    Intracranial solitary fibrous tumor (SFT) is a mesenchymal neoplasm that typically appears as a solid mass, whereas cases with predominantly cystic change are extremely uncommon. We report a case of intracranial SFT presenting as a cystic lesion. A 67-year-old woman presented with progressive memory impairment and gait instability over a 6-month period. CT and MRI revealed a mass located along the falx cerebri, composed predominantly of cystic components. The patient underwent surgical tumor resection, and histopathological examination confirmed the diagnosis of SFT, CNS WHO grade 2. Postoperative radiotherapy was administered, and no evidence of recurrence was observed during a 2-year follow-up period. Because SFT is an aggressive tumor with a high risk of recurrence and distant metastasis, accurate diagnosis is essential for appropriate treatment planning and postoperative surveillance. Therefore, SFT should be included in the differential diagnosis of intracranial tumors with cystic morphology.

    OBJECTIVE: To investigate regional 18F-flutemetamol accumulation differences between amyloid-positive (Aβ⁺) and amyloid-negative (Aβ⁻) brains, particularly in cerebral white matter, using a dedicated head PET (dhPET) scanner. METHODS: This retrospective study included 38 consecutive participants who underwent ¹⁸F-flutemetamol PET using a high-resolution dhPET scanner (SET-5002, Shimadzu). Participants were classified as Aβ⁺ or Aβ⁻ based on cortical uptake patterns. Voxels of interest were anatomically defined using OpenMAP-T1, an MRI-based automated brain parcellation software, and merged into 33 regions for SUVR calculation. SUVRs were calculated using the Centiloid whole cerebellar VOIs as the reference region. Group comparisons were assessed using two-way repeated-measures ANOVA, followed by Mann-Whitney U tests with false discovery rate correction. Age and MMSE were compared using nonparametric tests, and gender using Fisher's exact test. RESULTS: The Aβ⁺ (n = 25) and Aβ⁻ (n = 13) groups did not differ in age, gender, or MMSE. ANOVA revealed significant effects of group, VOI, and group×VOI interaction (all p < 0.001). After FDR correction, SUVRs were significantly higher in the Aβ⁺ group across widespread cortical gray matter, cerebral white matter, and subcortical structures, as well as in the choroid plexus and cerebellar gray matter (all q < 0.05). No significant differences were observed in cerebellar white matter, cerebellar peduncle, or brainstem regions. CONCLUSION: High-resolution dhPET revealed significantly higher SUVRs in Aβ⁺ compared with Aβ⁻ subjects in supratentorial structures, including cerebral white matter, and in cerebellar gray matter.

    BACKGROUND: Amyloid-β and tau deposition follow distinct spatial and temporal trajectories across the Alzheimer's disease (AD) continuum. Amyloid accumulation occurs early in the disease course, whereas tau pathology is more closely associated with neurodegeneration and clinical progression. Characterizing stage-specific regional divergence between amyloid and tau deposition may refine biomarker-based disease staging and improve prognostic assessment. METHODS: We analyzed participants from the BATON study who underwent amyloid PET with 18F-flutemetamol, tau PET with 18F-MK-6240, and 3D MRI within a three-month interval. Participants were classified into cognitively normal amyloid-negative controls (CNA, n = 101) and the AD continuum (n = 102), comprising preclinical AD (PCA, n = 47), mild cognitive impairment due to AD (MCA, n = 24), and AD dementia (ADD, n = 31). Standardized uptake value ratios (SUVRs) were calculated using the Centiloid and CenTauR frameworks, and voxel-wise tau-to-amyloid ratio (TAR) images were generated. Voxel-wise and region-of-interest analyses were performed to compare regional patterns of amyloid deposition, tau accumulation, and TAR across disease stages. RESULTS: Amyloid deposition was already widespread at the PCA stage, involving the frontal, posterior cingulate/precuneus, and temporal cortices. In contrast, tau deposition in PCA was largely confined to the medial temporal lobe. With progression to MCA and ADD, tau burden increased substantially and extended to the lateral temporal, parietal, and frontal cortices. TAR analysis demonstrated high values in the medial temporal cortex at the PCA stage, followed by decreasing TAR in frontal regions during MCA and a tau-dominant neocortical pattern in ADD. CONCLUSIONS: Amyloid and tau exhibit distinct and stage-dependent regional dissociation across the AD continuum. TAR imaging effectively captures this divergence, reflecting early medial temporal tau predominance and subsequent neocortical tau expansion. These findings support the utility of integrated amyloid-tau PET metrics for refined disease staging and longitudinal therapeutic monitoring.

    BACKGROUND: Intracranial hemorrhage is life-threatening and requires prompt and accurate diagnosis. Non-contrast head computed tomography is the standard first-line examination, but detecting small hemorrhages and classifying multiple subtypes require substantial expertise. Workforce shortages and increasing diagnostic workloads, especially in emergency settings, further challenge timely decision-making. Artificial intelligence (AI)-assisted interpretation has shown promise for improving accuracy and efficiency. This retrospective study evaluated the effect of AI assistance on the diagnostic performance of radiologic technologists (RTs). METHODS: We analyzed the data for 100 non-contrast head computed tomography examinations (50 positive and 50 negative for hemorrhage) obtained from the Japan Medical Image Database. The interpretations of the five RTs (5-12 years of experience) with and without AI assistance were compared with those of two radiologists. The detection targets were intraparenchymal, intraventricular, subarachnoid, subdural, epidural, and any hemorrhages. We calculated the Area Under the Receiver Operating Characteristic Curve (AUC), accuracy, sensitivity, and specificity. The differences in the AUC for the AI-assisted and unassisted readings were tested using the DeLong method with Bonferroni correction. RESULTS: Significant AUC improvements were observed for five of the 30 reader-task comparisons (17%) after Bonferroni correction. These improvements were all related to intraventricular (p = 0.0001 to 0.0071) and subdural (p = 0.0022 to 0.0071) hemorrhages. CONCLUSION: AI assistance significantly improved RT detection of challenging subtypes such as intraventricular and subdural hemorrhages. However, it did not improve the diagnostic accuracy for detecting any hemorrhage overall (p = 0.0689 to 0.9669). AI can strengthen the role of RTs within task-sharing models and help stabilize preliminary assessments, especially in emergency care and resource-constrained environments.

    Diffuse midline glioma (DMG), H3K27-altered, is an aggressive central nervous system tumor that usually arises in midline structures such as the pons, thalamus, or spinal cord. We present a rare case of intraventricular DMG, H3K27-altered in a 35-year-old woman. MRI showed a soap-bubble appearance, initially suggestive of central neurocytoma, but histopathological and immunohistochemical analyses confirmed the final diagnosis of DMG, H3K27-altered. This is the first reported intraventricular DMG, H3K27-altered with a soap-bubble appearance, underscoring the importance of including this tumor in the differential diagnosis of intraventricular masses.

    Syphilis, a bacterial infection caused by Treponema pallidum, is resurging globally. It is classified as acquired or congenital syphilis. Acquired syphilis progresses through primary, secondary, and tertiary stages. Pathologically, acquired syphilis is characterized by vasculopathy, perivascular infiltrates in syphilitic lesions, and reactive lymphadenopathy. Primary and secondary syphilis frequently manifest with cutaneous and mucosal lesions accompanied by extensive lymphadenopathy at the initial sites of sexual contact, such as the genitalia, oropharynx, and anorectum. These manifestations may mimic malignancies. Tertiary syphilis encompasses cardiovascular syphilis, late neurosyphilis, and gummas, each manifesting distinct imaging characteristics. Most acquired cases are suspected on the basis of dermatologic signs and patient history and are confirmed serologically. Imaging findings alone may not be diagnostic and can overlap with those of other infectious, inflammatory, or neoplastic conditions, necessitating clinical correlation and laboratory testing. However, in patients with atypical clinical presentations, imaging has an important role in providing valuable diagnostic clues and guiding appropriate clinical treatment. The incidence of congenital syphilis is also increasing in parallel with the overall increase in syphilis infections. A comprehensive understanding of the diverse imaging manifestations in multiple organ systems is vital for radiologists to accurately diagnose and guide appropriate management in the era of resurgent syphilis. The authors provide an overview of acquired and congenital syphilis, detailing the epidemiologic and pathophysiologic characteristics, diagnostic approaches, clinical courses, and imaging features. ©RSNA, 2025 Supplemental material is available for this article.

    BACKGROUND: Since the approval of disease-modifying drugs for Alzheimer's disease, the demand for amyloid positron emission tomography (PET) scans, which are crucial for determining treatment eligibility, is expected to increase significantly. We thus investigated the ability of an algorithm to predict amyloid accumulation from 18F-fluorodeoxyglucose (FDG)-PET images for use in amyloid PET screening. METHODS: We analyzed the images of 194 subjects with cognitive disorders who had undergone brain FDG-PET, amyloid PET using Pittsburgh compound-B (11C-PiB), and MRI scans at Kindai University Hospital between 2011 and 2018. Among them, 108 subjects showed positive amyloid accumulation; the other 86 did not. For the 108 positive cases, the input values were the region of interest-based calculated from the automatic anatomical labeling template, which divides the brain into 120 regions, and applied to the anatomically standardized FDG-PET images of each subject. We then used a support vector machine (SVM) machine learning algorithm and conducted a tenfold cross-validation to assess the algorithm's accuracy for predicting amyloid accumulation from FDG-PET images. RESULTS: We observed 81.5% accuracy, 78.5% sensitivity, 84.6% specificity, and an area under the curve (AUC) of 0.846 during training. The validation results for the trained model revealed 85.9% accuracy, 88.4% sensitivity, 81.0% specificity, and an AUC of 0.918. CONCLUSION: These results indicate that the performance of our algorithm to predict amyloid accumulation from 18FDG-PET images is adequate for use in amyloid PET scan screenings.

    Myelin oligodendrocyte glycoprotein antibody-associated disease is a group of central nervous system demyelinating disorders caused by autoantibodies. While myelin oligodendrocyte glycoprotein antibody-associated disease typically presents as optic neuritis and myelitis in adults, this case report details a patient with brainstem lesions. A 45-year-old male presented with episodes of vertigo, nystagmus, and diplopia in left lateral gaze, which had persisted for 2 months, accompanied by headache. Computed tomography showed hyperdensity extending from the left side of the pons to the middle cerebellar peduncle. Magnetic resonance imaging revealed lesions exhibiting heterogeneous diffusion restriction, with enhancement that included granular and linear patterns. 18F-fluorodeoxyglucose positron emission tomography demonstrated increased uptake in these lesions. Following further evaluation, myelin oligodendrocyte glycoprotein antibody-associated disease was diagnosed. Treatment with high-dose corticosteroids initially alleviated symptoms, but symptoms flared upon reduction of the steroids. This case underscores the importance of considering myelin oligodendrocyte glycoprotein antibody-associated disease in the differential diagnosis of brainstem lesions and discusses distinguishing imaging features from similar conditions.

    BACKGROUND: Alzheimer's disease (AD) is characterized by the accumulation of amyloid-β and tau proteins, leading to neurofibrillary tangles. A biomarker-based diagnostic method called the ATN system categorizes AD pathology into amyloid-β (A), tau (T), and neurodegeneration (N). The relationship between regional tau deposition and reduced glucose metabolism in the preclinical AD stage is not well understood. OBJECTIVE: We presented voxel-by-voxel metabolic/tau deposition ratio (MTR) images to investigate the effects of tau deposition on metabolism in AD brains on a stage-by-stage basis. METHODS: We selected 174 subjects who underwent 3D-MRI, FDG-PET, amyloid PET, and tau PET scans. MTR images were created by normalizing FDG-PET toMK6240 PET images. Voxel-wise comparisons among 63 cognitively normal amyloid-negative (CNA) subjects, 49 subjects with AD dementia (ADD), 23 subjects with mild cognitive impairment due to AD (MCA), and 39 preclinical AD (PRC) subjects were conducted. RESULTS: There was reduced glucose metabolism in ADD and MCA groups compared to CNA, predominantly in parietotemporal areas. Tau deposition was observed in wider areas in ADD and restricted to the medial temporal lobes in MCA. MTR exhibited significant reductions in broader regions in ADD and MCA, indicating simultaneous glucose metabolism decrease and tau deposition. At the MCA and PRC stages, glucose metabolism impairment and tau deposition were shown in separate regions by FDG PET and tau PET, respectively, while MTR images showed impairment in both regions. CONCLUSIONS: Our findings suggest that MTR imaging provides insights into AD pathophysiology by simultaneously assessing glucose metabolism and tau deposition. In the early stage of the AD continuum (MCA and PRC), metabolic decline and tau deposition occur independently in different brain regions.

    Symmetrical lesions in the temporal poles and external capsules on brain MRI are known as radiological markers of cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL); however, similar imaging findings have also been reported in neuromyelitis optica spectrum disorder (NMOSD), and this study investigated the frequency of such findings. The study included 55 NMOSD patients who met the 2015 international NMO diagnosis panel (IPND) criteria and were positive for aquaporin-4 antibodies (AQP4-Ab). Images were evaluated based on the consensus of two neuroradiologists, and brain lesions were detected in 33 patients, of whom 2 (6%) had symmetrical lesions in both the temporal poles and external capsules, and 1 (3%) had symmetrical lesions confined to the external capsules. Therefore, when symmetrical lesions in the temporal poles and external capsules are observed on MRI, NMOSD should be considered in the differential diagnosis.

    PURPOSE: Flow-diverter (FD) stents were developed to treat aneurysms that are difficult to treat with conventional coiling or surgery. This study aimed to compare usefulness of Silent MRA and TOF (time of flight) -MRA in patients with aneurysms after FD placement. MATERIALS AND METHODS: We retrospectively collected images from 22 patients with 23 internal carotid artery aneurysms treated with FD. Two radiologists conducted MRA and DSA experiments. In the first reading experiment, the radiologists evaluated the aneurysm filling by employing Silent MRA and TOF-MRA and utilizing the modified O'Kelly-Marotta (OKM) scale, a four-class classification system for aneurysms after FD placement. We then calculated the agreement between the modified OKM scale on MRA and the original OKM scale on DSA. In the second reading experiment, the radiologists rated blood flow within the FD using a five-point scale. RESULTS: The weighted kappa value of the OKM scale between DSA and TOF-MRA was 0.436 (moderate agreement), and that between DSA and Silent MRA was 0.943 (almost perfect agreement). The accuracies for the four-class classification were 0.435 and 0.870 for TOF-MRA and Silent MRA, respectively. The mean score of blood flow within FD for TOF-MRA was 2.43 ± 0.90 and that for Silent MRA was 3.04 ± 1.02 (P < 0.001). CONCLUSION: Silent MRA showed a higher degree of agreement than TOF-MRA in aneurysm filling with DSA. In addition, Silent MRA was significantly superior to TOF-MRA in depicting blood flow within the FD. Therefore, Silent MRA is clinically useful for the follow-up of patients after FD placement.

    PURPOSE: Multiple sclerosis (MS) and neuromyelitis optica spectrum disorder (NMOSD) are two major demyelinating diseases affecting the central nervous system (CNS). The objective of this study is to evaluate the prevalence of pontine trigeminal nerve lesions in patients diagnosed with MS and NMOSD using MRI. METHODS: This retrospective study included patients diagnosed with MS or NMOSD between July 2018 and July 2023. MS patients were clinically diagnosed using the 2017 McDonald criteria, while NMOSD patients were those who met the 2015 International Panel for NMO Diagnosis (IPND) criteria and were positive for Aquaporin-4 Antibody (AQP4-Ab). RESULTS: The study included a total of 90 patients, with 45 diagnosed with MS and another 45 with NMOSD. Pontine trigeminal nerve lesions were observed in both MS and NMOSD, but were more prevalent in MS patients (20 % vs. 2 %, p = 0.008). Root entry zone (REZ) lesions were found in 4 of 45 MS patients, accounting for 9 % (95 % CI: 3 %-17 %), and were absent in the NMOSD group; however, there was no significant difference between the two groups (p = 0.12). Of the MS patients with pontine trigeminal nerve lesions, 6 out of 9 (63 %; 95 % CI, 36 %-98 %) exhibited bilateral lesions, which was significantly more prevalent compared to the NMOSD group (13 % vs. 0 %, p = 0.03). CONCLUSIONS: The presence of pontine trigeminal nerve lesions, particularly when bilateral, are significantly more prevalent in MS patients than in those with NMOSD, suggesting their utility as a distinctive marker and potential diagnostic indicator specifically for MS.

    Solitary fibrous tumor (SFT) is a rare interstitial tumor that originates from various soft tissues, and SFTs occurring within the cranium are extremely rare. While intracranial SFTs with cerebral hemorrhage or subarachnoid hemorrhage have been reported, there have been no reports of intracranial SFTs causing subdural hematoma. In this case, we report on an intracranial SFT accompanied by a subdural hematoma. A 29-year-old female was emergently transported due to the sudden onset of persistent headache and vomiting that began the night before. CT and MRI imaging revealed a hemorrhagic tumor under the tentorium and an acute subdural hematoma extending along the tentorium. The excised tumor was diagnosed as an SFT through histopathological examination. After undergoing radiation therapy, no recurrence has been observed. This is the first case report of an SFT accompanied by a subdural hematoma, and it is vital to recognize that SFTs can be associated with subdural hematomas for proper diagnosis and treatment planning.

    OBJECTIVES: To evaluate a deep learning model for predicting gestational age from fetal brain MRI acquired after the first trimester in comparison to biparietal diameter (BPD). MATERIALS AND METHODS: Our Institutional Review Board approved this retrospective study, and a total of 184 T2-weighted MRI acquisitions from 184 fetuses (mean gestational age: 29.4 weeks) who underwent MRI between January 2014 and June 2019 were included. The reference standard gestational age was based on the last menstruation and ultrasonography measurements in the first trimester. The deep learning model was trained with T2-weighted images from 126 training cases and 29 validation cases. The remaining 29 cases were used as test data, with fetal age estimated by both the model and BPD measurement. The relationship between the estimated gestational age and the reference standard was evaluated with Lin's concordance correlation coefficient (ρc) and a Bland-Altman plot. The ρc was assessed with McBride's definition. RESULTS: The ρc of the model prediction was substantial (ρc = 0.964), but the ρc of the BPD prediction was moderate (ρc = 0.920). Both the model and BPD predictions had greater differences from the reference standard at increasing gestational age. However, the upper limit of the model's prediction (2.45 weeks) was significantly shorter than that of BPD (5.62 weeks). CONCLUSIONS: Deep learning can accurately predict gestational age from fetal brain MR acquired after the first trimester. KEY POINTS: • The prediction of gestational age using ultrasound is accurate in the first trimester but becomes inaccurate as gestational age increases. • Deep learning can accurately predict gestational age from fetal brain MRI acquired in the second and third trimester. • Prediction of gestational age by deep learning may have benefits for prenatal care in pregnancies that are underserved during the first trimester.

    We hypothesized that, in discrimination between benign and malignant parotid gland tumors, high diagnostic accuracy could be obtained with a small amount of imbalanced data when anomaly detection (AD) was combined with deep leaning (DL) model and the L2-constrained softmax loss. The purpose of this study was to evaluate whether the proposed method was more accurate than other commonly used DL or AD methods. Magnetic resonance (MR) images of 245 parotid tumors (22.5% malignant) were retrospectively collected. We evaluated the diagnostic accuracy of the proposed method (VGG16-based DL and AD) and that of classification models using conventional DL and AD methods. A radiologist also evaluated the MR images. ROC and precision-recall (PR) analyses were performed, and the area under the curve (AUC) was calculated. In terms of diagnostic performance, the VGG16-based model with the L2-constrained softmax loss and AD (local outlier factor) outperformed conventional DL and AD methods and a radiologist (ROC-AUC = 0.86 and PR-ROC = 0.77). The proposed method could discriminate between benign and malignant parotid tumors in MR images even when only a small amount of data with imbalanced distribution is available.

    Intracranial myxoid mesenchymal tumors harboring EWSR1 fusions with CREB transcriptional factor gene families were recently described in several case reports and a few case series and this tumor closely resembles the myxoid variant of angiomatoid fibrous histiocytoma. We herein present an intracranial mesenchymal myxoid tumor arising in the third ventricle of a middle-aged woman. The tumor displayed prominent myxoid features consisting of mildly atypical oval to round cells, arranged in reticular and cord-like structures, with starburst-like amianthoid fibers, whereas it lacked pseudoangiomatoid spaces, pseudocapsules and lymphoid cuffing. Immunophenotypically, tumor cells were positive for EMA, desmin, and ALK (focal). EWSR1 and CREB1 rearrangements were identified using FISH assay. The proliferation index was low. It is currently uncertain whether these myxoid tumors represent a variant of angiomatoid fibrous histiocytoma or a novel tumor entity.
