■ Paper- PURPOSE: Postoperative pain management is critical for psychological and physical patient outcomes. In April 2022, the Japanese government introduced a new Acute Pain Service (APS) fee addition into the health care reimbursement system to promote structured APS implementation. This study aims to describe the current status of APS addition and explore its association with patient and hospital outcomes. METHODS: We conducted a retrospective study using a nationwide claims database. Patients who underwent surgery with general anesthesia from April 2022 to June 2023 were included. Surgical procedures were categorized into 15 departments or classified as "mixed," and APS addition claim frequency was summarized. Propensity score matching and multivariable regression analyses were performed. RESULTS: A total of 14,022 patients with APS addition and 551,315 without were included. The relative frequency of APS addition was 2.48%, with the highest in esophageal surgery (11.8%). APS addition claims gradually increased over time. Propensity score matching showed no apparent differences in total hospitalization costs or length of stay between groups. However, significant interactions were observed between APS addition and patient age. Among 90-year-old patients, the estimated mean cost reduction [95% confidence interval (CI)] was 4.6% [2.1%, 7.0%] for males and 4.7% [2.4%, 6.9%] for females, while the estimated mean [95% CI] reduction in length of stay was 6.7% [3.0%, 10.3%] and 8.9% [5.6%, 12.1%], respectively. CONCLUSION: Although the overall effects of APS addition were minimal, age-stratified analyses suggest that it may be associated with improved outcomes among older patients. - PurposeThis study aims to evaluate causal relationships among eating rate, caloric intake, and weight gain using longitudinal data and to clarify how caloric intake mediates the effect of eating rate on weight gain.DesignA longitudinal cohort study with a 4-year follow-up.SettingData were collected from Daihatsu Co, Ltd employees, a major Japanese automobile manufacturer with 4 plants in Japan.SubjectsOf the 12,816 working-age Japanese individuals who underwent health checkups in 2020, 7,784 without excess weight or non-communicable diseases were included.InterventionThe eating rate was self-reported, and caloric intake was calculated based on cafeteria meal choices.MeasuresAnnual change in body mass index (BMI), calculated as the difference in BMI between consecutive years.AnalysisMediation analyses using a mixed-effects model were conducted to assess whether caloric intake mediates the effect of eating rate on BMI.ResultsCaloric intake in the fast-eating and ordinal-eating groups exceeded that of the slow-eating group by 19.21 kcal [9.28, 29.13] and 10.01 kcal [1.20, 18.81], respectively. Higher caloric intake was associated with a more significant BMI increase, rising by .09 [.06, .13] per interquartile range increase. The proportion of mediated impact among the impact of eating rate is .19% [.09, .32].ConclusionFaster eating independently contributes to BMI increases, with minimal mediation by caloric intake. - Abstract While generative artificial intelligence (AI) has shown potential in medical diagnostics, comprehensive evaluation of its diagnostic performance and comparison with physicians has not been extensively explored. We conducted a systematic review and meta-analysis of studies validating generative AI models for diagnostic tasks published between June 2018 and June 2024. Analysis of 83 studies revealed an overall diagnostic accuracy of 52.1%. No significant performance difference was found between AI models and physicians overall (p = 0.10) or non-expert physicians (p = 0.93). However, AI models performed significantly worse than expert physicians (p = 0.007). Several models demonstrated slightly higher performance compared to non-experts, although the differences were not significant. Generative AI demonstrates promising diagnostic capabilities with accuracy varying by model. Although it has not yet achieved expert-level reliability, these findings suggest potential for enhancing healthcare delivery and medical education when implemented with appropriate understanding of its limitations.Springer Science and Business Media LLC, Mar. 2025, npj Digital Medicine, 8(1) (1)[Refereed]Scientific journal

- OBJECTIVES: No bowel preparation for colonoscopy achieves optimal efficacy and tolerability. Combining polyethylene glycol plus ascorbic acid (PEG-Asc) with adjuvants has been explored to enhance cleansing efficacy and reduce the required volume. This study aimed to evaluate whether adding 0.5 mg linaclotide to 1-L PEG-Asc (1 L-PEG/AL) improves superior cleansing compared to adding 24 mg senna (1 L-PEG/AS), which we previously reported to be non-inferior to the standard regimen of 2 L PEG-Asc. METHODS: A multicenter, endoscopist-blinded, randomized controlled trial was conducted at five centers in Japan with outpatients scheduled for colonoscopy. The primary outcome was adequate bowel preparation (BP), evaluated using the Boston Bowel Preparation Scale. Analyses were stratified based on low- and high-risk groups for inadequate BP. RESULTS: Between April 2022 and April 2023, 1,464 patients were randomized to the 1L-PEG/AL (n=733) or 1L-PEG/AS (n=731). The 1L-PEG/AL demonstrated higher adequate BP rates (92% vs. 86%, P=0.001) compared to 1L-PEG/AS. Both groups experienced similar adverse events and expressed high willingness for repeat BP (83% vs. 81%, P=0.49). In high-risk patients (n=892), 1L-PEG/AL had significantly higher adequate BP rates (94% vs. 86%, P<0.001), whereas rates were comparably high in low-risk patients (n=539) (94% vs. 94%, P=0.66). CONCLUSIONS: The linaclotide regimen was superior to the senna regimen in terms of BP efficacy without reducing tolerability. - AIM: We aimed to compare which suture devices could reduce the incidence of incisional surgical site infections (SSIs) after gastrointestinal surgery using a systematic review and network meta-analysis. METHODS: The CENTRAL, PubMed, and ICHUSHI-Web databases were searched from January 1st, 2000, to December 31st, 2022, for randomised clinical trials (RCTs) comparing the incidence of incisional SSI after gastrointestinal surgery among patients treated with different surgical suture devices, including non-absorbable sutures, absorbable sutures, skin staplers, and tissue adhesives (last searched in August 23th 2023). The risk of bias was assessed using the criteria of the Cochrane Handbook for Systematic Reviews of Interventions. To estimate the pooled odds ratios (ORs) for each comparison, we employed a fixed-effect inverse-variance model based on the Mantel-Haenszel approach. This study was registered in PROSPERO and supported in part by the Japan Surgical Infection Society. RESULTS: A total of 18 RCTs with 5,496 patients were included in this study. The overall SSIs in absorbable sutures were significantly lower than the ones in skin staplers (OR: 0.77; 95% confidence intervals (CI): 0.63-0.95) and non-absorbable sutures (OR: 0.62; 95% CI: 0.39-0.99). On the other hand, SSIs in absorbable sutures were not statistically different from the SSIs in tissue adhesive. The highest P-score was 0.91 for absorbable sutures. A funnel plot for estimating the heterogeneity of the studies revealed that a publication bias would be minimal (Egger test, P = 0.271). - Estimation of individualized treatment effects (ITE), also known as conditional average treatment effects (CATE), is an active area of methodology development. However, much less attention has been paid to the quantification of uncertainty of ITE/CATE estimates in right-censored survival data. Here we undertake an extensive simulation study to examine the coverage of interval estimates from two popular estimation algorithms, Bayesian additive regression trees (BART) and causal survival forest (CSF). We conducted simulation designs from 3 different settings: first, in a setting where BART was developed for an accelerated failure time model; second, where CSF was developed; and finally, a ``neutral'' simulation taken from a setting where neither BART nor CSF was developed. BART outperformed CSF in all three simulation settings. Both the BART and CSF algorithms involve multiple hyperparameters, and BART credible intervals had better coverage than the CSF confidence intervals under the default values, as well as under optimized values, of these hyperparameters.Lead, 06 Apr. 2025

- 日本学術振興会, 科学研究費助成事業 若手研究, 若手研究, 大阪市立大学, Apr. 2020 - Mar. 2022, Principal investigator機械学習による自動的変数選択をもちいた新しい治療効果推定手法の提案因果推論における局外関数の推定に機械学習手法を応用することで,高次元の研究対象者の特性(共変量)を考慮できるというメリットがある一方,関心のある処置の効果推定に必要な情報の削減,局外関数の過適合によって平均処置効果推定量にバイアスが生じるデメリットがあることが知られている.これらのデメリットを低減させるために,計量経済学分野においてdouble/debiased machine learning(DML)推定量が提案された.このDMLは,従来の機械学習を用いた因果推論推定量に比べて優れた性質をもつと考えられるが,臨床研究に応用する際には課題が残る.処置変数に関する局外関数である傾向スコアモデルを推定する際に,lassoなど従来の機械学習手法を利用した場合,処置にのみ関連の強い因子(操作変数)が選択される可能性がある.従来の機械学習で操作変数の影響を強く考慮した傾向スコアモデルは処置変数を良く説明するが,関心のある平均処置効果推定量の分散やバイアスを増加させる恐れがある.臨床研究では操作変数など先験的に特定することは現実的に困難である.従って操作変数が存在するような状況においても,変数選択により頑健な推定量が利用できることが好ましい. 本年度は,上記の問題を改善すべく,以下の2点を目的として研究を行った. (目的1)結果変数に強く関連する因子を優先的に考慮した傾向スコアを利用しつつ,DML推定量の漸近的性質を損なうことがないoutcome-adaptive DML推定量を提案し,そのパフォーマンスを評価すること. (目的2)上記提案手法の臨床観察研究へ応用すること.

The users can experience the thinking process in statistical analysis, which consists descriptive analysis, univariate analysis, multivariate analysis. We examined the effect of this tool on learning statistical analysis when using in the program for learning clinical research. As results, test score about statistical analysis was not significantly improved by using this tool. However, the proportion of participants who completed the lecture of statistical analysis was increased compared to those without using this tool. Therefore, it was suggested that this tool can enhance the effect on learning of statistical analysis by reducing the feeling of difficulty for learning it.
