- Abstract Background Children and adolescents with intellectual and developmental disabilities (IDD) often experience reduced health-related physical function. However, the correlation between decline in physical function and reduced limb skeletal muscle mass among adolescents with IDD is not yet clear. This study aimed to clarify the relationship between limb skeletal muscle mass and physical function in adolescents with IDD. Methods This short-term longitudinal observational study included 53 adolescents (aged 12–18 years) with IDD from special needs schools who attended community sports classes in Kobe City, Japan. The assessment included body composition analysis (skeletal muscle mass index: SMI) and physical fitness tests (handgrip strength, standing long jump, sit-ups, sit-and-reach test, six-minute walk test, pulmonary function tests), complemented by physical activity level questionnaires. Results Significant positive correlations were observed between SMI and handgrip strength, as well as between SMI and standing long jump performance. The physical fitness assessment showed high reliability upon re-measurement after 4 weeks. Additionally, multiple linear regression analyses considering SMI as the dependent variable and adjusting for age, sex, body mass index, and physical activity level showed significant positive associations for handgrip strength and standing long jump. Conclusions This study revealed significant associations between skeletal muscle mass index and physical fitness indicators, such as handgrip strength and standing long jump, in adolescents with intellectual and developmental disabilities. - Background Ensuring effective return to work following acquired brain injuries is crucial from the perspectives of both quality of life and the economy. However, techniques of occupational therapy support for return to work remain relatively unelucidated. Aims/Objectives To clarify the specific contents of occupational therapy required for work and work support for clients with acquired brain injuries. Material and Methods An interview-based survey was conducted with participants who had >10 years of occupational therapy experience and had provided work support. We selected participants via snowball sampling. Data were analyzed using thematic analysis. Results A total of 20 participants (15 women and 5 men; 6, 12, 1, and 1 in their 30s, 40s, 50s, and 60s, respectively) were included. Six concepts were generated on reviewing the support for work items considered important by the occupational therapist as follows: “Support for vocational life,” “Support for interpersonal skills,” “Support for work,” “Support for illness, disability, and awareness,” “Support for utilization of compensation measures,” and “Support for goal setting.” Conclusions We clarified the specific contents of work support, including support for vocational life and support for work, that is administered by occupational therapists who provide work support for clients with acquired brain injury. Recently, the progress of everyday technology (ET) like home electronics is noticeable, that have multifunctional, high functionally and convenient value.
On the other hand, there are less benefits for the patients with cognitive impairments due to the distance pace of progress of ET. They are confused of how to use these ET, therefore it increases the care burden of caregivers.
We have visited the elderly people and the clients with higher brain dysfunctions /dementia at their own home, and researched the usage conditions of ET by using Everyday Technology Use Questionnaire (ETUQ) that was developed by Rosenberg and Nygård.
We have found that even normal elderly have trouble with new ET, and there were two reasons of discontinuing using these ET;one of them is the change of their lifestyles and the other one is the decrease of their cognitive functions.
The people with dementia are unable to the procedure of using ET and a lot of them were using the old ET. Furthermore we have researched usage of ET of the case of Gersmannʼs syndrome for over 4 years by ETUQ. She still has difficulties with the ET which are necessary to understand the meaning of the numbers or letters for operating.

Although the accumulation of the knowledge, they hardly reflect to the programs for clinical use. We developed a task set named "Atama-no-dojo," designed to activate multiple cognitive functions and enhance motivational incentives. The objective of our study is to confirm the effect of our program through a 6 months group intervention program. The intervention program conducted in a day service center for 6 months in the duration of 45 minutes per day, 4 days per month for a total of 25 sessions. Participants worked to the tasks on the screen all together with filling in the answering sheet. Neuropsychological tests, SF36 and GDS were assessed at pre-/post-intervention periods. Participants filled in a questionnaire about impression to the program at the last training session. Fourteen women (82.2 ± 2.9 years old) were analyzed and significant changes were found in the improvement of memory, attention, inhibition, GDS and some items of SF36. All participants recognized the program as fun and wanted to continue. Some of the participants' positive impressions to the program correlated to cognitive improvement. The improved cognitive functions by 6 months intervention of "Atama-no-dojo" were mainly related to prefrontal cortex and the motivational incentives seemed supported the effect of task contents. - 認知力低下に配慮した継続使用が可能な家電製品のデザイン方法に関する研究少子高齢化が進行している我が国において、認知症者の増加は予想をはるかに上回り、2012年時点で高齢者人口の15%にあたる約462万人が認知症を発症していると発表された(朝田隆、厚生労働省研究班、2013)。一方、2010年現在、高齢者のいる世帯は全体の4割を占め、独居高齢者は男性140万人、女性346万人と推計されている(平成24年版高齢者白書、内閣府)。加齢に伴う生活不安の一つは自身あるいは家族が認知症になることであり、認知症が進行すれば在宅生活をあきらめざるを得ない。アルツハイマーに代表される認知症は進行性の疾患であり、数年間に及ぶ初期症状の段階を経て要介護状態となる。この初期段階における日常生活上の困難や混乱によって生じる「問題行動」は生活行為を縮小させ、認知機能の廃用を引き起こし、認知症の進行を早める危険性がある。著者らは、生活環境とりわけ日常生活で使用する家庭電化製品等(以下家電等と略記)が認知機能の低下に配慮していないことに起因していると仮定し、独居もしくは日中独居の高齢者がどのような家電等を継続使用しているのか、使用を中断したものはないか、といった調査を行い、その結果から認知力が低下しても継続使用が可能な家電等のデザイン方法について7つの知見を得た。In 2013, a research unit of Ministry of Health, Labor and Welfare announced that more than 4.62 million are dementia and around 4 million are MCI (Mild Cognitive Impairment), extremely exceeding expectation. On the other hand, households which have elderly counted 42% in 2010 in Japan. The cabinet estimated 1.3 million men and 3.5 million women of over 65 live alone.One of the fears of aging people is to be a dementia. People must move to institute when the stage of dementia goes deep. The dementia like an Alzheimer's disease become worth in several years, after intermediate stage so called MCI. In this intermediate stage, if some kind of problems happens, family member tend to take his/her independent activities. This makes dementia worth.The authors interviewed 91 elderly who live alone to find what kind of everyday technology are still used or quite using. - A intraventricular hemorrhage patient of a 75-year-old woman was hospitalized with left hemiplegia. She impaired level of consciousness in onset, it showed III -200 in JCS, both constricted pupil, and decelerate state. In rehabilitation clinical pathway proposal for Japanese that a study group of Ministry of Health and Welfare developed, it is adapted a course for six months at that time a consciousness level in the onset of three columns in JCS. According to it, start of a toilet activities exercise is introduced 60 days after the onset. This time, however, we started rehabilitation from early stage with external ventricular drain placed. Because we possible preservation of disuse syndrome. - The purpose of this study was to investigate the relationship between the social activities and body functions and to clarify the factors associated with the social activities in the community-dwelling healthy older people. The subjects were 178 older people who live independently in the community and join the elderly welfare centers in Osaka City. The "Social Activity Index" was measured as the social activities, "grasping power", "ventriflextion with long seating position", "one-leg standing with eyes open & eyes close", "Functional Reach Test" and "Timed-Up & GO (TUG)" as the body functions, "height", "weight", "BMI" and "body fat" as the physical characteristics, and the associations between the social activities and each item of the body functions and the physical characteristics were examined. There was a statistical significance only in "TUG" (r=-0.391 p<.01) with the social activities in the young-old people (total 55:15 males, 40 females, mean age 71.1±2.0) and there were statistical significances in "age" (r=-0.179 p=.04), "body fat" (r=0.233 p=.01), "one-leg standing with eyes close" (r=0.230 p=.01) and "TUG" (r=-0.301 p<.01) in the old-old people (total 123:35 males, 88 females, mean age 80.6±4.8). The body functions and the physical characteristics in the old-old showed the association with the social activities than in the young- old. - An unignorable percentage of students have difficulty completing our clinical fieldwork program even though they have enough knowledge and skills from seminar programs. The lack of confidence in their clinical ability and negative impression of people with mental disability might have contributed to their discretion. Before going into the clinical fieldwork program the students were guided to attend a clinical interview with a person with mental disability. Twenty-three students (twenty female and three male students) who major in occupational therapy participated in this program. A questionnaire was given before and after each interview. This questionnaire consisted of ten items on self-confidence in the students', and eight items on impression of people with mental disability. These items were arranged from hearing of students' comments, and passed a trial. The students' confidence in communication skills, listening ability, and flexible response improved after this program. Seven items out of eight on an impression of psychiatric patients improved; fearfulness, incongruity with others in external appearance, behavioral incongruity with others, verbal insufficiency, difficulty in verbal communication, obsessiveness, and inattentive appearance. - 老人福祉センターに通所している地域高齢者の余暇活動とQuality of LifeThis study was aimed to investigate actual leisure-time physical activities and to analyze the relationship between the leisure-time physical activities and quality of life in the community-dwelling older people. The subjects were 132 persons who participated in this study. They lived independently and joined the Elderly Welfare Centers of Osaka city. They were examined for types of leisure-time physical activities, depression score, life satisfaction score and health-related quality of life. Seventy-one of 132 subjects (active group) were performing a certain activity more than once a week. Their activities included dancing (n = 22), walking (n = 17), table tennis (n = 16), Japanese-style croquet (n = 11), etc and dancing and table tennis are currently performed in each of the Elderly Welfare Centers. There were statistical significances in the depression score and the life satisfaction score between the active group and the non-active group who performed no activity or less than once a week. But, no significant difference was found as to health-related quality of life. - The purposes of this study were to measure and rank the importance-values of activities of daily living (ADL) in occupational therapy students, and to clarify reliability and validity of the measurement-tool. ADL in this study involved the following five self-care activities : feeding, grooming, dressing, bathing, and toileting. The subjects were 7 male, and 31 female occupational therapy students. The subjects were asked to rate the importance of the five self-care activities by pairwise comparison method. As for the male students, eating was valued highest followed by toileting, bathing, dressing, and grooming. The importance-values of eating and toileting were significantly higher than that of grooming (p<0.01), but there were no significant differences between the other ADL. As for the female students toileting was valued highest followed by eating, bathing, dressing, and grooming. The importance-value of toileting was significantly higher than that of the other ADL ( p <0.01), but there were no significant differences between the other ADL. Regarding the reliability of the measurement-tool, a significant correlation was demonstrated between test and retest of importance-values of the following four activities : feeding, grooming, bathing, and toileting (ρ=0.688-0.486, p<0.001-0.01). - The purpose of this study was to determine the differences in shoulder-abduction, elbow-flexion, and forearm-rotation, and differences in time between eating with chopsticks and spoon. Six students (2 men and 4 women, age : 20.3 ± 0.8 years [range : 19-21]) participated in this study. Using an electromagnetic tracking device system we performed a three-dimensional measurement of the motion of eating pickles with chopsticks and spoon. The subjects sat on a height adjustable chair with the olecranon at the same height as the 70 cm high wooden desk. The dish was placed a forearm-length away from the subject's trunk. As a result, it was revealed that abduction as well as maximum abduction of the shoulder, and elbow flexion when using spoon showed significantly larger ranges than those in using chopsticks (p<0.05). Then minimum elbow flexion was significantly smaller (p<0.05). There were no significant difference in remainder parameters between feeding activities with chopsticks and spoon. - For planning therapeutic programs it is important that therapists know the values of ADL for patients, patient's families and themselves. The purpose of this study was to calculate and rate values of the following five ADL : feeding, grooming, dressing, bathing, and toileting. The subjects were 22 female students of the department of occupational therapy (age : 21.9 ± 3.2). The hierarchy of value was toileting, feeding, bathing, dressing and grooming. The degree of value for toileting was significantly higher than other ADL for the whole cohort, but the degrees of value for other ADL varied per individual. Consequently it is important that therapists appreciate that values of ADL differ within generational and gender level. - The purpose of this study was to determine range of motion of forearm rotation and its velocity in eating. Eighteen young students (6 men and 12 women, age:20.4±1.0 years [range: 19-23]) participated in this study and were categorized into two groups. One group (A group) included 9 subjects with greater shoulder abduction range of motion (mean(±SD):9.6(±3.8) degrees) than the median value (5.5 degrees), while the other (B group) consisted of 9 with less range of motion (mean(±SD):3.4(±0.9) degrees). To measure three dimensional motions of eating pickles with chopsticks, we used an electromagnetic tracking device system. They sat on a height-adjusted chair. Thus olecranon was as height as a wooden desk with 70cm height. A dish was set at a distance of the distal of the elbow. The results indicated A group differed significantly in forearm rotation range of motion (mean (±SD):63.3(±7.1) degrees) from B group (mean(±SD):54.3(±8.9) degrees)(p<0.05), while there was no significant difference in time between the two groups. - The purpose of this study was to investigate the loss of resources and identify the psychological state of elderly victims 6 and 15 months after the Great Hanshin- Awaji Earthquake. The 72 elderly victims (16 men and 56 women; mean age, 82.3 and 80.3 years old, respectively) were interviewed with a structured questionnaire which included demographic information. The responses to the 17 questionnaire items were analyzed using the paired t-test and the Pearson's product moment correlation coefficient. The results indicated that their scores of resource loss were generally low at both 6 and 15 months after the earthquake. However, items such as "personal health", "feeling of having control over one's own life" and "motivation to get things done" showed higher scores than the other items. The resource loss scores revealed no significant differences between the two time points, but it showed a higher positive correlation at 15 than at 6 months. - The purpose of this study was to determine range of motion of forearm rotation and its velocity in eating. Six young students (age:20.5±0.5 years [range:20-21], 2 men and 4 women) participated in this study. To measure three-dimensional motions, we used an electromagnetic tracking device system. The first sensor was attached to the dorsal/distal of forearm with sprint, and the second sensor was attached to the dorsal/medial ulna with elastic belt. The subjects ate pickles with chopsticks. At a wooden desk with 70cm height they sat on a height-adjustable chair, so their olecranon was as high as the desk top. Three positions of a dish were measured; A: at the distance of length of distal to elbow, B: at the distance of length of the upper extremity, and C: at a middle position between A and B. The results revealed that forearm rotation, range of motion and its velocity were significantly different among three conditions (one-way repeated-measure ANOVA)(p<0.05). Characteristically the motion velocity of B was lower than C, and A was lower than C (p<0.05). Findings suggest that 1) limited forearm range of motion would decrease a burden of forearm by setting a dish at B or C than A, 2) the rotation (maximal pronation and supination: 11 and 49 degrees, respectively) was similar, to a fork (maximal pronation and supination: 10 and 51 degrees, respectively), 3) C may be efficiently operated, and 4) the system may be instructed as a good equipment for eating.Kobe University, 1998, 神戸大学医学部保健学科紀要, 14, 53 - 59, Japanese
- Micro FET : Norms for Upper Extremity Muscle Strength and Test-Retest ReliabilityMicro FET (Hoggan Health Industries, Inc.,USA) is a hand-held dynamometer designed for measuring human muscle strength. The purpose of this study was 1) to establish norms for upper extremity muscle strength employing the Micro FET and 2) to investigate the test-retest reliability of this instrument. Two handred and twenty healthy right-handed subjects (110 men and 110 women) ranging in age from 19 to 36 years were tested. Normative data collected can be compared between the muscle strength of a patient and that of a normal individuals at the same sex and age. Most correlation coefficients for the upper extremity muscle strength testing were high and significant. - The Effects of The Great Hanshin-Awaji Eathquake on Activities of Dialy Living of the Elderly and DisabledWe investigated changes of activities of daily living of the elderly and disabled by the Barthel Index scores before, immediately after, and six months after the Hanshin-Awaji Earthquake. According to variation in the overall Barthel Index scores before and after the quake, We divided the subjefts into two groups; The patients whose total scores had not changed after the quake were allocated to Groupl and the others to Group2. In Group2, physical condition got worse immediately after the quake and their bad condition continued for six months. The same tendency of the scores for the Barthel Index scores for mobility activities was recognized. These results suggested that after a big disaster the elderly and disabled who suffer from deterioration of physical conditions run a high risk of worsening dynamic movements in ADL. - Sanding Activity is frequently utilized in occupational therapy treatment program. The purpose of this study was to determine the cardiac responses to bilateral sanding activity by trickmotion of trunk. The parameters; blood pressure and heart rate were selected for 38 normal students. The 3-min activity consisted of five conditions; (1) sitting rest, (2) 0 degrees, 30 cycles per minute (cpm), (3) 0 degrees, 30 cpm by trickmotion of trunk, (4) 45 degrees, 30cpm, and (5) 45 degrees, 30 cpm by trickmotion of trunk. The results indicated that significant difference was found at (3), (4) and (5) of heart rate, and diastolic blood pressure at (3) showed statistically greater then (2). Moreover, no parameters revealed greater statistical significance between 0 and 45 degrees of board angle. - The purpose of this study was to clarify relationship between life style and bone mineral density (BMD) in the patients with postmenopausal osteoporosis. Thirty five women with osteoporosis were examined. Their mean age was 71.1±6.4 years old. Their bone mineral density of distal forearm was measured using by the Dual Energy X-ray Absorptiometry and their life style was evaluated by Breslow's Questionnaires items. As a result, those who showed life style score was not significantly correlated with BMD. The BMDs in those who showed a well life style were higher than those who did not show it in the breakfast, smoking, physical exercise and drinking milk. The women who were classified into "the well-slept group" showed worse BMD comparing with those who were "the poor-slept group". However, women keeping a higher physical activity in daily life as well as in holiday showed better BMD comparing with those who don't. - This is a clinical case report on a 70-year-old female who had poor acceptance of disability. She was referred to an occupational therapist for the treatment of Lacunar stroke in which damages involved the region of the right parietal branches of the middle cerebral artery. It was reported that motor dysfunction and failure of psychological function were ameliorated after the performance of endarterectomy of the right internal carotid artery. In addition to the occupational therapy in the hospital, local welfare services have been given her for the rehabilitation. However, her clinical picture was inferior, and equivocal symptoms increased when she reached the end of the therapy program. Her separation anxiety was caused by the poor acceptance of disability. We extended the duration of the therapy to enhance the acceptance level of the patient. As a result, the frequency of visiting the medical rehabilitation services was reduced. There were the alleviation of the patient's separation anxiety. We suggest the followings : 1) Despite the slight motor dys- function, patient could show poor acceptance of disability along with separation anxiety, and 2) It is necessary for patients with similar symptoms to receive empaphy for adequate period of time to accept their disability.Kobe University, 1995, 神戸大学医学部保健学科紀要, 11, 93 - 97, Japanese

- Japan Society for the Promotion of Science, Grants-in-Aid for Scientific Research, Grant-in-Aid for Scientific Research (C), Kobe University, 01 Apr. 2019 - 31 Mar. 2022Research on the method of matching assistive technology to elderly people focusing on cognitionWe investigated to identify the conditions that older adult with dementia use the ICT devices. We conducted a survey of family members of older adult residents in care centers to identify the conditions under which they access online visitation. There were three main differences between those who were using online visitation to communicate with their older adult relatives in care facilities and those who were not. Most participants using online visitation had information and communication technology support, enjoyed music and movies on mobile devices, and visited the nursing home many times before this pandemic compared to participants who did not engage in online visitation. We were able to find some possibilities when the older adult with dementia use ICT devices with these points.
- Japan Society for the Promotion of Science, Grants-in-Aid for Scientific Research, Grant-in-Aid for Scientific Research (B), Kobe Design University, 01 Apr. 2016 - 31 Mar. 2019Proposal of Dwelling Support System for the DementiaWe developed auto cue system which may support independent living for the persons with early stage of dementia as long as possible. The auto cue system is consisted by central unit and several IoT devices. We adopted the Raspberry Pi for the central unit, and the ESP-8266 micro controller with Wi-Fi function and each sensors for the IoT devices. In parallel, we constructed ten scenarios of auto cue and the demonstrative provisional auto cue system with the MESH Tag by Sony Co. Ltd. Then, the demonstration showed to whom concerned to the dementia at the model house. After the demonstration, we gathered questionnaire sheet from 53 participants. Around 70 percent of them answered useful to this. Especially, "the fire on the stove" and "Doge outing in night time" are thought as highly useful auto cue. The MESH Tag system have installed temporary in three hoses for three months each. Some troubles were found in these trial, not only due to the system but also due to behavior or habit of each.

- Japan Society for the Promotion of Science, Grants-in-Aid for Scientific Research, Grant-in-Aid for Scientific Research (B), Kobe Design University, 01 Apr. 2013 - 31 Mar. 2016Development of Prompting Device for DementiaWe interviewed elderlies who live alone or alone in daytime in their own houses by using ETUQ-Japan (Everyday Technology Usage Questionnaire - Japan) developed in Karorinska Institute Sweden. The amount of subjects is 185 through continuous research. We found some topics what appliances they use continuously and those features. Adding this, we listed troubles or annoyances complaint by their selves or their family, then we categorized them into hour: first is important and prompting may be effective, second is not so important but prompting may be effective, third is important but prompting may not support it, and fourth is not so important and prompting is not relating it. We developed some prompting devices related in first category. They are the gas stove, the washing machine, locking key, indoor atmosphere and confirming items when outing.

- Japan Society for the Promotion of Science, Grants-in-Aid for Scientific Research, Grant-in-Aid for Scientific Research (C), Kobe Design University, 2010 - 2012Design method for household appliances to use sustainably for the persons who has mutual cognitive disorderThe authors visited and surveyed 91 subjects who were 79.8 years old in average and lived alone in his/her own house by using ETUQ (Everyday Technology Usage Questionnaire) which was developed by Dr. Louise in Karolinska Institute Sweden. Thirty of the subjects were dementia and four have diagnosis of MCI (Mild Cognitive Impairment). Through this survey, the authors listed out household appliances and some features of these which were quitted to use or used continuously by the elderly with/without cognitive problems. Then, it was found that some knowledge of interface to allow continuous usage for the elderly persons with some cognitive disorder.In some found problems, the authors developed trial models to support them, and then found validity of voice guidance and better way of it also.
