    BACKGROUND: Recent research has suggested that general anesthetics may affect the immune system, but it is unclear how they affect the mitochondria of neutrophils. The effects of general anesthetics on neutrophil-like differentiated HL60 cells mitochondrial function and their production of reactive oxygen species (ROS) were examined in this study. METHODS: HL60 cells were differentiated into neutrophil-like cells by treating them with 1 μM all-trans-retinoic acid for 4 days. The differentiated HL60 cells were exposed to propofol (4 μg ml-1), midazolam (0.5 μg ml-1), or sevoflurane (3 %) for 1 h or 5 h. Following the exposure to the anesthetics, we analyzed the production of ROS, bactericidal effects, cell viability, and apoptosis of the neutrophil-like differentiated HL60 cells. We also investigated the effects of these anesthetics on mitochondrial morphology and function of the neutrophil-like differentiated HL60 cells. RESULTS: Administration of propofol or midazolam activated neutrophil-like differentiated HL60 cells mitochondrial respiration and increased ATP production (P < 0.05). However, there was no significant change in production of ROS or cell death. Exposure to sevoflurane activated mitochondrial respiration, ATP production (P < 0.05) and ROS production (P < 0.05). Additionally, exposure to sevoflurane led to a significant increase in phagocytosis. Filamentous mitochondria appeared after treatment with propofol, midazolam, and sevoflurane, while fragmented and filamentous mitochondria were seen in untreated neutrophil-like differentiated HL60 cells. CONCLUSION: We investigated the effects of three anesthetics on the mitochondrial and bactericidal functions of differentiated HL60 cells. Our results indicate that sevoflurane activates mitochondrial function in differentiated HL60 cells, promotes the production of reactive oxygen species, and enhances bactericidal activity.

    Low back pain during pregnancy is common. However, lumbar disc herniation in pregnancy is rare, and when severe, it can cause lower extremity pain that resists conservative treatment or lead to cauda equina syndrome. In this report, we describe a case of severe lower back pain that did not improve with conservative treatment, necessitating surgery at 12 weeks of gestation. Additionally, we reviewed the literature. A 35-year-old woman, 7 weeks pregnant, presented to our outpatient clinic with right lower extremity pain. Magnetic resonance imaging revealed a herniated disc at the right L4/5. Acetaminophen, a pregnancy-safe analgesic was prescribed. Despite initiating treatment, the pain persisted, and the patient was hospitalized for bed rest. After one month of conservative treatment with no relief, a repeat magnetic resonance imaging showed an enlarged herniated disc. After consulting with the patient and her family, surgery was performed at Kobe University Hospital, which offers comprehensive neurosurgery, obstetrics and gynecology, and anesthesiology services. At 12 weeks of gestation, the patient underwent a microscopic discectomy in the prone position under general anesthesia. Fluoroscopy for surgical level confirmation was minimized. The surgery lasted approximately 1 hour, and the right lower extremity pain was alleviated. Five months later, the patient delivered a healthy child. We report a case of surgery for a herniated disc during pregnancy, along with a review of the literature.

    Very long-chain acyl-coenzyme A dehydrogenase deficiency (VLCADD, OMIM 201475) is a congenital fatty acid oxidation disorder. Individuals with VLCADD should avoid catabolic states, including strenuous exercise and long-term fasting; however, such conditions are required when undergoing surgery. The perioperative management of VLCADD in infants has rarely been reported and details regarding the transition of serum biomarkers reflecting catabolic status have not been disclosed. Herein, we present the perioperative clinical and biological data of cryptorchidism in a 1.5-year-old boy with VLCADD. The patient was diagnosed through newborn screening and his clinical course was very stable. Genetic testing of ACADVL revealed compound heterozygous variants c.506 T > C (p.Met169Thr) and c.606-609delC (p.L216*). The enzyme activity of the patient with VLCAD was only 20% compared to that of healthy control. Left orchiopexy for the pediatric cryptorchidism was planned and performed at 1 and a half year of age. Induction anesthesia involved thiopental, fentanyl and rocuronium. The glucose infusion rate was maintained above 6.6 mg/kg/min starting the day before surgery until the operation was completed. Anesthesia was maintained with sevoflurane at approximately 2%. The serum concentration of tetradecenoylcarnitine were stable during the operation, ranging between 0.08 and 0.19 μM (cutoff <0.2 μM), and never deviated from the reference range. Concentration of other serum biomarkers including free fatty acid, 3-OH-butyrate, and creatine kinase, remained similarly unchanged. In this report, we describe the uneventful perioperative management of unilateral orchiopexy for left cryptorchidism in a 1.5-year-old boy with VLCADD using sufficient glucose infusion and volatile anesthesia.

    AIM: To investigate the anesthetic management of peroral endoscopic myotomy (POEM) and its associated complications. METHODS: This study was a single-center, retrospective, observational study comprising a case series of all patients who underwent POEM in our hospital from April 2015 to November 2016. We collected data regarding patient characteristics, anesthetic methods, surgical factors, and complications using an electronic chart. RESULTS: There were 86 patients who underwent POEM in our hospital during the study period. Preoperatively, patients were maintained on a low residue diet for 48 h prior to the procedure. They were fasted of solids for 24 h before surgery. There was one case of aspiration (1.2%). During POEM, patients were positioned supine with the upper abdomen covered by a clear drape so that pneumoperitoneum could be timeously identified. In three cases, the peak airway pressure exceeded 35 cmH2O during volume controlled ventilation with tidal volumes of 6-8 mL/kg and subsequent impairment of ventilation. These cases had been diagnosed with spastic esophageal disorders (SEDs) and the length of the muscular incision on the esophageal side was longer than normal. CONCLUSION: In the anesthetic management of POEM, it is important to prevent aspiration during induction of anesthesia and to identify and treat complications associated with CO2 insufflation.

  • [Controversy in the treatment of acid-base abnormalities].

    Sodium bicarbonate has been standard therapy for the treatment of acidosis. In lactic acidosis and hypercapnic acidosis, however, there is no clinical data supporting its effectiveness. We reviewed the literature of the efficacy of sodium bicarbonate on lactic acidosis and hypercapnic acidosis. On both conditions, we have no solid evidence supporting its beneficial effect. Conversely, acidosis or hypercapnia might be protective in acute lung and systemic organ injury. Therefore, the unprepared use of bicarbonate might be harmful in terms of fluid and sodium overload and excess lactate concentrations. According to current literature, we cannot recommend sodium bicarbonate administration for patients with lactic acidosis and hypercapnic acidosis.

  • 小児がんに対する麻酔薬の選択が予後および免疫機能へ与える影響

    本研究は、血液悪性腫瘍と診断され、全身麻酔を要する小児患者において、使用する全身麻酔薬として吸入麻酔薬と静脈麻酔薬を比較し、化学 療法効果率、再発率、骨髄幹細胞生存率(Cell viability), 末梢血中の好中球・単球・リンパ球数および白血球における割合、血中INF-γ濃度、TGF-β濃度、 IL-4濃度の推移を検討することで、麻酔薬の選択が小児悪性血液腫瘍患者における予後および免疫機能へ与える影響を明らかにすることを目的とした無作為化比較試験である。 当該年度には対象患者の抽出前に、過去に研究対象の条件を満たした症例を後方視的に検討した。過去の手術及び麻酔件数から研究対象患者の人数、術後の血球数など通常の診療の範囲内で測定される数値の推移につき情報を収集した。血液腫瘍患者で全身麻酔を受ける患者の数は研究で検討できると考えられた。また、過去の研究報告などから統計学的に必要とされる症例数を割り出した。これまでの小児血液腫瘍患者では、麻酔法の選択は担当麻酔医へ任されていたが、吸入麻酔薬、静脈麻酔薬どちらも使用されておりその比はやや吸入麻酔薬が多かった。 そのため、本研究にて吸入麻酔薬、静脈麻酔薬を使用する症例を同数ずつ割り付けても、これまで提供してきた麻酔とそれほど大きな隔たりはないものと考える。 現在院内での倫理委員会への申請準備を行っており、承認を得られ次第、順次対象患者の抽出、無作為化、割り付けへと進めていく予定である。
