■ Paper- Benefit finding is a concept that represents finding positive changes or benefits through difficult experiences. To map the currently available studies on benefit finding and growth among family caregivers of individuals with mental illness, we conducted a scoping review based on methodological frameworks. A systematic search for studies published in English was conducted using MEDLINE, PsycINFO, PsycARTICLES, CINAHL, and PTSDpubs from inception to August 2023. After screening, 21 articles were included in the review. Of these, 11 studies were cross-sectional studies, 7 were qualitative studies, 2 were mixed-methods studies, and a randomized controlled trial (RCT). Family caregivers experienced various positive psychological changes, such as becoming compassionate/tolerant and resilient, stronger family bonds, change in perspective on life and hope, acquisition of knowledge and coping strategies, and leveraging their experiences and appreciation by others, following various struggles and negative changes. Some indicators of benefit finding and growth, such as positive personal traits, positive reappraisal, good relationships with patients, and social support, were also revealed. The quality assessment demonstrated an acceptable level of quality in most studies, with a few outliers. Future longitudinal studies would reveal the mechanism of benefit finding and growth among family caregivers of individuals with mental illness.Jan. 2025, Journal of family nursing, 10748407241309138 - 10748407241309138, English, International magazineScientific journal

- The phenomenon of some patients with schizophrenia withdrawing and becoming hikikomori needs to be resolved. In some countries, outreach methods are being employed. In Japan, psychiatric home-visit nursing for patients with schizophrenia and hikikomori is being implemented. However, it is not based on sufficient evidence and relies on the experience and intuition of individual nurses. This study explored the underlying themes in the nursing practices of psychiatric home-visit nurses via semi-structured interviews with 10 nurses and a thematic analysis. Nine key themes emerged. Four themes—(i) understanding the patient’s world, (ii) supporting the patients as they are, (iii) providing a sense of relief, and (iv) having equal relationships—highlighted the nurses’ commitment to respecting patients’ individuality while building and sustaining relationships. Two themes—(v) exploring the right timing and (vi) waiting for the appropriate timing—illustrated the nurses’ anticipation of proactive patient engagement. Finally, three themes—(vii) working together on things, (viii) continuing care for expanding the patient’s world, and (ix) nursing care for the patient’s future—underscored the nurses’ gradual and methodical approach to working alongside patients. Nursing practices based on these nine themes cultivated meaningful relationships and secured a sense of relief for the patients. Additionally, they awaited patients’ proactive engagement and delivered timely support to facilitate positive daily life changes. - The current pre-/posttest pilot study investigated the impact of an individual nurse-led active listening intervention for spouses of individuals with depression (herein referred to as patients) on spouses' psychological states and patients' depressive symptoms. Sixteen couples participated in the study. Individual sessions were conducted over 10 weeks to help spouses express their thoughts and feelings. Psychological measurement scale scores did not change markedly postintervention for spouses; however, their subjective evaluations of the intervention were positive. In the qualitative analysis, spouses stated that they were able to express their thoughts and feelings and that the sessions were meaningful. Moreover, postintervention depressive scores of patients improved significantly. Findings suggest that the nurse-led intervention of active listening for spouses may provide a better environment for improving the depressive symptoms of patients. - 本研究の目的は、急性期患者の身体的拘束の早期解除に向けて、精神科熟練看護師が感じた困難と対処を明らかにすることである。A県内の精神科病院2施設に勤務する看護師6名に半構造化インタビューを実施し、質的記述的手法により分析した。急性期患者の身体的拘束の早期解除に向けて精神科熟練看護師が感じた困難として【拘束解除後の患者の行動を予測できない】等の6カテゴリ、対処としては【安全に配慮しながら一時拘束解除から始める】等の10カテゴリが明らかになった。精神科熟練看護師は、自殺企図や暴力のリスクを有する状態で早期解除を進めることに困難を感じ、また、職場風土や個人背景が合意形成に及ぼす影響や治療的な関わりの難しさを感じていた。対処として、リスク回避策を準備・実行することに加え、職場全体で患者・看護師双方にとって安全に拘束解除を推し進められる職場風土を醸成しており、これらの対処の重要性が示唆された。(著者抄録)(株)星和書店, 2021, 精神科治療学, 36(12) (12), 1451 - 1459, Japanese[Refereed]
- AIM: Several studies have reported a negative correlation between depressive symptoms and family caregivers' (FCs) subjective sleep status. However, there is a paucity of information on the association between objective/subjective sleep status, care burden, and related factors. METHODS: Participants were 23 pairs of care receivers (CRs; Mage = 82.7 ± 8.5 years; 69.6% women) receiving long-term care at home and their FCs (Mage = 66.9 ± 11.0 years; 69.6% women). At baseline, demographic data, subjective sleep status (Pittsburgh Sleep Quality Index; PSQI), WHO-5 well-being, depressive mood, and frequency of going outdoors were collected. FCs wore a small, wrist-worn device with an accelerometer to assess objective sleep status for a consecutive 24-h 2-week period, and they answered the Zarit Burden Interview short version (ZBI) every night before sleep. After 3 months, CR status was collected and analysed retrospectively. RESULTS: The mean total sleep time over 2 weeks was 349.5 ± 69.6 min. The mean ZBI score over 2 weeks was 8.8 ± 6.8, which was significantly correlated with total sleep time (r = -0.42; P < 0.05), total time in bed (r = -0.44; P < 0.05), PSQI (r = 0.62; P < 0.01), frequency of going outdoors by CRs (r = -0.42; P < 0.05), and WHO-5 well-being among CRs (r = -0.50; P < 0.05). Multiple regression analyses revealed that total sleep time (β = -0.51; P < 0.05) was significantly associated with care burden (adjusted R2 = 0.45). At the 3-month follow-up, four CRs had been hospitalised or died, and their FCs displayed significantly severe care burden and slept less than at baseline. CONCLUSIONS: Reduced objective total sleep time is significantly associated with the severity of care burden among FCs. - Home visit nurses (HVNs) are crucial in psychiatric home visit nursing (PHVN) in Japan. However, little is known about violence toward HVNs in PHVN and non–PHVN settings. The current study aimed to clarify nurses' experiences of violence in these settings, their implementation of preventive measures, and related associations. - Accessible Summary What is known on the subject? There is a developing body of research on violence in healthcare workplaces. Although psychiatric visiting nurses (PVNs) are an important group of professionals who provide medical services for people with mental disorders live in the community, little is known about the experiences and characteristics of violence exposure among PVNs, or the characteristics and work situations of PVNs related to violence exposure. What this study adds to existing knowledge? Approximately 40% of participants were exposed to violence during the previous 12months; approximately 50% had been exposed during their PVN careers in PVN settings. The most frequent violence was verbal abuse. Longer career length as a PVN and greater number of visits per month were both positively associated with verbal abuse during the previous 12months. Twenty-eight of the 34 participants (83%) who completed the IES-R-J survey had some residual psychological distress, and two (6%) had a potentially high risk of posttraumatic stress disorder. What are the implications for practice? In devising policies and strategies against violence, PVN organizations and administrators should consider the characteristics of the violence, especially verbal abuse, as well as the characteristics and work situations of PVNs that are related to verbal abuse. Furthermore, they might provide relevant information on violence in PVN settings within their violence-prevention manuals or education. It would be important to provide support and to construct a safe workplace environment for PVNs who are experiencing residual psychological distress. Introduction Psychiatric visiting nurses (PVNs) play a crucial role by providing medical services for community-living individuals with mental disorders in Japan. However, little is known about violence towards PVNs. Aim This cross-sectional study investigated violence during visits and the resulting psychological effects for PVNs. Methods PVNs were assessed using a violence exposure questionnaire and the Impact of Event Scale-Revised (IES-R-J); a measure of posttraumatic distress. Result Thirty-eight (41%) of 94 participants had experienced violence during the previous 12months and 49 (53%) over their entire career. The most frequent violence was verbal abuse. Career length as a PVN and number of visits per month were significantly positively associated with verbal abuse during the previous 12months. The IES-R-J scores indicated 28 of the 34 participants who completed the questionnaire exhibited psychological distress for the most traumatic violence during their career and two had a potentially high risk of posttraumatic stress disorder. Discussion and Implications Policies and strategies aimed at reducing violence in PVN settings should be developed according to characteristics of the violence, as well as the characteristics and work situation of PVNs. - 本研究は,うつ病をもつ夫と生きる妻の体験を記述し,うつ病患者の妻に対するケアへの示唆をうることを目的とした。うつ病と診断された夫の妻2名に対し,非構成的インタビューを行い,得られた語りをPatricia Bennerの解釈学的アプローチを用いて分析した。分析の結果,妻は夫がうつ病をもつことで,夫婦間のコミュニケーションの変容を体験していた。妻はうつ状態の夫といるとき,夫の症状と同様の身体的感覚を受けていた。これはMaurice Merleau-Pontyが提唱した間身体的次元でのコミュニケーションの影響であると考えられた。この間身体的な体験が,夫を理解することに重要な意味をもっていた。そして,妻は夫のうつ病症状,および医療者や親戚等が期待する妻の行動を意識するがゆえに,自己に非常に自覚的になっていた。妻は経験を重ねながら,知的理解と間身体的理解を通して妻なりに夫を理解し,自らのあり方を見出していた。(著者抄録)(一社)日本看護研究学会, 2016, 日本看護研究学会雑誌, 39(2) (2), 13 - 23, Japanese[Refereed]Scientific journal

- 日本学術振興会, 科学研究費助成事業, 基盤研究(C), 神戸大学, 01 Apr. 2019 - 31 Mar. 2023精神疾患を有する人の家族における、ベネフィット・ファインディングの関連要因の解明ベネフィット・ファインディングとは、慢性疾患などの逆境の経験を経て、得られたものや学んだものがあったと感じることを指す主観的な概念であり、困難な状況をポジティブに捉えようとするコーピングの一つとして解釈されている。ベネフィット・ファインディングは、ポジティブ心理学に含まれる概念の一つであり、精神疾患を有する人においてはパーソナル・リカバリーと密接に関連する。本研究課題は、精神疾患を有する人々の家族のベネフィット・ファインディングの関連要因を明らかにすることを目的とする。 コロナ禍の中で、精神疾患を有する人々の家族のセルフヘルプグループに参加している人がどのようなベネフィット・ファインディングを経験しているのかについてヒアリングを行うとともに、精神疾患を有する人々の家族を対象とした、ベネフィット・ファインディングおよびその近接概念に関する研究のレビューから、精神疾患を有する人々の家族のベネフィット・ファインディングの関連要因・アウトカムの整理を試みようとしている。 また、関連するテーマでは、精神疾患をもつ人のリカバリーとセルフケアの関連についてのこれまでの研究の知見を整理し発表した。また、コロナ禍におけるポジティブ心理学についての知見について整理し発表した。さらに、精神疾患を有する人々やその家族へのケアの質の向上に影響を与える要因の一つとして、精神科看護師のワークエンゲイジメントの関連要因について検討し、論文投稿を行った。
