    目的:高齢者の運動器障害疾患の罹患率は高く,下肢関節疾患は要介護の原因の上位を占めている。手術療法は疼痛や歩行機能改善への有効性が示されているが,高齢患者でのQOL改善や医療費経済効果の術後継続性は明らかでない。本研究では変形性膝・股関節症患者の関節手術療法5年以上経過後の医療費,QOL,介護状況を,保存療法継続患者と比較し明らかとすることを目的とした。 方法:対象者はA病院整形外科外来を受診した80歳から85歳までの変形性膝関節症,変形性股関節症患者とし,5年以上前に手術をした手術療法群(14名)と保存療法継続群(13名)を対象とした。最新2年間の患者医療費,患者QOL(SF-8),介護保険の利用状況の比較検討を行った。 結果:手術療法群の42.9%,保存療法群の15.4%が独歩可能であり,介護保険の利用は手術療法群で57.1%,保存療法群で69.2%であった。QOLスコアは手術療法群が保存療法群より有意に良く,最新2年間の年間医療費も手術療法群が低い結果であった。 考察:術後5年以上経過した手術療法群のQOLは保存療法群より良く,医療費も低かったことより,手術治療により数年間の高齢者の健康改善効果が継続する可能性が示唆された。

    In Japan mountainous areas’ population aging is more pronounced than in urban areas. Therefore, supporting community sustainability through health maintenance is critical. This study examines whether participating in community events leads to subjective health. We focused on the Yamanouchi area of Yumesaki-cho, Himeji City, Hyogo Prefecture, which is mountainous area and where population aging and depopulation is serious. With the theme of “town development for longevity and health,” multiple universities are participating in practical research projects in this area, such as a new health checkup, “Health Checkup Project through adding points to Goodness in Daily Life.” We hypothesized that joining community events actively and concerning to have connections with people outside their area, such as university students, lead good health. In 2019 and 2021, we conducted questionnaire surveys targeting all households in the Yamanouchi area and clarified the effects of joining community events and knowing local events related to university students on subjective health. We consider the endogeneity of variables related to events and conducted empirical analysis based on extended ordered probit model. Our empirical results supported our hypothesis. We found that inhabitants who join more community events or those who know of more events with university students tend to evaluate their health more favorably, and the effects were statistically significant. Thus, we deduce that actively planning community events will be beneficial for inhabitants’ health in the area. In addition, it is possible that university students’ participation in community development is effective for inhabitants’ health. In the mountainous areas where the population is decreasing, continuing community events could prove difficult, but it would be important to consider their impact on health and encourage various events as far as possible.

    Purpose: The Yoitoko check-up, a novel health check-up providing positive feedback, has been developed to promote health among older adults, and consists of several comprehensive geriatric assessment items. This report aimed to describe the details of the Yoitoko check-up and to explore the future possibility of the check-up by evaluating the participants' short-term behavioral changes in terms of comprehensive functioning, using a before-after study design. Patients and Methods: Four Yoitoko check-ups were conducted, at 3-month intervals, between December 2018 and September 2019. Study participants aged ≥65 years included those who had undergone ≥2 Yoitoko check-ups. The results of each visit after the second check-ups were retrospectively compared with those of the baseline, and the mean changes and the odds ratios were calculated using a paired t-test or a McNemar test, respectively. Results: Of 84 participants, the results of 16 (19.0%) participants were analyzed. The mean (standard deviation) age was 75.3 (4.7) years. The mean Tokyo Metropolitan Institute of Gerontology Index of Competence score, a measure of high-level functional capacity, increased 0.9 (95% confidence interval; range, 0.2-1.5) points between the first and second visits. Conclusion: We developed the Yoitoko check-up and introduced the details of it. Our study findings suggested that the Yoitoko check-up may further motivate older adults to improve their health and promote positive behavioral changes. Future studies are needed to evaluate the effectiveness of this novel assessment method.

    今日の日本では地方創生を掲げ中央政府や地方自治体,そして集落レベルでの様々な地域振興が行われている。本研究は中山間の限界集落である姫路市夢前町山之内地区を対象とした。全戸アンケート調査と集落訪問調査を行ったうえで,当該地域における現在までの経済社会の変遷,現在の人口構成から考えられる将来の地域課題,民間事業者が中心となって行っている「夢前花街道事業」の成果,こうした課題や活動を反映したうえでの地域住民による山之内地区の将来の展望を示した。

    本研究は日本の保健医療指標の提出数の増加を目的として,未報告の保健医療指標を調査し,既存の公的統計やレセプト情報・特定健診等情報データベース(NDB)を活用して推計を行う.OECD,WHO,国連等の国際機関は,医療および介護分野における政策立案に資する国際統計報告として様々な保健医療指標の迅速な提供を各国に求めている.各国から提供された国際統計報告は,例えばOECDのOECD.stat1)から一般公開されているが,日本が国際機関に提出している厚生労働統計分野の項目数は少なく,提出件数を増やすことが望ましい.

    In Japan, the increasing number of patients needing emergency medical care due to population aging is a major public health problem. Recently, emergency medicine in Japan has seen a growth in the number of Dedicated Emergency Physician Model style departments. We aimed to determine whether there is an association between Dedicated Emergency Physician Model emergency care and pre-hospital transportation time. We conducted a secondary analysis of a Japanese national pre-hospital database from 2010 to 2014. Three regions (group 1: Urayasu city and Ichikawa city in Chiba prefecture, group 2: Kamakura city, Chigasaki city, Fujisawa city and Zushi city in Kanagawa prefecture, and group 3: Fukui city in Fukui prefecture) were evaluated as Dedicated Emergency Physician Model emergency medicine areas. We compared transportation times in these areas with all municipalities in the same prefectures, and with a nearby area using multivariate linear regression with cluster adjustment. The variables used for adjustment are the time from Emergency Medical Services activation to the scene, month, day of the month, day of the week, time of day, age, gender, type of injury, severity, and location of call. Compared with all municipalities in each prefecture there were significant reductions in pre-hospital transportation time: 4.2 minutes (95% confidence interval, 0.9 to 7.5, p<0.05) in Group 1, 6.2 minutes (95%CI, 2.9 to 9.6, p<0.01) fin Group 2 and 7.5 minutes (95%CI, 6.0 to 9.0, p<0.01) in Group 3. Compared with nearby areas, there were statistically significant reductions in transportation time in Group 1, 6.8 minutes (95%CI, 0.7 to 12.8, p<0.05) and in Group 2, 6.8 minutes (95%CI, 3.7 to 9.9, p<0.05). There was a trend for reduced transportation time in Group 3, 2.3 minutes, (5.3 to -0.6, p<0.1). Areas with a Dedicated Emergency Physician Model are associated with reduced pre-hospital transportation time.

    IntroductionTransportation time by ambulance is one of the important factor of evaluating emergency medical service. But, the previous researches about the relation of transportation time and socioeconomically condition by district is insufficient.ObjectivesWe estimate an average transportation time and a ratio of daytime to nighttime in a transportation time in each district using various district socioeconomically indicators as independent variables.MethodThe dataset includes 3,538,275(71.9%) emergency patients who used emergency transportation by ambulance in 2011 and 587 districts. The independent variables for each district included a geographical component variable (population density and so on), ratio of transportation to another district, ratio of public hospital to emergency hospital, ratio of emergency patients to population, population aging rate and traffic congestion. We estimate two indicator by these independent variables for OLS.ResultFor the estimated transportation time (Adj.R-Squared = 0.294 P<.001), ratio of transportation to another district (P<.01), population aging rate (P<.01) and traffic congestion (P<.001) showed positive correlations. Geographical component variable (P<.001) and ratio of public hospital (P<.001) showed negative correlations. For the estimated a ratio of daytime to nighttime in a transportation time (Adj.R-Squared = 0.308 P<.001), geographical component variable (P<.001), ratio of transportation to another district (P<.001) and traffic congestion (P<.1) showed positive correlations, while ratio of public hospital (P<.01) showed negative correlations.ConclusionsThe result shows that a transportation time is longer in rural than urban and a ratio of daytime to nighttime in a transportation time is higher in ur ban than rural. These are the geographic subjects in emergency medical system. And, a transportation time and a ratio of daytime to nighttime are more influenced supply side factor rather than demand side. To solve the subjects, supply of emergency medical for elderly people is needed, because elderly people who is one of a cause of the subjects is increasing year by year in Japan.

    We collated "receipt data" with a questionnaire to "operators and employees" and clarified that harassment in the workplace environment is related to mental health impairment through analysis. It was also revealed that mental health is also related to lifestyle (Sleep Time), family composition by machine learning methods. It has been clarified that efforts to aim for "health management" for improving the workplace environment will lead to motivation for the work of employees and the intention to leave their jobs. In the Comprehensive Geriatric Assessment Check-up Business" in the mountainous area, we built our own health check-up votes to increase motivation for flail prevention and health promotion, and implemented "Yoitoko Health Check-up", a Novel Health Check-Up Providing Positive Feedback to Older Adults, and through this business, we clarified empirically the improvement of health promotion and motivation to participate in community activities.

    This study examines the status of economic evaluation, quality of life (QOL), and care of osteoarthritis patients undergoing surgery and conservative therapy. The subjects, 80- to 85-year-old patients with osteoarthritis of the knee and hip, were orthopedic outpatients in Hospital. In total, 54 older patients (43 women, mean age of 82.6 years) were considered and divided into three groups: surgery before five years (S1) or within five years (S2) and conservative therapy (NS). In the S1 and NS groups, 42.9% and 15.4% of the patients could walk independently. The QOL score of the physical component was 48.0 and 38.8 in the S1 and NS groups respectively. In the S2 group there was no significant difference in medical costs before and after surgery. However, medical costs decreased for 15 patients (55.6%).
