■ Paper- OBJECTIVE: In non-convulsive status epilepticus (NCSE), predicting patient outcomes is crucial in deciding treatment intensity. However, electroencephalograms (EEGs) of NCSE show various patterns, and their relationship with functional outcomes remains unclear. We aimed to examine the relationship between NCSE EEG patterns and functional outcomes. METHODS: We retrospectively collected data from 105 NCSE patients without cardiopulmonary arrest at two centers and compared functional outcomes in NCSE patients with electrographic seizures (ESz) and those with rhythmic/periodic patterns (RPPs) at discharge. Functional outcomes were assessed using the dichotomized modified Rankin scale (≥4 or less), with univariate and multivariate analyses conducted on patient backgrounds and therapeutic interventions. We compared background activities and discharge waveforms in periodic discharges (PDs) using the Fourier transform. RESULTS: ESz independently predicted better outcomes after adjusting for severity (Glasgow Coma Scale and Epidemiology-based Mortality Score). In PDs, the group with better outcomes exhibited larger alpha-band background activity and larger post-discharge slow waves. CONCLUSIONS: NCSE with ESz was associated with better functional outcomes than RPPs alone. Spared alpha-band background activity and larger amplitude of post-discharge slow shifts may indicate spared brain function in patients with PDs. - BackgroundUnderstanding associations between cognitive performance and blood biomarkers supports diagnosis and treatment of Alzheimer's disease (AD); however, this relationship remains unclear.ObjectiveExploring associations between the Amyloid Probability Score (APS; PrecivityAD® blood test) and cognitive performance, and between Mini-Mental State Examination (MMSE) and Cogstate Brief Battery (CBB) in cognitively impaired older adults.MethodsOlder adults aged ≥ 60 years with MMSE from 10 to 27, recruited from a cohort in Japan, were assessed with the CBB, and plasma biomarkers to calculate APS. We used Spearman correlation to explore relationships; receiver operating curves and cross-validation to identify the best model for predicting high APS.ResultsAmong 46 participants (mean age = 78.3 ± 5.9; mean MMSE = 20.3 ± 4.6), 39.1% did not complete at least one test in the CBB. Lower MMSE were observed in those who were non-completers. MMSE correlated with Detection (Psychomotor function), Identification (Attention), and their composite (r = 0.34-0.52; p < 0.05; 95%CI excluding null value). Higher APS was seen in those who did not complete the One Card Learning (OCL-visual learning and short-term memory). The best model for predicting High APS included OCL completion status, MMSE and Years of Education, achieving accuracy = 0.808 and kappa = 0.606 in participants with MMSE < 21.ConclusionsOur findings suggest a consideration of incompletion when using CBB in cognitively impaired older adults and a potential approach to differentiate High APS group among those with moderate to severe dementia based on visual learning and short-term memory, global cognition and education level.Sep. 2025, Journal of Alzheimer's disease : JAD, 13872877251372958 - 13872877251372958, English, International magazineScientific journal
- Intrathecal chemotherapy is often administered for prophylaxis and treatment of central nervous system involvement in hematological malignancies. However, it may rarely cause neurotoxicity as a side effect. We herein report a 74-year-old woman with diffuse large B-cell lymphoma including a spinal lesion. She received systemic and intrathecal chemotherapy. After five doses of intrathecal chemotherapy, she developed intrathecal chemotherapy-induced myelopathy. Intrathecal treatment was discontinued, and she was administered vitamin B12 and folic acid, along with steroid pulses. However, her symptoms did not improve. Intrathecal chemotherapy-induced myelopathy is rare, but may be irreversible; therefore, clinicians should be aware of this potential complication.Feb. 2024, Internal medicine (Tokyo, Japan), 63(4) (4), 547 - 551, English, Domestic magazineScientific journal

- INTRODUCTION: Functional neurological disorder (FND) has various clinical manifestations. Even though diagnostic criteria for FND have been proposed, FND characteristics with sensory manifestations have not been elucidated. Therefore, we aimed to investigate the association between sensory nerve action potential (SNAP) amplitudes and FND with sensory manifestations. METHODS: We included 76 outpatients with FND with sensory manifestations whose nerve conduction studies were performed retrospectively. Additionally, we defined 121 patients with other neurological diseases who did not have peripheral neuropathy as disease controls. The SNAP amplitudes were compared between the two groups. We also explored the relationship between SNAP amplitudes and FND-specific clinical symptoms in patients with FND. RESULTS: No differences were observed in SNAP amplitudes adjusted for age between patients with FND who had sensory manifestations and disease control patients. Additionally, no differences were observed between patients with FND who had and did not have FND-specific clinical symptoms. - IMPORTANCE: Repeat expansion of CGG in LRP12 has been identified as the causative variation of oculopharyngodistal myopathy (OPDM). However, to our knowledge, the clinicopathologic features of OPDM with CGG repeat expansion in LRP12 (hereafter referred to as OPDM_LRP12) remain unknown. OBJECTIVE: To identify and characterize the clinicopathologic features of patients with OPDM_LRP12. DESIGN, SETTING, AND PARTICIPANTS: This case series included 208 patients with a clinical or clinicopathologic diagnosis of oculopharyngeal muscular dystrophy (OPDM) from January 1, 1978, to December 31, 2020. Patients with GCN repeat expansions in PABPN1 were excluded from the study. Repeat expansions of CGG in LRP12 were screened by repeat primed polymerase chain reaction and/or Southern blot. MAIN OUTCOMES AND MEASURES: Clinical information, muscle imaging data obtained by either computed tomography or magnetic resonance imaging, and muscle pathologic characteristics. RESULTS: Sixty-five Japanese patients with OPDM (40 men [62%]; mean [SD] age at onset, 41.0 [10.1] years) from 59 families with CGG repeat expansions in LRP12 were identified. This represents the most common OPDM subtype among all patients in Japan with genetically diagnosed OPDM. The expansions ranged from 85 to 289 repeats. A negative correlation was observed between the repeat size and the age at onset (r2 = 0.188, P = .001). The most common initial symptoms were ptosis and muscle weakness, present in 24 patients (37%). Limb muscle weakness was predominantly distal in 53 of 64 patients (83%), but 2 of 64 patients (3%) had predominantly proximal muscle weakness. Ptosis was observed in 62 of 64 patients (97%), and dysphagia or dysarthria was observed in 63 of 64 patients (98%). A total of 21 of 64 patients (33%) had asymmetric muscle weakness. Aspiration pneumonia was seen in 11 of 64 patients (17%), and 5 of 64 patients (8%) required mechanical ventilation. Seven of 64 patients (11%) developed cardiac abnormalities, and 5 of 64 patients (8%) developed neurologic abnormalities. Asymmetric muscle involvement was detected on computed tomography scans in 6 of 27 patients (22%) and on magnetic resonance imaging scans in 4 of 15 patients (27%), with the soleus and the medial head of the gastrocnemius being the worst affected. All 42 muscle biopsy samples showed rimmed vacuoles. Intranuclear tubulofilamentous inclusions were observed in only 1 of 5 patients. CONCLUSIONS AND RELEVANCE: This study suggests that OPDM_LRP12 is the most frequent OPDM subtype in Japan and is characterized by oculopharyngeal weakness, distal myopathy that especially affects the soleus and gastrocnemius muscles, and rimmed vacuoles in muscle biopsy.Jul. 2021, JAMA neurology, 78(7) (7), 853 - 863, English, International magazineScientific journal
- Electroencephalogram (EEG) reading in clinical settings commonly uses three montage types: referential montage, bipolar montage, and average montage. Since each montage type has its advantages and disadvantages, there is no single best montage. To correctly read EEG, it is essential 1) to use the montage appropriate for the focus and distribution of epileptic activity and 2) to correctly recognize EEG waveforms that are often misdiagnosed as epileptic activity. In this article, we present the so-called "southern-hemisphere" epileptic activity, which is recognized as challenging to diagnose, along with two cases of temporal lobe epilepsy. Case 1 exhibited seizures that started with palpitations and epigastric discomfort, followed by loss of awareness and oral automatisms. Case 2 experienced recurrent episodes of syncope and was diagnosed with temporal lobe epilepsy based on EEG findings and the observed improvement with anti-epileptic medication. In both cases, the longitudinal bipolar montage ("double-banana montage") failed to visualize the epileptic activity, while the referential montages (ear lobe reference or average reference) clearly showed maximal activity at the earlobe electrode. - 症状が神経学的診察や他の補助検査を用いても説明不可能なとき, 機能性神経障害 (FND) が想定される。FNDは特徴的な所見を見いだして積極的に診断する疾患に変わってきている。感覚症状を呈するFNDは運動障害を呈する患者と比較すると, 特徴的な症候が乏しいとされているため, 生理検査も重要な位置を占める。Mayo ClinicのTiptonらは感覚症状を呈する神経伝導検査が基準範囲内の患者400名で多変量解析を行ったところ, 症状が多巣性に分布する患者の感覚神経活動電位 (SNAP) 振幅が大きかったと報告した。我々は感覚症状を呈するFND患者76名のSNAPを対照と比較したところ, 両群間に有意な差はみられなかったが, 説明できない運動症状を伴う群, 症状が持続的でない群は対照に比してSNAP振幅が有意に大きかった。ただし前者は有意に若年であり, 年齢が交絡因子として作用していた。今後より大規模な患者集団を用いた観察研究が必要と考えられる。(一社)日本臨床神経生理学会, 01 Dec. 2022, 臨床神経生理学, 50(6) (6), 492 - 496, Japanese
