■ Paper- BACKGROUND: Heart Failure (HF) is a common chronic disease that has a high readmission rate and is associated with worsening symptoms and major financial impacts. Disease management implemented during or after an HF hospitalization has been shown to reduce hospitalization and mortality rates. Particularly for outpatients, it is necessary to provide self-care interventions. Structured nurse-led support such as timely follow-ups, including phone calls, is beneficial for improving self-care assessments. Evidence for nurse-led support has been investigated but is less than conclusive. The aim of this study is to compare the effectiveness of a nurse-led structured intervention for outpatients with chronic HF against the usual medical care in terms of self-care behaviors and occurrence of symptom exacerbation or rehospitalization. METHODS AND ANALYSIS: This is a cluster-randomized controlled trial. A total of 40 facilities with certified HF nurses will be allocated to two-arm clusters at a 1:1 ratio, randomly to the intervention or usual care arms. A total of 210 participants will be assigned from the hospital. Participants will be adults aged 18 years or older diagnosed with chronic HF who are classified as Stage C according to the ACCF/AHA Heart Failure staging system. In the intervention group, patients will receive structured nursing support. This begins with weekly support, including phone calls, for the first month, then transitions to monthly support thereafter. The aim is to ensure the stability of their living conditions, promote medication adherence, and encourage self-management. In the control group, patients will receive the usual care. Primary outcomes will assess the improvement or continuation of self-care behavior as measured by changes in EHFScBS (European Heart Failure Self-Care Behavior Scale) scores. Secondary outcomes include occurrence of readmission within 30 days, 3 months, 6 months, and 1 year after discharge, duration of home care until readmission, and blood levels of BNP and NT-proBNP.Aug. 2024, Journal of personalized medicine, 14(8) (8), English, International magazineScientific journal
- Patients with heart failure have difficulty recognizing and identifying changes in bodily sensations, despite the importance of symptom monitoring. The way patients with heart failure experience their bodies from exacerbation to recovery is poorly understood. We aimed to describe the lived bodily experience of heart failure from exacerbation to recovery. Participatory observations and interviews were conducted in seven patients admitted to the intensive care unit with worsening heart failure. Benner's interpretive phenomenology was used for analysis. Four major themes were identified: a non-functional body becomes the central concern and an object; being conscious of bodily changes before hospitalization when asked; the central concern shifted to daily life and the body becomes the background; and having a feeling of death in the body that no longer functions or a weakened body after recovery. This study found that patients with heart failure were conscious and concerned about their bodies changing as they underwent rapid changes during exacerbations and recovery. In addition, immediately after their bodies recovered and until they were discharged from the hospital, they looked toward their daily lives through their bodily experiences during heart failure exacerbation. - BACKGROUND: To prevent rehospitalization for heart failure (HF), patients need to be able to perceive physical changes that occur at the onset of HF exacerbation and seek early help. OBJECTIVE: The aim of this study was to evaluate the effect of a self-monitoring intervention on patients' perceptions of physical sensations during daily activities in the context of HF via a randomized controlled trial. METHODS: Participants (N = 70) were randomly assigned to the intervention (received daily activity record-based self-monitoring intervention support; group A) or control (only explained the measured results from the records; group B) group. Group A reflected on and described the physical sensations in their daily activities within 1 month after discharge. Outcome measures were assessed at 1 month after the intervention using the European Heart Failure Self-care Behavior Scale, Evaluation Scale for Self-Monitoring by patients with Heart Failure, clinical events, physical activity, and sleep. RESULTS: There was no significant difference in the change in the "asking for help" subscale score of the European Heart Failure Self-care Behavior Scale between the groups (+0.7 vs +0.4 points, P = .716). Group A had improved score on the self-monitoring subscale related to "concern about how movements affect body" from baseline (from 12.7 to 14.0 points, P = .026). There was no significant effect of self-monitoring intervention support on the first rehospitalization related to HF and all-cause death (log-rank χ2 = 0.432, P = .511). A significant difference in moderate-intensity physical activity between the groups was observed (+4.6 vs -0.5 minutes, P = .029). - The essential skills required for nursing practice for patients on ventilator support include knowledge of ventilators, invasive techniques for preventing complications, and risk prediction for safe management. Therefore, management in the intensive care area is generally recommended for patients on ventilator support. In addition, the coronavirus disease 2019 pandemic has rapidly increased ventilator-related patients. Nursing practice for ventilator-associated patients is complex and requires advanced knowledge and skill. Therefore, this study aimed to develop a nursing practice self-assessment scale for clinical nurses and evaluate its reliability and validity. A survey of 629 nurses found that the scale could be classified into the machine and patient management. The management of the ventilator comprised the following 31 items with six factors: “initial confirmation,” “artificial airway,” “alarm management,” “humidification management,” “emergency management,” and “airway fixation.” For patient management, 27 items of five factors, including “complication prevention,” “prevent VAP,” “transfer safely,” “skin management,” and “assessment post transferred” were extracted. - Postoperative delirium, a common perioperative complication, is frequently observed in elderly surgical patients. Few studies have investigated the life rhythm of preoperative patients, and whether or not the preoperative life rhythm is associated with the development of postoperative delirium. The purpose of this study was to investigate the relationship between the preoperative circadian physical activity rhythm and postoperative delirium in cardiovascular surgery patients. A total of 43 patients who underwent cardiovascular surgery were included in this prospective study between July 2016 and September 2017 at Kobe University Hospital. All subjects used a "Life Microscope" wristband (a wristwatch-type terminal incorporating a 3-axis accelerometer monitoring) for 3-7 days at home before the planned surgery. Hourly mean values were calculated for the metabolic equivalents from the obtained activity amounts, and subsequently evaluated using cosine periodic regression analysis. The circadian rhythm parameters of mesor (24 h time series mean), amplitude (half the peak-trough variation), and acrophase (peak time) for the metabolic equivalents were obtained. The intensive care delirium screening checklist was used to assess for postoperative delirium. The acrophase significantly advanced in the postoperative delirium group (median, 11:55 h [interquartile range, 11:06-12:27 h]) compared to the group without postoperative delirium (median, 13:25 h [interquartile range, 12:52-14:13 h]) (p < .001). Furthermore, binary logistic regression analysis showed that advances in the physical activity phase remained independently associated with postoperative delirium (odds ratio, 0.003 [95% confidence interval, 0-0.63]). These results suggest that misalignment between advanced life rhythm before hospitalization and life rhythm after hospitalization might be associated with risk for developing postoperative delirium. - Lifestyle of Patients with Alcoholic Liver Disease and Factors Leading to Hospital Readmission: A Prospective Observational Study.The objective of this study was to clarify the lifestyle characteristics of patients with alcoholic liver disease (ALD) who were readmitted to the hospital, and to identify the background factors associated with these characteristics. This was a prospective observational study. Over a period of 3 months following hospital discharge, we conducted structured interviews to investigate the following five lifestyle characteristics based on our previous research: dietary intake, alcohol consumption or abstinence, psycho-emotional status, regularity of life habits, adherence to treatment. We also collected data on background factors from medical records and questionnaires. The analysis was performed using conceptual cluster matrices, with participants divided into two groups (at-home recovery and readmission). Lifestyle, health status, and background factors were compared between the two groups. Of the 34 patients with ALD recruited, 21 completed the one-month follow-up and were included in the analysis-14 patients were in the at-home recovery group and 7 in the readmission group. The at-home group's lifestyle was characterized by controlled alcohol consumption, but with maintenance of regular life and eating habits and adherence to treatment. In contrast, irregular eating habits (p=0.006) and the development of irregular life habits or the discontinuation of treatment very quickly after hospital discharge characterized the readmission group's lifestyle. Experiences of loss were a lifestyle-related background factor that was associated with readmission (p=0.017). - Aim: To develop the Questionnaire for Reflective Practice of Nursing Involving Invasive Mechanical Ventilation (Q-RPN-IMV), a Japanese self-evaluation instrument for ward nurses' IMV practices. Design: Cross-sectional survey. Methods: Participants were 305 ward nurses from five hospitals in Japan with nursing involving invasive mechanical ventilation. Items concerning the process of nursing practice, including the thought process related to ventilator care, were collected from the literature and observation and interviews with five IMV specialists. Construct validity, concurrent validity, internal consistency and test-retest reliability were tested. Results: Initially, 141 items were collected and classified into three domains (i.e., observation, assessment and practice). Examination of exploratory factor analysis yielded five factors in the observation domain, six factors in the assessment domain and six factors in the practice domain. The data exhibited internal consistency, stability and concurrent validity. - Purpose: To describe the illness experience of patients living with arrhythmia and implantable cardioverter defibrillator (ICD) and evaluate their experience after implantation of the device for gaining insight on care suitable for their condition.Methods: In this qualitative descriptive study, we conducted semi-structured interviews of 22 outpatients who were visiting certified implant facilities.Results: Three categories and seven subcategories emerged from the interviews. The ICD patients were (1) Bewilderment Stemming from Arrhythmia and ICD Implant, (2) Facing the Reality of Arrhythmia, the ICD and Being Able To Continue Life, and (3) Giving Meaning to Living With Arrhythmia and an ICD. The ICD patients first experience bewilderment in the form of [Uncertainty about One's Own Bodies], [Fear of Arrhythmia Ending My Life], and [Dissatisfaction with Unforeseen Results of the ICD]. After discharge from the hospital following ICD implantation, they slowly resumed their own lifestyles and [Facing the Reality of the ICD and Being Able to Continue Life] and [Confirming and Managing Lifestyle Activities]. As they recognized, objectified, and faced changes in their lifestyles, they began to practically see themselves as living with arrhythmia which required ICD-aided treatment, [Giving Meaning to One's Illness] and [Recognition of One's Disease]. And so, they reknit their lives integrated with arrhythmia and an ICD. - We studied the relationship between dietary self-management behavior and actual energy intake by Type 2 diabetic individuals based on clinical backgrounds and profiles of gender, age, HbA1c, body mass index (BMI), and waist size. Subjects were 99 men and 77 women. Using data from the dietary self-management behavior questionnaire (DSBQ) and semiquantitative food frequency questionnaire, which lists 122 dishes, we initially divided clinical profiles into two groups. We then assessed the rank correlation coefficient between DSBQ factors and dietary intake using Student's t-test and Spearman's technique. We noted three trends: (1) Men aged <60 showed a noticeably low frequency of behavior such as “Device to enhance satisfaction during meals,” “Device to reduce the use of high-calorie seasoning,” and “Device to eat tasty food while maintaining nutritional balance,” Similarly, those with a BMI of >25 and a waist of >85 cm demonstrated a noticeably low frequency of behavior such as “Device to avoid excessive food intake,” “Device to enhance satisfaction during meals,” and “Device to eat tasty food while maintaining nutritional balance,” consequently reflected in increased dietary intake. (2) Women with HbA1c <7.0% showed a noticeably high frequency of behavior such as “Device to reduce the amount of cooking salt,” consequently reflected in decreased dietary intake. Women with a BMI of >25 and a waist of >90 cm showed a high frequency of “Behavior that each allows one's own value system to take priority even if it means eating inappropriate food” in behavior to cope with factors interfering with dietary regimens, consequently reflected in increased dietary intake. - The purpose of this study was to investigate the loss of resources and identify the psychological state of elderly victims 6 and 15 months after the Great Hanshin- Awaji Earthquake. The 72 elderly victims (16 men and 56 women; mean age, 82.3 and 80.3 years old, respectively) were interviewed with a structured questionnaire which included demographic information. The responses to the 17 questionnaire items were analyzed using the paired t-test and the Pearson's product moment correlation coefficient. The results indicated that their scores of resource loss were generally low at both 6 and 15 months after the earthquake. However, items such as 'personal health', 'feeling of having control over one's own life' and 'motivation to get things done' showed higher scores than the other items. The resource loss scores revealed no significant differences between the two time points, but it showed a higher positive correlation at 15 than at 6 months. - The purpose of this study was to examine the effects of the visiting nursing by the laboratory-unit nurses to the ward of precardiac catheterization patients. The Visiting Nursing is that on the day before planned cardiac catheterization, a nurse from the laboratory-unit visited the patient at his/her ward to collect information about the patient's situation, to provide the patient with information about the test or treatment, and to reduce the patient's anxiety. The subjects of this study were 10 patients who received cardiac catheterization during their hospital stay. There were 8 males and 2 females, with ages ranging from 46 to 73 years (mean: 61.3 + 2.45 years). Of these patients 5 had undergone no cardiac catheterization before, and 5 had.received it once or more before. The effects of the nurse's visit before catheterization were assessed using an interview-based qualitative survey method. Two or three days after catheterization, the patient was interviewed by an examiner. The patients experienced this kind of visiting nursing to have the following effects: 1) cognitive effects (supply of information about procedure, supply of sensory information, presentation of the role of the laboratory-unit nurses), and 2) emotional effects (helping the patient to express his/her anxiety, understanding the patient's anxiety, assuring patients about the safety and comfort of the procedure, etc.) During the nurse's visit, the patients understood the information supplied, spoke about their anxiety, and felt support, dependency, reliance and a sense of relief. Most patient accepted the visiting nursing positively, saying that it 'was valuable', 'made me feel relieved', and 'made me feel comfortable'. Emotional effects of the visit were positively accepted by all patients. Regarding the cognitive effects of the visit, some patients who had received cardiac catheterization before said that repeated explanation of details was unnecessary. - A survey was conducted to investigate the effects of the-great/Awaji earthquake on thedaily lives of diabetic patients, as well as their awareness of post-earthquuake self-management and the extent to which they carried out such activities. The subjects of this survey were 243 patients with diabetes mellitus who had been managed at our outpatient diabetes clinic before the earthquake and who visited our clinic during the three-month period after Febrary 1995 (the month following the earthquake). Of all respondents, 36.5% had their houses either severely or completely destroyed. As for regaining access to life lines, one month was needed before 31 % of all respondents regained access to water suupply and before 51% of all respondents regained access to their gas spply. Patients who had been on drug therapy usually recognized the necessity of self-management on the very day of the earthquake, while patients who had been on diet or exercise therapy usually became aware of this necessity 8 days to one month after the earthquake. The percentage of respondents who took more than one month to begin self-management was 38% for the diet therapy group, 34% for the exercise therapy group, 26% for the oral drug therapy group and 19% for the insulin injection group. Undertaking self-management was found tobe significantly related to the restoration of the water and gas supplies. HbAlc levels were higher for patients whose access to water and gas took a longer time to recover. When the HbAlc level of patients on diet therapy was examined three months after the earthquake, it tended to be higher in patients who were slower to undertake self-management. Where the damage caused by the earthquake was more severe, the time needed for diabetic patients to undertake self-management after the earthquake tended to be longer and the HbAlc level was higher. This suggests that the earthquake had a large effect on the self-management of diabetic patients. - 日本学術振興会, 科学研究費助成事業 基盤研究(C), 基盤研究(C), 神戸大学, 01 Apr. 2021 - 31 Mar. 2026心不全患者の「身体に根差した知性」に着目した身体経験に基づくセルフケア支援の開発本研究の目的は、心不全患者のセルフケア能力向上のために、患者の身体経験に基づいたセルフケア支援方法を開発し、その評価を行うことである。 具体的には、心不全患者のセルフケア能力向上のために、1)急性増悪による緊急入院から退院後の生活(およそ半年間)における身体経験を調査する。その際に、心不全患者が明確に意識することなく行っているセルフモニタリングや生活調整などの「身体に根ざした知性(embodied intelligence)」を明らかにする。2)1)の分析結果とこれまでに作成した「慢性心不全患者の症状体験に働きかけ、セルフモニタリングを強化する看護支援」、文献を基に、『心不全患者の身体経験に基づいたセルフケア支援方法』を作成し、3)2施設の循環器病棟と外来で実施して、セルフモニタリングおよびセルフケア能力、退院後2年間の再入院率を評価する予定である。 当初の計画では、令和3年度より研究活動を開始する予定であったが、令和3年4月1日~令和3年3月1日迄産前産後・育児休業を取得したため、令和4年4月1日より1年遅れで、当初計画のとおり研究活動を実施する予定である。

- 日本学術振興会, 科学研究費助成事業, 基盤研究(B), 聖路加国際大学, 01 Apr. 2021 - 31 Mar. 2025がん治療後の循環器疾患患者の心不全発症・増悪予防の自己管理支援プログラムの構築前年度までの研究成果を踏まえ、腫瘍循環器領域における看護の役割モデル案を作成し、その看護ケア内容、および評価項目を検討した。具体的な項目として以下の内容について、がん治療前、がん治療中、がん治療後に設定に看護支援と患者評価項目の設定を行った。 1)ハイリスク患者・がん治療経過の把握、2)生活習慣、自己管理状況、3)がん治療と循環器疾患への認識と家族の理解、4)がん治療前、治療中、治療後の症状、検査値モニタリング、5)がん治療前、治療中、治療後の循環器症状のセルフモニタリング状況、6)身体活動状況、7)メンタルヘルスの状況、8)がん治療に関連した健康状態の把握 また、本プログラムの実行可能性について研究分担者、ならびにスウェーデンにおける循環器看護研究者との協議を行い、1)上記モデルでのセルフモニタリングへの支援を行うための方略、ならびに2)継続した看護支援の重要性と啓発、3)日本での教育体制としての腫瘍循環器領域における看護師の役割として、予防から終末期に至るまで、患者ケアを調整し、患者、家族、介護者、その他の医療チームメンバーを含め教育を提供することの重要性が示された。特に外来以降のアセスメント項目の評価体制については、患者とのツールとしてchat機能を活用するなどの安価なコミニュケーションツールを用いることでの効果について議論を行った。上記を踏まえ、作成したモデルを用いた教育プログラムには、基本的に必要な内容を確認できる外来での看護職や医療者への啓発内容について検討を行っていくこととした。

- 日本学術振興会, 科学研究費助成事業 基盤研究(B), 基盤研究(B), 聖路加国際大学, 01 Apr. 2021 - 31 Mar. 2025がん治療後の循環器疾患患者の心不全発症・増悪予防の自己管理支援プログラムの構築本研究の目的は、がん治療後の循環器疾患患者の心不全の発症・増悪予防への自己管理支援プログラムの構築を行い、その中・長期効果を検証することである。2021年度は、がん治療後の循環器疾患患者の心不全発症・増悪因子を探索するため、国内外の文献検討を行い、マルチヒット仮説の分類に基づき増悪因子を検討した。さらに、がん治療後の循環器疾患患者の心不全発症・増悪因子のアセスメント方法、自己管理支援方法の検討を行うために、 日本循環器学会ホームページに掲載の循環器専門医研修施設・研修関連施設、ならびにがん診療連携拠点病院を対象とし、循環器疾患を有するがん治療後患者の自己管理支援の状況について、教育に携わる看護師を対象とした無記名質問紙調査を郵送法にて実施した。調査内容は、がん治療後の循環器疾患患者の身体面、日常生活、生活習慣、心理面、社会面のアセスメントと自己管理支援方法、自己管理への教育内容と方法について調査項目を設定し、アセスメント方法と自己管理支援の特徴を把握した。その結果、がん治療後の循環器疾患患者の心不全発症・増悪予防への現状と課題が示された。増悪因子と看護支援を明らかにするための、がんと循環器疾患の併存疾患をもつ患者の看護経験を有するがん診療施設・循環器診療施設に勤務する看護師へのインタビュー調査を検討中であり、これらの結果より2023年度のがん治療後の循環器疾患患者の心不全の発症・増悪予防への自己管理支援プログラムの策定に繋げていく。
- Japan Society for the Promotion of Science, Grants-in-Aid for Scientific Research Grant-in-Aid for Scientific Research (B), Grant-in-Aid for Scientific Research (B), University of Hyogo, 01 Apr. 2019 - 31 Mar. 2023Development of a multi-professional collaborative program to support the transition of family roles including children with chronic diseases幼少期から若年成人期までの子どもの発達段階別に,Synthesis research(既存の研究結果の統合)を行い,保護者から子どもへの役割移行における家族内の相互作用について明らかにすることを目的とし,本年度は,慢性状態の子どものセルフケア習得における親子相互作用について系統的レビューを行った.文献の検索にはPubMedとCINAHLを使用し,青年期、子供、幼児、核家族、ひとり親家族、親子関係、セルフケアなどをインデックスとして検索を行った.その結果,多くの論文が青年とその親に焦点を当ており,慢性的な状態で生活している子どもは,楽しいアクティブな学習と家族の社会的プロセスを通したセルフケアのサポートがされていた.親にとっての主要な問題は,子どもの苦痛を防止または最小限に抑えることであり.これは,親自身の感情的な不快感と密接に関連していることが明らかとなった. また,慢性疾患を持つ子どもが意思決定を行う際に保護者どのような相互作用が行われているかを明らかにすることを目的とし,0~14歳の間にⅠ型糖尿病,ネフローゼ症候群、先天性心疾患の診断を受けた現在も治療の継続・外来フォローを受けている15歳から24歳である患者とその保護者を対象とした半構成面接調査を実施した.2019年度は慢性疾患をもつ子ども6名、保護者5名(計11名)にインタビューを行った.11名のインタビューについては,逐語録を作成し,グランデッドセオリーアプローチに基づき分析中である.2020年以降,COVID-19の感染拡大防止のため病院施設での調査を中断している.調査再開が可能となり次第インタビュー調査を再開予定である.
- 日本学術振興会, 学術研究助成基金助成金/基盤研究(B), 基盤研究(B), 神戸大学, Apr. 2019 - Mar. 2023, Principal investigator慢性心不全患者の急性増悪予防に向けて看護師が行う退院時病態アセスメントとその評価本研究は、急増している慢性心不全患者の急性増悪予防(再入院予防)のために、退院後の患者・家族の療養継続の要(かなめ)となる看護師 が行う退院時の病態アセスメントに基づく支援に焦点をあて、これらを強化する支援ツールを開発し、その評価を行うことを目的としている。今回、慢性心不全患者の退院時支援における看護師(病棟看護師12名、退院調整看護師4名、訪問看護師5名を対象に、退院時のアセスメントについて面接調査を行い行い、質的帰納的に分析を行った。慢性心不全患者の退院時支援における医療をつなぐ看護実践を中心に分析した結果、病態把握として病棟看護師は[症状や検査結果を確認する]、退院調整看護師は重症度に応じて[病態を理解できる在宅担当者につなぐ]、訪問看護師は[入院中の症状と検査結果の推移を知る努力]をしていた。病棟看護師は生活上の[改善点を重点的に指導]し、[看護サマリーで情報提供][在宅担当者や外来Nsと情報共有]をし、訪問Nsは病態の情報を得るため[医学的知識のある者同士で連携したい]と語り、[生活上の心負荷を調整][増悪徴候に気づき対処する力を引き出す]をしていた。退院調整Nsは医療保険と介護保険の相違を考慮し[医療と暮らしがみえる訪問看護につなぐ]をしていた。心不全患者の急性増悪を予防し在宅療養を継続するには、病態に基づく身体の安定性を考慮した生活調整のため、Ns同士の協働が重要であることが示唆された。なお、これらの調査を通じて、病院看護師を中心とした慢性心不全患者の退院支援の実際(特に病棟退院時における看護師の支援と選択肢、および活用可能な医療制度に関するリテラシー)に関する実態調査の必要性が明らかになった。Competitive research funding
- 日本学術振興会, 学術研究助成基金助成金/基盤研究(C), 基盤研究(C), 神戸大学, Apr. 2018 - Mar. 2022本研究の目的は、各領域の患者に必要かつ特化した人工呼吸器ケアを、看護師が継続的に実践するための「人工呼吸器ケア教育支援モデル」の構築を目指して、以下のことを検討することである。 ⅰ.人工呼吸器ケア教育支援プロトコール案の作成をする。具体的には、集中治療領域での人工呼吸器ケアの看護実践過程を基盤に、各領域(一般病床、療養病床、介護施設、地域・在宅)に必要かつ特化した人工呼吸器ケアの看護実践過程(観察・判断・実施)を抽出する。各領域で実践されている人工呼吸器ケアと看護師の背景要因との関連を検討・分析することである。ⅱ.各領域での人工呼吸器ケア教育支援プロトコール案をもとに、人工呼吸器ケア実践教育モデルを作成し、その効果検証を行うことである。 本年度は、一般病棟・療養病床における人工呼吸器ケアに関する看護実践評価票が完成し、それを基にした教育プログラムを立ち上げ、年3回の実践教育を実施し、その効果検証を行った。評価基準は人工呼吸器ケアの安全管理とし、施設内での人工呼吸器ケアに関するインシデント・アクシデントの発生件数とした。これにより、看護師の背景要因による看護実践の差異が確認でき、具体的な教育プログラムを評価・再構成中である。次年度は、地域・在宅などでのフィールド調査を中心に調査を進めていく予定である。Competitive research funding
- 日本学術振興会, 学術研究助成基金助成金/基盤研究(C), 基盤研究(C), Apr. 2017 - Mar. 2021「心臓デバイス(植込み型除細動器、心臓再同期療法)植込み患者の治療安定期における生活リズム評価に基づく長期的な生活適応への支援」の構築のための第1段階として、2施設における心臓デバイス(CRT,CRT-D,ICD)新規植込み患者約50名を対象に、治療安定期(植込み6か月~1年後頃)の生活リズムと生活適応の実態調査を行った(2018年9月まで)。調査内容は、①生活リズム計による活動と休息・睡眠の質・加速度計(アクチグラフ)の7日間装着②質問紙による生活適応(自記式の心理社会的適応尺度:先行研究で作成した「デバイス植込み患者の療養生活適応アセスメント票」(4因子.25項目))③心機能・腎機能、基本属性や社会的要因などである。 2019年度は、まず、上記実態調査に関して、患者背景要因と活動・睡眠の関連を分析した。活動・睡眠と背景要因の関連は、歩数と適応感・要介護、座位・低強度活動と年齢、中等度活動と年齢・要介護、中途覚醒回数と植込み期間・eGFRとの間に有意な弱い相関があった。有職者は、歩数と中等度活動が多いが、睡眠効率が悪く中途覚醒も多かった。心リハ通院者は、睡眠効率が良かった。ショックデバイス患者は、歩数が多く座位行動が少なかった。したがって、年齢、腎機能、職業、心リハ利用等の背景要因に着目し、生活スタイルや心機能に合わせた活動調整支援を検討する必要があることが考察された。 そして、上記分析を基に、長期生活適応のための支援プログラムを作成した。さらに、当プログラムの効果検証研究実施のために、調査施設の医学介入研究倫理審査委員会の審査を受け、2020年2月に承認を受けた。同年3月より調査開始予定であったが、COVID-19による対象患者の医療中断に伴い、調査開始に至っていない。Competitive research funding

Coupled with that, elderly people have to deal with climate change and the secondary effects on their chronic diseases. Home-visit nurses are expected to organize elderly patients' living environments from a specialists' perspective and empower their patients to self-care as much as possible. To assist in that effort, we have developed medical-based nursing assessment support software to better organize the process. Our software includes functions such as: A system to understand the correlation between changes in a patient's condition and weather patterns such as temperature, humidity, and atmospheric pressure and An interactive database about a patient's prescriptions, meals, bladder and bowel functions, daily activities, resting times, appearance or management of any infections, and any behavioral patterns or overall condition changes.Competitive research funding

A nationwide questionnaire survey of cardiovascular care facilities showed no regional differences in patient information. However, regional characteristics were indicated in patient education items provided by nurses. An interview survey revealed the need for education based on regional characteristics. Using these results, we developed a self-management support program based on regional characteristics for patients with chronic heart failure.
- Japan Society for the Promotion of Science, Grants-in-Aid for Scientific Research Grant-in-Aid for Scientific Research (B), Grant-in-Aid for Scientific Research (B), Tohoku University, 01 Apr. 2016 - 31 Mar. 2020Effect of integtared body temperature management on sleep of community-dwelling frail older adults.We reviewed the literature on sarcopenia/frailty and sleep in older adults, as well as evaluated muscle mass, muscle strength, walking speed, and body composition and conducted a questionnaire survey of community-dwelling older adults. Furthermore, we performed interviews and web surveys with home health nurses to ascertain the potential risk of adverse effects associated with the use of sleep medications in older adults who require home care. The older adults requiring home care were more likely to use sleep medications than the general adult population and there were adverse events that were related to the home-based environment. Multidisciplinary collaboration is necessary to identify and respond to adverse events in frail older adults who require home care.

The protocol is based on the characteristics of recuperation behavior in patients with CHF and who are repeatedly hospitalized, and abstracted nursing support contents which are considered effective in the prevention of acute exacerbation. 1) Recognition of the reason for re-admission by patients with CHF. 2) Contents in nursing which make home care possible. 3) Physical activity and self-management of patients with CHF in early phase after discharge.Research results were unified, and a support protocol, which focused on symptom recognition and life activities of the patient, physical sensation accompanying physical activity during the recovery period, and an approach to self-monitoring was developed.

This study aims to develop a delirium prevention program which begins prior to surgery, by identifying the relationship between pre- and post-operative sleep activity indices and post-operative delirium onset, while focusing on the patient’s rhythm of life before hospitalization. Data from 52 patients who received cardiovascular surgery were analyzed, and pre-operative factors considered to be related to the presence or severity of post-operative delirium were considered. As a result, age, sleep efficiency before hospitalization, time to perform moderate activity before hospitalization, and pre-operative renal function and total protein were identified. From this, development of a nursing support program to maintain and improve sleep efficiency and moderate activity before hospitalization was examined.Competitive research funding

As a first step and to better understand the relationship between weather fluctuations and CANA, we monitored the sleep of 39 healthy adults in a controlled environment for one year. We collected about 1000 data and analyzed the relationship between their CANA while sleeping and data from meteorological sensors (℃,%,hPa) that were set up in their rooms. The results showed no overall similarities of sleeping patterns within the group however, sleeping patterns were influenced by irregular meal times. In addition, the CANA was decreased when experiencing low atmospheric pressure and high humidity. In the months of March and April, we observed the power of CANA was intensified, suggesting there is a correlation between seasonal changes, inclement weather and worsening physical condition.Competitive research funding

We performed a subjective assessment based on evaluation of the autonomic nerve, analysis of sleep-wake rhythms, quantitative evaluation of oxidative stress levels, and self-administered questionnaire surveys to observe chronological changes and features. We devised an educational program utilizing face-to-face interviews with the purpose of preventing risks of heart failure. We provided support at one, two, three, and six months after the conclusion of the study and monitored the effects one year later. We also formulated the educational content for professional nursing training.

In the new implantation patients, we utilized "the Japanese modified- Implanted Device Adjustment Scale" that we made in this study newly, and twice telephone visiting early after discharge, and supported an interview in the device outpatient department until one year after implantation. We used the anteroposterior comparison design which assumed profile of mood state, cardiac function and the arrhythmia event . For fourteen patients participated in this program, only " anxiety" was significantly improved until seven months later as compared with a discharge, and one year later, "confusion" "vigor" "anxiety" were significantly improved. It was suggested that this program was effective in the psychology adaptation of the device implantation patients.Competitive research funding
- Japan Society for the Promotion of Science, Grants-in-Aid for Scientific Research Grant-in-Aid for Scientific Research (C), Grant-in-Aid for Scientific Research (C), Kobe University, 2011 - 2013, Principal investigatorThe purpose of this study was to develop a nursing support program that focuses on the recuperation actions of CHF patients who are frequently readmitted to the hospital and will be applied during the early stage after the patients are discharged from their hospitals to prevent a need for readmission.The study was conducted by interviewing 18 nurses who provided home nursing support to help patients with severe CHF continue with their home treatment for one year or longer.The nurses were asked about the details of their nursing practices and a qualitative analysis was conducted thereafter.The result of the interviews showed that the following kinds of support were effective: 1) Bowel management; 2) Infection prevention; 3) Specific guidance on physical exertion in everyday life; and 4) Improvement of self-monitoring ability. Since these kinds of support were considered an indispensable part of CHF patients'home treatment in order to prevent acute exacerbation of their symptoms.Competitive research funding

In developing this software program, results from a previously used self-management behavior questionnaire for diabetic patients was used to assess each patients'self-management behaviors, the relationship between food intake and the amount of physical activity and the relationship between the degree of obesity and the ability to control blood sugar. This program will enable us to assist each diabetic patient based on their particular situation and we can therefore recognize and assist patients behavior more effectively. We believe the further development of this software program will allow us to more accurately evaluate the kind of self-management behavior each patient needs. This software program is currently being tested for its validity of RCT(Randomized Controlled Trial). We believe this software program will be applicable for clinical nursing practice.Competitive research funding

In our Questionnaire, we could reveal the self-management behaviors related to dietary intake and amounts of physical activity for different patient characteristics, investigating the association between patient's self-management behaviors and clinical variable, such as the patient's age and BMI, as well as abdominal circumference, glycemic control. Using this Questionnaire, we created a support protocol which was appropriate for each diabetic patients. As nursing support is particularly important in the early stages after the initiation of treatment, we studied how well self-management behaviors related to dietary and exercise therapy were being implemented by patients with type 2 diabetes for one year after initial diagnosis during which we also studied the changes in the patients' glycemic control index. The results suggest that in order to maintain a good glycemic control index, rather than relying on drug therapy alone, it is necessary to pro-actively implement both dietary therapy and exercise therapy, and that good control cannot be obtained from either one of these therapies alone.Competitive research funding

Research Design and Methods Outpatients with coronary heart disease (n = 76) completed a self-administered questionnaire supported by a semi-structured interview based on literature review. The content, factor, and concurrent validity and internal consistency and reproducibility of the scale were analyzed. Pearson's correlation coefficients for the ES-SMBPA-CHD and International Physical Activity Questionnaire (IPAQ) subscale scores were calculated to evaluate the concurrent validity, and also the relationship between the ES-SMBPA-CHD and resting energy expenditure values (REE: measured by Portable Indirect Calorimeter
) were investigated. Results The ES-SMBPA-CHD was divided into two parts, the first dealing with self-management behavior to enhance physical activity in daily life and the second with behavior to maintain the level of physical activity. Factor analysis showed that the first part comprised four factors and the second five. The ES-SMBPA-CHD correlated with the IPAQ subscales. Cronbach's α coefficient was between 0.71 and 0.90 and the intraclass test-retest correlation coefficient of the subscales was between 0.73 and 0.93. There was no evident relationship between resting energy expenditure and the ES-SMBPA-CHD Conclusions The ES-SMBPA-CHD is reasonably reliable and valid and we expect that will prove useful for the assessment of patients' self-management behavior and for individualized instruction. - 2006糖尿病をモデルとしたシグナル伝達病拠点Competitive research funding

- Japan Society for the Promotion of Science, Grants-in-Aid for Scientific Research Grant-in-Aid for Scientific Research (C), Grant-in-Aid for Scientific Research (C), Kobe University, 2001 - 2003Research on the Framework of Developing an After Discharge Daily Life Assessment Tool and Nursing Intervention Model for Women with Ischemic Heart Disease1.Patients of both sexes were queried at two different times : during their hospital stay while receiving heart rehabilitation and at the time of discharge, to ascertain their level of anxiety about daily life after discharge. They were found at both points of time to have a great deal of anxiety about relapses, the onset of chest symptoms, and sudden changes in their condition. At the time of discharge, they were also anxious about continuing social activities after discharge, e.g., worried about continuing their hobbies, planning meals and sustaining human relationships. These tendencies were observed among both men and women. 2.Women with ischemic heart disease, who were visiting outpatient clinics, were analyzed as to their daily lives, on the basis of their responses to questions about their daily experiences. It was found that their most pressing feeling was fear of having a heart attack. It was also found that these women were being careful to confine their activities to a narrow range, suppress their tendency to be active, and minimize social interactions. Furthermore, they took pains to keep stress on their hearts to a minimum when doing housework or engaging in other activities. Although they had initially resisted accepting support from other people to deal with their daily needs, it was confirmed by their responses that the women had come to accept such support. If these results of the present study are combined with the fact that the age of onset of ischemic heart disease among women is relatively high and that most women with this disease are housewives who do not work outside the home, we may say that a basic feature of a valid nursing intervention model for women with ischemic heart disease should be the creation by medical care providers of a regional medical support network.
- 日本学術振興会, 科学研究費助成事業 若手研究(B), 若手研究(B), 神戸大学, 2001 - 2002女性虚血性心疾患患者の冠危険因子是正に影響する食行動に関する研究1.研究目的および研究内容 本研究は、女性虚血性心疾患患者(以下女性IHD患者という)への効果的な患者教育に資するために、まず、女性IHD患者の食行動を測定するための調査票を作成することを目的に発症5年を経過した女性IHD患者を対象に面接調査を行い、内容の整理・分類を行った。同時に冠危険因子是正に影響する食行動を調査するための調査時期(対象者の発症後の年数)を検討するために、発症後の食行動の定着時期と脂質代謝などの冠危険因子の推移について面接調査ならびに診療録による資料収集および分析を実施した。次いで上記の予備調査をもとに、発症2〜5年を経過した女性IHD患者100名を対象に作成した食行動調査票を用いて冠危険因子(BMI・体脂肪・血中脂質代謝)是正に影響する食行動について検討を行っている。 2.研究成果 女性IHD患者20名を対象に面接調査を行った結果、女性IHD患者の食行動は、「食事制限を継続するための調理の工夫」「食品の入手方法および食品の保管に関する工夫」「家事・介護など家族内の役割調整上の工夫」「食生活上の楽しみを見いだすための工夫」「食品の選択と摂取頻度」「外的要因を調整するための工夫」の6領域に分類できた。食行動の定着時期については、退院後すぐに定着した、退院後2〜3か月、半年後、1年以降、5年以降などの回答にばらつきが認められた。脂質代謝の推移は、T-CHOで初回発症退院時〜2年後までは、変化が認められないが発症3年〜4年にかけて発症2年後に比べ低下する傾向が認められた。現段階での対象数50名(最終予定数は100名)において冠危険因子是正指標のBMI低下量と総コレステロール低下量について複数の関連が認められた食行動は「間食のとり方」「汁物をとる場合の工夫」であった。今後、最終予定対象数まで調査を継続し、女性IHD患者の冠危険因子是正に影響する食行動に関する詳細な分析を行う予定である。
