■ Paper- Background: Aging profoundly alters host immunity, yet how age-associated immune changes in the liver influence the growth of metastatic tumors remains incompletely understood. Liver metastasis is a major cause of cancer-related mortality, particularly in elderly patients, for whom aggressive treatments are often not feasible. This study aimed to clarify how aging reshapes the hepatic immune microenvironment and to identify age-associated host factors that influence liver metastasis growth. Methods: Tumor-naïve and tumor-bearing young and aged mice were analyzed using a syngeneic MC38 liver metastasis model. Immune cell composition in the liver was assessed by flow cytometry, and gene expression was evaluated by quantitative reverse transcription PCR (RT-qPCR). Public transcriptomic datasets were screened to identify age-associated inflammatory factors. The functional relevance of the S100A8/A9 axis was examined using the small-molecule inhibitor paquinimod. Results: Aging was associated with a distinct baseline immune cell composition in the liver. During liver metastasis, overall growth was comparable between young and aged mice; however, metastatic lesions in aged hosts showed increased expression of multiple inflammation-related genes and prominent accumulation of Ly6G+ cells. In silico screening identified S100a9 as one of the most highly upregulated inflammation-related genes in aged livers, which was confirmed in both tumor-naïve and metastatic liver tissues. Pharmacological modulation of the S100A8/A9 axis with paquinimod significantly reduced liver metastasis growth in aged, but not young, mice, and was accompanied by a shift in immune cell composition, including an increased representation of CD8+ T cells. Conclusions: These findings indicate that aging is associated with a distinct hepatic immune context that shapes the inflammatory and cellular composition of the tumor microenvironment during liver metastasis. S100A9 emerges as a key age-associated, host-derived factor that is functionally relevant to the growth of liver metastases in aged hosts, supporting the S100A8/A9 axis as a context-specific therapeutic target.May 2026, Cancers, 18(10) (10), English, International magazineScientific journal
- BACKGROUND/AIM: Extracellular vesicles (EVs), including exosomes, are abundant in body fluids, and EV surface proteins can be profiled as potential minimally invasive biomarkers. CD147 (EMMPRIN/Basigin) is a tumor-associated surface glycoprotein implicated in cancer progression. CD9 and CD63 are tetraspanin membrane proteins widely used as EV markers. The present study aimed to evaluate the clinical significance of serum EV subsets double-positive for CD9 and CD63, or for CD9 and CD147 in patients with colorectal cancer (CRC) before and after tumor resection. MATERIALS AND METHODS: Sixteen patients with CRC were recruited, and paired pre- and postoperative serum samples were analyzed for CD9+ CD63+ EVs and CD9+ CD147+ EVs. Serum EVs were quantified using the ExoCounter system with antibody-conjugated beads. CD147 expression in resected tumor tissue was analyzed by immunohistochemistry. RESULTS: CD9+ CD63+ EVs were significantly reduced in postoperative samples compared with preoperative samples (p=0.0151). CD9+ CD147+ EVs also showed a significant postoperative decrease (p=0.0186). CD147 immunostaining was positive in 13 out of 16 resected tumors (81.3%). In patients with CD147 negative tumors (n=3), serum CD9+ CD147+ EV levels were low at both pre- and postoperative time points and remained unchanged. CONCLUSION: Serum CD9+ CD63+ and CD9+ CD147+ EV subsets decreased after CRC surgery, supporting their potential utility as minimally invasive liquid biopsy biomarkers of postoperative tumor burden. - BACKGROUND: There is substantial global variation in demographics, disease burden, and treatment for gastric cancer patients. Benchmarking is an instrument to assess such variation and enables to investigate to which extent case-mix and treatments explain differences in outcomes. We aimed to evaluate hospital-level variation in surgical outcomes following gastrectomy for gastric cancer before and after adjusting for case-mix and treatment-related factors. METHODS: Data were retrieved from the GastroBenchmark and GASTRODATA databases, including consecutive gastric cancer resections performed between 2017 and 2021 from 43 centers. Patients who underwent a (sub)total gastrectomy for adenocarcinoma were identified. Outcomes included 30-day mortality, severe complications (Clavien-Dindo grade ≥ 3a), > 15 lymph nodes retrieved, negative resection margin (R0), prolonged hospitalization (> 14 days), readmissions (< 30 days), reoperations, and escalation of care. We assessed absolute inter-hospital variation for outcomes, and estimated outcomes using mixed-effect logistic regression models with a random intercept. We estimated crude, case-mix adjusted, and case-mix and treatment adjusted hospital effects. The conditional and marginal pseudo-R2 were used to quantify the variance in outcome explained by case-mix and treatment-related factors. RESULTS: A total of 7818 patients from 41 hospitals were included, with contributions ranging from 12 to 2554 patients per hospital (IQR: 49-146). Observed 30-day mortality and severe complications ranged from 0 to 9.7% (IQR: 3.2%) and 5.3 to 31% (IQR: 7.7%), respectively. Larger variation between hospitals was observed for retrieval of > 15 lymph nodes (IQR: 12.3%), prolonged hospitalization (IQR: 14.4%) and readmissions (IQR 11.3%). This variation was reduced in the crude model, while adjusting for case-mix and treatment-related factors did not significantly reduce variation for any outcome. Case-mix factors had a limited contribution to the explained variance, except for 30-day mortality (33.9%) and negative resection margins (31.7%). Adding treatment-related factors increased the explained variance for 30-day mortality by 40.8%, but had low impact (< 10%) on the variance in most surgical outcomes. CONCLUSIONS: Case-mix and treatment factors are not the primary drivers of variation in surgical outcomes following gastrectomy. - BACKGROUND: Esophageal squamous cell carcinoma (ESCC) and hepatic cirrhosis (HC) share major risk factors, including alcohol consumption and smoking. Consequently, patients with HC exhibit an elevated incidence of ESCC. Although HC is widely recognized to contribute to poor prognosis following esophagectomy for ESCC, the underlying mechanisms remain poorly understood. This study aimed to investigate the association between hepatic fibrosis evaluated using the fibrosis-4 (FIB-4) index and postoperative outcomes among patients with ESCC undergoing esophagectomy. METHODS: We retrospectively analyzed 461 patients with ESCC who underwent minimally invasive esophagectomy at Kobe University Hospital. Hepatic fibrosis was evaluated using the FIB-4 index, which estimates liver fibrosis based on routine blood test parameters. Patients were stratified into two groups: fibrosis-positive (FIB-4 > 2.67) and fibrosis-negative (FIB-4 ≤ 2.67). RESULTS: Among patients with cT1 tumors, overall survival (OS) did not differ significantly between the two groups. However, among those with ≥cT2 tumors, the fibrosis-positive group showed significantly worse OS than the fibrosis-negative group (P = 0.0148). Additionally, the fibrosis-positive group had a significantly higher incidence of poor response to preoperative chemotherapy (P = 0.0075). Postoperative serum albumin levels were markedly lower at 3, 6, and 12 months in the fibrosis-positive group (P = 0.015, 0.043, and 0.0066, respectively). While albumin levels recovered to baseline in the fibrosis-negative group within 12 months, no such recovery was observed in the fibrosis-positive group. CONCLUSIONS: Preoperative hepatic fibrosis is associated with poor response to preoperative chemotherapy and prolonged postoperative malnutrition, contributing to poorer long-term survival outcomes among patients with ESCC undergoing minimally invasive esophagectomy.Jan. 2026, Annals of surgical oncology, 33(1) (1), 444 - 452, English, International magazineScientific journal
- BACKGROUND: Large type III gastric cancer (GC) ≥ 8 cm has conventionally been categorized with type IV GC in Japan, leading to alternative treatment strategies such as neoadjuvant chemotherapy and staging laparoscopy (SL). However, whether 8 cm is the correct cutoff remains unclear. METHODS: We retrospectively analyzed patients clinically diagnosed with advanced GC who underwent surgery at our department. Patients were classified by Borrmann type, and clinicopathological characteristics including survival outcomes and peritoneal metastasis incidence were analyzed based on tumor size to determine the optimal cutoff for large type III GC. RESULTS: Tumor size correlated with overall survival in type III GC. Although hazard ratios (HRs) for "large" and "small" type III vs. type IV remained comparable up to the 8 cm cutoff (0.60 and 0.41, respectively), HR for "large" type III GC increases sharply to 0.74 with a 10 cm cutoff. Subgroup analysis based on histological subtype revealed similar results in the undifferentiated type. Conversely, a larger cutoff value appeared more appropriate for the differentiated type. The largest difference in the incidence of peritoneal metastasis was observed with a 6 cm cutoff (36.1% in "large" type III and 10.2% in "small" type III), and similar results were observed in the undifferentiated type at the same cutoff. CONCLUSIONS: In terms of survival, a 10 cm cutoff may more accurately define large type III GC than the conventional 8 cm. - BACKGROUND/AIM: Previous studies demonstrated the safety of laparoscopic gastrectomy in patients with locally advanced gastric cancer. However, the feasibility of minimally invasive surgery (MIS) following neoadjuvant chemotherapy (NAC) for locally advanced gastric cancer remains unclear. To our knowledge, no previous study has conducted a comprehensive analysis that evaluated NAC and surgical approach simultaneously. This study aimed to assess the feasibility of MIS for locally advanced gastric cancer after NAC using a four-arm comparison. PATIENTS AND METHODS: This study retrospectively analyzed 174 patients who underwent gastrectomy at Kobe University Hospital between January 2011 and July 2024. The patients were categorized into four groups based on NAC and surgical approaches: No NAC/open surgery (n=63), no NAC/MIS (n=49), NAC/open surgery (n=20), and NAC/MIS (n=42). We compared surgical and survival outcomes among the four groups. RESULTS: Patients in the group treated with NAC/MIS experienced less blood loss, fewer transfusions, and a shorter hospital stay. Additionally, compared with the NAC/open (20%) and no NAC/MIS (6.1%) groups, the rate of severe postoperative complications (grade ≥III) was lowest in the group treated with NAC/MIS, with no incidence of pancreatic fistula. The R0 resection rate was highest (97.6%) in the group treated with NAC/MIS. Although 5-year relapse-free survival and overall survival were higher in the group treated with NAC/MIS, the differences were not statistically significant. - PURPOSE: Minimally invasive esophagectomy (MIE) is the standard surgical procedure. Robot-assisted MIE (RAMIE) was developed to overcome the problems associated with conventional MIE. This study investigated whether or not standardizing surgical procedures for RAMIE using soft coagulation scissors and Maryland bipolar forceps could improve short-term outcomes. METHODS: This study included 130 patients who underwent RAMIE. Clinicopathological characteristics and surgical outcomes were compared before and after standardization by two expert surgeons as well as between the standardized and trainee groups. A cumulative sum control chart (CUSUM) was used to evaluate changes in operative time. RESULTS: The console time during the thoracic procedure was shorter in the standardized group than in the pre-standardized group (P < 0.001). The rate of recurrent laryngeal nerve palsy was lower in the standardized group than in the pre-standardized group (P = 0.039). No significant differences were observed between the standardized and trainee groups. In the standardized and trainee groups, CUSUM clearly indicated that the console time peak occurred in the 12th and 16th cases. CONCLUSIONS: Standardization of surgical procedures using an appropriate device for each surgical step could improve surgical outcomes in RAMIE. - BACKGROUND & AIMS: Oral nutritional supplements (ONS) administered after gastrectomies can prevent postoperative weight loss. However, the amount of additional energy to be added to energy requirements after gastrectomy is unclear. This study aimed to clarify dietary energy intake and its association with weight loss in postgastrectomy patients. METHODS: We performed a single-center, single-arm, open-label, non-randomized clinical trial. Fifty patients with gastric cancer who underwent distal or total gastrectomy were included in this study. Until 3 months postoperatively, the patients continuously consumed ONS and their intake was recorded. Additionally, the patients took photos of their regular diets. Hospital visits were made every 2 weeks to investigate calorie intake (from regular diet plus ONS), physical findings, and nutritional status. The primary endpoint was the percentage of body weight loss (%BWL) at 3 months after gastrectomy. RESULTS: Data were available for 45 patients. %BWL was 7.5 ± 5.1 % at 3 months postoperatively. The group with ONS intake <200 kcal/day tended to show a lower %BWL; however, this was not significant. No correlation was observed between ONS and regular dietary calorie intake. The required calorie intake was 1588 ± 157 kcal/day. Calorie intake from regular diet alone and from regular diet plus ONS were 1330 ± 280 and 1487 ± 300 kcal/day, respectively. The sufficient group, whose total calorie intake met the requirements, had significantly reduced body weight loss, compared with the insufficient group. CONCLUSIONS: After gastrectomy, regular diet alone often fails to meet nutritional requirements, and ONS supplementation may reduce %BWL at 3 months postoperatively by correcting energy deficits. - Abstract Background To reduce the clinical and economic burden of complications after gastrectomy for gastric cancer, specific complications should be targeted to effectively allocate healthcare resources for quality improvement and preventive measures. The aim of this study was to assess the impact of complications on clinical outcomes. Methods This was a retrospective multicentre study of patients who underwent (sub)total gastrectomy for gastric or junctional adenocarcinoma at 43 centres in 16 countries between 2017 and 2021. Outcomes were escalation of care, reoperation, prolonged hospital stay (greater than the 75th percentile), readmission, and 30-day mortality. Adjusted relative risks and population attributable fractions were estimated for specific complication–outcome pairs. The population attributable fraction represents the percentage reduction in the frequency of an adverse outcome if a complication could be completely prevented in the population. Results In total, 7829 patients were included. Postoperative complications occurred in 1884 patients (24.1%). The most frequent complications were pulmonary complications (436 patients (5.6%)), anastomotic leakage (363 patients (4.6%)), and abdominal collection (301 patients (3.8%)). Anastomotic leakage, cardiac complications, and pulmonary complications had the greatest impact on 30-day mortality (population attributable fraction 26.6% (95% c.i. 14.5% to 38.6%), 18.7% (95% c.i. 9.4% to 28.0%), and 15.6% (95% c.i. 12.0% to 30.0%) respectively). Anastomotic leakage and pulmonary complications had the greatest impact on escalation of care (population attributable fraction 26.3% (95% c.i. 20.6% to 32.0%) and 18.4% (95% c.i. 11.7% to 25.2%) respectively), whereas anastomotic leakage and intra-abdominal bleeding had the greatest impact on reoperation (population attributable fraction 31.6% (95% c.i. 26.4% to 36.9%) and 8.5% (95% c.i. 5.5% to 11.5%) respectively). Most of the studied complications contributed to a prolonged hospital stay, whereas the contribution of complications to readmission did not exceed 15.9%. Subgroup analysis showed regional variation in the impact of complications. Conclusion Anastomotic leakage had the largest overall negative impact on clinical outcomes after gastrectomy for gastric adenocarcinoma. Reducing the incidence of anastomotic leakage and pulmonary complications would have the most impact on the burden of complications.Oxford University Press (OUP), Mar. 2025, British Journal of Surgery, 112(4) (4)Scientific journal
- OBJECTIVE/BACKGROUND: Various anastomotic and reconstruction techniques are used for minimally invasive total (miTG) and distal gastrectomy (miDG). Their effects on postoperative morbidity have not been extensively studied. METHODS: MiTG and miDG patients were selected from 9356 oncological gastrectomies performed in 2017-2021 in 43 centers. Endpoints included anastomotic leakage (AL) rate and postoperative morbidity tested by multivariable analysis. RESULTS: Three major anastomotic techniques [circular stapled (CS); linear stapled (LS); and hand sewn (HS)], and 3 major bowel reconstruction types [Roux (RX); Billroth I (BI); Billroth II (BII)] were identified in miTG (n=878) and miDG (n=3334). Postoperative complications, including AL (5.2% vs 1.1%), overall (28.7% vs 16.3%), and major morbidity (15.7% vs 8.2%), as well as 90-day mortality (1.6% vs 0.5%) were higher after miTG compared with miDG. After miTG, the AL rate was higher after CS (4.3%) and HS (7.9%) compared with LS (3.4%). Similarly, major complications (LS: 9.7%, CS: 16.2%, and HS: 12.7%) were lowest after LS. Multivariate analysis confirmed anastomotic technique as a predictive factor for AL, overall, and major complications. In miDG, AL rate (BI: 1.4%, BII 0.8%, and RX 1.2%), overall (BI: 14.5%, BII: 15.0%, and RX: 18.7%), and major morbidity (BI: 7.9%, BII: 9.1%, and RX: 7.2%), and mortality (BI: 0%, BII: 0.1%, and RY: 1.1%%) were not affected by bowel reconstruction. CONCLUSIONS: In oncologically suitable situations, miDG should be preferred to miTG, as postoperative morbidity is significantly lower. LS should be a preferred anastomotic technique for miTG in Western Centers. - Despite the recent development of chemotherapeutic agents, the prognosis of colorectal cancer (CRC) patients with peritoneal dissemination (PD) remains poor. The tumor immune microenvironment (TIME) has drawn attention as a key contributing factor of tumor progression. Of TIME components, myeloid‑derived suppressor cells (MDSCs) are considered to play a responsible role in the immunosuppressive characteristics of the TIME. MDSCs are classified into two major subsets: Monocytic MDSCs (M‑MDSCs) and polymorphonuclear MDSCs (PMN‑MDSCs). Therefore, we hypothesize that MDSCs would play important roles in the PD‑relevant TIME and PD progression. To address this hypothesis, we established PD mouse models. As the PD nodules consisted scarcely of immune cells, we focused on the peritoneal cavity, but not PD nodule, to evaluate the PD‑relevant TIME. As a result, intraperitoneal PMN‑MDSCs were found to be substantially increased in association with PD progression. Based on these results, we phenotypically and functionally verified the usefulness of CD244 for identifying PMN‑MDSCs. In addition, the concentrations of interleukin (IL)‑6 and granulocyte‑colony stimulating factor (G‑CSF) were significantly increased in the peritoneal cavity, both of which were produced by the tumors and thought to contribute to the increases in the PMN‑MDSCs. In vivo depletion of the PMN‑MDSCs by anti‑Ly6G monoclonal antibody (mAb) significantly inhibited the PD progression and reverted CD4+ and CD8+ T cells in the peritoneal cavity and the peripheral blood. Summary The number of elderly patients with esophageal cancer has increased in recent years. The use of thoracoscopic esophagectomy has also increased, and its minimal invasiveness is believed to contribute to postoperative outcomes. However, the short- and long-term outcomes in elderly patients remain unclear. This study aimed to elucidate the safety and feasibility of minimally invasive esophagectomy in elderly patients. This retrospective study included 207 patients who underwent radical thoracoscopic esophagectomy for thoracic esophageal squamous cell carcinoma at Kobe University Hospital between 2005 and 2014. Patients were divided into non-elderly (<75 years) and elderly (≥75 years) groups. A propensity score matching analysis was performed for sex and clinical T and N stage, with a total of 29 matched pairs. General preoperative data, surgical procedures, intraoperative data, postoperative complications, in-hospital death, cancer-specific survival, and overall survival were compared between groups. The elderly group was characterized by lower preoperative serum albumin levels and higher American Society of Anesthesiologists grade. Intraoperative data and postoperative complications did not differ between the groups. The in-hospital death rate was 4% in the elderly group, which did not significantly differ from the non-elderly group. Cancer-specific survival was similar between the two groups. Although overall survival tended to be poor in the elderly group, it was not significantly worse than that of the non-elderly group. In conclusion, the short- and long-term outcomes of minimally invasive esophagectomy in elderly versus non-elderly patients were acceptable. Minimally invasive esophagectomy is a safe and feasible modality for elderly patients with appropriate indications.[Refereed]Scientific journal - BACKGROUND/AIM: The aim of this study was to investigate PD-L1 expression and its association with prognosis in esophageal squamous cell carcinoma (ESCC) before and after neoadjuvant chemotherapy (5-fluorouracil and cisplatin, NAC-FP). PATIENTS AND METHODS: Using a database of 69 ESCC patients, we analyzed PD-L1 expression on tumor cells (TCs) and immune cells (ICs), as well as the density of CD8+ tumor-infiltrating lymphocytes (TILs) in pretreatment biopsy specimens-versus-surgical specimens after resection. We determined the prognostic significance of these factors. RESULTS: The fraction of ESCC containing ICs expressing PD-L1 and having a high CD8+ TIL density was significantly increased after neoadjuvant treatment. However, PD-L1 expression on TCs or ICs, and CD8+ TIL density, was not significantly associated with patient survival in ESCC patients. CONCLUSION: NAC-FP induced PD-L1 expression on ICs and CD8+ TILs in ESCC patients. - In tumor immunity, invariant natural killer T (iNKT) cells play a pivotal role as a link between the innate and adaptive immune systems. With a precisely regulated activation mechanism, iNKT cells have the ability to respond quickly to antigenic stimulation and rapidly produce cytokines and chemokines, and subsequently an effective antitumor immune response. The development of iNKT cell-targeted active immunotherapy enables, not only an antitumor immune response through innate and acquired immunity, but also the conversion of an immunosuppressive into an immunogenic microenvironment. This review is focused on the activation mechanism and the role of iNKT cells after therapeutic active immunization. Among them, Myeloid-derived suppressor cells (MDSCs) are suppressor immune cells and are involved in the establishment of the tumor microenvironment. On the other hand, there have been few reports on peritoneal seeding, and the studies are still insufficient. In this study, we created a mouse model of peritoneal dissemination of colorectal cancer, analyzed the dynamics of intraperitoneal MDSCs, and examined their involvement in the pathogenesis of peritoneal dissemination. The results showed that intraperitoneal MDSCs increased with disease progression after intraperitoneal administration of MC38. The proportion of PMN-MDSCs increased especially in the peritoneal cavity. Anti-Ly6G antibody-induced MDSC disappearance studies inhibited tumor progression.

T cells exhaust by chronic antigen-stimulation in cancer patients. The function of exhausted T cells decrease and can’t remove cancer specific antigen. At present, functional evaluation of T cells takes several days to obtain results. It is important to establish a system that can quickly assess the functional depression and exhaustion of T cells. Therefore, in this research project, we propose new method which detect IS formation of ex vivo T cells by using imaging cytometry. The study was conducted using T cells in human peripheral blood.
