- INTRODUCTION: COVID-19 emergency declarations and ensuing lockdowns affected lifestyle and glycemic control in individuals with diabetes mellitus, with some studies finding improvement and others deterioration of glucose management. The effects of such emergency declarations in Asian populations, including individuals with type 1 diabetes (T1D) managed with a sensor-augmented pump (SAP), have been unclear, however. We here investigated the impact of a COVID-19 emergency declaration on glycemic control in Japanese individuals with T1D treated with a SAP device. METHODS: This retrospective investigation included individuals with T1D who were managed with SAP technology and had continuous glucose monitoring including sensor glucose levels and time above range, time in range [TIR], and time below range and insulin dose data available in electronic health records from February to July 2020, corresponding to periods before, during, and after the declared emergency. RESULTS: The 78 enrolled individuals had a mean age of 44.6 years, diabetes duration of 15.4 years, and hemoglobin A1c level of 7.3%. Average sensor glucose levels improved from 152.0 mg/dL before to 148.0 mg/dL during and 147.6 mg/dL after the emergency. TIR increased from 68.8% before to 71.3% during and 71.4% after the emergency. Total daily insulin dose decreased from 40.9 U before to 39.6 U after the emergency. After the emergency declaration, individuals with an initial TIR of ≤ 70% showed increased sensor adherence as well as a greater improvement in glycemic control compared with those with a TIR of > 70%. CONCLUSION: Individuals with T1D treated with a SAP device showed improved glycemic management after the COVID-19 emergency declaration. - OBJECTIVE: To investigate the effect of visualizing blood glucose variability by intermittently scanned continuous glucose monitoring (isCGM) on weight reduction in overweight individuals with impaired glucose tolerance (IGT) or mild type 2 diabetes mellitus (T2DM). MATERIALS AND METHODS: Forty overweight (BMI, ≥25 kg/m2) individuals with IGT or T2DM (drug naïve; HbA1c, ≤7.0 %) were included in this 24-week randomized controlled trial. Participants were randomly assigned to the control group (diet and exercise therapy) or the isCGM group (diet and exercise therapy plus isCGM). The primary endpoint was the change in body weight during the 24-week intervention period. RESULTS: One participant in the isCGM group withdrew consent. We therefore analyzed 19 individuals in the isCGM group and 20 in the control group. Baseline BMI was significantly higher in the isCGM group (35.2 ± 5.7 kg/m²) compared to the control group (31.6 ± 6.8 kg/m²). Weight change in the isCGM and control groups (-1.8 and -2.2 kg) did not differ. However, the change in coefficient of variation (-0.9 and 2.9 %) of sensor glucose differed significantly between the two groups. isCGM scan frequency was positively correlated with time above range (TAR) during the first month, positively correlated with the change in protein intake, and negatively correlated with that in TAR. CONCLUSION: While isCGM use in overweight individuals with IGT or mild T2DM did not reduce body weight, it might have influence dietary behavior. The negative correlation between scan frequency and TAR, and the positive correlation between scan frequency and protein intake suggest that self-awareness of glucose fluctuations contributed to behavioral change.2025, Obesity research & clinical practice, 19(1) (1), 70 - 76, English, International magazineScientific journal
- AIMS/INTRODUCTION: An early-morning elevation of blood glucose levels known as the dawn phenomenon and consequent postbreakfast hyperglycemia occur in some individuals with type 1 diabetes (T1D). Whereas insulin pump therapy can mitigate this phenomenon, some individuals prefer or are limited to alternative treatments. We have now assessed the effectiveness of early-morning administration of rapid-acting insulin for amelioration of the dawn phenomenon in individuals with T1D. MATERIALS AND METHODS: Thirteen individuals with T1D who experienced the dawn phenomenon as determined by continuous glucose monitoring (CGM) and who received a small dose of rapid-acting insulin on waking were included in this retrospective study. We evaluated the change in sensor glucose levels during a 2-h period from before to after breakfast consumed at 0700 h. The change in blood glucose levels during additional time intervals, average daily sensor glucose values, CGM indices, and insulin dose were also evaluated. RESULTS: The early-morning administration of 0.5-1 unit of rapid-acting insulin was associated with a significant reduction in 2-h glucose variability between before (0700 h) and after breakfast from a median of 90.7-51.0 mg/dL. The glucose variability from 0300 to 0700 or 0900 h was also significantly decreased, from 67.7 to 29.0 mg/dL and from 172.5 to 78.3 mg/dL, respectively. Average sensor glucose levels throughout the day were significantly reduced (from 192.7 to 156.7 mg/dL), as was the daily total insulin dose. CONCLUSION: Early-morning administration of rapid-acting insulin effectively managed the dawn phenomenon and subsequent postbreakfast hyperglycemia in individuals with T1D. - Coronavirus disease 2019 (COVID-19) is associated with endocrine disorders, but their long-term clinical course remains unclear. We here report the 15-month clinical course for an individual with multiple endocrine disorders of the pituitary gland and testis likely triggered by COVID-19. A 65-year-old man with no history of endocrinopathy was admitted for acute COVID-19 pneumonia. Although his respiratory condition improved after administration of antiviral drugs, his blood pressure dropped suddenly to a preshock level and was refractory to vasopressors. The circulating adrenocorticotropic hormone (ACTH) and cortisol concentrations were low, and secondary adrenal insufficiency was suspected. Administration of hydrocortisone rapidly ameliorated the hypotension, and the patient was discharged taking 15 mg of hydrocortisone daily. An insulin tolerance test performed 3 months later revealed impaired ACTH, cortisol, and growth hormone (GH) responses, indicative of combined hypopituitarism. The patient also manifested symptoms of hypogonadism, and a hormonal workup suggested primary hypogonadism. At 12 months after discharge, GH and ACTH responses had recovered completely and partially, respectively. After another 3 months, basal ACTH and cortisol levels had been restored to the normal range and the patient discontinued hydrocortisone replacement without exacerbation of symptoms, although his hypogonadism persisted. The patient thus developed transient GH and ACTH deficiency that lasted for more than a year as well as persistent primary hypogonadism during intensive care for COVID-19. - We report a case of classical Hodgkin lymphoma type post-transplant lymphoproliferative disorder (HL-PTLD) after kidney transplantation to highlight the difficulty of distinguishing this disorder from Hodgkin lymphoma-like PTLD (HL-like PTLD). Through this case report and literature review, we seek to clarify definitive pathologic features to differentiate these two conditions. A 38-year-old male kidney transplant recipient who had been receiving immunosuppressants was admitted to our hospital with unidentified high fever. Computed tomography images and blood tests indicated a lymphoproliferative disorder. Abdominal lymph node biopsy was performed, and microscopic examination revealed the presence of many large atypical cells in a background of dense T cell accumulation. The large, atypical cells were positive for Epstein-Barr Virus (EBV)-encoded small RNAs (EBER) in situ hybridization, EBV-LMP1, CD30 and PAX5, but negative for CD15, CD20 and CD45. Except for CD15-negativity, this immunohistochemical pattern was consistent with that of classical Hodgkin lymphoma. By close examination of the above immunoreactivities and the patient's subsequent chemosensitive clinical course, we finally made a diagnosis of HL-PTLD.Aug. 2018, International journal of hematology, 108(2) (2), 218 - 227, English, Domestic magazineScientific journal
