• Image Quality and Diagnostic Performance of Accelerated T2-weighted Imaging of Prostate with Deep Learning Reconstruction: A Comparative Study.

  PURPOSE: To compare accelerated T2-weighted turbo spin-echo imaging with deep learning reconstruction (DLR-TSE) with conventional T2-weighted TSE (conv-TSE) and accelerated TSE without DLR (non-DLR-TSE), and to evaluate image quality and diagnostic performance of Prostate Imaging Reporting and Data System version 2.1 (PI-RADS v2.1)-based T2 scoring in prostate MRI. METHODS: This single-center retrospective study included 60 patients who underwent prostate MRI with all 3 T2-weighted image sets acquired in the same examination. Qualitative image quality was independently assessed by 2 radiologists using 6 parameters on a 5-point Likert scale. Quantitative metrics included apparent SNR (aSNR), apparent contrast-to-noise ratio (aCNR), and contrast ratio (CR). Diagnostic performance of PI-RADS T2 scores for transition zone lesions was evaluated using receiver operating characteristic (ROC) analysis, sensitivity, specificity, and accuracy. Noninferiority of DLR-TSE relative to conv-TSE was tested with predefined margins. Inter-reader agreement was assessed using weighted kappa statistics. RESULTS: DLR-TSE demonstrated noninferiority to conv-TSE for all qualitative parameters and quantitative metrics for both readers. Both DLR-TSE and conv-TSE showed significantly higher image quality scores and quantitative values than non-DLR-TSE. For PI-RADS T2 scoring, DLR-TSE achieved diagnostic performance comparable to conv-TSE. For reader 1, the area under the ROC curve (AUC) was identical for DLR-TSE and conv-TSE (0.83; 95%CI 0.69-0.95), and significantly higher than for non-DLR-TSE (0.70; 95%CI 0.56-0.83). Specificity and overall accuracy were markedly reduced with non-DLR-TSE for both readers, whereas sensitivity did not differ significantly among methods. Inter-reader agreement was substantial to almost perfect for DLR-TSE and conv-TSE, and lower for non-DLR-TSE. DLR-TSE reduced acquisition time by approximately 60% compared with conv-TSE. CONCLUSION: Accelerated T2-weighted imaging with DLR allows substantial reduction in scan time while maintaining image quality, diagnostic performance, and inter-reader agreement comparable to those of conventional T2-weighted imaging. DLR-TSE may serve as a practical option for improving examination efficiency in clinical prostate MRI.

• Intraindividual comparison of photon-counting detector CT and third-generation dual-source energy-integrating detector CT in multiphasic contrast-enhanced abdominal CT.

  This study aimed to compare image quality between photon-counting detector computed tomography (PCD-CT) and third-generation dual-source energy-integrating detector CT (EID-CT) in multiphasic contrast-enhanced abdominal imaging using intraindividual analysis. This retrospective study included 88 patients who underwent both arterial phase (AP) and portal venous phase (PVP) abdominal CT with PCD-CT and EID-CT. Virtual monoenergetic images (VMIs) from 50 to 90 keV were reconstructed in PCD-CT, and 120 kVp images were applied in EID-CT. Quantitative image analysis evaluated CT attenuation, image noise, and contrast-to-noise ratio (CNR) in abdominal organs and vessels. Qualitative image quality, including vessel and parenchymal contrast, noise, and overall image quality, was assessed by two radiologists using a 5-point Likert scale. Compared with EID-CT, VMIs at 50–70 keV obtained from PCD-CT demonstrated significantly higher CT attenuation values (P < 0.001) across all evaluated abdominal organs and vessels. CNRs at 70 keV were significantly greater than those in EID-CT for all objects (P < 0.001), with increases of 13.3‒41.6% on AP and 40.4‒49.3% on PVP. Image noise in 70 keV VMI was lower than EID-CT (P < 0.001). Optimal image quality was achieved using VMIs at 60–70 keV, which achieved significantly higher qualitative scores (P < 0.001) than those from EID-CT. Substantial interobserver agreement was observed (κ = 0.60–0.79). The CT dose volume index was slightly lower with PCD-CT compared with EID-CT (P < 0.001), whereas dose-length product values remained similar. PCD-CT significantly improved image quality in multiphasic abdominal CT compared with third-generation dual-source EID-CT under equivalent radiation dose conditions. The superior iodine enhancement and higher CNR in VMIs at 60–70 keV highlight the advantages of PCD-CT.

• Mri-based diagnostic model integrating clinical features for placenta accreta spectrum in non-previa placenta.

  OBJECTIVES: To identify clinical and MRI features useful for diagnosing placenta accreta spectrum (PAS) in non-previa placenta and to develop diagnostic models integrating these features. METHODS: This retrospective study included 101 pregnant women with non-previa placenta who underwent MRI between January 2022 and June 2024. Nineteen were confirmed as PAS. Clinical variables and 11 MRI findings were evaluated using intraoperative or pathological results as the reference standard. Diagnostic performance was assessed using univariable analysis and repeated cross-validation of a random forest (RF) model, with ROC analysis used to assess discriminative performance. RESULTS: Hormone replacement cycle-frozen embryo transfer (HRC-FET) (sensitivity 0.89, specificity 0.63) and abnormal placental bed vascularization (sensitivity 0.63, specificity 0.90) showed the strongest univariable performance. The RF model using six variables with acceptable interobserver agreement achieved an AUC of 0.88, sensitivity 0.92, specificity 0.79, demonstrating higher discriminative performance than individual predictors. Feature importance analysis highlighted HRC-FET and abnormal placental bed vascularization as the most influential factors. CONCLUSIONS: Integrating clinical and MRI features improves PAS diagnosis in non-previa placenta. The RF model demonstrated a more balanced diagnostic profile than individual predictors in this exploratory cohort and may aid preoperative risk assessment. HRC-FET and abnormal placental bed vascularization were key contributors, supporting their relevance for risk stratification.

• Role of 18F-FDG-PET in renal tumors: insights from WHO 2022 classification.

  The objective of this article is to provide a comprehensive overview of the imaging characteristics of various renal cell tumors using 18F-fluorodeoxyglucose (FDG)-positron emission tomography (PET), based on the latest WHO-2022 classification. Due to the physiological accumulation of FDG in the kidneys, the clinical utility of FDG-PET in the evaluation of renal tumors has traditionally been considered limited. However, recent studies have re-evaluated its potential value. FDG-PET has demonstrated particular utility in detecting metastases and postoperative recurrence of renal cell carcinoma (RCC), as well as in identifying RCC in patients with chronic kidney failure, where FDG excretion into the urinary tract is reduced. Renal tumors are occasionally detected incidentally on FDG-PET, and FDG uptake varies depending on the tumor subtype. Therefore, a comprehensive understanding of these imaging characteristics is clinically important, as it may serve as a valuable guide for subsequent diagnostic evaluations. Furthermore, recent advancements in the development of novel PET tracers hold promise for future applications in the imaging of renal tumors. We believe that the insights gained from this study will contribute to routine diagnostic practice and the planning of future research.

• Time-synchronized 2-deoxy-2-[18F]fluoro-D-glucose PET/MRI with MR-active trigger and Bayesian penalized likelihood reconstruction: Diagnostic utility for locoregional extension of endometrial cancer.

  PURPOSE: Minimal misregistration of fused PET and MRI images can be achieved with simultaneous positron emission tomography/magnetic resonance imaging (PET/MRI). However, the acquisition of multiple MRI sequences during a single PET emission scan may impair fusion precision of each sequence. This study evaluated the diagnostic utility of time-synchronized PET/MRI using an MR active trigger and a Bayesian penalized likelihood reconstruction algorithm (BPL) to assess the locoregional extension of endometrial cancer. METHODS: Fifty-five patients with endometrial cancer who underwent pelvic 2-deoxy-2-[18F]fluoro-D-glucose PET/MRI were retrospectively evaluated. The PET emission time for the BPL reconstruction was determined by the MR active trigger of each MR sequence. The concordance rates of image interpretation with pathological T-staging, diagnostic performance for deep myometrial invasion (MI), and diagnostic confidence levels were evaluated by two readers and compared between time-synchronized, overlapping (conventional and simultaneous, but not time-synchronized), and sequential (not simultaneous) PET/MRI and MRI with diffusion-weighted imaging. Misregistration of the PET/MRI-fused images was determined by evaluating the differences in bladder dimensions. RESULTS: The T classification by time-synchronized PET/MRI was the most concordant with the pathological T classification for the two readers. Time-synchronized PET/MRI had a significantly higher diagnostic performance for deep MI and higher confidence level scores than overlapping PET/MRI for the novice reader (p = 0.033 and p = 0.038, respectively). The differences in bladder dimension on sequential PET/MRI were significantly larger than those on overlapping and time-synchronized PET/MRI (p <0.001). CONCLUSION: Time-synchronized PET/MRI is superior to conventional PET/MRI for assessing the locoregional extension of endometrial cancer.

• Uterine diverticulum mimicking endometriotic cyst of the ovary.

  Uterine diverticulum is a rare congenital malformation caused by abnormal fusion of the Müllerian ducts. The diagnosis of uterine diverticulum is difficult, and it is often misdiagnosed as a Müllerian duct anomaly, degenerated uterine fibroid, or ovarian cyst. We herein report a case of uterine diverticulum mimicking an ovarian endometriotic cyst. A multiparous woman with a history of normal vaginal delivery underwent magnetic resonance imaging for investigation of lower abdominal pain and fever. A 155-mm cystic lesion was observed on the ventral side of the uterus. The content of the cyst showed high signal intensity on T1- and T2-weighted images with precipitates of low signal intensity on the dorsal side, suggesting an endometriotic cyst of the ovary. Surgical and pathological findings revealed that the cyst was pedunculated from the anterior uterine body and composed of 3 layers: CD10-positive endometrium, a smooth muscle layer, and serosa. A uterine diverticulum was definitively diagnosed.

• Contrast enhancement of intracranial lesions at 1.5 T: comparison among 2D spin echo, Black-Blood (BB) Cube, and BB Cube FLAIR sequences.

  OBJECTIVES: The purpose of this study was to investigate the usefulness of T1W black-blood Cube (BB Cube) and T1W BB Cube fluid-attenuated inversion recovery (BB Cube-FLAIR) sequences for contrast-enhanced brain imaging, by evaluating flow-related artefacts, detectability, and contrast ratio (CR) of intracranial lesions among these sequences and T1W-SE. METHODS: Phantom studies were performed to determine the optimal parameters of BB Cube and BB Cube-FLAIR. A clinical study in 23 patients with intracranial lesions was performed to evaluate the usefulness of these two sequences for the diagnosis of intracranial lesions compared with the conventional 2D T1W-SE sequence. RESULTS: The phantom study revealed that the optimal parameters for contrast-enhanced T1W imaging were TR/TE = 500 ms/minimum in BB Cube and TR/TE/TI = 600 ms/minimum/300 ms in BB Cube-FLAIR imaging. In the clinical study, the degree of flow-related artefacts was significantly lower in BB Cube and BB Cube-FLAIR than in T1W-SE. Regarding tumour detection, BB Cube showed the best detectability; however, there were no significant differences in CR among the sequences. CONCLUSIONS: At 1.5 T, contrast-enhanced BB Cube was a better imaging sequence for detecting brain lesions than T1W-SE or BB Cube-FLAIR. KEY POINTS: • Cube is a single-slab 3D FSE imaging sequence. • We applied a black-blood (BB) imaging technique to T1W Cube. • At 1.5 T, contrast-enhanced T1W BB Cube was valuable for detecting brain lesions.

• T-staging of gastric cancer of air-filling multidetector-row CT: comparison with hydro-multidetector-row CT.

  PURPOSE: The purpose of this study was to evaluate the accuracy of T-staging of gastric cancer by air-filling multidetector-row CT (air-MDCT) compared with water-filling MDCT (hydro-MDCT). MATERIALS AND METHODS: One hundred fifteen patients with histologically diagnosed gastric cancer were included in this study. Fifty-eight patients underwent air-MDCT, and the remaining 57 had hydro-MDCT using a 64-channel scanner. Based on the volumetric data of contrast-enhanced MDCT obtained about 75 s after intravenously injecting 525 mg iodine per kilogram patients weight (525 mgI/kg) nonionic contrast material at the rate of 2 ml/s, oblique coronal and oblique sagittal multi-planar reformatted images perpendicular to the stomach wall, including the tumor, were reconstructed on a workstation. Mural invasion of gastric cancer into the gastric wall, as visualized by CT, was classified according to the TNM classification, and the results of T-staging by MDCT were compared with those by pathologic analysis after surgery. RESULTS: Correct assessment of T-staging by air-CT was achieved in 48 of 58 patients (83%), and that by hydro-MDCT was 49 of 57 patients (86%). The sensitivity, specificity, and accuracy of the technique in determining the invasion of serosa were 88%, 93%, and 91% for air-CT and 83%, 95%, and 91% for hydro-CT. There were no significant differences between hydro-MDCT and air-MDCT in sensitivity (P=0.73), specificity (P=0.71) and accuracy (P=0.98). CONCLUSION: Air-MDCT is a very valuable tool in T-staging of gastric cancer as well as hydro-MDCT.

• Optimal dose of contrast medium for depiction of hypervascular HCC on dynamic MDCT.

  PURPOSE: The purpose of this study is to prospectively investigate the optimal dose of contrast medium for the depiction of hypervascular hepatocellular carcinoma (HCC) during the hepatic arterial phase (HAP), portal venous phase (PVP) and delayed phase (DP) of dynamic MDCT. MATERIALS AND METHODS: The study included 128 patients, out of these patients, 36 patients were found to have 56 hypervascular HCCs. Sixty-three patients were assigned to receive a dose of 525 mgI/kg with protocol A, and 62 received a dose of 630 mgI/kg with protocol B. Measurements of the attenuation values of the abdominal aorta, portal vein, hepatic vein, hepatic parenchyma and HCC during the HAP, PVP and DP were taken. Tumor-liver contrast (TLC) was calculated from the attenuation value of the hepatic parenchyma and HCC. RESULTS: The aortic attenuation value with protocol B (351, 166, and 132 HU) was significantly higher than that with protocol A (313, 153, and 120 HU) during all the phases, (P<0.01 for all phases). The hepatic enhancement from unenhanced baseline with protocol B (25.2, 63.6, 50.6 HU) was significantly higher than that with protocol A (20.2, 55.1 and 43.0 HU) during all the phases, (P<0.01 for all phases). The TLC with protocol B (37.4, -11.8 and -13.6 HU) was significantly higher than that with protocol A (28.0, -9.8 and -12.1 HU) during HAP (P=0.042). CONCLUSION: The administration of 630 mgI/kg of body weight depicts hypervascular HCC more clearly during HAP and shows sufficient hepatic enhancement of 50 HU during DP.

  PURPOSE: To investigate the usefulness of T2* mapping of liver on gadolinium ethoxybenzyl diethylenetriamine pentaacetic acid (Gd-EOB-DTPA)-enhanced MRI for estimating liver function. MATERIALS AND METHODS: 33 patients were classified into 3 groups as follows: normal liver function (NLF) (n = 7); mild liver damage (MLD) (n = 16) with Child-Pugh A; severe liver damage (SLD) (n = 10) with Child-Pugh B. T2*-weighted gradient-echo (T2*W-GRE) and T1-weighted gradient-echo (T1W-GRE) images were obtained before and after Gd-EOB-DTPA administration (3, 8, 13, and 18 min; 5, 10,15, and 20min; respectively). T2* mapping of liver was calculated from T2*W-GRE, then T2* values of liver and T2* reduction rates of T2* value between pre- and post-contrast enhancement were measured. The increase rates of liver-to-muscle signal intensity (LMS) ratio on T1W-GRE between pre- and post-contrast enhancement were calculated. RESULTS: T2* values on pre- and post-contrast showed no significant differences among three groups. Significant differences in T2* reduction rates were found among groups, and those of LCB were lower than those of other groups (NLF:MLD:SLD, 3.8:6.0:0.6% at 3 min, 8.2:10.3:1.0% at 8 min, 10.7:11.5:1.2% at 13 min, and 16.1:13.2:3.5% at 18 min, respectively) (P<0.05). Significant differences in increase rates of LMS ratio on T1W-GRE were identified (NLF:MLD:SLD, 1.53:1.46:1.35 at 5 min, 1.68:1.64:1.37 at 10 min, 1.79:1.76:1.44 at 15 min, and 1.89:1.78:1.49 at 20 min, respectively). CONCLUSION: T2* reduction rate and increase rate of LMS ratio on T1W-GRE may allow us estimation of liver function according to Child-Pugh score.

• T2* relaxometry mapping of the uterine zones.

  BACKGROUND: Previous literature demonstrated that the T2* value of the uterine junctional zone was lower than that of peripheral myometrium by using BOLD MR imaging. We expect T2* mapping image may add more information to T2-weighted images of the uterine myometrium. PURPOSE: To evaluate whether T2* mapping software would reproduce the result of previous report, and to apply the software to benign uterine diseases. MATERIAL AND METHODS: Five healthy volunteers and 19 patients clinically suspected of having benign pelvic disease were imaged using a 1.5T MR system. All women were of reproductive age, and all provided informed consent. Sagittal T2* images using a multishot EPI sequence were obtained. T2* values were calculated and color T2* maps reconstructed using a T2* fitting tool. RESULTS: The uterine zones could be identified in all 24 examinations on the T2* maps. In addition, a thin "4th zone" was seen between the endometrium and the JZ (junctional zone) in 19 of 24 examinations. The T2* value of JZ was significantly lower than that of peripheral myometrium (PM) (P < 0.001). No significant difference in the T2* value of the JZ or of PM was noted between normal uterus and uterus with leiomyomas and/or adenomyosis. CONCLUSION: A quantitative T2* map can easily be obtained using the PRIDE software T2* fitting tool, and the software reproduces the result from previous report. T2* value of the junctional zone was lower than that of peripheral myometrium regardless of having benign myometrial diseases.

• Primary adenomyoma of the fallopian tube mimicking tubal malignant tumor.

  We report a rare case of adenomyoma localized only in the left fallopian tube mimicking tubal malignant tumor. A 45-year-old woman presented with mild pelvic pain, dysmenorrhea and left adnexal mass. Magnetic resonance imaging showed a solid tumor, suspected primary cancer of the fallopian tube, and serum carbohydrate antigen 125 was elevated to 72 U/mL (normal; 0-35). At surgery, the tumor was revealed as a left fallopian tube tumor without torsion. Postoperative histopathology showed that the tumor included bundle-like growing non-atypical leiomyoma cells and ectopic normal endometrium accompanied with endometrial stroma and we diagnosed primary adenomyoma of the left fallopian tube. Adenomyoma localized only in the fallopian tube is a rare entity and it can occur only in the fallopian tube.

• Unenhanced fat fraction ratios obtained by MR and enhanced T2* values with liver-specific MR contrast agents for diagnosis of non-alcoholic steatohepatitis in rats.

  BACKGROUND: Non-invasive MR imaging is expected to be used for accurate diagnosis and quantification of non-alcoholic steatohepatitis (NASH), because NASH is a progressive fatty liver disease. New MR techniques, such as fat fraction ratio (FFR) and T2* value measurement, have attracted an increasing attention, because those techniques can measure quantitative parameters of fibrosis, fat and iron deposition in the liver. PURPOSE: To investigate the potential of FFR and T2* value in NASH with pre-enhancement, gadolinium-ethoxybenzyl-diethylenetriamine pentaacetic acid (Gd-EOB-DTPA) or super-paramagnetic iron oxide (SPIO)-enhanced MRI. MATERIAL AND METHODS: Twenty-eight rats were divided equally into four groups (one control group and three NASH groups). All rats underwent unenhanced, Gd-EOB-DTPA, and SPIO-enhanced MRI. The T2* value of the liver was measured for each image sequence, and then changes in T2* values before and after each injection were analyzed using Dunnett's test. The reduction rate of T2* value before and 13 min after injection of Gd-EOB-DTPA or SPIO was analyzed using Mann-Whitney's U test. Moreover, FFR of the liver was measured before enhancement, and the relationship between fat fraction and the calculated fat area percentage on a pathological specimen was examined using Spearman's correlation test. RESULTS: On pre-enhancement, FFR and T2* value were 26.0% ± 12.0% and 21.5 ± 4.2 ms for all NASH groups, and 0.9% ± 0.5% and 30.8 ±-5.5 ms for control, respectively. Both FFR and T2* values were significantly different between the NASH and control groups. The reduction rate of T2* value was significantly lower in the NASH groups than in the control group on SPIO-enhanced MRI, though there was no significant difference on Gd-EOB-DTPA-enhanced MRI. FFR was correlated with the calculated fat area percentage for the pathological specimen. CONCLUSION: Pre-enhancement FFR, T2* value measurement and reduction rate of T2* value on SPIO-enhanced MRI may help estimate the progress of liver fat deposition and fibrosis in NASH.

• 卵巣漿液性腺癌と成熟嚢胞性奇形腫のcollision tumorの１例

  25歳女。不正出血を主訴とした。MRIで骨盤内に脂肪を含んだだるま型の腫瘤を認め、その腹側に不整形の充実性腫瘍が存在していた。充実性腫瘍はT1強調像で低信号、T2強調像で高信号、内部樹枝状の低信号を示し、乳頭状に発育していた。造影CTでは充実性腫瘍が淡い増強効果を示し、複数の腹膜播種を疑う病変もあり、上皮性癌または境界悪性腫瘍を疑い、切除術を施行した。右卵巣に脂肪を含むだるま型の腫瘤の間に充実性腫瘍を認め、一部漿膜が破綻し、露出して体網が癒着していた。病理組織所見で、だるま型腫瘤は角化重層扁平上皮に囲まれたcystがあり、壁に皮膚付属器や軟骨組織を認め、成熟嚢胞性奇形腫と診断した。これとは別に大小不同の濃染核をもつ円柱状の異型細胞が繊毛状或いは乳頭腺管状に配列して増殖・浸潤しており、卵巣漿液性腺癌と診断した。二つの組織は近接していたが移行像は認めず、collision tumorと考えられた。

  PURPOSE: To prospectively compare magnetic resonance imaging (MRI) at 3.0 T and 1.5 T in the same patients for preoperative evaluation of endometrial carcinoma. MATERIALS AND METHODS: Thirty consecutive patients with endometrial carcinoma underwent MRI at both 3.0 T and 1.5 T as well as surgery. Quantitative and qualitative analyses were performed. Two radiologists independently evaluated images. MR findings were compared with surgicopathologic findings. RESULTS: Image homogeneity of T2-weighted images at 3.0 T was significantly inferior to that at 1.5 T (P = 0.007). The scores of image homogeneity and susceptibility artifacts were not significantly different between 3.0 T gadolinium-enhanced imaging and 1.5 T imaging (P = 0.09 and 0.36). Kappa statistics showed good interobserver agreement between the two radiologists for local-regional staging on T2-weighted images (kappa>0.6). The area under the receiver operating characteristic curve (Az) values for T2-weighted imaging in terms of myometrial invasion, cervical invasion, and lymph node metastases were 0.88 (3.0 T) versus 0.91 (1.5 T), 0.84 versus 0.83, and 0.94 versus 0.95 for reader 1, respectively. There were no significant differences between imaging at 3.0 T and at 1.5 T in Az values for either reader (P > 0.35). CONCLUSION: 3.0 T MRI is an equivalent imaging modality to 1.5 T imaging for presurgical evaluation of endometrial carcinoma, although not significantly superior to 1.5 T imaging.

• Adrenal masses: the value of additional fluorodeoxyglucose-positron emission tomography/computed tomography (FDG-PET/CT) in differentiating between benign and malignant lesions.

  OBJECTIVE: To investigate whether integrated fluorodeoxyglucose-positron emission tomography/computed tomography (FDG-PET/CT) can differentiate benign from adrenal malignant lesions on the basis of maximum standardized uptake value (SUV(max)), tumor/liver (T/L) SUV(max) ratio, and CT attenuation value (Hounsfield Units; HU) of unenhanced CT obtained from FDG-PET/CT data. METHODS: We studied 30 patients with 35 adrenal lesions (16 adrenal benign lesions, size 16 +/- 5 mm, in 15 patients; and 19 adrenal malignant lesions, 24 +/- 12 mm, in 15 patients) who had confirmed primary malignancies (lung cancer in 23 patients, lymphoma in 2, esophageal cancer in 2, hypopharyngeal cancer in 1, prostate cancer in 1, and 1 patient in whom lesions were detected at cancer screening). All patients underwent PET/CT at 1 h post FDG injection. Diagnosis of adrenal malignant lesions was based on interval growth or reduction after chemotherapy. An adrenal mass that remained unchanged for over 1 year was the standard used to diagnose adrenal benign lesions. Values of FDG uptake and CT attenuation were measured by placing volumetric regions of interest over PET/CT images. Adrenal uptake of SUV(max) >/= 2.5 was considered to indicate a malignant lesion; SUV(max) < 2.5 was considered to indicate a benign lesion. In further analysis, 1.8 was employed as the threshold for the T/L SUV(max) ratio. Unenhanced CT obtained from PET/CT data was considered positive for adrenal malignant lesions based on a CT attenuation value >/= 10 HU; lesions with a value < 10 HU were considered adrenal benign lesions. Mann-Whitney's U test was used for statistical analyses. RESULTS: SUV(max) in adrenal malignant lesions (7.4 +/- 3.5) was higher than that in adrenal benign lesions (2.1 +/- 0.5, p < 0.05). The CT attenuation value of adrenal malignant lesions (27.6 +/- 11.9 HU) was higher than that of adrenal benign lesions (10.1 +/- 12.3 HU, p < 0.05). In differentiating between adrenal benign and malignant lesions, a CT threshold of 10 HU corresponded to a sensitivity of 57%, specificity of 94%, accuracy of 74%, positive predictive value of 92% and negative predictive value of 65%. An SUV(max) cut-off value of 2.5 corresponded to a sensitivity of 89%, specificity of 94%, accuracy of 91%, positive predictive value of 94% and negative predictive value of 88%. The T/L SUV(max) ratio was 1.0 +/- 0.2 for adrenal benign lesions and 4.5 +/- 3.0 for adrenal malignant lesions. And T/L SUV(max) ratio cut-off value of 1.8 corresponded to a sensitivity of 85%, specificity of 100%, accuracy of 91%, positive predictive value of 100% and negative predictive value of 83%. CONCLUSIONS: FDG-PET/CT with additional SUV(max) analysis improves the diagnostic accuracy of adrenal lesions in cancer patients.

• Ring-shaped lateral ventricular nodules: an incidental finding on brain magnetic resonance imaging.

  INTRODUCTION: We have occasionally seen ring-shaped lateral ventricular nodules <1 cm in diameter during routine brain magnetic resonance imaging (MRI). We investigated retrospectively clinical and MRI findings of the nodules. MATERIALS AND METHODS: Review of radiological records was performed for 39,607 patients who underwent brain MRI between January 2001 and April 2008. Nodules were assessed for number, location, shape, and signal intensity, which was determined based on the range of signal intensity from gray to white matter on T1- and T2-weighted imaging. Fluid-attenuated inversion recovery (FLAIR) and diffusion-weighted imaging (DWI), contrast enhancement characteristics, and serial MRI changes of nodules were assessed when available. RESULTS: Nine of 39,607 patients (0.023%) showed the nodules. No symptoms associated with the nodules. Among the nine patients, 11 nodules were identified (one nodule in seven patients (77.8%) and two nodules in two patients (22.2%)). Location was limited to the roof of the body for six nodules (54.5%) and the frontal horn for five patients (45.5%). All nodules (100%) were round and isointense on T1- and T2-weighted imaging. On FLAIR imaging of eight nodules, six (75%) were hyperintense, and two (25%) were isointense. On DWI of seven nodules, all nodules (100%) were isointense. None of seven nodules (0%) examined using postcontrast MRI showed enhancement. None of eight nodules (0%) examined using serial MRI (range, 4-60 months) showed changes in morphology over time. CONCLUSION: These nodules were incidentally encountered and shared similar MRI features. Although pathological confirmation was lacking in our cases, these nodules may be of nonaggressive nature.

• Comparison between one-route and two-route injection for liver and aortic enhancement using MDCT.

  OBJECTIVE: The purpose of our study was to evaluate whether simultaneous injection into cubital veins bilaterally at one half of the standard injection rate achieves similar hepatic and aortic enhancement on MDCT as the conventional injection rate into a single cubital vein. MATERIALS AND METHODS: Thirty-two patients underwent multiphase MDCT because they were suspected of having a hepatic tumor. Patients were assigned to one of the following two groups: group A, 100 mL of 370 mg I/mL of contrast medium injected into a unilateral cubital vein (one-route) via a 20-gauge cannula at a rate of 4 mL/s; or group B, 50 mL of contrast medium injected into the cubital veins bilaterally (two-route) via 24-gauge cannulas at 2 mL/s. Peak contrast enhancement of the liver and abdominal aorta for groups A and B was measured using regions of interest and compared; arrival time of the contrast media was also compared using a bolus-tracking system. Analysis was performed using Wilcoxon's signed rank test. RESULTS: Peak aortic enhancement of groups A and B was 367 +/- 67 H and 361 +/- 113 H (p = 0.61, not significant), respectively, and peak hepatic enhancement of groups A and B was 56 +/- 11 H and 56 +/- 16 H (p = 0.88, not significant), respectively. Mean arrival time to the aorta of group B (19.4 +/- 3.4 seconds) was significantly later compared with that of group A (15.5 +/- 3.5 seconds) (p = 0.005). CONCLUSION: The slower two-route injection produced the same aortic and hepatic enhancement as the faster one-route method with faster injection, but the arrival time of the contrast medium was later using the two-route method.

• [Advancement of the imaging for hepatic malignancies].

  The development of CT and MR imaging technology represents a significant advancement. On CT imaging,the advent of multi-detector row CT (MDCT) scanners enables an increase in both scanning speed and spatial resolution. Multiphasic dynamic study is useful for the detection of hepatocellular carcinomas (HCCs) which are demonstrated as hyperattenuated lesions during the hepatic arterial phase. Many investigators assess the optimal timing for detection of HCCs by using the evolving MDCT, and have demonstrated the high detection rate for HCCs. Thanks to the increase in spatial resolution along the z-axis,we are now able to generate high-quality CT angiography and multiplanar reformations without complicated interpolation steps. These CT angiography and MPR images of patients with malignant hepatic tumors (e.g., HCCs, cholangiocellular carcinomas) are very useful for preoperative evaluation. On the other hand, the development of MR fast imaging techniques allows rapid breath-hold, whole-liver imaging. Superparamagnetic iron oxide (SPIO) is a liver-specific particulate magnetic resonance (MR) imaging contrast agent that is taken up by the reticuloendothelial system of the liver. SPIOenhanced MR imaging is useful for the detection of hepatic metastases. The advent of parallel imaging techniques improves the quality of images obtained with diffusion weighted (DW) imaging of the liver. DW imaging demonstrates high accuracy in the detection of hepatic metastases. Recently, in Japan, positron emission tomography (PET)-CT has also been employed for evaluation of hepatic metastases.

• A case of localized pseudomyxoma peritonei associated with mucinous cystadenoma of the appendix

  症例は53歳女性で、3年前から3回下腹部痛を主訴に来院し、保存的治療にて軽快していたが、当時撮影された骨盤MRIでは約4cm大の左卵巣腫瘍が認められ経過観察されていた。今回、経過観察のためのMRIにて左卵巣腫瘤とは別に、骨盤腔に一部充実性成分を有する多房性嚢胞性腫瘤を認め、卵巣腫瘍の術前診断にて手術目的で入院となった。腹腔鏡下腫瘤摘出術が施行され、右骨盤底部に白色手拳大、ゼリー状物質の透見できる嚢胞状腫瘤を認め、腫瘤壁は脆弱で腹腔鏡操作で容易に被膜損傷をきたして腹腔内にゼリー状物質が流出した。腫瘤は虫垂先端に連続しており、虫垂粘液性腫瘍より発生した限局性腹膜偽粘液腫と考えられたため開腹術に移行した。腫瘍壁は回腸と強固に癒着しており、回腸も一部合併切除し、直腸・仙骨前面にも強固な癒着を認めたが剥離摘出した。摘出標本の病理組織学的所見から卵巣腫瘍との鑑別が困難であったが虫垂粘液性嚢胞腺腫破裂による限局性腹膜偽粘液腫と診断された。

• MRI evaluation of the uterine structure after myomectomy.

  Myomectomy is a good indication for women with uterine leiomyoma who desire to preserve their child-bearing potential. However, there are still no reports about how long it takes the uterus to reach a stable state after myomectomy. We evaluated the changes in uterine structure during the recovery process after myomectomy by MR images. MR images were used to analyze the time-dependent changes in the length of the uterine cavity, the volume of the uterus, recovery of the junctional zone, prevalence of modification of the endometrium, and uterine structure in the region of the enucleated myoma. The cavity length and the volume of the uterus, and the myometrium were stabilized at six weeks after the myomectomy. With regard to the endometrium, 12 weeks were required for it to achieve a stable state after myomectomy. However, even at 12 weeks postoperatively, 14.2% of the cases showed an unusual view near the uterine incision on MR images. We concluded that the recovery process is complete at 12 weeks after the operation if there are no clear findings of hematoma or edema formation in the myometrium on MR images.

  脂肪抑制を併用したsegmented True FISPによる門脈,肝静脈,胆管描出のための至適条件について,ファントムおよび健常ボランティアを対象として基礎的な検討を行い,次いで臨床応用を試みた.1.5T超伝導MR装置を用い,受信用コイルとして,ファントムの撮像にはCPヘッドアレイコイルを,ボランティアを対象とした撮像には,CPボディアレイコイルを使用した.肝臓と門脈,肝静脈,胆管とのコントラストは,high flip angleでsegment数を増加させての撮像で良好であった.しかし,脂肪抑制効果を加味するとsegment 75程度での使用が適当であった.健常ボランティア全例で造影剤を使用することなく門脈,肝静脈および総胆管の良好な画像を得ることが可能であった

• 腹膜漿液性乳頭状腺癌の３例：MR所見について

  症例1は61歳女で,腹部エコーにて腹水貯留が指摘された.腹水細胞診はclass V,adenocarcinomaと診断された.血清CA125は高値であり,MRIでは多量の腹水貯留とダグラス窩にシート状に腫瘤が認められ,大網にも腫瘤形成を認めた.病理で卵巣は腹膜漿液性乳頭状腺癌(PSPC)と診断された.症例2は49歳女で,腹水細胞診はclass V,adenocarcinomaと診断された.血清CA125は高値で,MRIでは多量の腹水貯留とダグラス窩・膀胱子宮窩にT2強調像で淡い高信号を示す腫瘤を認めた.病理でPSPCと診断された.症例3は52歳女で,右下腹部痛を主訴とした.血清CA125は高値であり,MRIのT2強調像でダグラス窩,回盲部に淡い高信号を呈する腫瘤を認め,右総腸骨,外腸骨リンパ節の腫大を伴っていた.病理でPSPCと診断された

• [Fundamental study of the relation between flow velocity and signal intensity in the TrueFISP sequence].

  The purpose of this study was to evaluate the effect of inflow phenomenon on TrueFISP. We created a phantom using a vinyl tube and distilled water, and applied a pump-oxygenator to the phantom to obtain stationary flow. First, to evaluate the effect of inflow and the dephase phenomenon on signal intensity, the phantom was measured for the signal intensity of variable flow velocity. Second, the relation of TR/TE with signal intensity was analyzed. The results showed that a flow velocity of less than 15 cm/sec did not participate in signal reduction; however, signal intensity was reduced when flow velocity was more than 30 cm/sec. Moreover, the reduction of signal intensity was remarkable with a flow velocity of 50-100 cm/sec, which corresponds with arterial flow velocity. In the analysis of TR/TE, signal intensity was increased when TR of less than 5 ms was applied to the slow velocity of 15 cm/sec. Signal intensity was decreased when the same TR was applied to the high velocity of 50-100 cm/sec. When TR was 6-9 ms, peak signal intensity was recognized at the high velocity of 50-100 cm/sec. This peak, however, might correspond only to the inflow phenomenon, and steady state might have already collapsed. Based on these results, we concluded that TrueFISP is suitable for the imaging of slow flow velocity. A short TR of less than 5 ms was effective for obtaining high signal intensity. Our next goal will be to apply TrueFISP to MR venography, although further investigation will be necessary.

• TrueFISPにおける流速と信号強度に関する基礎的検討

  TrueFISP(fast imaging with steady state free precession)における流れの影響について,定常流の流体ファントムを用いて検討した.使用装置はSiemens社製MAGNETOM Symphony 1.5T,受信用コイルとして,スライス面内の流速による信号変化の検討にボディコイルを用い,その他の検討にはCP四肢コイルを用いた.使用流体ファントムは,内径9.5mmの塩化ビニルチューブと人工心肺送血ポンプ(定常流)を用いた.内溶液として蒸留水を使用した.TrueFISPはパラメータによって様々な信号特性を示すが,基本的に流れに対して位相補正が行われていないことが判明した.しかし,比較的遅い流れに対し流入効果による信号の上昇が確認され,静脈血流等の流れが遅い血流の描出に有用であると示唆された

• [Usefulness of 3D-VIBE method in breast dynamic MRI: imaging parameters and contrasting effects].

  MR imaging (MRI) has been reported to be a useful modality to characterize breast tumors and to evaluate disease extent. Contrast-enhanced dynamic MRI, in particular, allows breast lesions to be characterized with high sensitivity and specificity. Our study was designed to develop three-dimensional volumetric interpolated breath-hold examination (3D-VIBE) techniques for the evaluation of breast tumors. First, agarose/Gd-DTPA phantoms with various concentrations of Gd-DTPA were imaged using 3D-VIBE and turbo spin echo (TSE). Second, one of the phantoms was imaged with 3D-VIBE using different flip angles. Finally, water excitation (WE) and a chemical shift-selective (CHESS) pulse were applied to the images. Each image was analyzed for signal intensity, signal-to-noise ratio (1.25*Ms/Mb) (SNR), and contrast ratio [(Ms1-Ms2)/[(Ms1+Ms2)/2]]. The results showed that 3D-VIBE provided better contrast ratios with a linear fit than TSE, although 3D-VIBE showed a lower SNR. To reach the best contrast ratio, the optimized flip angle was found to be 30 degrees for contrast-enhanced dynamic study. Both WE and CHESS pulses were reliable for obtaining fat-suppressed images. In conclusion, the 3D-VIBE technique can image the entire breast area with high resolution and provide better contrast than TSE. Our phantom study suggests that optimized 3D-VIBE may be useful for the assessment of breast tumors.

• 乳腺Dynamic MRIにおける3D-VIBE法の有用性ー撮像パラメータと造影効果ー

  乳腺ダイナミックMRIのThree dimensional volumetric interpolated breath-hold examination(3D-VIBE)法において,至適flip angleと脂肪抑制法について検討し,又,造影効果について従来のturbo spin echo(TSE)法及びSE法と比較検討した.その結果,最短TR,TE下での3D-VIBEの至適flip angleは,コントラストを重視した30°が適切であった.脂肪抑制効果については時間分解能を重視したCHESS法が望ましく,又,造影効果については,コントラストではTSE,SE法に比べて優れていることが示された.3D-VIBEは時間分解能に優れ,従来法と比較してスライス方向の分解能がよく,同時相での多方向からの観察に優れ,3D撮像による付加価値も多く,乳管内進展病巣の描出や乳癌の広がり診断に有用と考えられた

  天理よろづ相談所病院放射線部MR部門では手作りのMR診療録参照パソコンシステムを計画し実用化した.即ち,MR部門開設以来から現在に至る迄の12年間のMR検査施行患者全てのデータベースファイルを,Filemaker Pro(FMP)(Claris Corporation)を利用して作成した.MR診療録は,診断報告書,依頼箋,問診表,撮像法記録の4種のデータから構成され,これら4種類の各々のデータ内では,5000患者毎で分割して1ファイルとし,結果的に複数のファイルとして保存した.MR診療録の検索は,FMPのスクリプト機能,リレーション機能及びボタン機能を駆使して簡単でスピーディーに行えるように工夫した

  天理よろづ相談所病院放射線部MR部門のMRレポーティングシステムを計画し実用化した.その結果,MR部門の日常業務でレポート作成が非常に円滑に行えるようになった.システムは必要に応じて比較的容易に改良でき,機能拡張も可能なシステムである.システムの特徴は市販のデータベースソフトであるFilemakerPro.ver.4.1(FMP)を使用して計画から作成,実用化まで放射線部MR部門職員の手作りで行ったことである.細部の変更もソフトハウスなどの援助をまったく必要としない

  第283回日本医学放射線学会関西地方会, Jun. 2006, 第283回日本医学放射線学会関西地方会, 大阪, 症例は73歳男性で、主訴は下痢。大動脈解離(DebakeyI型)術後1週間より発症。CTで解離の及んでいた下腸間膜動脈の閉塞を認め、虚血性腸炎と考えた。解離術後に発生する腸管虚血は稀な合併症で、上腸間膜動脈閉塞が原因となることが多く、下腸間膜動脈が原因となることは極めて稀（本邦１９８５～０３年の間で３例）である。虚血性腸炎発症時期は術後数週からという報告が多い。本症例でも術後1週間から発症しており、虚血性腸炎は解離術後から発症までにはタイムラグがあることを知っておくことは重要である。, Domestic conference
