- Background & objectives: A high incidence of gallbladder cancer (GBC) is observed in northern India. This study was aimed to identify the factors involved in developing GBC in this region. Methods: A gallstones-matched, case-control study was conducted in northern India. Ninety nine patients with GBC and gallstones (33 men and 66 women, mean age of 51.4 yr) comprised the case group, while 99 patients with cholelithiasis (40 men and 59 women, mean age of 45.7 yr) comprised the control group. All participants were interviewed to complete 183 questionnaire items that included 105 food items. Potential risk factors were identified using a multivariate analysis adjusted for age and sex. Significant risk factors were identified using a stepwise logistic-regression analysis. Results: Age (≥50 yr), education (illiterate), socioeconomic status (≤below poverty line), bowel habits (≤once a day), hypertension history, hypotensive drug use, non-vegetarian diet, use of firewood for cooking, tap water drinking, hand pump water drinking and high consumption of coffee and sweets were identified as the potential risk factors. In women, factors included menarche (<13 yr), number of pregnancies (≥3 pregnancies) and parity (≥3 babies). Of these factors, age, education, bowel habits, tap water drinking and multiple pregnancy and/or multiparity were identified as significant risk factors, whereas a high consumption of coffee and sweets or hypotensive drug use and/or hypertension history were protective factors. Interpretation & conclusions: Poor bowel habits and drinking unsafe water appear to be the main risk factors for developing GBC. - BACKGROUND: Typhoid (Salmonella typhi and paratyphi) carriers and gall bladder cancer (GBC) are endemic in northern India. Results of previous studies about association of typhoid carriers with GBC are inconsistent. We studied antibodies against Salmonella typhi and paratyphi in serum samples of patients with GBC. METHODS: We performed modified Widal test for antibodies against Salmonella typhi (Vi and O) and Salmonella paratyphi (AO and BO) antigens in patients with GBC (n=100), xanthogranulomatous cholecystitis (XGC, n=24), chronic cholecystitis (CC, n=200) and healthy controls (HC, n=200). RESULTS: Serum antibodies against Salmonella were more frequently positive in GBC (22%) and XGC (29%), particularly in males in age ≥50 years (GBC: 47% and XGC: 50%) vs. HC (0) (p <0.01). Vi antibody was more common in GBC (13%, OR:9.8) and XGC (8%, OR:5.9) than HC (2%). O antibody was more common in GBC (8%, OR: 8.6) and XGC (8%, OR: 9.0) than HC (1%). O antibody was also more common in males with GBC (12%) than CC (1%) and HC (1%) (P=0.02 and p <0.001, respectively). AO (6%) and BO (4%) antibodies were detected in GBC, particularly in males, than HC (0), (p <0.01). Salmonella antibodies were more frequent in GBC with GS than those without GS (50% vs. 20%, OR=3.94, P=0.01). CONCLUSIONS: Salmonella carrier state was more common in GBC and XGC, particularly in elderly males than HC. The Vi antibody was more common in GBC and XGC than HC. - BACKGROUND: Carcinogen metabolism pathway and tumor suppressor gene polymorphisms have been reported to be associated with increased gallbladder cancer risk. However, the association of genetic variants and gallbladder cancer risk in Indians are not well studied. We examined whether genetic polymorphisms of metabolic enzymes cytochrome P450 1A1 and glutathione S-transferase and tumor suppressor gene p53 (TP53) are associated with an increased risk of gallbladder cancer in North Indians. METHODS: This hospital-based case-control study was conducted in 96 gallbladder cancer patients with gallstones (cases) and 93 cholelithiasis patients (controls) at the Sanjay Gandhi Postgraduate Institute of Medical Sciences in Lucknow, India from July 2014 through May 2017. Genomic DNA was extracted from white blood cells of each patient using a simple salting-out procedure. The genotypic frequencies of CYP1A1 rs4646903, CYP1A1 rs1048943, and TP53 rs1042522 polymorphisms were investigated using TaqMan SNP Genotyping Assay and GSTM1 and GSTT1 polymorphisms were analyzed using the multiplex PCR assay. RESULTS: The frequency of CC genotype of TP53 rs1042522 polymorphism was 27.1% (26/96) in cases and 12.9% (12/93) in controls. The CC genotype was associated with an increased risk of gallbladder cancer in North Indians (age- and sex-adjusted odds ratio, 2.81; 95% confidence interval, 1.19-6.61; P = 0.02). No significant differences in genotypic and allelic frequencies of the metabolic pathway gene polymorphisms were found between cases and controls. CONCLUSIONS: Our data provide preliminary evidence that the CC genotype of the TP53 rs1042522 polymorphism may be associated with an increased risk of gallbladder cancer in North Indians.Dec. 2019, Asian Pacific journal of cancer prevention : APJCP, 20(12) (12), 3643 - 3647, English, International magazine[Refereed]Scientific journal

- Drinking water survey to elucidate risk factors in a region with high incidence of gallbladder cancer in IndiaPeople living near the Ganges River Basin located in northern India show a high incidence of gallbladder cancer, but the onset mechanism for this cancer in this region has not been well defined. Therefore, we conducted a drinking water survey of people living in this region to obtain clues regarding the causes of gallbladder cancer. Samples of Ganges River water, public tap water, public well water from two spots, and well water from four gallbladder cancer patients' homes were collected in September 2017. Lead levels, bacterial counts, levels of agricultural chemicals, iron, copper, nitrate, nitrite, and chlorine, pH, and hardness were measured using a simple commercial water analysis test kit. The results obtained were then compared with the water quality standards for drinking water in Japan. Of the 10 items, bacteria, iron, and nitrate were present at levels beyond the standard levels. Additionally, three of the four well water samples collected from gallbladder cancer patients' homes were contaminated with bacteria, and two had iron levels higher than the standard levels. - 胸郭圧迫した時、肺機能にどのような影響が及ぼすか健常者を対象に検証した。健常成人男性29名年齢20~24歳、女性28名年齢20~24歳を胸郭圧迫しない状態と胸郭圧迫(胸郭をコルセットを装着)した状態でスパイロメトリーから%肺活量と1秒率をフローボリューム曲線から最大呼気流量とV50/V25を計測し検証した。%肺活量と最大呼気流量では胸郭圧迫しない状態と胸郭圧迫した状態では有意差がみられた。1秒率とV50/ V25は有意差がみられなかった。胸郭圧迫した状態は拘束性換気障害と同じ状態の検査データが得られ、シミュレーション教育に活用できることが示された。北陸大学, Mar. 2016, 北陸大学紀要 = Bulletin of Hokuriku University, (40) (40), 154 - 159, Japanese

- We determined the conditions of preventive measures against <i>Norovirus</i> infection among food handlers in Niigata Prefecture and the infection rate among healthy carriers. A questionnaire with 27 questions was sent to 33 food handlers. Of the 27 questions, 22 were direct yes-or-no questions. The data obtained were compared between two groups (group A: employers with ≤ 10 employees; group B: employers with ≥ 11 employees). <i>Norovirus</i> genotypes I and II were detected in feces samples collected from 377 employees (204 men and 173 women) by real-time reverse transcription-polymerase chain reaction analysis. The numbers of items for which all food handlers in groups A and B acted in accordance with the teaching guidelines were two (2/22, 9.1%) and seven (7/22, 31.8%), respectively (<i>p</i> < 0.05). The rates of compliance with the guidelines, namely, "use of a nail brush in hand washing" and "their own manual preparation regarding the prevention of food poisoning", were significantly lower in group A than in group B (<i>p</i> < 0.05). Genotype II was detected in the samples from two men (0.98%) in group A. - Evaluating The Result of Learning in The Students at The Department of Clinical Engineering and Medical Technology Using S-P Score Table AnalysisWe have tried to introduce the method of Student-Problem Score Table Analysis(S-P Table Analysis)in order to evaluate more objectively the degree of learning in the students of The Department of Clinical Engineering and Medical Technology. As a source of data for evaluation, the results of examination concerning General Clinical Medicine, which were obtained from the mark-sheets used for answering according to the state examination style, were used. These mark-sheets were copied with the copying machine equipped in our university, from which their imaging data were transmitted to my personal computer in my room via the run network of university, followed by transformation into the CSV files. These data collected were analyzed according to S-P Table Analysis, using the software developed in our university group. From the analyzed data, several informations were extracted to use for educating the students individually and to devise my lectures and examinations given. In conclusion, the analysis using S-P Table Analysis is very useful not only for evaluating and grasping the degree of learning at the levels of individual student as well as of whole students, but also for devising our teaching methods.新潟医療福祉大学臨床技術学科では学生の修学効果をより客観的に評価するためにS-P表分析法の導入を試みた。臨床医学総論の定期試験を国家試験に準じた5 選択肢形式で行い、解答マークシートを新潟医療福祉大学設置のコピー機で読み込み、その画像データを研究室のPCで学内ランインターネットを通し受け取る。その後にこの画像情報を市販ソフトでCSV化した。さらに収集データを新潟総合学園で開発されたソフトを用いS-P表分析を行った。得られた結果を基にして受験学生の個人データを抽出して、学生の個人教育指導を行った。また出題した問題の解析により授業内容、問題作成の改善に活用した。結論としてS-P表分析は学生の修学評価を学年全体、個人レベルで把握する上でも、修学効果を上げる教育法の改善という面からも有益であると考えられた。新潟医療福祉学会, Mar. 2015, 新潟医療福祉学会誌, 14(2) (2), 28 - 32, Japanese

- Parasite infections among people living in a high altitude area of La Paz, BoliviaDetermining the accurate rate of human parasite infections is of great importance in developing a strategy to prevent the infections. We aimed to demonstrate the parasite infection rates in people living in a high altitude area of La Paz, Bolivia. Feces were collected from 301 people, but 274 (137 males and 137 females) were considered as our study subjects because in 27 the age or sex was unknown. Feces were examined by the formalin-ether medical sedimentation method to find helminth eggs and protozoan cysts. The rates of helminth, protozoal and mixed infection with both of them were 1.8% (5), 39.1% (107) and 2.9% (8) cases among 274 examinee, respectively. The rates of the 4 helminth detected were as follows: 0.7% (2), Ascaris lumbricoides; 2.2% (6) Trichuris trichiura; 2.2% (6) Hymenolepis nana; 0.4% (1) Strongyloides stercoralis; and those of 5 protozoan parasites were 15.0% (41) Giardia lamblia; 3.3% (9) Entamoeba histolytica: 22.6% (62) Entamoeba coli; 4.7% (13) Endlimax nana; 1.1% (3) Iodamoeba buetschlii, respectively. The Infection rates were 42.3% (58/137) in males and 45.3% (62/137) in females. No significant differences were recognized in the detection rates between the males and the females. However, the infection rate of 7.7% (2/26) of E. coli in the females at age of 15 years and elder was significantly lower than that of 29.7% (33/111) in those younger less than 15 years old. Protozoal infection accounted for a large portion of the parasite infections among the subjects. - Usefulness of Bethesda system for uterine cervical cancer screening in Niigata PrefectureUterine cervical cancer screening using the Bethesda system was put into use in Niigata Prefecture in 2010. However, the effectiveness of the screening using the Bethesda system has not been well evaluated. We aimed to compare the results of smears diagnosed using the Bethesda system in 2010 with the results of smears diagnosed using the Nichibo classification from 1995 through 2009. A total of 337,029 women were screened for cancer. The diagnoses obtained using the Bethesda system (18,449 examinees) were compared with those obtained using the Nichibo classification at the period of 15 years (318,580 examinees). The rate of abnormalities and the detection rate of dysplasia were displayed significant difference between Nichibo classification and Bethesda system (p<0.05). However, no significant difference was found in the cancer detection rate between the two classification methods. - Japan Society for the Promotion of Science, Grants-in-Aid for Scientific Research, Grant-in-Aid for Scientific Research (C), Niigata University, 01 Apr. 2023 - 31 Mar. 2026Streptococcus anginosus group bacteria in the gallbladder bile and developing gallbladder cancer2023年度の予定は、インドと日本の研究者の打ち合わせ(アンケート内容、胆汁採取法・保存法、リアルタイムPCR法、統計解析法、学会報告及び論文執筆の分担、その他)後、Mahatma Gandhi Medical College and Hospitalと新潟大学で倫理委員期の承認を得て、150人の胆嚢がん患者と200人の胆石を有する胆嚢炎患者からアンケート調査と胆汁採取を行うことであった。 2023年12月にインド、ジャイプール市で開催された第3回Jaipur Surgical Festivalに参加し、日本とインドの胆嚢がん患者における遺伝要因に関する研究成果の発表を行った (The polymorphism of p53 tumor suppressor gene and the risk of gallbladder cancer)後、インド研究者との研究打合せを行い、本研究実施についての合意を得た。その後、インドと日本で倫理委員会の承認を得てアンケート調査と胆汁採取を開始した。年度内に胆嚢がん患者45人、胆嚢炎患者168人からアンケートと胆汁試料を採取できた。しかし、目標とした胆嚢がん患者150人、胆嚢炎患者200人には達することができなかった。
- Japan Society for the Promotion of Science, Grants-in-Aid for Scientific Research, Grant-in-Aid for Scientific Research (B), Juntendo University, 01 Apr. 2021 - 31 Mar. 2025Establishment of an evaluation system for assuring the quality of education for clinical engineers医療の高度化、医療機器の発展に伴い、臨床工学技士の業務は拡大を続けており、今後さらなる知識や技術が必要とされる。その教育課程(卒前教育)について、臨床現場が求める教育水準との乖離が懸念されているが、これまで臨床工学技士養成教育の到達度を客観的に評価するための基準は作成されておらず、教育の質の実態は正しく把握されていない。そうした状況下、2021年3月に取りまとめられた「臨床工学技士学校養成所カリキュラム等改善検討会報告書」(厚労省医政局)では、臨床実習において学生が経験すべき行為(到達目標)が初めて示され、客観的な評価基準としての機能も期待される。今後、全国の養成施設が社会的使命を果たせるよう教育の質を担保していくためには、卒前教育の全領域において評価基準を整備し、教育の現状を正しく評価していくことが望まれる。
2021年度は、(一社)日本臨床工学技士教育施設協議会の教育委員25名の協力を得て、2021年8月~10月に意見集約のためのワークショップを2段階に分けて実施した。第1段階ではオンラインツールを用いたグループ討議により、教育項目ごとの課題とコンピテンシーの素案について検討した。さらに第2段階では、対面形式でのワークショップによりコンピテンシーの素案に対する協議を行った。その結果を踏まえて臨床工学技士養成教育におけるコンピテンシーの原案として、養成校卒業時の到達目標となる40項目の行動・態度を選定した。
さらに、臨床現場の視点からも各項目に求められる到達水準を検証するため、(公社)日本臨床工学技士会の協力を得て、同会に所属する理事、代議員、各委員会の役員など359名を対象に、オンライン形式での質問紙調査を実施した。その結果、249名(回収率69.4%)から回答が得られ、その結果を踏まえて『臨床工学技士養成教育コア・コンピテンシー』を作成した。 - 日本学術振興会, 科学研究費助成事業 基盤研究(C), 基盤研究(C), 新潟大学, 01 Apr. 2019 - 31 Mar. 2022飲食物の微生物汚染と胆嚢がん発症の関係及び本症早期診断のための血液検査体系の確立2021年度は本研究の最終年度に当たり、2020年度に予定していたバナラス・ヒンドゥー大学(BHU)、医学部、外科学教室教授のProf. Puneetから胆嚢がん患者100名、胆石症患者100名、及び健常者100名から採取した血清試料を受け取り、胆道系の検査項目として総ビリルビン、直接ビリルビン、アルカリフォスファターゼ、γGT、AST、ALT、腫瘍マーカー他の項目としてCEA、CA19-9、Span-1、DUPAN-2、CRP、アルブミンの測定を行うことと、2021年度に予定していた血清中アフラトキシンB1濃度を測定することであった。 しかし、2021年度内にインドから日本への試料輸送許可の承認がインド政府内のインド医療評議会(Indian Council of Medical Research: ICMR)から得られなかったためこれらの測定はすべてできなかった。また、インド研究者からの報告では、2021年10月時点で、胆嚢がん患者から30件、胆石症患者から40件の血清試料を採取しているとの連絡を受けたが、これ以降は現在までに採取した試料数に関する報告はなく、採取した試料数を確認できない状態となっている。 試料採取が遅れている原因として、採取した試料をインドから日本への輸送許可をICMRから得ることができないために、採取が滞っているものと考えられる。2020年頃から、インド政府は、インドで可能な生体試料を用いた解析はインドで実施することを原則とする方針を打ち出したためである。当初予定していた検査項目のうち、インドでは測定不可能の項目がいくつかあるので、それらの項目だけでも実施可能か、研究期間を1年延長して、再度ICMRへの申請をお願いする予定である。
- Japan Society for the Promotion of Science, Grants-in-Aid for Scientific Research, Grant-in-Aid for Scientific Research (C), Niigata University, 01 Apr. 2017 - 31 Mar. 2020Metagenomic analysis of bacterial flora in the bile of patients with gallbladder cancer or cholelithiasis and verification of bacterial infection historyTo elucidate the association between bacterial infection and developing gallbladder cancer (GBC), we examined the bacterial flora in the bile of patients with Indian GBC and chronic cholelithiasis (CC) using metagenomic analysis. Furthermore, serum or plasma antibody titers were measured to clarify the association between Salmonella typhi and paratyphi or Helicobacter pylori infections and developing GBC. Streptococcus anginosus group bacteria were specifically detected in the bile of GBC patients, suggesting a relationship with the development of this disease. Although a relationship between Salmonella typhi infection and developing GBC was observed, no association between Helicobacter pylori infection and developing this disease could be confirmed. Additional studis are needed to verify the association between the bacteria with specifically detected in the bile of GBC patients and developing this disease.

Although our findings need confirmation whether incidence of gallbladder cancer would be reduced by preventing these factors, our data show that lack of vegetables and fruits intake and supply of unsafe drinking water are modifiable risk factors for the development of gallbladder cancer in north Indians. We also found that the CC genotype of TP53 rs1042522 polymorphism is associated with an increased risk of gallbladder cancer. The GG genotype of the TP53 polymorphism was previously shown to have a more potent apoptosis-inducing activity than the CC genotype. Carriers with the CC genotype among North Indians appear to show an increased risk of developing gallbladder cancer, which may be due to the lower apoptosis-inducing activity.

In the examination of the antibody titer, it was proven that three kinds of proteins of TUBAL3, SFN and STMN1 were effective. However, although 50 ~ 300 ng/ml of protein could be detected by the ELISA method, the detection limit was about 50 ng/ml, which did not lead to the detection of protein on the order of pg/ml as originally intended. In the future, the establishment of the detection system by the SRM method which can determine the high-precise quantity is required.

The first year of this research, the research reader had changed the laboratory, from Kumamoto University to Niigata University of Health and Welfare. We were granted human iPS cells fron RIKEN, and we strarted differentiation- induced experiment from iPS cells to hepatocytes. And then, we confirmed differentiation to hepatocytes by means of immunological staining, hepatocyes marker ALB, SERPINA1 and HNF4A. This achivemnnt would help education and research for unversity students trying to be medical techcologysts, the reseach reader instructed there.

We aimed to identify SNPs involved in the survival times of GBC patients. A total of 96 GBC patients in Chile were analyzed using a Cancer SNP Panel (Illumina Inc.) that contained 1,421 SNPs in 408 cancer-related genes. SNPs associated with the cancer-specific survival were screened by log-rank test. Six SNPs were identified to have statistically significant association with cancer-specific survival. The cancer-specific survival times of patients grouped according to the number of risk genotypes of 6 SNPs differed significantly (0-1 vs 2-3 vs 4-6 risk genotypes; P=3.05x10-10). The number of risk genotypes was independently associated with survival of GBC patients in a multivariate analysis that included age and stage (P=2.26x10-8).

Other bacterial infections may also be associated with GBC. We aimed to investigate the microbial communities in gallbladder bile of patients with GBC and cholelithiasis (CL). DNA was extracted from bile in seven GBC patients and 30 CL patients, and the V3-V4 region of 16S rRNA was amplified. The sequencing results were compared with the 16S database. DNA was detected in the bile of three GBC (42.9%) and four CL patients (13.3%). As the predominant species, Fusobacterium nucleatum, Escherichia coli, and Enterobacter sp. were detected in bile from GBC patients. Those in bile from CL patients were Escherichia coli, Salmonella sp., and Enterococcus gallinarum. Whether the three bacteria detected in bile from GBC patients would associated with the development of GBC just like colorectal cancer remains unknown.

The mean concentrations of OTA in red chili peppers from Chile, Bolivia, and Peru were higher in the order of the incidence of gallbladder cancer (GBC) among the three countries. These findings suggest the possibility that mycotoxin contamination of red chili peppers are associated with developing GBC. The frequency of TP53 and K-ras mutations in DNA extracted from paraffin embedded tissues of GBC patients were examined. TP53 mutations were found in 7 patients (7/18, 39%), namely 4 of transition type, 2 of transversion type, and 1 of insertion type. However, G:C to T:A transversion suggesting the presence of exogenous carcinogens, and K-ras mutation were not found. Our findings suggest that the development of GBC in Peruvians is more strongly associated with endogenous mechanisms rather than exogenous carcinogens.
