■ Paper- INTRODUCTION: Studies have not sufficiently clarified the differences in citation impact between funded and non-funded clinical research papers. Hence, this study seeks to evaluate the relation between research funding status and clinical research papers' citation impact in different research fields using multiple evaluation indices. METHODS: In this cross-sectional bibliometric study, clinical research papers published by core clinical research hospitals in Japan were compared retrospectively in terms of times cited (TC), category normalized citation impact (CNCI), citation percentile (CP), journal impact factor (JIF), the Software to Identify, Manage, and Analyze Scientific Publications (SIGAPS) category, and whether they were the funded clinical research. The association between research funding status or the SIGAPS category and CNCI ≥ 2 was analyzed using logistic regression analysis. RESULTS: 11 core clinical research hospitals published 553 clinical research papers, of which 120 were non-funded and 433 were funded (public institution-funded and industry-funded). The study found that funded clinical research papers (public institution-funded and industry-funded) had significantly higher TC, CNCI, CP, and JIF than non-funded ones [TC: 8 (3-17) vs. 14 (8-31), p < 0.001; CNCI: 0.53 (0.19-0.97) vs. 0.87 (0.45-1.85), p < 0.001; CP: 51.9 (24.48-70.42) vs. 66.7 (40.53-88.01), p < 0.001; JIF: 2.59 (1.90-3.84) vs. 2.93 (2.09-4.20) p = 0.008], while the proportion of A or B rank clinical research papers of the SIGAPS category was not significantly different between the two groups (30.0 vs. 34.9%, p = 0.318). In the logistic regression analysis, having a CNCI ≥ 2 was significantly associated with research funding (public institution-funded and industry-funded) and publication in A or B rank journals of the SIGAPS category [research funding: Estimate 2.169, 95% confidence interval (CI) 1.153-4.083, p = 0.016; SIGAPS category A/B: Estimate 6.126, 95% CI 3.889-9.651, p < 0.001]. CONCLUSION: Analysis via multiple indicators including CNCI and the SIGAPS category, which allows for a comparison of the papers' citation impact in different research fields, found a positive relation between research funding status and the citation impact of clinical research papers.2022, Frontiers in medicine, 9, 978174 - 978174, English, International magazine[Refereed]Scientific journal

- Nifedipine limits infarct size via NO-dependent mechanisms in dogs.OBJECTIVES: Amlodipine increases NO levels in coronary vessels and aorta via bradykinin-dependent mechanisms in vitro. We have previously reported that nifedipine increases cardiac NO levels in the ischemic canine hearts, suggesting that nifedipine may also have protective effects against ischemia and reperfusion injury, because the enhancement of NO production limits infarct size. We tested whether nifedipine limits infarct size via NO-dependent mechanisms. METHODS: In open chest dogs, the left anterior descending coronary artery was perfused with blood through a bypass tube and occluded for 90 min followed by 6 hours of reperfusion. Infarct size was assessed at 6 hours of reperfusion. Nifedipine of 3 or 6 microg/kg/min was infused into the bypass tube between 10 min prior to the onset of ischemia and 60 min of reperfusion. RESULTS: Neither systemic blood pressure nor heart rate changed during infusion of nifedipine. Infarct size was reduced by the administration of nifedipine (3 or 6 microg/kg/min) compared with the untreated condition (25.6+/-2.6 and 19.1+/-3.5 vs. 43.4+/-5.6%, respectively), which was completely blunted by L-NAME (45.0+/-3.6 and 45.4+/-4.2 vs. 47.9+/-3.9% in the nifedipine (3 or 6 microg/kg/min) with L-NAME groups vs. the L-NAME group). Myeloperoxidase activity of the myocardium increased after 6 hours of reperfusion, which was attenuated by nifedipine. The limitation of infarct size and the attenuation in myeloperoxidase actiivity were completely blunted by L-NAME. There were no significant differences in collateral blood flow at 45 min of ischemia between each group. - Benidipine, a long-acting Ca channel blocker, limits infarct size via bradykinin- and NO-dependent mechanisms in canine hearts.Amlodipine increases NO levels in coronary vessels and aorta via bradykinin-dependent mechanisms in vitro. We have previously reported that a long-acting Ca channel blocker, benidipine, increases cardiac NO levels in ischemic canine hearts, suggesting that benidipine may also protect against ischemia and reperfusion injury via bradykinin- and NO-dependent mechanisms. We examined this possibility. In open chest dogs, the left anterior descending coronary artery was perfused with blood through a bypass tube and was occluded for 90 min followed by 6 hours of reperfusion. Infarct size was assessed by TTC staining at 6 hours of reperfusion. When benidipine doses of 50, 100, and 200 ng/kg/min were infused via the bypass tube between 10 min prior to the onset of ischemia and after 60 min of reperfusion, systemic blood pressure did not change significantly. Infarct size decreased with the administration of benidipine (50, 100, and 200 ng/kg/min) when compared to the untreated condition (24.8+/-2.5, 17.3+/-3.1, and 16.5+/-2.0 vs. 43.4+/-5.6%, respectively) associated with the increased release of NO and bradykinin in the coronary venous blood upon reperfusion. Myeloperoxidase activity of the myocardium increased after 6 hours of reperfusion, which was attenuated by benidipine. The limitation of infarct size and the increase in myeloperoxidase activity were completely blunted by either L-NAME or HOE140. There were no significant differences in collateral blood flow assessed by the microsphere method after 45 min of ischemia for any of the groups. - 先行する短時間心筋虚血 (ischemic preconditioning) は, 長時間虚血により生じる心筋壊死サイズを縮小する.本研究では, 麻酔開胸イヌを用いそのメカニズムを検討した.その結果, 1) ischemic preconditioningによる心筋梗塞サイズ縮小効果には, アデノシン産生酵素活性化が関与する, 2) protein kinase C活性化に加えてATP感受性K+チャネル開口がアデノシン産生酵素活性化に関与する, 3) 細胞膜・ミトコンドリアに存在するATP感受性K+チヤネルが独立して相加的にischemic preconditioningに関与する, ことが明らかになった.以上の結果より, ischemic preconditioningによる心筋保護作用にはアデノシンーアデノシン産生酵素・protein kinase C・ATP感受性K+チャネル開口が連関している可能性が示された.The Japanese Society of Electrocardiology, Mar. 2000, Japanese Journal of Electrocardiology, 20(2) (2), 103 - 111, Japanese

We tested whether IL-33 protects myocardium against HF induced by mechanical overload. Following TAC, myocardial mRNA expressions of Th2 cytokines were diminished while those of Th1 were enhanced in IL-33KO mice than in WT mice. After 8-weeks, IL-33KO mice exhibited exacerbated LV hypertrophy, increased chamber dilation, reduced fractional shortening, aggravated fibrosis and impaired survival compared with WT littermates. Accordingly, myocardial mRNA expressions of fibrogenic (TGF-b1/Collagen3a1) as well as hypertrophic (c-Myc/BNP) molecular markers were also significantly enhanced in IL-33KO mice than those in WT mice. In conclusion, the ablation of IL-33 directly and significantly lead to exacerbate cardiac remodeling with impaired cardiac function and survival upon mechanical stress. These data reveal a potential therapeutic role for IL-33 in non-ischemic HF.

Such structural remodeling occurred in the cell nucleus can be observed as changes in high electron density areas called chromatin by electron microscopy. Alterations in nuclear chromatin structure have been reported some relation with physiological changes. Based on our experimental findings of chromatin remodeling as an upstream of pathological gene expression in cardiomyocytes, we focused on changes in nuclear chromatin structure of cardiomyocytes in patients with dilated cardiomyopathy (DCM). Using our original quantitation methods to analyze the ultrastructure of chromatin, we conducted a retrospective observational study assessing the ability to identify the DCM patients with severe heart failure at risk for poor outcomes, and to make an accurate and an early determination of the indication for some ventricular assist devices to support the failing heart.

In the present study, to clarify the role of ER oxidoreductin-1 (Ero1), a major ER oxidase, in the development of abdominal aortic aneurysm, we have tried and succeeded to create Ero1 knockout mice by ES trap method. Interestingly, we found that Ero1 expression increased in human samples from abdominal aortic aneurysm. We also found that continuous infusion of angiotensin II induced abdominal aortic aneurysm in Ero1 knockout, but not normal, mice, suggesting that Ero1 plays an important role in the development of abdominal aortic aneurysm.
- 日本学術振興会, 科学研究費助成事業, 基盤研究(C), 大阪大学, 01 Apr. 2012 - 31 Mar. 2014ブタ心筋梗塞、心不全モデルでの腎動脈周囲高周波焼灼術による心機能改善効果の検討平成25年度の研究実施計画は、ブタ左右腎動脈内に焼灼を実施し、6か月後の腎動脈および腎動脈周囲の傷害の程度を腎動脈造影および病理学的に評価を行うことである。また、腎動脈周囲の交感神経の傷害の程度を生理的に評価するために昇圧反応の有無からアデノシン(一般名:アデノシン三リン酸二ナトリウム)の至適投与量および投与方法の検討を行うことである。 これらを評価、検討するために、2頭のブタ腎動脈に対して、麻酔下に心臓用の不整脈焼灼カテーテルを用いて1頭目右腎動脈に12時方向を6W1分間、左腎動脈に12時方向を4W1分間、2頭目右腎動脈6時方向4W1分間、左腎動脈6時方向6W1分間にそれぞれ焼灼を行った。2頭とも左右腎動脈の近位部と遠位部に焼灼を行った。左右両腎動脈にアデノシンの急速動脈投与および、アデノシンの持続投与を焼灼6か月後にそれぞれ実施した。すべての手技が終了後、ブタを安楽死させ、腎動脈を摘出し、長軸方向に切開し、焼灼部位周辺部を組織固定した。6か月後、腎動脈造影上、いずれの血管においても狭窄像を認めなかった。ブタを安楽死させた直後に採取した腎動脈において、マクロ像として焼灼部位が白く瘢痕化していた(n=1)。組織標本については現在、作成中のため、今後評価予定。アデノシン10mgをゆっくりまたは急速に腎動脈内投与したところ、10秒後に5秒程度の昇圧反応が観察された。この反応は5mg、20mgの急速腎動脈内投与でも観察された。40mgを腎動脈内に急速投与すると5秒後に昇圧反応が観察されたが、その後、徐脈が出現。心臓に同薬剤が到達したために生じたと考えられた。2頭目の豚ではすべての用量においても昇圧反応は観察されなかった。両腎動脈ともアデノシン1γの持続投与により8分後より7~10mmHg程度の昇圧が観察されたが、アデノシン投与中止後も血圧は低下しなかった。

Thus, the development of new treatment for fluminant myocarditis is medical unmet need. We found that nano-size particles were accumulated in myocarditis hearts. Then, we have developed FK506(an immunosuppressant drug) coated with nano-size liposome. In rat myocarditis model, we found that liposomal FK506 prevented the cardiac dysfunction more than FK506 did. Liposomes are promising for developing new treatments for fluminant myocarditis.
