    LESSONS LEARNED: The results of the APPEARANCE trial indicate that adapalene does not prevent acne-like rash over placebo when added to topical moisturizer and oral minocycline but instead may have a detrimental effect. Therefore, adapalene is not recommended as prophylaxis against acne-like rash induced by anti-epidermal growth factor receptor therapies.Given that acne-like rash was completely controlled with placebo in approximately half of patients, predictive measures to identify patients needing intensive prophylaxis are required. BACKGROUND: Anti-epidermal growth factor receptor (EGFR) therapies are frequently associated with acne-like rash. To evaluate the prophylactic efficacy of adapalene, a topical retinoid used as first-line therapy for acne vulgaris, we conducted a randomized, placebo-controlled, evaluator-blinded, left-right comparative trial. METHODS: Patients with non-small cell lung, colorectal, or head and neck cancer scheduled to receive anti-EGFR therapies were randomly assigned to once-daily adapalene application on one side of the face, with placebo on the other side. All patients had topical moisturizer coapplied to both sides of the face, and received oral minocycline. The primary endpoint was the difference in total facial lesion count of acne-like rash at 4 weeks. Secondary endpoints included complete control rate (CCR) of acne-like rash (≤5 facial lesions) and global skin assessment (Investigator's Global Assessment [IGA] scale, grade 0-4) at 4 weeks. Two blinded dermatologists independently evaluated the endpoints from photographs. RESULTS: A total of 36 patients were enrolled, of whom 26 were evaluable. Adapalene treatment was associated with a greater lesion count than placebo at 4 weeks, although the difference was not statistically significant (mean, 12.6 vs. 9.8, p = .12). All four patients with a difference >10 in lesion count between face sides had a greater count on the adapalene-treated side. No significant differences were observed in CCR of acne-like rash (54% vs. 50%) or IGA scale (mean grade, 1.9 vs. 1.7) between the adapalene and placebo sides. CONCLUSION: Adapalene is not recommended as prophylaxis against acne-like rash induced by anti-EGFR therapies.

    Transformation to small cell lung cancer is one phenomenon of acquired resistance to anaplastic lymphoma kinase (ALK) tyrosine kinase inhibitors in ALK rearrangement-positive non-small cell lung cancer (NSCLC). Few case reports have focused on other types of histological transformation. We report a case of transformation of ALK rearrangement-positive adenocarcinoma to NSCLC with neuroendocrine differentiation during alectinib therapy. A 36-year-old woman presented with a tumor in the left lower lobe and bone metastases. She was diagnosed with ALK rearrangement-positive adenocarcinoma by histopathology of the primary tumor. Alectinib had been effective for 8 months before new lesions appeared. Histopathological re-examination of a recurrent tumor revealed poorly differentiated carcinoma with insulinoma-associated protein 1 (INSM1) expression, which remained ALK-positive. Expression of CD133, BCL-2, and SOX2 was positive in comparison to the initial tumor. Expression of SOX2 became more strongly positive than it was before treatment. The immunohistochemical findings of these markers associated with cancer stem-like cells and/or neuroendocrine differentiation suggest that cancer stem cells play a role in the mechanisms of histological transformation and acquired resistance of ALK rearrangement-positive cancer. To our knowledge, this is the first report to suggest an association between cancer stem-like cells and histological transformation in ALK rearrangement-positive lung cancer.

    BACKGROUND/AIM: Immune checkpoint inhibitors (ICI) are a novel medication for non-small cell lung cancer (NSCLC). Recent reports indicated that baseline tumor size (BTS) relates to the efficacy of ICI therapy for melanoma, but no study exists for NSCLC. This study aimed to evaluate the utility of BTS for ICI therapy. PATIENTS AND METHODS: Data from 58 patients diagnosed with NSCLC who underwent ICI monotherapy, were retrospectively analyzed. Patients were divided into two groups according to BTS (below 101 mm, above 101 mm). The primary endpoint was progression-free survival (PFS) and the secondary endpoint was overall survival (OS). RESULTS: PFS of patients with a large BTS was significantly shorter than that of those with a small BTS (median; 2.07 [95% confidence interval [CI]=0.99-6.77] months versus 6.39 [95%CI=4.17-11.50] months) (p=0.044). OS of patients with large BTS was also significantly shorter (p<0.01). CONCLUSION: BTS is a predictive and prognostic negative factor of ICI therapy for NSCLC.

    ROS proto-oncogene 1 receptor tyrosine kinase (ROS1)-rearranged lung cancer is rare and comprises only 1% of lung adenocarcinoma cases. It has recently been reported to have good response to crizotinib, a tyrosine kinase inhibitor of anaplastic lymphoma kinase. Driver oncogene mutations with approved therapies seldom coexist with a high expression of Programmed death-ligand 1 (PD-L1). The present case report describes a rare case of ROS1 rearrangement with high-PD-L1-expressing occult lung adenocarcinoma. A 32-year-old woman presented with chest pain and a prolonged cough. Chest computed tomography (CT) revealed a 57×36-mm tumor in the mediastinum, with no tumors detected in other regions. Positron emission tomography (PET)-CT showed a strong fluorodeoxyglucose accumulation in the tumor (SUVmax 13.2). Mediastinal tumor resection was completely resected using a video-assisted thoracic surgery approach. Pathological examination showed the tumor cells were positive for thyroid transcription factor 1, Napsin-A, ROS1, and PD-L1 (tumor proportion score >99%). ROS1 rearrangement was confirmed by fluorescence in situ hybridization. The mediastinal tumor was diagnosed as mediastinal lymph node metastasis of ROS1-rearranged PD-L1 high-expression undifferentiated lung adenocarcinoma (pathological stage 3, TxN2M0). Two months after the operation, the CT scan showed multiple mediastinum lymph nodes metastases with rapid tumor growth. The patient achieved a complete response after three cycles of S-1 plus cisplatin with concurrent radiotherapy 60 Gy/30 Fr.

  • Bronchoscopy Using Virtual Navigation and Endobronchial Ultrasonography with a Guide Sheath (EBUS-GS) with or without Fluoroscopy for Peripheral Pulmonary Lesions

    OBJECTIVE: Endobronchial ultrasonography and guide sheath (EBUS-GS) technique has high diagnostic yield in lung nodules. Virtual bronchoscopic navigation (VBN) can lead bronchoscope to the target bronchi. The aim of this prospective study was to compare the diagnostic yield of two bronchoscopic procedures: bronchoscopy under EBUS-GS and VBN with or without x-ray fluoroscopy in small peripheral pulmonary lesions (PPLs, ≤30mm) with apparent CT-bronchus sign. METHODS: 31 patients with PPLs which had apparent CT-bronchus sign were randomly assigned to the X-ray or the non-X-ray groups (18 with and 13 without fluoroscopy) between September 1, 2012, and September 30, 2015. A bronchoscope was introduced into the target bronchus using the VBN system. Sites of specimen sampling were verified using EBUS-GS with or without fluoroscopy. RESULTS: The overall diagnostic yield was 83.3% in the X-ray and 69.2% in the non-X-ray group. The diagnostic yield of malignancy was 88.2% and 81.8%, respectively. The duration of the examination and time elapsed until the first EBUS visualization were similar in the X-ray and the non-X-ray group (9.0 (5.8-20.) min vs 11.0 (5.3-17.3) min, and 2.5 (1.3-14.2) min vs 4.1 (1.4-8.1) min, respectively). The fluoroscopy exposure time was 3.7 (2.9-10.56) min. The only adverse event was mild pneumothorax in a patient from the non-X-ray group, who had consequent TBB under fluoroscopy. CONCLUSIONS: There was a possibility that VBN-guided EBUS-transbronchial diagnosis without fluoroscopy might be equivalent to that under fluoroscopy. Further multi-center randomized study may be desired. (UMIN000008592).

    We report herein on a case of allergic bronchoalveolar aspergillosis (ABPA) with hyphae and Charcot-Leyden crystals against a necrotic background stained with Papanicolaou stain. Lung carcinoma was initially suspected before bronchoscopic brush cytology. Lung carcinoma or tuberculosis is sometimes suspected first in cases of aspergillosis. The pathological diagnosis is important because the seroprevalence and culture-positive rate are very low, and degenerated or vacuolated Aspergillus hyphae do not stain well with Papanicolaou stain. Careful observation of cytological specimens in a necrotic background is therefore required, considering the possibility of aspergillosis.

    背景.肺末梢小型病変に対する仮想気管支鏡ナビゲーションシステムの有用性は広く知られてきているが,肺門・縦隔気管支壁外病変に対する穿刺ガイドとしての報告はない.症例. 49歳,女性.呼吸困難を自覚して近医を受診した.胸部造影CTにて左肺動脈に接して腫瘍が認められたため,当科紹介受診となった.気管支鏡検査では腫瘍は気管支に表出しておらず,腫瘍が存在すると考えられたB^<1+2>c背側部を穿刺したが,病理学的な診断に至らなかった.再検として仮想気管支鏡ナビゲーションシステムを併用した経気管支針吸引(TBNA)を行うこととした.仮想気管支鏡ナビゲーションでは腫瘍はB^<1+2>a入口部上背側に位置しており,同部位を穿刺したところ,出血も少なく,組織も採取でき,肺MALT(mucosa-associated lymphoid tissue)リンパ腫の確定診断をしえた.結論.肺門・縦隔気管支壁外病変に対する経気管支鏡的診断は,腫瘍の存在部位が同定できないため困難となる場合が多い.仮想気管支鏡ナビゲーションシステムは,肺門・縦隔気管支壁外病変に対するTBNAの診断支援にも有用であると考えられた.

    背景.肺末梢病変に対する気管支鏡検査において,ガイドシース併用気管支腔内超音波断層法(EBUS-GS)の使用が普及し始めている.しかし,気管支擦過と経気管支生検の施行順に決まりはない.目的.EBUS-GSを使用した気管支鏡検査において,擦過と生検のどちらを先行させたほうが診断率が高いかにつき,前向きに検討した.対象と方法.対象はEBUS-GSで病変の描出を確認し,擦過および生検を施行した病変.検体採取順は期間で区切り,コホートA:擦過のあとに生検(2007.6.1〜2008.3.31),コホートB:生検のあとに擦過(2008.4.1〜2009.1.31)とした.診断率を評価項目とし,付随的に検体採取の順番が,擦過細胞診の適切標本作製に影響を与えるかにつき検討した.結果.コホートAでは,EBUS-GSを使用した病変は81病変あり,うちEBUS描出可能であった病変は72病変(EBUS描出率:88.9%),かつ擦過および生検を施行した病変は69病変であった.同様にコホートBでは,EBUS-GS使用病変は71病変,描出病変は68病変(95.8%),両者施行病変は66病変であった.最大径の平均値はコホートA,Bとも28.6mmであった.全135病変の診断率は83.0%(112/135病変),全悪性疾患の診断率は90.8%(108/119病変)であった.悪性病変119例の生検診断率・擦過診断率・両者合わせた診断率はそれぞれ,コホートA:77.2%,78.9%,87.7%,コホートB:85.5%,87.1%,93.5%であり,有意差はみられなかった.また,検体採取の順は細胞診の標本作製および診断に影響を与えなかった.結論.EBUS-GSを使用した気管支鏡検査において,EBUSで病変が描出できれば,擦過・生検の施行順は診断率に影響しない.

    Lung cancer is the leading cause of cancer death worldwide. Molecular targeted therapy has greatly advanced the field of treatment for non-small cell lung cancer (NSCLC), which accounts for the majority of lung cancers. Indeed, gefitinib, which was the first molecular targeted therapeutic agent, has actually doubled the survival time of NSCLC patients. Vigorous efforts of clinicians and researchers have revealed that lung cancer develops through the activating mutations of many driver genes including the epidermal growth factor receptor (EGFR), anaplastic lymphoma kinase (ALK), c-ros oncogene 1 (ROS1), v-Raf murine sarcoma viral oncogene homolog B (BRAF), and rearranged during transfection (RET) genes. Although ALK, ROS1, and RET are rare genetic abnormalities, corresponding tyrosine kinase inhibitors (TKIs) can exert dramatic therapeutic effects. In addition to anticancer drugs targeting driver genes, bevacizumab specifically binds to human vascular endothelial growth factor (VEGF) and blocks the VEGF signaling pathway. The VEGF signal blockade suppresses angiogenesis in tumor tissues and inhibits tumor growth. In this review, we also explore immunotherapy, which is a promising new NSCLC treatment approach. In general, antitumor immune responses are suppressed in cancer patients, and cancer cells escape from the immune surveillance mechanism. Immune checkpoint inhibitors (ICIs) are antibodies that target the primary escape mechanisms, immune checkpoints. Patients who respond to ICIs are reported to experience longlasting therapeutic effects. A wide range of clinical approaches, including combination therapy involving chemotherapy or radiation plus adjuvant therapy, are being developed.

    Treatment with epidermal growth factor receptor tyrosine kinase inhibitors (EGFR-TKIs) improves the overall survival of patients with EGFR-mutated non-small-cell lung cancer (NSCLC). First-generation EGFR-TKIs (e.g., gefitinib and erlotinib) or second-generation EGFR-TKIs (e.g., afatinib and dacomitinib) are effective for the treatment of EGFR-mutated NSCLC, especially in patients with EGFR exon 19 deletions or an exon 21 L858R mutation. However, almost all cases experience disease recurrence after 1 to 2 years due to acquired resistance. The EGFR T790M mutation in exon 20 is the most frequent alteration associated with the development of acquired resistance. Osimertinib-a third-generation EGFR-TKI-targets the T790M mutation and has demonstrated high efficacy against EGFR-mutated lung cancer. However, the development of acquired resistance to third-generation EGFR-TKI, involving the cysteine residue at codon 797 mutation, has been observed. Other mechanisms of acquired resistance include the activation of alternative pathways or downstream targets and histological transformation (i.e., epithelial⁻mesenchymal transition or conversion to small-cell lung cancer). Furthermore, the development of primary resistance through overexpression of the hepatocyte growth factor and suppression of Bcl-2-like protein 11 expression may lead to problems. In this report, we review these mechanisms and discuss therapeutic strategies to overcome resistance to EGFR-TKIs.

    第98回肺癌学会近畿地方会, 2013年02月, 日本語, 日本肺癌学会, 大阪, 【背景】 傍腫瘍性神経症候群(paraneoplastic neurological syndrome: PNS)は担癌患者に生じる、自己免疫学的機序によると考える神経障害である。PNSの頻度は約1％で、肺小細胞癌患者に頻度が高いとされている。PNSによる症状は多彩であるが、今回は肺小細胞癌に合併した癌関連網膜症の2例に関して報告する。【症例1】 80歳女性。左視力低下で発症し、その後右目の視力の低下もきたした。網膜電図で網膜全体に著しい電位の低下を認め癌関連網膜症が疑われ、全身精査にて肺癌と診断された。【症例2】 68歳男性。左視力低下で眼科を受診し、自己免疫性網膜炎を疑われて全身精査を行った結果肺小細胞癌と診断された。, 国内会議

    第5回肺機能イメージング研究会学術集会, 2013年01月, 日本語, 肺機能イメージング研究会, 徳島, 【背景】肺末梢病変に対する仮想気管支鏡ナビゲーションシステム（Bf-NAVI, OLYMPUS）の有用性は知られてきている．またBf-NAVIは肺門・縦隔病変に対する穿刺ナビゲーションの機能も有している．今回，我々は，肺末梢病変の診断および中枢病変に対する経気管支診断において，Bf-NAVIが有用であった症例を経験したので報告する．【症例1】72歳, 男性．食道癌・喉頭癌の既往があり，歯肉癌の精査加療目的に入院中，右S1胸膜直下に9×7mm大の結節が認められ，気管支鏡検査を施行した．Bf-NAVIにて5次分岐まで確認し，生検施行，原発性肺腺癌と診断した．【症例2】49歳，女性．呼吸困難感を主訴に来院．胸部CTにて左肺動脈に接して28mm大の結節を認め，気管支鏡検査を施行した．可視範囲に腫瘍の表出はなく，X線透視下にて穿刺を行ったが，出血のみで確定診断, 国内会議

    第80回 日本呼吸器学会畿地方会, 2012年12月, 日本語, 呼吸器学会近畿地方会, 神戸, 2011年4月から2012年3月までの間に当院呼吸器内科入院中に死亡した26例の検討を行った。原疾患は悪性腫瘍が22件、間質性肺炎が4件であった。死因は原疾患の進行に伴うものが21例で、治療関連死と考えられるものが5例であった。3例でＮＰＰＶの使用が行われたが、喀血に対する気道確保目的で挿管された1例をのぞいては、挿管下での人工呼吸器管理は行われなかった。ミダゾラムの使用は11件。オピオイドの使用は21件。持続的深い鎮静を行った症例は6件であった。間質性肺炎の終末期についてもオピオイドや鎮静薬の使用を必要とする症例が多く、非癌呼吸器疾患についても苦痛緩和のための鎮静に関するガイドラインが必要と考えられた。, 国内会議

    第92回日本気管支内視鏡学会近畿支部会, 2012年12月, 日本語, 日本呼吸器内視鏡学会, 大阪, 【背景と目的】当院では2011年6月より，肺末梢病変に対し，仮想気管支内視鏡ソフトウェア（Bf-NAVI，オリンパスメディカルシステムズ）を併用して気管支内視鏡検査を施行している．今回，Bf-NAVIを用いて気管支鏡検査を施行した症例について，有効性の検討を行うとともに，その使用経験を報告する．【結果】病変は30病変で，病変の平均長径は21.4mm（8-42mm）であった． 21例で確定診断（肺癌19例，炎症性疾患2例）がつき，診断率は70%であった．細胞診は全症例で施行され，診断率は66％（20/30例）であった．組織診は25例で施行され，診断率は60％（15/25例）であった．また、30例中20例はガイドシース併用気管支腔内超音波断層法（EBUS-GS）を併用した．うち，EBUS-GSを併用し病変が描出できた症例は18症例であり，診断率は72％（, 国内会議

  • Bf-NAVI併用によるTBNAにて確定診断しえた肺MALT Lymphomaの一症例

    第35回日本呼吸器内視鏡学会学術集会, 2012年05月, 日本語, 日本呼吸器内視鏡学会, 東京, 症例は49才、女性。平成20年12月頃に呼吸困難感を自覚し近医受診。胸部造影CTにて左肺動脈内の血栓を疑われ、肺塞栓症の診断にて抗凝固療法を開始された。その後の経過中症状は消失し、経過観察されていたが平成22年1月に再度呼吸困難感が出現。再検査の結果左肺動脈内の血栓様陰影が残存していたため精査目的に当院当科紹介となる。各種画像やその他検査所見から肺血栓塞栓症は否定的と考え、左肺動脈に近接する腫瘍を疑ったため気管支鏡検査を施行するも病理学的に有意な所見を得られず。腫瘍は肺動脈に近接したB1+2c入口部背側の粘膜下付近に存在し、再検として盲目的なTBAC針による細胞採取の危険性を鑑み、仮想気管支ナビゲーションシステム（Bf-NAVI OLYMPUS）を用いて腫瘍の位置、TBAC針の穿刺部位を設定した上で再度気管支鏡によるアプローチを試み細胞を採取、確定診, 国内会議
