■ Paper- RATIONALE & OBJECTIVES: Cognitive function is impaired in patients with chronic kidney disease, particularly in those undergoing hemodialysis, although the precise mechanisms are not well understood. This cross-sectional study examined the possible relationship between serum electrolyte levels and cognitive function in patients receiving maintenance hemodialysis. STUDY DESIGN: Cross-sectional study. SETTINGS & PARTICIPANTS: Eligible patients were selected from the participants of the Osaka Dialysis Complication Study, a multicenter study conducted in Japan. EXPOSURES: Serum sodium, potassium, and chloride levels. OUTCOMES: Cognitive function assessed by the Modified Mini-Mental State examination (3MS) which was mathematically transformed to 3MS' to follow a normal distribution. ANALYTICAL APPROACH: Multivariable-adjusted linear regression analysis. RESULTS: For this study, 1,207 patients were selected. Their median age, 3MS score, serum sodium, potassium, and chloride concentrations were 67 (IQR, 60-74) years, 91 (IQR, 82-97) points, 139 (IQR, 137-141) mEq/L, 4.7 (IQR, 4.2-5.2) mEq/L, and 102 (IQR, 99-105) mEq/L, respectively. After adjustment, sodium (P = 0.23) or potassium (P = 0.38) were not associated with the 3MS score, whereas serum chloride was positively associated with the 3MS score (3MS', regression coefficient 0.100 [0.062, 0.138] per 1 IQR (6 mEq/L), P < 0.001) independently of potential confounders. The positive association between serum chloride and cognition was consistent in subgroup analysis. The difference in the 3MS score by 1 IQR of chloride was comparable to that of the difference by 7 years of age. LIMITATIONS: Causality cannot be determined because of the cross-sectional design. - The correct diagnosis of tetrahydrobiopterin (BH4, sapropterin dihydrochloride)-responsive phenylketonuria (PKU) and treatment with BH4 are important for prognosis and quality of life. We examined whether age affects biopterin bioavailability following oral BH4 administration in PKU and whether this influences BH4 responsiveness. A retrospective analysis was conducted in 255 Japanese PKU patients who underwent a 24-h BH4 loading test (mostly ≤ 2 months old) and/or a 1-week test (all ≥ 5 months) between 2008 and 2023. Correlations were evaluated among age, peak blood biopterin, and the phenylalanine (Phe) reduction rate. In the 24-h test, analyses of day-of-age versus peak biopterin after oral BH4 showed that peak levels were highest during the early neonatal period and declined significantly with age (p = 0.008). In the 1-week test, peak levels increased during school age through adolescence (6-19 years) (p = 0.001), with no material age trend in adults (≥ 20 years). In the 24-h test, peak biopterin correlated positively with the Phe reduction rate (p = 0.029), and baseline Phe correlated negatively with peak biopterin (p = 0.001). These findings indicate that BH4 loading tests performed in infants and preschool-aged children (1 month-5 years)-in whom biopterin peaks tend to be low-or in patients with high baseline Phe levels may yield suppressed biopterin peaks and false-negative results. - BACKGROUND: Antenatal administration of magnesium sulfate (MgSO4) is employed for fetal neuroprotection, and its extended use for tocolysis remains prevalent in certain regions. The impact of MgSO4 on neonatal gastrointestinal function is contentious, with evidence indicating variability across subpopulations. METHODS: A single-center retrospective cohort study examined preterm infants with a birth weight below 1500 g, admitted to the neonatal intensive care unit at the University of Fukui Hospital, Japan, between April 2015 and October 2023. Among 141 eligible infants, 83 received antenatal MgSO4 within 1 month before delivery, while 58 did not. Exposure was defined as duration, cumulative dose, and neonatal serum magnesium concentration. The primary outcome was time to achieve full enteral feeding (100 mL/kg/day). Associations were estimated using Cox proportional hazards models with inverse probability weighting to evaluate the effects of gestational and maternal age. RESULTS: Antenatal MgSO4 exposure was not significantly associated with time to full enteral feeding (HR, 0.80; 95% CI, 0.59-1.10). Multivariable weighted models suggested heterogeneity of association across gestational and maternal age strata (P for interaction < 0.001). HRs increased with gestational age in each maternal age stratum, suggesting antenatal MgSO4 exposure was more associated with delayed full enteral feeding at earlier gestational ages, especially among older mothers. CONCLUSIONS: Antenatal MgSO4 exposure was not significantly associated with delayed enteral feeding, and gestational and maternal ages may explain the conflicting results. - OBJECTIVES: To quantify the short-term impact of an unexpected stay-at-home exposure caused by the shipment suspension on body mass index (BMI) and cardiometabolic markers among employees with different levels of occupational physical activity. METHODS: Health-check records from 8307 workers at a large Japanese automobile manufacturer were linked to company attendance data covering the shipment suspension (January-April 2024). An interrupted time-series assessed BMI trajectories before, during and after the halt. Among 614 employees who underwent an additional examination in April 2024, mixed-effects models related the duration of stay-at-home to changes in BMI and blood pressure within low-, medium- and high-intensity job categories. RESULTS: Compared with pre-halt trends, medium-intensity and high-intensity workers showed significant level rises (0.96 kg/m2; 95% CI 0.56-1.36 and 0.64 kg/m2; 95% CI 0.24-1.04, respectively) at the onset of the suspension.. Mixed-effects analyses showed a positive dose-response between the duration of stay-at-home and BMI gain in high-intensity jobs (0.47 kg/m2 per 20% absent days; 95% CI 0.37-0.58. Per 20% of scheduled workdays absent, systolic blood pressure was higher in the medium- and high-intensity groups. No significant effects were observed among sedentary workers. CONCLUSIONS: Employees whose daily energy expenditure relies on job-related physical activity are especially susceptible to weight gain and blood-pressure elevations during forced work interruptions. - Few studies have investigated the distribution of blood pressure (BP) and associated factors in pediatric patients with X-linked hypophosphatemic rickets (XLH). We analyzed snapshot baseline data from the SUNFLOWER study, a longitudinal observational cohort study of patients with XLH in Japan and South Korea (NCT03745521/UMIN000031605). We used data from pediatric participants aged 5-17 years who were 120-189.9 cm (males) and 120-179.9 cm (females) in height and had a history of conventional treatment. Systolic and diastolic BP (SBP and DBP, respectively) were categorized into percentile ranks based on age, sex, and height. Ordinal logistic regression analyses were performed to investigate the association between BP and exposure factors, including estimated glomerular filtration rate, serum intact parathyroid hormone levels, serum intact fibroblast growth factor 23 levels, urinary calcium/creatinine ratio, and body mass index standard deviation score (BMI-SDS). Forty-five participants were eligible for the subgroup analysis. Of these, 44 were evaluated after one patient with missing BP data was excluded. After height adjustment, three patients (6.8%) were at or above the 95th percentile for SBP, and five (11.4%) were at or above the 95th percentile for DBP. Regarding age adjustment, one patient (2.3%) was at or above the 95th percentile for SBP and three (6.8%) were at or above the 95th percentile for DBP. In the association analysis, age- and height-adjusted BP was positively correlated with BMI-SDS. - OBJECTIVES: No bowel preparation for colonoscopy achieves optimal efficacy and tolerability. Combining polyethylene glycol plus ascorbic acid (PEG-Asc) with adjuvants has been explored to enhance cleansing efficacy and reduce the required volume. This study aimed to evaluate whether adding 0.5 mg linaclotide to 1-L PEG-Asc (1 L-PEG/AL) improves superior cleansing compared to adding 24 mg senna (1 L-PEG/AS), which we previously reported to be non-inferior to the standard regimen of 2 L PEG-Asc. METHODS: A multicenter, endoscopist-blinded, randomized controlled trial was conducted at five centers in Japan with outpatients scheduled for colonoscopy. The primary outcome was adequate bowel preparation (BP), evaluated using the Boston Bowel Preparation Scale. Analyses were stratified based on low- and high-risk groups for inadequate BP. RESULTS: Between April 2022 and April 2023, 1,464 patients were randomized to the 1L-PEG/AL (n=733) or 1L-PEG/AS (n=731). The 1L-PEG/AL demonstrated higher adequate BP rates (92% vs. 86%, P=0.001) compared to 1L-PEG/AS. Both groups experienced similar adverse events and expressed high willingness for repeat BP (83% vs. 81%, P=0.49). In high-risk patients (n=892), 1L-PEG/AL had significantly higher adequate BP rates (94% vs. 86%, P<0.001), whereas rates were comparably high in low-risk patients (n=539) (94% vs. 94%, P=0.66). CONCLUSIONS: The linaclotide regimen was superior to the senna regimen in terms of BP efficacy without reducing tolerability. - AIM: We aimed to compare which suture devices could reduce the incidence of incisional surgical site infections (SSIs) after gastrointestinal surgery using a systematic review and network meta-analysis. METHODS: The CENTRAL, PubMed, and ICHUSHI-Web databases were searched from January 1st, 2000, to December 31st, 2022, for randomised clinical trials (RCTs) comparing the incidence of incisional SSI after gastrointestinal surgery among patients treated with different surgical suture devices, including non-absorbable sutures, absorbable sutures, skin staplers, and tissue adhesives (last searched in August 23th 2023). The risk of bias was assessed using the criteria of the Cochrane Handbook for Systematic Reviews of Interventions. To estimate the pooled odds ratios (ORs) for each comparison, we employed a fixed-effect inverse-variance model based on the Mantel-Haenszel approach. This study was registered in PROSPERO and supported in part by the Japan Surgical Infection Society. RESULTS: A total of 18 RCTs with 5,496 patients were included in this study. The overall SSIs in absorbable sutures were significantly lower than the ones in skin staplers (OR: 0.77; 95% confidence intervals (CI): 0.63-0.95) and non-absorbable sutures (OR: 0.62; 95% CI: 0.39-0.99). On the other hand, SSIs in absorbable sutures were not statistically different from the SSIs in tissue adhesive. The highest P-score was 0.91 for absorbable sutures. A funnel plot for estimating the heterogeneity of the studies revealed that a publication bias would be minimal (Egger test, P = 0.271). CONCLUSION: This study showed that absorbable sutures reduced incisional SSIs in gastrointestinal surgical operations compared to any other suture devices.Jun. 2024, The Journal of hospital infection, English, International magazine[Refereed]Scientific journal

- Generalisable prediction model of surgical case duration: multicentre development and temporal validationBackground: Accurate prediction of surgical case duration underpins operating room (OR) scheduling, yet existing models often depend on site- or surgeon-specific inputs and rarely undergo external validation, limiting generalisability. Methods: We undertook a retrospective multicentre study using routinely collected perioperative data from two general hospitals in Japan (development: 1 January 2021-31 December 2023; temporal test: 1 January-31 December 2024). Elective weekday procedures with American Society of Anesthesiologists (ASA) Physical Status 1-4 were included. Pre-specified preoperative predictors comprised surgical context (year, month, weekday, scheduled duration, general anaesthesia indicator, body position) and patient factors (sex, age, body mass index, allergy, infection, comorbidity, ASA). Missing data were addressed by multiple imputation by chained equations. Four learners (elastic-net, generalised additive models, random forest, gradient-boosted trees) were tuned within internal-external cross-validation (IECV; leave-one-cluster-out by centre-year) and combined by stacked generalisation to predict log-transformed duration. Results: We analysed 63,206 procedures (development 45,647; temporal test 17,559). Cluster-specific and pooled errors and calibrations from IECV are provided with consistent performance across centres and years. In the 2024 temporal test cohort, calibration was good (intercept 0.423, 95%CI 0.372 to 0.474; slope 0.921, 95%CI 0.911 to 0.932). Conclusions: A stacked machine-learning model using only widely available preoperative variables achieved accurate, well-calibrated predictions in temporal external validation, supporting transportability across sites and over time. - Estimation of individualized treatment effects (ITE), also known as conditional average treatment effects (CATE), is an active area of methodology development. However, much less attention has been paid to the quantification of uncertainty of ITE/CATE estimates in right-censored survival data. Here we undertake an extensive simulation study to examine the coverage of interval estimates from two popular estimation algorithms, Bayesian additive regression trees (BART) and causal survival forest (CSF). We conducted simulation designs from 3 different settings: first, in a setting where BART was developed for an accelerated failure time model; second, where CSF was developed; and finally, a ``neutral'' simulation taken from a setting where neither BART nor CSF was developed. BART outperformed CSF in all three simulation settings. Both the BART and CSF algorithms involve multiple hyperparameters, and BART credible intervals had better coverage than the CSF confidence intervals under the default values, as well as under optimized values, of these hyperparameters.Lead, 06 Apr. 2025

- Abstract Background The rapid advancement of generative artificial intelligence (AI) has led to the wide dissemination of models with exceptional understanding and generation of human language. Their integration into healthcare has shown potential for improving medical diagnostics, yet a comprehensive diagnostic performance evaluation of generative AI models and the comparison of their diagnostic performance with that of physicians has not been extensively explored. Methods In this systematic review and meta-analysis, a comprehensive search of Medline, Scopus, Web of Science, Cochrane Central, and MedRxiv was conducted for studies published from June 2018 through December 2023, focusing on those that validate generative AI models for diagnostic tasks. The risk of bias was assessed using the Prediction Model Study Risk of Bias Assessment Tool. Meta-regression was performed to summarize the performance of the models and to compare the accuracy of the models with that of physicians. Results The search resulted in 54 studies being included in the meta-analysis. Nine generative AI models were evaluated across 17 medical specialties. The quality assessment indicated a high risk of bias in the majority of studies, primarily due to small sample sizes. The overall accuracy for generative AI models across 54 studies was 56.9% (95% confidence interval [CI]: 51.0–62.7%). The meta-analysis demonstrated that, on average, physicians exceeded the accuracy of the models (difference in accuracy: 14.4% [95% CI: 4.9–23.8%], p-value =0.004). However, both Prometheus (Bing) and GPT-4 showed slightly better performance compared to non-experts (-2.3% [95% CI: -27.0–22.4%], p-value = 0.848 and -0.32% [95% CI: -14.4–13.7%], p-value = 0.962), but slightly underperformed when compared to experts (10.9% [95% CI: -13.1–35.0%], p-value = 0.356 and 12.9% [95% CI: 0.15–25.7%], p-value = 0.048). The sub-analysis revealed significantly improved accuracy in the fields of Gynecology, Pediatrics, Orthopedic surgery, Plastic surgery, and Otolaryngology, while showing reduced accuracy for Neurology, Psychiatry, Rheumatology, and Endocrinology compared to that of General Medicine. No significant heterogeneity was observed based on the risk of bias. Conclusions Generative AI exhibits promising diagnostic capabilities, with accuracy varying significantly by model and medical specialty. Although they have not reached the reliability of expert physicians, the findings suggest that generative AI models have the potential to enhance healthcare delivery and medical education, provided they are integrated with caution and their limitations are well-understood. Key Points Question:What is the diagnostic accuracy of generative AI models and how does this accuracy compare to that of physicians? Findings:This meta-analysis found that generative AI models have a pooled accuracy of 56.9% (95% confidence interval: 51.0–62.7%). The accuracy of expert physicians exceeds that of AI in all specialties, however, some generative AI models are comparable to non-expert physicians. - In any observational study where an exposure of interest, such as pharmaceutical treatment, is not randomly assigned to subjects, a bias is often introduced in estimating the true effect of treatment. This is because patients who receive treatment usually have more severe medical conditions than those without treatment. Failure to control such inherent bias in patient characteristics when assessing for the true effect of treatment across comparison groups, may lead to confounding. The presence of confounding makes it difficult to evaluate the true treatment effect. - 日本学術振興会, 科学研究費助成事業, 若手研究, 大阪市立大学, Apr. 2020 - Mar. 2023, Principal investigator機械学習による自動的変数選択をもちいた新しい治療効果推定手法の提案因果推論における局外関数の推定に機械学習手法を応用することで,高次元の研究対象者の特性(共変量)を考慮できるというメリットがある一方,関心のある処置の効果推定に必要な情報の削減,局外関数の過適合によって平均処置効果推定量にバイアスが生じるデメリットがあることが知られている.これらのデメリットを低減させるために,計量経済学分野においてdouble/debiased machine learning(DML)推定量が提案された.このDMLは,従来の機械学習を用いた因果推論推定量に比べて優れた性質をもつと考えられるが,臨床研究に応用する際には課題が残る.処置変数に関する局外関数である傾向スコアモデルを推定する際に,lassoなど従来の機械学習手法を利用した場合,処置にのみ関連の強い因子(操作変数)が選択される可能性がある.従来の機械学習で操作変数の影響を強く考慮した傾向スコアモデルは処置変数を良く説明するが,関心のある平均処置効果推定量の分散やバイアスを増加させる恐れがある.臨床研究では操作変数など先験的に特定することは現実的に困難である.従って操作変数が存在するような状況においても,変数選択により頑健な推定量が利用できることが好ましい. 本年度は,上記の問題を改善すべく,以下の2点を目的として研究を行った. (目的1)結果変数に強く関連する因子を優先的に考慮した傾向スコアを利用しつつ,DML推定量の漸近的性質を損なうことがないoutcome-adaptive DML推定量を提案し,そのパフォーマンスを評価すること. (目的2)上記提案手法の臨床観察研究へ応用すること.

The users can experience the thinking process in statistical analysis, which consists descriptive analysis, univariate analysis, multivariate analysis. We examined the effect of this tool on learning statistical analysis when using in the program for learning clinical research. As results, test score about statistical analysis was not significantly improved by using this tool. However, the proportion of participants who completed the lecture of statistical analysis was increased compared to those without using this tool. Therefore, it was suggested that this tool can enhance the effect on learning of statistical analysis by reducing the feeling of difficulty for learning it.
