■ Paper- PURPOSE: Hepatectomy is a curative treatment for hepatocellular carcinoma (HCC) but deteriorating postoperative liver function can limit its benefits, especially for patients with borderline resectable (BR) HCC. We evaluated postoperative liver function trajectories and their impact on long-term survival. METHODS: The subjects of this retrospective analysis were 790 patients who underwent initial curative hepatectomy, stratified by resectability (R, BR1, BR2) and modified albumin-bilirubin (mALBI) grade. Liver function was assessed via ALBI scores pre- and postoperatively. Deterioration was defined as an ALBI score ratio < 0.9. RESULTS: Median overall survival (OS) differed significantly, being 109.9, 54.4, and 40.4 months for the R, BR1, and BR2 groups, respectively (p < 0.001). While functional status at POD 90 influenced recurrence-free survival, deterioration at POD 180 was identified as a robust independent predictor of OS (HR 1.53, p = 0.005). Notably, no single preoperative or perioperative factor was a significant predictor of this long-term functional decline at POD 180. Postoperative recovery was heterogeneous, with 47.5% of patients with mALBI grade 2a improving to grade 1 by POD 180. CONCLUSIONS: Post-hepatectomy liver function at POD 180 is a key determinant of long-term survival. As this late deterioration is difficult to predict perioperatively, continuous monitoring of hepatic reserve is essential to maintain eligibility for subsequent multidisciplinary therapies.Jun. 2026, Surgery today, English, Domestic magazineScientific journal
- We describe a case of refractory chylous ascites (RCA) successfully managed with a Denver shunt (DS) after conversion surgery for unresectable locally advanced pancreatic cancer. A 55-year-old woman developed RCA 6 months after distal pancreatectomy with celiac axis and portal vein resection. Conservative therapy, lymphangiography, and repeated paracentesis were ineffective. After confirming negative cytology, culture, and endotoxin tests, DS placement on postoperative day 425 resulted in rapid resolution of ascites, accompanied by improvement in body weight and hypoalbuminemia. No early major complications were observed after DS placement, and she was discharged safely. Although the shunt was removed 29 months later because of internal jugular vein thrombosis, no reaccumulation of ascites was observed thereafter. The patient remains alive without reaccumulation of ascites or tumor recurrence 60 months after curative resection. Although careful patient selection is essential because of potential complications, DS may serve as an effective salvage option for postoperative RCA when conservative approaches fail.Jun. 2026, Clinical journal of gastroenterology, English, Domestic magazineScientific journal
- BACKGROUND: Portomesenteric vein (PMV) resection is performed for pancreatic ductal adenocarcinoma with venous invasion. However, positive transection margins may lead to local recurrence. This study aimed to evaluate whether routine intraoperative frozen section analysis (FSA) reduces positive PMV transection margins. METHODS: This retrospective study reviewed patients who underwent pancreatectomy with PMV resection at Kobe University Hospital from 2010 to 2020. Routine FSA of the PMV transection margin was introduced in 2017. Clinicopathological factors and the incidence of positive PMV transection margins were compared between patients who underwent FSA (PMV frozen [+]) and those who did not (PMV frozen [-]). Cox regression was used to assess the prognostic effect of margin status. RESULTS: A total of 115 patients were included (57 in the PMV frozen [+] group and 58 in the PMV frozen [-] group). The rate of positive PMV transection margins was significantly lower in the PMV frozen (+) group than in the PMV frozen (-) group (1.8% vs 17.2%, respectively; P =.003). Multivariate logistic regression analysis demonstrated that intraoperative FSA of the PMV transection margin was an independent factor associated with a reduced risk of positive PMV transection margins on final pathological diagnosis (odds ratio, 13.7 [95% CI, 1.61-116.7]; P =.017). However, the positive PMV transection margin was not associated with overall survival (hazard ratio, 0.96 [95% CI, 0.31-2.50; P =.937). CONCLUSION: Routine FSA of the PMV transection margin markedly reduces positive PMV transection margins after pancreatectomy. However, the prognostic effect of a positive PMV transection margin remains unclear. Larger prospective studies are needed to determine its prognostic relevance.Jun. 2026, Journal of gastrointestinal surgery : official journal of the Society for Surgery of the Alimentary Tract, 30(6) (6), 102410 - 102410, English, International magazineScientific journal
- BACKGROUND: Central pancreatectomy (CP) is a function-preserving procedure for benign or low-grade malignant lesions in the pancreatic neck or proximal body. However, endocrine function after CP has not been objectively evaluated. METHODS: We retrospectively analyzed 123 patients who underwent CP (n = 38) or distal pancreatectomy (DP; n = 85) for benign or low-grade malignant tumors. Short- and long-term outcomes were compared between groups. Among 37 patients (CP; n = 12, DP; n = 25) underwent a 75-g oral glucose tolerance test (OGTT) before and 1 month after surgery to evaluate endocrine function. RESULTS: CP was associated with a higher incidence of postoperative pancreatic fistula than DP (40% vs. 8%; p < 0.001). All CP patients achieved R0 resection with no perioperative mortality or tumor recurrence. The incidence of new-onset diabetes mellitus was significantly lower after CP than after DP (14.8% vs. 50.9%, p = 0.002). In the DP group, the area under the curve (AUC) for blood glucose (BG) increased postoperatively (p < 0.001), whereas AUCs for immunoreactive insulin (IRI) and C-peptide immunoreactivity (CPR) decreased (p = 0.010 and p = 0.022, respectively). Conversely, the CP group showed preserved endocrine function, with no significant postoperative changes in AUCs for BG (p = 0.351), IRI (p = 0.723), or CPR (p = 0.900). CONCLUSION: Compared with DP, which has acceptable short-term outcomes, CP preserves pancreatic endocrine function. In patients with benign or low-grade malignant pancreatic tumors with expected long-term survival, CP should be considered over DP to preserve function.May 2026, Annals of gastroenterological surgery, 10(3) (3), 827 - 834, English, Domestic magazineScientific journal
- BACKGROUND: Relationship between timing of initiating adjuvant chemotherapy (AC) and clinical outcomes after surgery for pancreatic cancer remains controversial. METHODS: In this ancillary analysis of the JASPAC 01 trial, 187 patients were classified according to the interval between surgery and chemotherapy initiation to Early (< 6 weeks, n = 45), Standard (6-8 weeks, n = 70), and Delayed (> 8 weeks, n = 72). Overall survival (OS) and relapse-free survival (RFS) were analyzed. RESULTS: Baseline characteristics were comparable among three groups. The Standard group showed significantly longer OS than the Early group (median, 66 vs. 37 months; HR 0.61, 95% CI 0.38-0.99; p = 0.041), and relatively longer OS compared with the Delayed group (median, 45 months; HR 0.68, 95% CI 0.44-1.05; p = 0.077). RFS was longer in the Standard group (median, 46 months) compared with the Early group (20 months; HR 0.61, 95% CI 0.38-0.99; p = 0.040) and the Delayed group (20 months; HR 0.59, 95% CI 0.39-0.89; p = 0.011). Multivariate analysis identified operative procedure, R1 resection, lymph node metastasis, and nonstandard initiation (< 6 or > 8 weeks) as independent adverse prognostic factors. CONCLUSIONS: Initiation of S-1 AC at 6-8 weeks after pancreatectomy was associated with favorable survival outcomes.Apr. 2026, Journal of hepato-biliary-pancreatic sciences, 33(7) (7), 568 - 575, English, Domestic magazineScientific journal
- We report a rare case of cefepime-induced encephalopathy (CE) after pancreaticoduodenectomy. An 83-year-old female patient underwent pancreaticoduodenectomy for a distal bile duct tumor. On postoperative day (POD) 6, she developed a pseudoaneurysm secondary to postoperative pancreatic fistula, for which interventional radiology (IVR) treatment was performed. A liver abscess developed after IVR treatment on POD 15. Multidrug-resistant P. aeruginosa was detected in a drain culture, prompting a change of antibiotic therapy to cefepime on POD 30. Antifungal therapy was initiated for fungemia; however, her renal function progressively deteriorated. On POD 56, the patient presented with vomiting, expressionlessness, and hypokinesia. However, brain computed tomography revealed no abnormalities. Involuntary movement of upper and lower limbs were noted on POD 64, but both magnetic resonance imaging and cerebrospinal fluid examination showed none remarkably abnormalities. On POD 66, epileptic waves were detected via an electroencephalogram (EEG). CE was suspected, and cefepime administration was discontinued on POD 69. The involuntary movements disappeared promptly after cefepime discontinuation (POD 70). By POD 80, her communication ability improved, and EEG findings normalized. The patient was discharged on POD 260. Clinicians should consider CE in any patient receiving cefepime who presents with altered mental status and neurological symptoms.Feb. 2026, Clinical journal of gastroenterology, 19(1) (1), 100 - 105, English, Domestic magazineScientific journal
- BACKGROUND/AIM: The prognostic impact of the timing of adjuvant chemotherapy initiation in patients with resected pancreatic cancer is unclear. Therefore, this study aimed to investigate whether delayed initiation of S-1 adjuvant chemotherapy affects the survival of patients with resectable pancreatic cancer. PATIENTS AND METHODS: Patients who received S-1 adjuvant chemotherapy after undergoing R0/R1 resection were grouped according to whether adjuvant chemotherapy was initiated <60 (n=36) or ≥60 days (n=27) after surgery. Correlations between the time to chemotherapy initiation, clinicopathological factors, and survival were analyzed. RESULTS: The median and mean times to chemotherapy initiation were 58 and 61 days, respectively. The delayed group had worse 2-year overall (57% vs. 80%, p=0.032) and recurrence-free survival (35% vs. 54%, p=0.044) rates than the early group. These results were similar in patients who completed S-1 adjuvant chemotherapy. In multivariate analysis, delayed initiation of chemotherapy, R1 resection, and tumor size (≥40 mm) were independent prognostic factors for poor overall survival. The delayed group had more patients with severe postoperative complications (Clavien-Dindo grade ≥III) (30% vs. 8%, p=0.028) and delayed recovery of serum albumin levels (postoperative days 7, 14, and 30: p=0.040, 0.004, and 0.003, respectively) than the early group. CONCLUSION: Delayed initiation of S-1 adjuvant chemotherapy had an adverse impact on survival and was correlated with severe postoperative complications and delayed recovery of postoperative nutritional status.Feb. 2026, Anticancer research, 46(2) (2), 987 - 999, English, International magazineScientific journal
- BACKGROUND: Resection of pancreatic ductal adenocarcinoma (PDAC) after prior esophagectomy for esophageal cancer is technically demanding and its oncologic value is unclear. We evaluated the peri-operative safety and long-term survival after pancreatectomy for PDAC in this setting. METHODS: All patients undergoing pancreatectomy for PDAC at Kobe University Hospital (2017-2024) were reviewed. Outcomes in patients with previous esophagectomy for esophageal cancer were compared with those in patients undergoing contemporaneous resectable PDAC without such history. Primary endpoints were postoperative morbidity/mortality and overall survival. RESULTS: Of 415 pancreatectomies, nine (2.2%) had a history of esophagectomy for esophageal cancer (pancreaticoduodenectomy five; distal pancreatectomy four). Preservation of the gastrointestinal-conduit vessels was achieved in seven patients; two required right gastroepiploic artery and vein reconstruction. Major morbidity occurred in one patient (11.1%); there was no 90-day mortality and no conduit-related complication. However, only three patients (33.3%) received adjuvant chemotherapy. Median overall survival was significantly shorter in post-esophagectomy patients (n = 9) than in controls (n = 249) (8.4 vs. 41.7 months, p = 0.001). Survival after pancreaticoduodenectomy was especially poor (7.4 vs. 47.2 months, p < 0.001), whereas outcomes after distal pancreatectomy did not differ (16.4 vs. 38.9 months, p = 0.781). CONCLUSIONS: Pancreatectomy after esophagectomy can be performed safely, even with minimally invasive techniques, yet long-term survival remains dismal, particularly after pancreaticoduodenectomy. Careful patient selection with specific attention to the feasibility of delivering perioperative chemotherapy may be required to improve outcomes in this highly selected cohort.Feb. 2026, Annals of surgical oncology, 33(2) (2), 918 - 924, English, International magazineScientific journal

- AIM: Regarding the resectability of pancreatic adenocarcinoma (PDAC), not only anatomical factors but also biological and conditional factors have come to be considered. This study examined the impact of the Global Leadership Initiative on Malnutrition (GLIM) criteria on prognosis after resection of anatomically resectable PDAC. METHODS: The medical records of consecutive patients who underwent resection for resectable PDAC between January 1, 2014, and December 31, 2022, were retrospectively reviewed. Patients were classified as normal, moderately, or severely malnourished according to the GLIM criteria. RESULTS: In total, 194 patients were included in the analysis. According to the GLIM criteria, 61 (31.4%), 49 (25.2%), and 84 (42.3%) patients were normal, moderately, and severely malnourished, respectively. Patients with malnutrition had shorter overall, recurrence-free, and disease-specific survival (OS, RFS, and DSS) than normal patients (OS, normal vs. moderate, p = 0.015; normal vs. severe, p < 0.001; RFS, normal vs. moderate p = 0.012, normal vs. severe, p < 0.001; DSS, normal vs. moderate, p = 0.023; normal vs. severe, p < 0.001). In multivariate analysis regarding OS using all factors, moderate or severe malnutrition according to the GLIM criteria (p = 0.007), performance status (p = 0.086), preoperative diabetes (p = 0.017), tumor diameter ≥ 3 cm (p = 0.002), lymph node metastasis (p < 0.001), and postoperative adjuvant therapy (p = 0.027) were independent prognostic factors. In multivariate analysis using preoperative factors, malnutrition according to the GLIM criteria remained a significant prognostic factor (p = 0.003). CONCLUSION: The GLIM criteria are effective prognostic predictors in patients with resectable PDAC undergoing upfront surgery. Preoperative nutritional assessment using these criteria may contribute to determining treatment plans for resectable PDAC.Nov. 2025, Annals of gastroenterological surgery, 9(6) (6), 1351 - 1361, English, Domestic magazineScientific journal
- BACKGROUND: In this study, we investigated the criteria that predict the long-term survival benefits after surgical resection in patients with pancreatic cancer accompanied by liver oligometastasis. METHODS: In total, 60 patients from 34 high-volume Japanese centers who underwent surgical resection for liver oligometastasis between 2005 and 2020 were included. Univariate and multivariate methods of survival analyses were performed. All patients were followed up for at least 36 months. RESULTS: Overall survival (OS) was significantly longer in the preoperative chemotherapy group than in the up-front surgery group (37.4 vs. 20.4 months, p = 0.001). In the operation with preoperative chemotherapy group, a complete response was observed in eight patients (28.6%). The 1-, 3-, and 5-year OS rates were 92.9%, 50.0%, and 35.7%, respectively. The multivariate analysis showed that low CA19-9 (< 100 U/mL; HR: 0.25; 95% CI: 0.06-0.96; p = 0.043), low CEA (< 5 U/mL; HR: 0.14; 95% CI: 0.04-0.48; p = 0.002), and resectable (R) or borderline resectable pancreatic cancer invading the portal vein (BR-PV) status (HR: 0.19; 95% CI: 0.07-0.51; p < 0.001) were positive prognostic factors. The median OS of the patients who met all three criteria was 106.6 months. CONCLUSION: Preoperative chemotherapy is essential for the treatment of liver oligometastases. Despite the high recurrence rates, patients who met the specific criteria have a favorable prognosis with liver resection.Aug. 2025, Journal of hepato-biliary-pancreatic sciences, 32(8) (8), 611 - 619, English, Domestic magazineScientific journal
- BACKGROUND: This study evaluated the clinical significance of an unplanned total pancreatectomy (TP) by comparing surgical outcomes of planned and unplanned TP for pancreatic ductal adenocarcinoma (PDAC). METHODS: Forty-two patients who underwent TP for PDAC between 2009 and 2020 at the Kindai University Hospital were retrospectively analyzed. We evaluated the differences in the prognosis and related factors between planned and unplanned TP, and independent prognostic factors of TP for PDAC. RESULTS: Twenty-three patients underwent planned TP, and 19 underwent unplanned TP. There were no significant differences in clinicopathological data between the two groups, except for microscopic vascular invasion. However, the unplanned TP group had significantly better overall survival (OS) and recurrence-free survival (RFS) times than the planned TP group. In the multivariate analysis of the prognostic factors of TP for PDAC, a planned TP and not receiving or completing postoperative adjuvant chemotherapy (AC) were independent poor prognostic factors. CONCLUSION: This study supports a rigorous approach to margin clearance in TP for PDAC to achieve curative negative margins in cancer-positive pancreatic resection stumps. However, the prognosis of a planned TP for PDAC is extremely poor, and adequate preoperative and multidisciplinary treatment may be necessary to prolong the prognosis.Jul. 2025, Journal of hepato-biliary-pancreatic sciences, 32(10) (10), 777 - 786, English, Domestic magazineScientific journal
- AIM: Evidence regarding the reliability of endoscopic ultrasound-guided tissue acquisition (EUS-TA) for assessing histological proliferation and WHO grading of small (≤20 mm) pancreatic neuroendocrine tumors (PanNETs) is limited. METHODS: In this multicenter retrospective study, we analyzed data from 122 patients with small PanNETs who underwent EUS-TA followed by surgical resection between 2006 and 2022. We compared the histopathological proliferation assessment and WHO grading between preoperative EUS-TA and surgical definitive specimens. RESULTS: Among the 122 patients with small PanNETs (80% with surgical definitive WHO grade G1 and 20% with G2), EUS-TA histology identified neuroendocrine tumors in 101 (83%) patients and provided WHO grading in 85 (70%) patients. Histopathological WHO grading for EUS-TA was concordant with surgical definitive grading in 86% (73/85) of cases, overstaged in 4% (3/85), and understaged in 11% (9/85). Moderate, severe, and fatal adverse events associated with EUS-TA, as classified by the lexicon, were not reported in this cohort. CONCLUSION: EUS-TA is a reliable method for assessing histopathological proliferation and WHO grading of small PanNETs. However, grading discordance may occur, and a risk-benefit evaluation on a per-patient basis is recommended.Mar. 2025, Annals of gastroenterological surgery, 9(2) (2), 339 - 346, English, Domestic magazineScientific journal
- INTRODUCTION: Hepatoid carcinoma (HC) is a rare type of malignant tumor that shares similar serological, morphological, and immunohistochemical features with hepatocellular carcinoma. Pancreatic HC exhibits aggressive biological behavior and is classified as pure type, combined type, and mixed type. Unlike the pure type, the combined and mixed types refer to HC with other histological components. While the most common component is the neuroendocrine tumor (NET), no case of coexistence of neuroendocrine carcinoma (NEC) and NET with hepatoid differentiation has been reported. We herein report an extremely rare case of coexisting NEC and NET of the whole pancreas with hepatoid differentiation. CASE PRESENTATION: A 15-year-old woman had epigastric pain and impaired glucose tolerance, without any medical or family history. Computed tomography (CT) revealed a 6.4-cm mass in the pancreatic head, an enhanced mass throughout the pancreas, and a 1.0-cm liver lesion. Positron emission tomography (PET)/CT and somatostatin receptor scintigraphy (SRS) suggested a NET in the pancreatic body and tail, and a NEC in the pancreatic head. Biopsies confirmed NEC in the pancreatic head and liver with possible hepatoid differentiation. The patient underwent combination chemotherapy with carboplatin and etoposide. Due to the partial response achieved with chemotherapy, which led to significant tumor shrinkage on CT and no uptake on PET/CT in the pancreatic head tumor, the patient proceeded with conversion surgery, including a total pancreatectomy with portal vein resection and partial hepatectomy. However, serum α-fetoprotein (AFP) levels rapidly rose, and multiple liver metastases of NEC were detected on CT at 5 months after surgery. Liver metastases worsened despite further chemotherapy. The patient died 10 months after surgery. CONCLUSIONS: We herein present an extremely rare case of coexisting NET and NEC of the whole pancreas with hepatoid differentiation. Due to a remarkable response to chemotherapy, conversion surgery was performed. However, early recurrence of liver metastases accompanied by a rapid increase in serum AFP levels occurred, and the prognosis was poor. Pancreatic HC should be considered when encountering a bulky tumor of the pancreas with elevated serum AFP levels, and further case series and analysis are needed to determine the appropriate treatment strategy.2025, Surgical case reports, 11(1) (1), English, Domestic magazineScientific journal
- PURPOSE: To clarify the prevalence, risk factors, and clinical implications associated with zinc deficiency in patients undergoing pancreatic surgery. METHODS: The serum zinc levels were measured in 329 patients post-pancreatic surgery between January and April 2021. The postoperative serum zinc levels and clinicopathological variables were retrospectively analyzed. RESULTS: The median serum zinc level was 73 µg/dL (33-218). Zinc deficiency (zinc level < 60 µg/dL) was observed in 52 patients (16%). A total of 329 patients were classified into zinc-deficient (n = 52) and non-deficient (zinc ≥ 60 µg/dL, n = 277) groups. A univariate analysis revealed significant differences in sex, postoperative body mass index, serum albumin, total cholesterol, creatinine, aspartate aminotransferase (AST), HbA1c levels, diabetes, surgical procedures, and operative blood loss. According to a multivariate analysis, male sex [odds ratio (OR) 3.70; 95% confidence interval (CI) 1.67-8.20; p = 0.001], postoperative serum albumin levels < 3.9 g/dL (OR 6.39; 95% CI 3.30-12.37; p < 0.001), postoperative serum AST ≥ 51 U/L (OR, 4.6; 95% CI 0.07-0.29; p < 0.001), and total pancreatectomy (OR 3.68; 95% CI 1.37-9.85; p = 0.009) were found to be independent predictors of zinc deficiency after pancreatic surgery. CONCLUSIONS: Zinc deficiency frequently occurs in patients undergoing pancreatic surgery. Lower postoperative zinc levels could be linked to sex, the serum albumin and AST levels, and surgery type.Sep. 2024, Surgery today, 55(4) (4), 552 - 559, English, Domestic magazineScientific journal

- BACKGROUND: Optimal management of non-functioning pancreatic neuroendocrine tumors (PanNETs) ≤20 mm is controversial. The biological heterogeneity of these tumors poses challenges when deciding between resection and observation. METHODS: In this multicenter, retrospective cohort study, we analyzed all patients (n = 78) who underwent resection of non-functioning PanNETs ≤20 mm at three tertiary medical centers from 2004 to 2020 to assess the utility of preoperatively available radiological features and serological biomarkers of non-functioning PanNETs in choosing an optimal surgical indication. The radiological features included non-hyper-attenuation pattern on enhancement computed tomography (CT; hetero/hypo-attenuation) and main pancreatic duct (MPD) involvement, and serological biomarkers included elevation of serum elastase 1 and plasma chromogranin A (CgA) levels. RESULTS: Of all small non-functioning PanNETs, 5/78 (6%) had lymph node metastasis, 11/76 (14%) were WHO grade II, and 9/66 (14%) had microvascular invasion; 20/78 (26%) had at least one of these high-risk pathological factors. In the preoperative assessment, hetero/hypo-attenuation and MPD involvement were observed in 25/69 (36%) and 8/76 (11%), respectively. Elevated serum elastase 1 and plasma CgA levels were observed in 1/33 (3%) and 0/11 (0%) patients, respectively. On multivariate logistic regression analysis, hetero/hypo-attenuation (odds ratio [OR] 6.1, 95% confidence interval [CI] 1.7-22.2) and MPD involvement (OR 16.8, 95% CI 1.6-174.3) were significantly associated with the high-risk pathological factors. The combination of the two radiological worrisome features correctly predicted non-functioning PanNETs with high-risk pathological factors, with about 75% sensitivity, 79% specificity, and 78% accuracy. CONCLUSIONS: This combination of radiological worrisome features can accurately predict non-functioning PanNETs that may require resection.Feb. 2023, Annals of surgical oncology, 30(6) (6), 3493 - 3500, English, International magazineScientific journal
- Aim: Pancreaticodigestive tract anastomotic stricture is a long-term complication of pancreticoduodenectomy (PD). However, optimal treatment has not yet been defined. We conducted longitudinal pancreaticojejunostomy (LPJ) in symptomatic patients with anastomotic stricture after PD. This study aimed to evaluate the efficacy of this procedure. Methods: Pancreticoduodenectomy was performed in 605 patients at our institution between January 2005 and April 2020. Of these, 15 patients (2.5%) developed symptomatic pancreaticodigestive tract anastomotic stricture after PD. Three patients were referred to our institution owing to recurrent pancreatitis with anastomotic stricture after PD. LPJ was indicated for these 18 patients, and they were enrolled in this study. Results: The median time from the initial operation to LPJ was 2.0 y. Preoperative clinical presentations included obstructive pancreatitis in 10 patients, a rapid deterioration of glucose tolerance in nine, and severe steatorrhea in two. Surgical morbidity ≥grade III defined by the Clavien-Dindo classification was not observed. After LPJ, preoperative symptoms improved in 16 patients (89%) during a median follow-up of 39 mo. Nine of the 10 patients with obstructive pancreatitis achieved complete pain relief. All nine patients with a rapid deterioration of glucose tolerance showed improved endocrine function. Daily insulin requirement was significantly decreased after LPJ (11.6 ± 3.3 vs 3.4 ± 4.3 units, P = .0239). Four of the seven patients who required insulin injections were free of insulin after LPJ. Conclusion: LPJ is a safe and effective surgical procedure for symptomatic patients with stricture of the pancreaticodigestive tract anastomosis after PD.May 2022, Annals of gastroenterological surgery, 6(3) (3), 412 - 419, English, Domestic magazineScientific journal
- PURPOSE: Postoperative pancreatic fistula (POPF) remains the most clinically relevant complication of laparoscopic distal pancreatectomy (LDP). The present study evaluated the efficacy of the "slow firing method" using a reinforced triple-row stapler (Covidien, Tokyo, Japan) during LDP. METHODS: This retrospective single-center study included 73 consecutive patients who underwent LDP using the slow firing method. A black cartridge was used in all patients. The primary endpoint was the rate of clinically relevant POPF (CR-POPF) after LDP. Secondary endpoints included perioperative outcomes and factors associated with CR-POPF as well as the correlation between the transection time and thickness of the pancreas. RESULTS: Four patients (5.5%) developed CR-POPF (grade B). Overall morbidity rates, defined as grade ≥ II and ≥ III according to the Clavien-Dindo classification, were 21 and 11%, respectively. The median postoperative hospital stay was 10 days. Preoperative diabetes (13.6 vs. 0.2%, P = 0.044) and thickness of the pancreas ≥ 15 mm (13.8% vs. 0%, P = 0.006) were identified as independent risk factors for CR-POPF. The median transection time was 16 (8-29) min. CONCLUSION: The slow firing method using a reinforced triple-row stapler for pancreatic transection is simple, safe, and effective for preventing CR-POPF after LDP.Feb. 2022, Surgery today, 52(2) (2), 260 - 267, English, Domestic magazineScientific journal

- BACKGROUND: Particle radiotherapy has increasingly gained acceptance for locally advanced pancreatic cancers owing to superior tumor conformity and dosimetry compared to conventional photon radiotherapy. However, the close proximity of the pancreas to the stomach and duodenum leads to radiation-induced gastrointestinal toxicities, which hinder the delivery of curative doses to the tumor. To overcome this problem, a surgical spacer was placed between the tumor and gastrointestinal tract, and subsequent proton radiotherapy was performed in this study. METHODS: Data from 9 patients who underwent surgical spacer placement and subsequent proton radiotherapy were analyzed. The safety and feasibility of the spacer placement surgery were evaluated; the impact of the spacer on dosimetry was also assessed using dose volume histogram (DVH) analyses, before and after surgical spacer placement. RESULTS: Surgical spacer placement and subsequent proton radiotherapy were successfully completed in all cases. Surgical spacer placement significantly improved the dose intensity covering 95%, mean, and minimum doses for the gross tumor volume, and the clinical and planning target volume based on the DVH, while respecting the dose constraints of the gastrointestinal tract. Based on the Common Terminology Criteria for Adverse Events, two patients (22.2%) developed gastrointestinal ulcer (Grade 2) at 1 and 35 months, and one patient (11.1%) developed gastric perforation (Grade 4) at 4 months after proton radiotherapy. CONCLUSIONS: Surgical spacer placement in the locally advanced pancreatic body and tail cancers is relatively safe and technically feasible. Comparing radiation plans, surgical spacer placement seems to improve the dose distribution in the locally advanced pancreatic body and tail cancers, which are close to the gastrointestinal tract.Jan. 2021, Radiation oncology (London, England), 16(1) (1), 3 - 3, English, International magazineScientific journal
- PURPOSE: The purpose of this study was to determine the factors influencing conversion from laparoscopic distal pancreatectomy (LDP) to open surgery, and the effect of such conversion on the outcome. METHODS: This retrospective single-center study included 70 consecutive patients undergoing LDP. The primary endpoint was the rate of conversion to open surgery during LDP. The secondary endpoints were determining the reasons for conversion to open surgery, with detailed analyses of these cases and a comparison of the surgical outcome with and without conversion. RESULTS: Seven patients (10%) required conversion to open surgery during LDP. Pancreatic ductal adenocarcinoma (PDAC) was identified as a risk factor for conversion (p = 0.010). The reasons for conversion included technical difficulty (two bleeding, one severe adhesion) and pancreatic stump-related issues (two margin-positive, two stapling failures). Although the overall morbidity rate (29 vs. 11%, p = 0.48) and the rate of clinically relevant postoperative pancreatic fistula (14 vs. 5%, p = 0.82) were no different for the patients with or without open conversion, the postoperative hospital stay was significantly longer in the former (median 15 vs. 10 days, p = 0.03). CONCLUSIONS: Careful preoperative assessment is required when planning LDP for PDAC. Although conversion to open surgery does not result in failure of LDP, efforts to reduce the duration of postoperative hospital stay and the occurrence of complications are desirable to improve the outcome of LDP.Jan. 2021, Surgery today, 51(1) (1), 70 - 78, English, Domestic magazineScientific journal

- AIM: To evaluate the clinical significance of lateral pelvic lymph node (LLN) size in predicting pathological metastasis and prognosis in rectal cancer treated with preoperative chemoradiotherapy (CRT) followed by surgery. PATIENTS AND METHODS: Fifty-two patients with rectal cancer who underwent curative surgery after preoperative CRT were included. Fifteen patients underwent total mesorectal excision (TME) alone, while 37 patients underwent TME with LLN dissection for clinical LLN metastasis. RESULTS: Pathological metastasis was identified in seven (2.6%) out of 270 resected LLNs in six (16.2%) out of 37 patients. The cut-off value of the short-axis diameter was 7.0 mm before and 6.0 mm after CRT. The 5-year recurrence-free survival rate was significantly higher in patients with LLNs <7.0 mm than in those with LLNs ≥7.0 mm (85.7% versus 56.8%, p=0.038). CONCLUSION: Short-axis diameter of LLNs of 7.0 mm seems to be an optimal cut-off value before CRT for predicting pathological metastasis and prognosis.Feb. 2019, Anticancer research, 39(2) (2), 993 - 998, English, International magazineScientific journal

- [A Patient with Multiple Pancreatic Metastases Undergoing Total Pancreatectomy 18 Years after Renal Cell Carcinoma Resection].A 71-year-old woman underwent right nephrectomy for the treatment of clear cell renal cell carcinoma at the age of 53. After 15 years, surgical removal of a solitary tumor was performed in the right adrenal gland and thyroid gland; both were diagnosed as metastases of renal cell carcinoma. Eighteen years after the initial resection, computed tomography(CT) showed multiple hypervascular tumors spreading across the entire area of the pancreas. She was referred to our hospital, and endoscopic ultrasound-guided fine needle aspiration biopsy(EUS-FNA)revealed that they were metastases from the renal cell carcinoma. Total pancreatectomy and splenectomy were performed, and the patient remains alive and well with no evidence of recurrent disease 7 months after the pancreatectomy. Furthermore, her blood glucose level is well controlled with insulin therapy.Dec. 2018, Gan to kagaku ryoho. - [A Surgical Resected Case of VIPoma with Para-Aortic Lymph Node Involvement].A 72-year-old woman had severe watery diarrhea and weight loss. Computed tomography demonstrated a 55mm tumor in pancreatic tail with enlargement of para-aortic lymph nodes. There was no apparent liver metastasis. Endoscopic ultrasound demonstrated a well-circumscribed heterogenous tumor, which was diagnosed neuroendocrine tumor by endoscopic ultrasound-guided fine needle aspiration biopsy. For suspected VIPoma with para-aortic lymph node involvement, distal pancreatectomy and para-aortic lymphadenectomy was performed. The tumor was diagnosed as VIPoma by immunohistochemistry. The diarrhea improved after surgery. No evidence of recurrence was detected after a follow-up of 11 months.Nov. 2017, Gan to kagaku ryoho. - Japan Society for the Promotion of Science, Grants-in-Aid for Scientific Research Grant-in-Aid for Challenging Exploratory Research, Grant-in-Aid for Challenging Exploratory Research, Kobe University, 01 Apr. 2016 - 31 Mar. 2019The incidence of pancreatic fistula from the cut end of the pancreas in distal pancreatectomy is still as high as 20% to 50%, sometimes leading to fatal complications. We aimed to develop a novel bio-absorbable device for pancreatic stump closure that has never been available before. First, we developed a new bio-absorbable magnesium alloy and verified its safety in vivo. The magnesium alloy was processed into a stapler type and loaded into a staple cartridge of automatic suture instrument. Nest, distal pancreatectomy was performed on 4 beagle dogs. All 4 animals survived and no pancreatic fistula occurred. Autopsy was performed 2 and 6 weeks after surgery. As a result of autopsy, the pancreatic stump was completely closed at 2 weeks and 6 weeks. The magnesium alloy was almost absorbed into vivo at 6 weeks after surgery. These results suggest that development of this novel bio-absorbable device for pancreatic stump can be continued in the future.
