    Abstract Background Competency is used to channel abilities into successful processes and is employed in the medical field. Globally, several laboratory competencies exist, but the job descriptions of Japanese medical laboratory scientists differ from those of other countries and little evidence-based information on novice medical laboratory scientist competency is available in Japan. This study clarified the competencies of novice medical laboratory scientists based on various expert opinions in Japan. Methods The Delphi method was used to achieve an expert consensus on novice medical laboratory scientist competencies. We asked the participants to evaluate the importance of each item using the Likert scale and set 70% as the final consensus rate. Results We obtained 106/400 (26.5%) and 95/106 (89.6%) responses from participants in rounds 1 and 2, respectively. Their professional experience mean ± standard deviation was 32.4 ± 6.0 years (range: 13–41). The average of each category consensus rate was > 99.1%. Ninety-five expert opinions converged and agreed that the competency comprised 8 categories and 54 items. Conclusions The survey results revealed that novice medical laboratory scientists were expected to have relatively higher main laboratory skill competencies in the ‘Preparation and analysis’ category than in other categories. Nevertheless, competencies in other categories required basic skills. In addition, our competencies contained unique competencies compared with others due to their divergent roles and their environment. Further research is warranted to explore assessment tools by developing a competency scale, thereby helping clarify the differences between ability and correlated factors. The unique competencies scale can help assess the efficacy of educational programmes for Japanese medical laboratory scientists.

    Abstract Background Underage smoking and drinking are public health issues in Vanuatu. This study aims to describe the behavior, knowledge, attitudes, and perceptions of parents, siblings, and peers regarding smoking and drinking among urban and rural public-school students in Vanuatu. Methods This cross-sectional study included 358 students (urban, 217; rural, 141; aged 12–14 years) from the public schools in Efate Island, Vanuatu. Data were collected using self-administered questionnaires. Chi-square and Mann–Whitney U tests were used to determine the rural–urban differences. Results Urban students showed a higher prevalence of ever smoking (13.5%), ever drinking (16.9%), intention to smoke (11.1%), and intention to drink (14.0%) compared to rural students (10.3%, 8.3%, 5.8%, and 9.5%, respectively); although a significant difference was only observed in the prevalence of ever drinking. Urban students were more likely to be aware of the health hazards of substance use and showed higher self-efficacy to refuse tobacco and alcohol compared to rural students. Parents in rural areas were less likely to talk about the health hazards of substance use with their children and were more likely to offer tobacco or alcohol to them compared to parents in urban areas. Conclusions The results provide evidence of rural–urban differences in the behavior, attitude, knowledge, and perceptions of parental behavior regarding smoking and drinking. The findings suggest that issues related to underage smoking and drinking differ between urban and rural students. Future intervention programs for reducing underage smoking and drinking should be adapted in recognition of urban and rural differences.

  • Factors Related to the Intake of Store-bought Foods among Upper-Grade Primary School Children in Vanuatu

    Objective: To identify the factors related to the intake of store-bought foods in upper-grade primary school children in Vanuatu. Method: The targets of this study were sixth-, seventh-, and eighth-grade students attending primary schools in both urban and rural areas in Vanuatu. We conducted a self-administered questionnaire survey. The main questionnaire items included intake of store-bought foods, knowledge related to non-communicable diseases (NCDs), health practice attitudes, and guardians' health-related parenting attitudes. Relationships between scores for store-bought food items and each variable were analyzed using a Mann-Whitney U-test and Spearman's rank correlation coefficient. Factors related to store-bought food intake were analyzed using multiple regression analysis. The significance level was set at p<0.05. Results: More than 90 % of the targets of this study consented to participate in the questionnaire survey. The questionnaires were distributed only to the targets who agreed, and 415 questionnaires were distributed (194 in urban areas and 221 in rural areas). All 415 questionnaires were collected and admitted as valid responses. The mean store-bought food intake score was 8.9 (SD=3.1) of a maximum possible score of 16. The univariate analysis shows that residential area (p<0.001), finding local foods delicious (p<0.05) and favorite (p<0.05) ; finding store-bought foods delicious (p<0.01), favorite (p<0.001), and healthy (p<0.05); perception scores for store-bought foods (p<0.001); and scores for household possessions (p<0.01) were significantly associated with the intake of store-bought foods. Discussion: The findings suggest that living in urban areas, disliking local foods, enjoying store-bought foods, and having economic power promote the intake of store-bought foods.

    Objective
    To identify the lifestyles of upper grade primary school students in urban and rural areas of Vanuatu
    Methods
      All 415 students (urban, 194; and rural, 221) from 6th, 7th, and 8th grade students of primary school from one in the urban area and three in the rural area of Vanuatu participated in this survey. We conducted a self-administered questionnaire survey that included items addressing food consumption, exercise, smoking and drinking experience, health knowledge, attitudes toward health practices, guardians' parenting attitudes related to health, prevalence of family employment, and number of household possessions. Urban and rural areas were compared on each item by performing chi-square tests for categorical data and Mann-Whitney U tests for ordinal data. Analysis was conducted manually and subsequently checked using SPSS version 18 for Windows. The significance level was set at p < .05.
    Results
      A response rate of 100% was obtained from a total target sample students. All responses were valid. Comparisons between urban and rural areas revealed that consumption frequencies of rice, bread, fresh meat, soft drinks, and sweets were higher in urban areas, while root crops, fresh fish, and fruits were consumed more frequently in rural areas. The percentage of students who had ever tried alcohol was significantly higher in urban areas than in rural areas, as were levels of health knowledge related to noncommunicable diseases, favorable health attitudes, the availability of health advice from guardians, the number of family members employed, and the number of household possessions.
    Conclusion
      The results indicated that significant differences exist between urban and rural areas for frequency of food consumption, drinking experience, health knowledge, attitudes toward health practices, guardians' parenting attitudes related to health, the prevalence of family employment, and household possessions.

    Objectives To identify factors related to smoking and consumption of alcohol and kava in children attending the upper grades of primary schools in Vanuatu.
    Methods We conducted a self-administered survey of 6th, 7th, and 8th grade students attending primary schools in both urban and rural areas of Vanuatu. The main survey items included questions on the personal attribute (sex, age, grade); experience of smoking and consumption of alcohol and kava; food consumption (local food/store-bought food); perceptions of local foods and store-bought foods; attitudes toward smoking and consumption of alcohol and kava; knowledge related to non-communicable diseases; attitudes toward health practices; guardians' health-related parenting attitudes; and family members' use of tobacco, alcohol, and kava.
    The responses for the main outcome variables (smoking and consumption of alcohol and kava) were dichotomized as 'ever' versus 'never'. Factors related to smoking and consumption of alcohol and kava were examined using logistic regression analysis. The significance level was set at P<0.05.
    Results A total of 415 (194 urban and 221 rural) students participated in our study that had total and valid response rates of 100% for both. Of the participants, 8%, 12.4%, and 5.8% had previously smoked, consumed alcohol, or consumed kava, respectively. Students' experience of smoking and consumption of alcohol and kava were mutually associated. Student sex and family members' smoking status were significantly associated with the participants' smoking status. Student grades, attitudes toward drinking, and perceptions of local and store-bought food were significantly associated with alcohol consumption. Lastly, attitudes toward kava and alcohol consumption and perceptions of local food were significantly associated with kava consumption.
    Conclusion Our results indicate that the food consumption, attitudes toward smoking and consumption of alcohol and kava, and family members' smoking status were associated with the participants' smoking and consumption of alcohol and kava. In conclusion, it may be necessary to consider these factors when establishing measures to prevent smoking and consumption of alcohol and kava among primary school students.

  • Re-establishment Motivation in Patients with Long-term Diseases Living at Home, Focusing on Parkinson's Disease and Spinocerebellar Degeneration in Early-stage

    In order to establish effective public health nurse support, the present study assessed motivation during the early stage in patients with Parkinson's disease or spinocerebellar degeneration living at home; 7 patients were interviewed individually in a semi-structured manner. Based on verbatim transcriptions of the interviews, qualitative and descriptive analyses were performed. The results indicated that, among the 9 subcategories of patients, including "patients accept their current disease status and situation," "patients continue to maintain a relationship with others and society," and "patients attempt to carry out their role and responsibilities as much as possible," the patients demonstrated 3 levels of motivation: "As the disease worsens, the patient actively deals with the situation," "The patient is involved socially and interacts with others," and "The patient tries to achieve self-realization by discovering the meaning of the disease." The early-stage patients tried to understand the reality of having an intractable disease and attempted to cope with the presence of the disease through different approaches. These results suggest that such patients should be supported through symptom management, identifying the limitations imposed by their impaired functions, creating opportunities for interaction with others, including patients with the same disease, and by helping the patients find new meaning in their lives, so as to be able to live with their disease. It is necessary to conduct a comprehensive assessment of these patients' motivation. Continuous support measures should be implemented for in-home patients in the early stage of illness, based on the content and level of their motivation.
