Background: An international physical activity (PA) questionnaire is beneficial to make cross-country comparisons among children and adolescents. This study assesses the validity of the PA questions in the World Health Organization Health Behavior in School-Aged Children (WHO HBSC) survey in Japanese children and adolescents. Methods: Participants were fifth- to sixth-grade Japanese primary school students (67 students aged 10.8 [0.5] y) and first- to third-grade junior high school students (108 students aged 13.0 [0.7] y). The Japanese version of the PA questions in the WHO HBSC (WHO HBSC-J) was used. To assess the validity of the PA questions, the authors used a partial correlation adjusted for sex, age, and relative weight between the answers to the survey questions and objectively measured moderate to vigorous PA (MVPA) by an accelerometer. Results: A significant positive correlation was found between accelerometer-measured MVPA and the number of reported days with at least 60 minutes/day of MVPA in primary school students (r = .39, P = .002) and junior high school students (r = .32, P < .001). Conclusion: The HBSC-J has moderate validity for evaluating MVPA in Japanese primary school and junior high school students.

Abstract Background Smoking among Japanese adolescents has decreased noticeably. However, little is known whether the decreasing trend in adolescent smoking can be seen across all socioeconomic status (SES) groups. This study aimed to examine trends in socioeconomic inequalities in smoking among Japanese adolescents between 2008 and 2016. Methods We conducted a repeated cross-sectional study using data from three surveys of high school students in Okinawa, Japan, in 2008, 2012, and 2016. The study participants consisted of 7902 students in grades 10 through 12 (15–18 years). Smoking was assessed as current cigarette use. SES indicators included familial SES (parental education and family structure) and student’s own SES (school type). To evaluate absolute and relative inequalities, prevalence differences (PDs) and ratios (PRs) between low and high SES groups were estimated. The slope index of inequality (SII) and relative index of inequality (RII) were also calculated. Results Smoking prevalence among boys and girls significantly declined from 11.5% and 6.2% in 2008 to 4.7% and 1.9% in 2016, respectively. Similar decreasing trends in smoking were found among most of the SES groups. The PDs and SII for parental education in boys and family structure in girls decreased over time while those for school type persisted among boys and girls. The PRs and RII for school type in boys increased while those for other SES indicators among both sexes remained stable over time. Conclusions Smoking among Japanese adolescents has been declining and time trends of socioeconomic inequalities in smoking varied by absolute and relative measures. Further policies and/or interventions to reduce smoking inequalities should focus on the context of schools, especially in vocational high schools.

Objectives: To assess the quality of classroom-based physical activity intervention that integrated academic
contents (active lesson program: ALP), and to update the effectiveness of the interventions on physical activity, health, or educational outcomes.
Methods: Studies, which published from April 2015 to September 2018, were identified in five electronic data-
bases (ERIC, PubMed, Science Direct, Cochrane Library, and EMBASE). Inclusion criteria were: 1) classroom lesson containing both physical activity and educational contents; 2) physical activity, physical constitution, fitness components, academic achievements, or facilitators-of-learning were evaluated as outcome values; 3) intervention studies featuring randomized controlled trials, quasi-experimental design, or pre-post study design; 4) school-aged children (5–18 years) for study participant; 5) at least one week for the intervention period. Physical activity breaks without educational content, the ALP as part of complex interventions, and studies investigating special populations (such as disabled or obese children) were excluded. Risk of bias was assessed using the the Cochrane Collaboration "risk of bias" assessment tool.
Results: All ten studies found increased physical activity following the ALP. Compared with inactive teaching, two studies showed positive effects of the ALP on standardized academic performance test. Four studies found the effectiveness of the ALP on on-task behavior, which is one of the facilitators-of-learning outcomes. Risk of bias was totally high.
Conclusions: Quality of study design has been improving, but the risk of bias is high. Overall, the ALP can increase physical activity level without sacrificing academic performance, but further research is needed to refer to the effectiveness on the health outcome.

Positive association between physical fitness and academic achievement in adolescents has been suggested yet the causal effect of physical fitness on academic achievement remains unclear. This study examined if longitudinal changes in physical fitness were associated with changes in academic achievement among junior high school students. Analyses were based on a two-year with three time-point data of 567 students (aged 12–13 years old at the baseline-point; 303 boys) who entered in five Japanese junior high schools in 2015. Academic achievement was evaluated using the student’s overall grade point average. Comprehensive physical fitness score was summed up from eight fitness tests: 50-m sprint, standing broad jump, repeated side-steps, sit and reach, sit-ups, hand-grip strength, handball throw, and 20-m shuttle run or endurance run. The hybrid regression model was applied to examine the impact of change in physical fitness on change in academic achievement using multiple imputation to account for non-response at follow-up. The changes in fitness score within-person and the differences in average of fitness score of three-time points between-person were associated with change in overall grade point average for boys. No significant association between fitness score and overall grade point average was observed in girls. Opportunities for increased physical fitness may be important to support academic achievement, particularly in junior high school boys.

• Study of Actual Lifestyle of Junior High School Students in Northern Okinawa and Associated Factors Based on Their Health, Academic Skill with Focus on Single Parent Household

Kobayashi Minoru, Agarie Hiroshi, Kyan Akira, Shimabukuro Kei, Uema Tatsuya

The main purpose of this study is to clarify the actual lifestyle of junior high school student in Northern Okinawa and associated factors through focusing more on topics of health, study and single parent households. The study investigated a total of 519 students from 2 local junior high schools, comprising of 263 boys and 256 girls from early June 2017 until the middle of July 2017.An analysis of findings indicated that the girls have a better result of sleeping time and daily exercise time compared with Japan as a whole. Analysis also indicates that improvements are required in areas such as breakfast consumption factor, smartphone usage, study time after school and time of boys daily exercise. In addition, the logic regression results indicate that regular life and gender are related to sleep quality. In contrast, academic skill showed differences between boys and girls. However, the association between cram school attendance and the level of academic skill showed the same result for both boys and girls. Moreover, the single parent household resulted in irregular lifestyle behavior such as playing smartphone games too long. Also, in single parent households with boys, the student was less likely to join a sports club. Mental health was also a element for some single parent households with girls. Overall it is confirmed that junior high school students' lifestyle and their academic skill are impacted by socioeconomic factors.

Kobayashi Minoru, Fujita Tsutomu, Kakazu Kengo, Takakura Minoru, Kyan Akira

The thinking and decision making abilities evaluation toolkit can be applied on exercise activities ofphysical education (PE) of different outlooks. Therefore, the purpose of the present study was to evaluatethe influence of thinking and decision making abilities on motivation in PE class and its antecedent ofbehavior change. The study also aimed to provide physical activity guidance for adolescents. The surveywas conducted in May 2015 among 1,144 students (561 boys and 583 girls) enrolled in public junior highschools in Okinawa, Japan. The questionnaire included thinking and decision making abilities in PE classand "Achievement Motivation in Physical Education Test"(AMPET).However, the pass analysis wasperformed in 842 students (401 boys and 441 girls) with no missing values. The significant positive effectsthe plotted pass outcomes included "learning strategy" against information gathering (β =.32); mutuallearning (β=.45); problem-solving (β=.40); exercise observation and analysis (β=.48); and foundationof learning establish (β=.36), and "overcome obstacles" against problem solving(β=.21); and exerciseobservation and analysis (β=.24).Overall, to foster thinking and decision making abilities, having onlyhigh competence is not sufficient, but well-balanced general motivation is also necessary

Miyagi Masaya, Kyan Akira

The purpose of this study was to examine the effect of stress management program in the exam just before (about 2 weeks until the high school entrance examination). Investigation of the subject was a total of 162 students of the third year that belong to the public junior high school (men 86 students and women 76 students). The results were as follows: (a) The "stress response" indicates a significant decrease in the score, it showed a significant reduction in particular "fatigue" "stiff shoulders" "over-food and drink" among them. (b) There was no significant change for the total score of the "mood". And especially "motivation" and "intensive score" significant changes also was not observed.(c) For a subscale of the mood "tension", it showed a significant increase in scores after the intervention program. In conclusion, the practice of stress management programs of the high school entrance examination just before reduces the possibility of the physical stress response has been suggested. However, there is no change in the mood, especially for tension and increases rather suggesting it is difficult to reduce.

• Trends in current alcohol use among Japanese adolescents by sociodemographic groups

M Takakura, A Kyan, M Miyagi, M Kobayashi

14th European Public Health Conference 2021, 20 Oct. 2021, English, オンライン, Abstract Background Alcohol is the most commonly used substance among Japanese adolescents, although its use has been declining recently. However, little is known whether the declining trend in adolescent drinking can be found across all sociodemographic characteristics. This study explored time trends in alcohol use among Japanese adolescents by sociodemographic characteristics between 2008 and 2016. Methods A repeated cross-sectional study was conducted using data from three prefectural-wide surveys of public high schools in Okinawa Prefecture, Japan, in 2008, 2012, and 2016. The study participants comprised 7,807 students in grades 10 to 12 (15-18 years). Alcohol use was defined as drinking one or more alcoholic beverages on at least one day in the past month. Demographic characteristics included grade, sex, and region, and socioeconomic status (SES) was assessed by family structure, parental education, and school type. Time trends in alcohol use over time by each sociodemographic group were assessed by the Cochran-Armitage test for trend. Results The overall prevalence of adolescent alcohol use significantly reduced from 20.7% (95% CI 19.1-22.3%) in 2008 to 9.5% (95%CI 8.4-10.6%) in 2016. Similar reducing trends in adolescent alcohol use were found across all of the demographic characteristics as well as the SES groups (all P for trends <0.001). During the survey periods, there were no sex differences in alcohol use prevalence, while alcohol use was more prevalent among old graders. Alcohol use was also more prevalent among low SES groups than high SES groups. However, family structure in 2016 did not show significant differences in alcohol use. Conclusions Alcohol use among Japanese adolescents has been declining over time and this downward trend was similar for all demographic and socioeconomic subgroups. Further adolescent drinking prevention efforts should be implemented across all population subgroups. Moreover. monitoring of adolescent alcohol use should be continued. Key messages This study found that adolescent alcohol use in Japan has been decreasing over time across all population subgroups. The finding is a desired outcome from the viewpoint of the public health impact.

• Adherence to 24-hour movement guidelines among Japanese elementary and junior high school students

A Kyan, M Takakura, M Miyagi, M Kobayashi

14th European Public Health Conference 2021, 20 Oct. 2021, English, オンライン, Abstract Background For optimal health benefits, the Canadian 24-Hour Movement Guidelines for Children and Youth (aged 5-17 years) recommend an achievement of ≥ 60 minutes of moderate-to-vigorous physical activity (MVPA), <2 h of recreational screen time (ST), and 9-11 h for 5-13 years or 8-10 h for 14-17 years of sleep time (SLT) each day. There is little evidence showing the proportion of the recommendation in Japan, so we need to investigate. The aim of this study thus was to reveal the compliance to the recommendation with a large sample of Japanese elementary and junior high school children. Methods This study included 2408 students enrolled in fifth grade (aged 10-11 years) in 31 elementary schools, and 4360 students enrolled in eighth grade (aged 13-14 years) in 30 junior high schools. Each behaviour was assessed by a self-reported questionnaire. To analyze the differences in adherence to the recommendation between the school-aged group, we conducted a chi-analysis. Results Overall, only 4% of participants complied with all three recommendations, and 20% of the students did not comply with any of the recommendations (χ2=1390.87;p<.001). Compared with MVPA and ST, SLT had a trend to be likely to achieve the recommendation in each school-age group. Elementary school students had a higher percentage of non-adherence to all recommendations (39%) than junior high school students (10.3%). The adherence to the recommendation of the ST & SLT simultaneously had no differences in each age group (11.5% for elementary schoolers and 12.7% for junior high schoolers). However, adherence to the recommendation of MVPA only, ST only, and MVPA & ST were higher in elementary schoolers, and the adherence to the recommendation of SLT only and MVPA & SLT were higher in junior high schoolers. Conclusions The proportion of adherence to at least one of the three recommendations is better in junior high school students than elementary school students in Okinawa, Japan. Key messages Health behaviour required improvement may differ by age group. It is necessary for particularly elementary school children in Okinawa to take proper countermeasures to improve the movement behaviours.

This study aimed to cross-sectionally and longitudinally examine the relationship between physical activity, physical fitness and academic achievement among youth in Okinawa. We also reviewed and developed active lesson programs integrating physical activity with academics in the classroom. A cross-sectional study showed that aerobic fitness mediated the relationship between physical activity and academic achievement among junior high school boys. A follow-up study for three years in junior high schools showed that the changes in physical fitness within- and between-person were positively associated with change in overall grade point average for boys. As physical fitness is unlikely to negatively influence academic performance, opportunities for improving physical fitness may generally support academic achievement among junior high school students. Lastly, we reviewed intervention studies on active lesson programs and proposed the programs adapted to the course of study for arithmetic.
