• Measurement of laryngeal elevation time using a flexible surface stretch sensor

  BACKGROUND: Dysphagia is a growing health problem in aging societies. An observational cohort study targeting community-dwelling populations revealed that 16% of elderly subjects present with dysphagia. There is a need in elderly communities for systematic dysphagia assessment. OBJECTIVE: This study aimed to verify whether laryngeal elevation in the pharyngeal phase could be measured from the body surface using thin and flexible stretch sensors. METHODS: Thirty-two elderly subjects (17 males, 15 females; mean age ± SD: 89.2 ± 6.2 years) with suspected dysphagia underwent a swallowing contrast examination in which seven stretch sensors were attached to the front of the neck. The elongation of the sensors was measured and compared to the laryngeal elevation time values obtained using videofluorography. The sensor signal detected the laryngeal elevation start time, conclusion of the descent of the larynx, and the laryngeal elevation time. The respective laryngeal elevation times obtained using videofluorography and using the sensor were compared using the Bland-Altman method. RESULTS: The laryngeal elevation time was 1.34 ± 0.46 s with the stretch sensor and 1.49 ± 0.56 s with videofluorography. There was a significant positive correlation between the duration obtained by both methods (r = 0.69, p < 0.0001). A negative additional significant bias of -0.15 s (95% confidence interval -0.30 to -0.03, p = 0.046) was noted in the laryngeal elevation time from the videofluorography measurement. CONCLUSION: Laryngeal elevation time can be measured noninvasively from the neck surface using stretch sensors.

• 保健専門職による住民組織のコミュニティ・エンパワメント過程の質的評価指標の開発

  Purpose : The purpose of the present study was to develop a qualitative evaluation index on the empowerment process of residents' organizations, from the perspective of public health professionals. This study examined the credibility, transferability, and usability of the index. Method : The index was developed based on the results of preliminary research. Following this, the author conducted self-report questionnaires twice ; these questionnaires were mailed to public health professionals who supported community development in health promotion. The respondents were required to evaluate each residents' organization with the index, score the state of the community empowerment process, and evaluate the development of the organizations' activity in five stages. They were also asked to consider the usability of the index. The data were analyzed statistically with the help of descriptive statistics, correlation coefficient, and t-test. Results : The index was comprised 3 areas and 14 items. The first survey had 129 respondents (response rate: 33.3%), and 125 valid responses were analyzed. A total of 46 responses were studied (response rate : 12.0%). The credibility of the evaluation index was confirmed by significant differences in all 14 items at the beginning and end of the health professionals' support. Further, the applicability of the index was confirmed because 97.8% of the respondents in this survey answered that the index was useful for their practice, and 91.3% replied that the index was of practical use in their activities. Conclusion : The result of this study suggests that this index has high applicability for evaluating the empowerment process of residents' organizations.

  本研究の目的は,恒久住宅で一人暮らしをしている阪神・淡路大震災の被災高齢者の生活力量の形成過程と,それに影響する要因を明らかにすることである.被災高齢者10名を対象に半構造化面接を行い,質的・帰納的方法にて分析を行った.その結果,被災高齢者の生活力量形成過程は,他者の支援利用段階,隣人支えあい段階,地域貢献段階の3段階がみられた.生活力量形成に影響した要因は,社会資源の存在,助け合える隣人・友人の存在,助け合える家族の存在,住居等の生活環境,情報の存在の5つであった.被災高齢者の生活力量形成過程とは,被災高齢者が自分の置かれた状況に気づき,隣人同士で助け合い,地域の問題にかかわっていく過程と定義できた.

• 通所ケア施設における痴呆性高齢者中心の介入方法－看護・介護職におけるアクティビティ・プログラム実施に焦点をあてて－

  The purpose of this study is to clarify the intervention methods by the nurses and caregivers in finding activity programs to draw out activity from the elderly people with dementia. As for the research method, interviews were carried out towards 18 skilled nurses and caregivers in the field of giving care for the elderly suffering from elderly people with dementia by using quasi open-ended type questionnaire sheet, and the analysis was implemented with revised Grounded Theory. From this study, it was found that there are 2 methods of making intervention by the nurses and caregivers to draw out activity from those elderly people with dementia; one not based on the dates of each of the individual, and the other based on such dates. In addition to this, the models of activity programs could be formulated based on the data of the individual elderly people with dementia. Therefore, it is possible to conclude that the decision-making regarding activity programs can be done, not based on the choices of the available programs but on the needs of the elderly people with dementia.

• 痴呆性老人を介護する嫁の介護負担感と現状認識との関連

  The purpose of this study was to clarify the relationships between the subjective burden of caregivers and the perceived stages of dementia care or the characteristics of caregivers and dementia patients. The subjects were daughters-in-law who were taking care of their fathers-or mothers-in-law with dementia using day care services in the Kinki districts. A questionnaire survey was conducted on 302 dementia patients in 90 randomly selected day care centers, and evaluable replies were obtained from 78 patients in 33 centers. As a result, CBS factors that influenced the perceived stages were assistance of daily life and correspondence to dementia symptoms. In addition, during assistance of daily life, CBS was found to increase with the age of daughters-in-law (caregivers), absence of experience in care giving, and uncleanly behaviors or a delusion of persecution in dementia patients. During correspondence to dementia symptoms, CBS was found to correlate with the absence of experience in care giving by daughters-in-law, necessity of assistance during movement of dementia patients, and uncleanly behaviors by dementia patients. These findings suggest that assistance promoting the understanding of dementia and methods of dealing with dementia symptoms such as communication with dementia patients is essential to increase the perceived stages of dementia care and QOL in dementia patients and caregivers. The results also suggest that promoting the use of day care services, communication with fellowmen and women, and assistance in learning are necessary to increase the perceived stages of care giving by daughters-in-law, caregivers.

• 女性介護者の就労継続を阻害する要因と利用者要因

  The purpose of this study was to evaluate the optimal method of assistance during the future use of care services by clarifying the relationships between factors that interfere with the continuation of work by female caregivers and the actual status of using care services. Regarding 41 items related to factors that interfere with the continuation of work, a questionnaire survey was conducted on 113 women who were actually taking care of the frail elderly as primary caregivers. As a result, factors related to the details of work and those related to caregivers themselves were found to greatly influence the continuation of work by female caregivers. In addition, these factors were closely associated with the degree and duration of care giving. Factors related to the frail elderly they were taking care of, those related to care services, those related to their family, and those related to their job did not significantly influence the continuation of work by female caregivers. Although the subjects did not frequently use care services, they demanded various care services, demonstrating insufficiency of the current care service system. The results of this study may help regional nurses to assist female caregivers in continuing their work, as well as to consider the viewpoint of providing care services and the method of intervention.

• 阪神・淡路大震災被災高齢者の生活力量形成過程－恒久住宅に住む一人暮らし高齢者３事例の被災後のプロセスから－

  The purpose of this study is to reveal shaping process of the ability to control life of the elderly suffering from the Great Hanshin-Awaji Earthquake. The elderly sufferers continue the living in spite of disastrous environmental change helping each other and solving problems by themselves. In this research, such positive side is examined in term of the ability to control life. Semi-structured interviews were conducted with 3 elderly sufferers living in the eternal apartment houses for the sufferers. Qualitative analysis method was employed. As a result, the shaping process of the ability to control life is divided into 3 times, the earthquake and the evacuation, the temporal housing unit, and the eternal apartment houses. The ability of abandonment was formed in all processes. In the earthquake and the evacuation, 6 abilities including the acceptance of disaster victims helping each other were formed. In the temporal housing unit, 5 abilities such as building relation with disaster victims and helping each other were formed. In the eternal apartment houses, 5 abilities, for example, reconstruction of relation with disaster victims and mutual support were shaped.

• 阪神・淡路大震災被災高齢者の生活力量に関する研究－被災４年半後の恒久住宅における一人暮らし高齢者の生活から－

  The purpose of this study is to make evident the elements affecting of the independence of elderly victims of the Great Hanshin-Awaji Earthquake after four and half years, who were living alone in eternal apartment compleyes. Semi structured interviews were conducted with ten subjects. Qualitative analysis method was employed. As a result, the elements found were the acceptance of the suffering experience, giving up, relationship and mutual support with disaster victims; utilization of social resources, relaxation behavior, and the management of living alone. Specifically, the acceptance and surrender were features for the elderly victims. Utilization of social resources was linked with keeping relationship with other victims. This result suggests that nursing staff should pay attention to the ability of self determination and make the social resources available for the elderly to have this ability.

• 在宅高齢者ニーズとサービス利用に関する利用者とケアマネージャーの認識の違い

  The purpose of this study is to consider the ways to support care service users after the implementation of the Long Term Care Insurance System, by examining the survey which was conducted before the introduction of the above scheme to look into the difference between care service users and care managers in terms of grasping the care needs and usage of the care services. The subjects included care managers of health care centers in the municipalities and those of the home care support centers, and 481 care service users to whom they offer the care management. The results of this study are as follows. Care managers were grasping diverse needs especially regarding those which relate to the health issues of the persons who receive nursing care and of their families, difficulty in their managing daily lives, and the burden caused by the nursing. These needs were found to be consistent with those held by care service users. The number of needs grasped by care managers was higher than the one grasped by the care service users. In particular, regarding the needs for using care services and the needs for having opportunities by care service receivers to be involved in the social interchange or participation in the society, these points were mainly grasped only by care managers. Regarding informal care services and usage of nursing-related subsidies, public health nurses were grasping more deeply than the care service users. In the future, it is necessary for the care managers to be empowered more and to increase their skills of providing information in order to encourage more positive participation by the care service users in the care service scheme.

• 介護予防アセスメントツールの開発―試案項目の妥当性の検討―

  This study was to develop a tool for public health nurses to assess risk of theelderly who need care, and show guides to prevention activities against requiring care. The elderly also find factors blocking successful aging by using the tool for themselves. The current study focused on examination of validity of the tool's items. The questionnaires of the tool were conducted with the two groups. The first group was consisted of the elderly who participated in care prevention services. The second was made of the healthy and independent elderly. Besides, public health nurses rated risk of the elderly who need care. The difference of the average value of two groups was tested. And correlations of the total score of the tool and the judgment items were analyzed. As a result, significances were found in 31 items including interaction with society, active health promotion, daily life, and health condition. Correlation was between the judgment item and the total score.

• 介護予防アセスメントツールの開発―項目収集と試案作成―

  The purpose of this study was to develop a tool that enables community care specialists, such as public health nurses, to perform comprehensive assessment to identify elderly people in whom future health care needs may develop, and also enables elderly people themselves to simply assess factors that affect healthy long life. To collect items for comprehensive assessment, we conducted a group interview with 8 public health nurses and evaluated the literature. From the obtained items, some were selected by the researchers and the public health nurses, and a survey was performed on public health nurses (effective replies, 137) with 5 or more years of experience to perform weighting of each item and determine the validity of the contents. As a result, 50 items associated with care prevention needs were divided into the following 8 categories: 1) health promotion by elderly people themselves, 2) physical condition, 3) nature of daily living, 4) social contact, activity, 5) housing and surrounding environment, 6) family conditions, 7) changes in the surroundings or feelings, and 8) supporters and the use of services. This assessment tool contained items from the professional view of community nursing that are not incorporated in the existing assessment tools, such as health promotion by elderly people themselves, living rhythm, and life events. Further studies are necessary on assessment criteria and measures to make the best use of obtained results for the activities of public health nurses.

• A Study on a Model of Support by Elderly People in the Community to Support Children with Developmental Disabilities and Their Families in a Community Symbiotic Society

  本研究の目的は地域共生社会における発達障害児と家族を地域で支える高齢者による支援モデルを検討することである。本研究における支援モデルの検討に当たっては、「育てにくさに寄り添う支援の重要性」と「地域共生社会における多様な支援のあり方を検討する必要性」に重点を置いている。発達障害児とその家族が居住する地域で長く健やかに生活していくためには、療育等に加えて地域全体での支えが重要と考えられる。 2021年度は昨年度に引き続くCovid-19のさらなる感染拡大により、研究協力を得る予定であった保健所等が非常時レベルの状況であったこと等から関係機関との調整が難しい状況が続いていた。また、まだワクチン接種の対象でなかった子どもを含む親子へのアプローチへは慎重な対応が必要であった。そこで、まず、先行研究における調査結果を活用し、さらなる基礎的検討を行った。本研究では組織的活動に長けていると考えられる男性に注目している。サポートの種類別に関連要因を探ったところ、女性はサポートの種類によって関連要因が異なっていたが、男性はどのようなサポートにおいても「社会貢献への関心の高さ」が関連していた。そこで社会貢献意欲と発達障害をもつ子と親へのサポートに関連する文献検討を行ったところ、関連するキーワードとしてこれまでのキーワードに加えてstigmaが抽出され、一般的な子育て支援に加えて熟慮すべき可能性が考えられた。今後はこの点に留意しながらインタビュー調査等を進めていく計画である。


• 保健師の住民参加促進力量向上教育プログラムの開発

  本研究の目的は、行政保健師の住民参加を促進するための力量を向上させる教育プログラムの開発することである。本年度は学習ニーズを把握し教育プログラムを作成するために、住民参加の促進する技術と関連要因を明らかにすることを目的に、A県内の行政機関に所属する保健師を対象に郵送による無記名自記式調査を実施した。 当初、無作為抽出により全国調査を計画していたが、新型コロナウイルス感染症の拡大による行政機関の業務逼迫の状況を踏まえ、1つの都道府県内の悉皆調査に変更した。調査項目は、精選を繰り返し、48項目から33項目に集約した。行政機関を定年退職した保健師、行政保健師経験を持つ医療機関の保健師にプレテストを実施し、調査票を作成した。 調査項目は、属性（年齢、所属、職位、基礎教育、最終学歴）、住民参加を促進する技術の自己評価項目33項目、保健師の専門職務遂行能力尺度20項目、保健師の自信尺度17項目である。住民参加の促進技術の自己評価33項目の結果から、技術の因子構造を明らかにし、特定した項目を得点化し、属性毎の比較検討、高得点群と低得点群の比較等により、住民参加の促進技術の関連要因を明らかにする。保健師の専門職務遂行能力尺度及び自信尺度との相関の確認し、尺度の妥当性を検証する。明らかにした因子と各尺度の因子の関連を分析し、住民参加の促進技術の特徴を把握する。約1200名に配布し、約250名から回答を得て、現在結果の分析中である。

  The purpose of this study was to examine an effective child-rearing support program that emphasizes community ties by the elderly generation. As a result of various considerations, we decided to focus on elderly men who are less connected to the community and who face challenges such as maintaining their health after retirement. This is because, finally, it aims to contribute to the health of the elderly themselves. Based on the results of a survey of older male respondents and the collection of opinions from relevant organizations, we discussed an introductory training program for older males to support child rearing in their communities.

  The objective of this study was to create an education program for public health nurses (PHNs) in order to improve their competency as clinical instructors. The results indicated that an education program which could improve both the educational skills and professional competency of PHNs was required. Moreover, in order to further foster the collaboration between faculties and PHNs, it was suggested that education programs conducted by educational institutions could be more effective and practical than those conducted by governments. Because PHNs from small- and middle-sized local governments have less experience of attending preparatory programs for clinical instructors and less self-confidence to perform the role of clinical instructors, education programs should be especially targeted for such PHNs.

  We developed the Self-evaluation Scale for Roles of a Public Health Nurse as a Clinical Instructor (SSRPC) to assess the performances required for public health nurses (PHNs) to fulfil the role of clinical instructor.According to multiple regression analysis, professional identity level, benefits from the clinical instructors’ role experience, previous education on the role ofa clinical instructor and co-workers’ support were positively associated with SSRPC. A number of participants in this study required education for the role preparation. There is no empirical study of support program for PHNs as clinical instructors. Therefore,further research is required to develop more effective preparation programs for PHNs.


• Development of Useful Method that PHNs (Public Health Nurses) Collaborate and Coordinate between Medical and Community HealthInstitutions to Prevent the Abuse of High-risk Children

  We conducted the mail survey in order to clarify the collaboration and coordination that aims to prevent the abuse from high-risk children between the medical institutions and the community health organizations. The subjects were three hundred nineteen coordinators of Perinatal/Regional Maternal and Children's Medical Center. As a result, most coordinators had collaborated with the partners in the community health organizations to prevent the abuse from high-risk infants. The interview research was also conducted to explore the factors that enable public health nurses (PHNs) to collaborate with the partners of relevant organizations. We selected two prefectural public health centers, one health center in Tokyo Metropolis, and two city health centers. Because we made sure by the literature review that PHNs of these centers had constructed the useful community support network for preventing the child abuse. Participants were eight PHNs of those centers. Data were collected through 5 in-depth interviews that explore participants’ experiences about collaboration with the partners during the process of constructing the useful network. These were identified as the important factors that PHNs participate in the planning department and PHNs exert the coordinating function as the professional.


• Development of educational programme and teaching materials to enhance professional and comprehensive competency on Public Health Nurse(PHN)who work in public health centre.

  The purpose of this study was to develop the educational programme and the teaching materials in order to strengthen professional and comprehensive competencies of PHNs working at public health centers. We developed the educational programmers, the teaching materials and the evaluation tools, according to the results of former studies such as "Specific PHN competencies which are required to strengthen", "Action Research in order to enhance PHN competencies to evolve their activities", and the collection of existent educational methods/programmers and existent PHN competency standards of advanced nation through visitation and literature review. As educational programmers, "the tentative programme to develop 5 specific competencies", "the programme to motivate to do study for continuing professional development", "the programme to improve the competencies to demonstrate the necessity and the effects of actions with accountability", "the programme to upgrade PHN competencies through one year facilitation", "the programme to develop competencies using narrative DVD materials by a experienced PHN", "assorted work-sheets for these programmers", "home page to upload these materials" were developed. As evaluation tools, "the scale to measure competencies to protect equality in resident health and well-being", "the scale to measure competencies to generate / create policies and social resources", "the scale to measure competencies to empower the population's capacity", "the indicator to assess competencies to demonstrate the necessity and the effects of actions with accountability", "the scale to measure competencies to develop and establish PHN professionalism", "the indicator to assess the suitable way of facilitation and to evaluate PHN learning outcomes" were developed. These results can be utilized for on the job training, postgraduate education and basic education to upgrade PHN competencies, and it is promising to contribute to promoting public health and health promotion.

• 住民のヘルスプロモーション活動強化のためのモデル開発-保健師による支援モデル提言-

  本研究の目的は、住民のヘルスプロモーション活動強化のために、保健師による支援モデルを開発することである。 研究は、四段階で行った。第一、二段階の目的は、ヘルスプロモーションのめざすゴールの明確化とその妥当性の検討であった。調査方法は、フォーカス・グループディスカッションであり、2回の調査を行った。調査対象者は、ヘルスプロモーション学識経験者3名と熟練保健師1名(1回)と、健康な地域づくりに熟練している住民5名(1回)であった。データ分析後、対象者に対し、妥当性の確認を行った。また、カナダのヘルスプロモーション先進地での研修を行った。 第三、四段階の目的は、活動モデル試案の作成とその妥当性の検証であった。方法は、質的・帰納的方法であった。対象者は、ヘルスプロモーション先進地域の保健師であり、面接による調査を行った。調査結果は、対象者に文書で提示し、妥当性の確認と意見に基づく修正を行った。 本研究の結果、ヘルスプロモーションがめざすゴールとして、コミュニティ・エンパワーメントの状態が明らかになった。その状態は、個人、対人関係、組織、地域の領域に大別され、相互作用がみられた。また、住民組織が、自ら社会資源となり、地域社会に働きかける状態がみられた。カナダの先進地研修では、カナダのヘルスプロモーション実践や、保健師に求められる能力等の情報収集を行い、海外事情として日本に紹介した。 さらに、住民のヘルスプロモーション活動を強化する保健師の活動には、「活動準備期」と「活動開始から主体的な活動確立まで」、「活動確立期」の過程がみられた。保健師の技術には、「住民が主体的に取り組むニーズを捉え」、「住民の参加と気づきを促す」という特徴がみられた。また、保健師は、「行政内で活動予算を確保する」、「行政職員に対して住民活動への参加を促す」等の、住民の活動を支援する環境を整備する技術を発揮していた。


• Development of public health nursing practice models through action research to fulfill the needs not covered by the Long-term-care Insurance Law

  The purpose of this study was to develop public health nursing practice models to fulfill health needs of the elderly and disabled people not covered by the Long-term-care Insurance Law. The method of this study was action research and the subjects were public health nurses. Descriptive data about their practice processes included defining the needs, planning, implementation and evaluation was collected by observation, their practice documents and reflective discussion between subjects and researchers. The needs concerned with the specialty of public health nursing not covered by the Law were addressed as following, 1)Setting the basic system of specialized assessment and team approach for intractable diseases, elderly abuse, etc., 2)Planning and execute the skill up training program for quality improvement of the profession concerned, such as care-managers, home-visiting nurses, etc., 3)Integrating similar serious needs of each case and developing the prevention and support system for solving them in each municipality, 4) Facilitating the decision making of each municipality and enhancing the participation of citizens to improve those systems and actions. These case data about subjects' activities worked out effectively were analyzed to clarify the better ways of activity for fulfilling the needs not covered by the Law. Now we have been discussing about the practice models for each need and after creating the experimental models, we will verify their practicability.


• Care Management for the Needs of Intractable Plural Involving Public Health nurses

  1. Purpose : This study aims to clarify those needs of preventive health care that involve public health nurses and to systematicallyput in order the developingprocess of caremanagement for those cases. The needs were (1) intractable illness, (2) frail elderly, (3) before and after an application for the. Long Term Care Insurance. 2. Methods : We used a researching method of personal interviews. Care management process action index was used in the analysis framework in order to read situations, and processes by units, of activities taken by them with their own words. 3. Result : (1) The 13 cases who had intractable illness needs were analyzed. The needs of 10 have been extracted as needs concerning the betterment of relevant medical care, assistance to user' s home care and daily living, improvement of home care environments. The characteristics of the care management were development of a service, taking action on the subjects. Also included were individual interrelations so that service will best fit the needs, taking action to improve the regime of related institutions, (2) The 12 cases who had low level care needs were analyzed. The needs of 11 were classified as that result. It thought that the needs of the daily life confirmation, covering the service which was not provided officially, and development of a service were especially characteristic of that. For managing these needs, they developed informal support network, provided service by themselves temporally, and tried to formulate newpolicy. (3) The 6 cases who supported by them before and after an application for the Long Term Care Insurance were analyzed. The needs of 11 were classified. The characteristics of the care management were as the next. They supported service timely when inhabitant used the Long term Care.
