強い発現と、Gqの共発現している所見を見出し、外毛根鞘および毛乳頭細胞におけるOPN4およびGnaqの強い発現を認めた。これは、青色光がOPN4を介して細胞内シグナルへと変換されることを示唆している。青色光の照射により、 細胞増殖シグナルであるERK1/2のリン酸化を促進すること、細胞レベルでの生体時計を調節することを見出していることから、青色光の①体内時計と毛周期の連動、②毛乳頭細胞への影響、③毛髪のメラニン保持の促進について検討することとした。 ① 毛髪から、実体顕微鏡下に毛乳頭部分を分離・培養し、毛乳頭細胞・毛母細胞の単離培養を行う。hBmal1およびhPer2遺伝子のプロモーター領域を組み込んだルシフェラーゼ発現ベクターを導入し、培養下にreal-time Luciferase assayを行い、青色光の照射により時計遺伝子の発現がリセットされることを確認した。 ② ①で得られた毛乳頭細胞を利用し、青色光を照射後、ウェスタンブロッティング法によりERK1/2のリン酸化の促進を確認する。次にend-point Luciferase assayを行い、SRF(ERK1/2の下流)の転写促進を確認する。さらにscratch assayおよびBRdU法により、細胞が青色光の照射により増殖することを確認する。③ 青色光によりMITFの転写促進を、real time PCRにより確認する。次に、reporter assayによりtyrosinaseの転写の促進を確認する。

    われわれは抗菌ペプチドであるdefensinに注目し、これを培養細胞において強制的に発現させることで細胞が感染への抵抗性を示すことを確認することを第一の目標とした。human defensinはアルファ型とベータ型とに分類され。さらにそのサブファミリーが存在する。これらはそれぞれに抗菌スペクトラムや発現部位が異なることが明らかとなっている。われわれはhuman beta defensin-3に注目した。本ペプチドは広い抗菌スペクトラムを持つためである。殺菌活性は細胞外に分泌して作用しなくてはならず、シグナルペプチドを組み合わせる必要があると考えられた。一方で、各defensinには一次構造にはシグナルペプチドが含まれており、この配列をベクターに組み込むこととして発現ベクターを設計した。この時、CMVプロモータ下流に位置させることとしたが、これとは別に発現が傾向タンパク質により確認できるように双方向性の発現ベクターを選択した。 まずカスタム合成によりhuman defense-3の全長配列を作成した。これに対してPCRを行うことで全長配列を増幅した。この配列をpBIおよびpBAプラスミドベクターに組み込みを行なっている。得られたクローンの塩基配列を現在確認している。 今後、プラスミドベクターへの組み込みが確認されたのち、ヒト培養皮膚線維芽細胞に導入し、ELISA法などにより培養上澄中に含まれるdefensinの発現量の測定や細菌耐性について検討を行う予定である。この細菌耐性の検討を行うにあたり、病原性を持つ細菌によるモデル化は汎用性に乏しいため、納豆菌によるバイオフィルム形成実験も併せて行なっている。

  • Analysis of metastasis and growth of melanoma cells under irradiation of specific light exposure with Gq mutation

    虹彩に発生する悪性黒色腫(以下Melanoma)のうち、遠隔転移を起こした群ではGタンパク質ファミリーのうちGqαサブユニット群(以下Gq)の遺伝子変異が高率に認められること、また動物実験により、Gq遺伝子変異株ではMAPキナーゼ系の活性化(細胞増殖の活性化)および有意に転移をきたしたことが報告された。その後、Melanomaのみならず消化器系や呼吸器系に発生した悪性腫瘍においても同様の知見が報告されつつある。本研究の目的は、光とMelanomaの関連においてこれまで示されてきた「光線暴露が発生の危険因子である」ことに加え、「光線暴露は増殖・転移の危険因子でもある」ことを細胞生物学的に示すことにある。われわれは2つのMelanoma細胞株(MEWO株およびG361株)を研究対象として選んだ。これらの細胞株をモデルとし、Gq変異を加えることで、Melanomaの転移活性への影響を検討するため、まず、それぞれの細胞株におけるGq(GnaqおよびGq11)の遺伝子変異について探索を行なった。それぞれの細胞株から抽出したmRNAを元に、cDNAライブラリーを作成し、これに対して全長のGqに対するPCRを行なった。これをクローニングすることで、これらの遺伝子配列を解析した。その結果、いずれの細胞株においてもDNAレベルでは変異が認められるもののアミノ酸配列には影響を持たなかった。次に、変異を導入するためにゲノム編集を行うこととした。まずはGnaqをターゲットに、R183Q変異の導入を計画した。これに沿って、CRISPR/CAS9を用いたベクターの配列を設計した。同時にssODNの配列(1塩基置換)を設計した。これに沿って、Cas9と同時発現ベクターを構築した。

  • Biological analysis of smooth muscle cells in soft tissue vascular malformation

    脈管奇形は先天性の脈管形成異常である。特に動静脈奇形や静脈奇形では軟部組織の醜状変形にとどまらず、筋・骨格系の変形、病変からの出血など組織破壊的となることもあり、日常生活に著しい制限を生じる。病変の多くは進行性であり、難治病変では進行を止める手段さえない場合がある。動静脈奇形や静脈奇形を含めた脈管奇形の病態生理および悪化の機序にはなお不明な点が多い。われわれは本研究を通じて、脈管奇形のうち特に静脈奇形における悪化機序について、主に「血管平滑筋細胞」に焦点を当てて分子生物学的側面より評価し、本疾患の分類・診断基準作成の補助ならびに疾患治療の確立へ結びつけることを目的とした。本年度は静脈奇形において免疫組織学的手法を用いて、主に形態についての検討を行った。摘出した標本についてHE染色ならびにEVG染色を行い、画像処理技術を用いてBackground subtraction操作を行い、嚢胞部分を含めた病変全体の面積(total area)と、嚢胞部分を除いた断面の面積(non-cystic area)を測定した。加えて、EVG染色画像をもとに膠原線維、弾性繊維、筋繊維が主に染色された結果である赤、紫、黄色の3色でcolour deconvolutionを行い、各色で染色された組織の面積(red stained area, purple stained area, yellow stained area)を測定した。膠原繊維を反映する赤色部位は36.8%、弾性線維を反省する紫部分は23.7%、筋線維を反映する黄色部分は13.9%であった。加えて「血管平滑筋細胞」の細胞培養を中心に行い免疫組織学的検討ならびにMTTアッセイによる増殖能の確認を行い細胞培養法に関する手技の確立を行った。

  • 糖尿病性足潰瘍の骨髄炎の発症には終末糖化産物による微小血管障害が関与するか

    足趾標本を用いた細菌の感染経路解明の研究:骨髄炎を合併した糖尿病性足潰瘍患者から、足趾切断術の際に摘出した12趾に対しグラム染色を施行し、11趾で染色された菌体を確認し、うち10趾は骨端での菌体の存在を確認した。骨端部は成長期の急性化膿性骨髄炎の好発部位として知られているが、成長軟骨板の消失した成人では、どこにでも骨髄炎が生じると考えられている。成人においても骨端部が骨髄炎発症の好発部位であることを示すために今後、より多くの症例でのグラム染色を行い、またグラム染色での明瞭な菌体の証明方法を検討し、施行する予定である。2.足趾の腱におけるAGEの局在の研究:足趾潰瘍で断端形成術を施行した38例38趾(うち糖尿病患者25例)に対し、抗AGE抗体(ab23722, Abcam, Cambridge, UK)用いた足趾腱の免疫組織化学染色を行い、34趾で腱束の間の結合組織内に存在する腱の栄養血管内皮細胞にAGEの沈着を認めた。糖尿患者の方が非糖尿病患者よりAGEを認める割合が高かった(92 vs 84.6%)。これまで腱の脆弱性の原因は、腱への直接的なAGEの蓄積が原因と言われてきたが、我々の仮説通りAGEによる微小血管障害が原因であれば、これまでの定説を覆す可能性がある。AGEは糖尿病だけでなく加齢によっても増加し、AGEによる腱、筋障害は高齢者の肩腱盤断裂や転倒の原因とされている。我々の予測が正しいとすると、糖尿病患者だけでなく加齢によるAGE研究にも大きく影響を与えることになる。今回の症例は高齢者が多く、加齢によってもAGEは増加するため、差が少なかった可能性が高い。症例数を増やし年齢的検討も加えて検討したい。

  • Involvement of stem cells in infantile hemangiomas -Elucidation of proliferation / differentiation control mechanism-

    Infantile hemangioma-derived vascular endothelial cell (IHVEC) samples were cultured primarily by the explant method from the infantile hemangioma spiecemen in the proliferative stage, Total RNA was collected from IHVECs. Similarly, as a control, total RNA was collected from radial artery-derived vascular endothelial cells (RAVECs) and human normal umbilical vein-derived normal vascular endothelial cells (HUVECs), and microarray analysis was performed. As a result, as markers related to vascular growth, VEGFA, PDGFRA and his PDGFRB showed no significant difference in expression between RAVEC and HUVEC, but IHVEC was strongly expressed against RAVEC. Similarly, immunohistological examination showed strong expression in IHVEC.

  • Exploring the Significance of Skin Circadian Rhythm Formation by Light

    We have previously shown that photoreceptor protein (OPN4) is expressed in human skin. OPN4 is involved in the formation of internal clocks. Peripheral circadian clock is usually thought to be reset by a central signal, but we investigated whether it is affected by environmental light. Cortisol was used as the central signal, and 480 nm light was used as the peripheral signal. After stimulation by each, stimulation by the other failed to adjust the rhythm in either case. It was suggested that the rhythm once formed is remembered for a while.

  • Investigation of the light signal conversion mechanism of the skin and the function

    The human skin, unlike other mammals, is mainly hairless and therefore the most receptive to external light in body tissues. We explored whether light signals received by human skin could be converted into intracellular signals and used. Thus far, we have shown that a photoreceptor protein called OPN4 is expressed in the skin. In this study, the light signal received via OPN4 was converted into an intracellular signal, which resulted in 1) an increase in intracellular calcium concentration and 2) a dose-dependent increase in ERK 1/2 phosphorylation, 3) forms peripheral circadian rhythm.

  • Analysis of angiogenesis in soft tissue arteriovenous malformation

    Cultured cells were obtained from arteriovenous malformation specimens obtained from surgical specimens. RNA extracted from these cells was comprehensively examined by microarray and further immunohistochemical study showed higher expression of selectin-P (SELP) than human normal artery-derived endothelial cells and human umbilical vein-derived endothelial cells. In refluxing culture, the growth of the cultured cells did not spread in the flow direction of the culture medium, but rather grew in a reverse manner. SELP seemed to have the high possibility of being concerned in the abnormal blood vessel breeding of AVM of the body surface. It was proven that the flow had some effects on the cell proliferation, because the cell proliferated against the flow of the culture solution.

  • Development of bio-skin, an innovative three-dimensional skin evaluation model with skin appendages

    In this project, we aimed to develop a bioengineered three-dimensional integumentary organ system, which mimics tissue structure and function of the natural skin. 1) We established a technology to regenerate mature hair follicles in vitro and succeeded in constructing a skin organ system (bio-skin) in vitro by assembling the mature hair follicles into functional three-dimensional human skin. 2) The bio-skin constructed in 1) was able to maintain hair growth and sebum secretion, demonstrating its usefulness as an in vitro evaluation model, which have the similar function as a natural skin organ system. 3) We further evaluated the function as an in vitro evaluation model and demonstrated that the constructed bio-skin reproduces the physiological function of natural skin. These results indicated that we have demonstrated the basic technology for clinical application and the applicability of a wide range of organ regenerative medicine.

  • Control of melanoma with irradiation of specific wave length light

    The malignant melanoma is one of the high malignancy tumor occurring on skin. So far, we have shown that a cell of the skin received light from the outside environment through photoreceptor protein. Similarly, photoreceptor protein might be expressed in a melanoma cell and considered the possibility of the control of the cell proliferation by the light irradiation. In this study, we had cleared the following; (1) melanocyte and a melanoma cells express photoreceptor protein, malanopsin, (2) G protein as a second messenger is co-expressed with melanopsin, (3 )Blue light irradiation influenced cytosolic calcium uptake.

  • Relationship of hair growth induction with melanotic nevus

    Melanocytic nevus sometimes exist with bristle. Histologically, hair follicles are surrounded by nevus cells, which indicates the relationship of hair growth induction with melanotic nevus. We found the protein, Melanopsin and Gnaq, expression that was common between an outer root sheath and nevus. This indicates that photo signal has some effect (positive or negative) for hair growth. Cells in outer root sheath and nevus receive the same outer signal, however, it is unclear that those cells share the same signal or no.

  • Micro-anatomy atlas using quantitative protein expression analysis

    Whole rat brain mRNA was extracted and amplified a part of the rat (Wistar rat) Hn-1 gene by the polymerase chain reaction. After performing the sequence analysis, we confirmed that this was known Hn-1 gene with homology. cRNA probe was constructed and in situ hybridization was performed for a rat brain slice after the unilateral facial nerve damage using this cRNA probe. Expression of Hn-1 mRNA was observed at facial nerve nucleus and trigeminal nerve nucleous on the damaged side.

  • The etiology of expanding in vascular malformations -focusing on growth hormone receptor-

    It was reported growth hormone receptor was overexpressed in the vessels of vascular malformations, especially in arteriovenous malformations. Tissue specimens(arteriovenous malformation(AVM), normal artery) were collected from patients undergoing resection or free flap transfer. AVM-derived vascular endothelial cell(AVMVEC) were prrimally cultured by explant method. Immunohistchemistry was used in paraffin‐embedded tissue and AVMVEC. Growth hormone receptor expression was observed in half the paraffin‐embedded AVM tissue, but in no AVMVEC. Proliferative ability of AVMVEC was as the same degree as human umbilical vein endothelial cells. In our study, growth hormone might not contribute to the expansion of arteriovenous malformation. However the proliferative ability of AVMVEC was very high, so some internal or external factor might effect the AVMVEC. Further investigation is needed.

    The expression of Melanopsin in human skin were detected. The localization of its expression were; keratinocyte, melanocyte in epidermis, fibroblast in dermis. Irradiation of 450nm of LED light cause the up regulation of phosphorylation of ERK and degree of phosphorylation were related to the irradiation time. On the other hand, irradiation of 570nm light cause the down regulation of phosphorylation of ERK.Irradiation of both lights causes the more phosphorylation of ERK.

    We analyzed the expression of 5 kinds of prostanoid receptors on lymphatic vessel, TP, IP, EP2, EP4, DP1, which are related to the contraction and extension of vasoactivity. All of these receptors were expressed on lymphatic vessel and patterns of expression were compatible to arteries. Our result suggest that prostanoid mayrelate to the activity of lymphatic vessels.

In our result, hyper osmotic method isthe one of the option for cross-link nerve graft for rat model.

    We developed a novel decellularization method using a hyperosmotic electrolytic solution. The rat abdominal aorta was decellularized and histologically evaluated. These materials, 15 mm in length, were transplanted into the rat abdominal aorta. The hematoxylin and eosin staining and scanning electron microscopic images showed complete decellularization. All transplanted acellular vessels were confirmed to be patent(1 week to 14 months), except in the case of 1 animal, which died 14 months following transplantation. The harvested vessels displayed formation of tunica intima(endothelial cells) and tunica medulla(smooth muscle cells) layers. We also examined the physiological properties of the vessels 3 months after transplantation with a wire myograph system. The transplanted vessels did not react to norepinephrine, but did relax when exposed to sodium nitroprusside. In conclusion, our acellular vessels produced with hyperosmotic electrolytic solution showed extremely high and long-term patency with regeneration of the tunica intima and tunica medulla. In addition, we found that these regenerated acellular vessels possess the physiological properties associated with vascular reflection.

    The small diameter(3mm) conduit consisted with PVA hydrogel was transplanted to rat abdominal artery. Immediately after the transplantation, beating was observed, however, all conduits were occulted in a few days. Next, endothelial cells were co-cultured with PVA sheet. No endothelial cells were adhered to PVA sheet. The collagen fragment was embedded into PVA hydrogel and endothelial cells were co-cultured with rotation. These cells were, then, adhered and migrated. In addition, smooth muscle cells were co-cultures, and these cells also adhered to the collagen-PVA sheet.

    It has been known that silver and it's ions suppress the bacterial growth and works as an anti bacterial reagent. Recently some dressing materials and ointments contains silver ions, however, some reports indicate its toxicity for human cells and may induce the delay of wound healing. Fibroblast growth factor(bFGF) is one of the cytokine that stimulates cell migration and recombinant bFGF is used as an external medicine. The purpose of this study is to confirm if the silver ion act as cytotoxic, if the silver ion affect to the protein such as bFGF. We prepared 4 models for fibroblast culture ; model 1 : only medium with 1% CFS, model 2 : medium with bFGF and 1% CFS, model 3 : medium with bFGF, 1% CFS and silver ions, model 4 : medium with 1% CFS, bFGF which was incubated for 24 hours with silver ions, and silver ions. In this study, firstly, we confirmed that low concentration of silver ions doesn' t affect to cell growth and high concentration of silver ions does. Second, Second, bFGF mitigate the cytotoxic effect of silver ion. Third, the silver ion directly interact with bFGF and interacted bFGF became more harmful to cells.

    We developed novel decellularizing method using hypertonic sodium chloride solution. The histological evaluation for acellularization was performed by HE staining, immunostaining and electron microscope images. The acellular nerve was also transplanted to rats and evaluated histologically. The cells were washed out completely and extracellular matrix was maintained. Two months after the transplantation, Schwann cells thrust into the graft and neurofilament was running through the graft. This acellular nerve presents a useful scaffold for peripheral nerve regeneration.

    The wound is divided to acute wound and chronic wound. The treatment is totally different in between these two types of wound. We focused on the exudates of wound and try to analyze and control it. First, exudates were analyzed with two-dimensional gel electrophoresis and it was cleared that a lots of amount of albumin and IgG were contained in the exudates. To more extensive analysis, we had to eliminate the effect of these proteins. Second, we established a new method of wound treatment which showed a lot of exudates. We continuously supplied the air in to the dressing and brew up the moisture. Under this method, the exudates were well controlled and guided to the cure of intractable chronic wound.

    遊離筋皮弁などによる再建を行うにあたり、筋を支配する運動神経を切断したことに続発する筋の萎縮は思わぬ変形・機能喪失を来たす場合がある。臨床ではこの様な場合において如何に筋の萎縮を抑えることができるか、は重要な課題となる。本研究では動物実験モデルを利用して、いくつかの切断後の神経の修復方法とそれぞれの間での筋体萎縮予防効果に関して検討を行った。知覚神経あるいは端側神経縫合を介した他の運動神経による交叉支配により、脱神経後の筋体量を維持できることが示された。自家遊離移植後の筋体萎縮予防は臨床上、大きな課題となるが、手術手技の単純な工夫によって予防できる可能性が強く示唆された。
